Levels of fibrinopeptide A (FPA) and fibrinopeptide B beta 15-42 (FPB beta) in urine and plasma after urokinase administration in patients with chronic glomerulonephritis.

A study on the significance of the measurement of fibrinopeptide A (FPA) and fibrinopeptide B beta 15-42 (FPB beta) in urine and plasma before and after urokinase (UK) administration in patients with chronic glomerulonephritis is described. FPA and FPB beta in urine and plasma were measured by radioimmunoassay. The levels of fibrinogen degradation product (FDP) in urine and sera were also determined before and after the UK administration. UK was administered at a dose of 48,000 IU/20 ml in saline intravenously, followed by an intravenous infusion of 250 ml of 5% glucose for one hour. It was demonstrated that the levels of FPB beta in urine and plasma showed a significant increase after UK administration in patients with chronic glomerulonephritis. However, there was no significant difference in the levels of FDP in urine and sera before and after UK administration. It was concluded that the measurement of FPB beta in urine and plasma may be useful for evaluating the efficacy of UK therapy in patients with chronic glomerulonephritis.

A case of acute renal failure associated with type A acute hepatitis responds dramatically to plasmapheresis.

A case of acute renal failure (ARF) associated with type A acute hepatitis followed by a dramatic response to plasmapheresis is described. A female patient was transferred to our hospital with a diagnosis of ARF associated with acute hepatitis. Since radioimmunoassay for IgM antibody to hepatitis A virus showed a positive reaction, recent infection with hepatitis A virus was suspected. Hemodialysis immediately performed after admission was not effective. Plasmapheresis was started on the 5th hospital day and there was a dramatic improvement in ARF and hepatitis. It is concluded that plasmapheresis is effective in the treatment of ARF associated with liver disease if it is applied in the early stage.

Parameters of altered plasma glucose levels during short intervals in diabetic patients.

The percentages of glycosylated proteins or albumin, or glycohemoglobin A1 (HbA1) in sera from patients with noninsulin dependent diabetes mellitus (NIDDM) were measured to elucidate whether such measurements are useful for evaluation of the mean levels of blood glucose during short intervals in these patients. Forty patients with NIDDM and twenty healthy adults were examined. The percentages of glycosylated proteins or albumin were quantitated by a Glyc-Affin system and those of HbA1 were measured by electrophoresis. It was indicated that the percentages of glycosylated proteins or albumin, and HbA1 in sera from patients with NIDDM were significantly higher than those in sera from healthy adults. There was a significant correlation between the percentages of HbA1 in sera and the mean levels of fasting blood sugar (FBS) during short intervals in patients with NIDDM. It was concluded that the measurement of HbA1 is a useful method for evaluation of the mean levels of blood glucose during short intervals in patients with NIDDM compared with those of glycosylated proteins or albumin.

Specific binding of circulating IgA antibodies in patients with IgA nephropathy.

Detection of circulating IgA antibodies which are specific in patients with IgA nephropathy is described. Freeze and thawed extracts of pharyngeal cells obtained from patients with IgA nephropathy, other glomerular diseases, and healthy adults were cultured with fibroblasts such as Vero or Hel cells at 37 degrees C for 2 weeks. Serum samples were obtained from these patients and healthy adults. The cultured fibroblasts were fixed on slide glasses, and then incubated with the serum samples from the same or other patients with IgA nephropathy. The cells were stained with FITC-labeled heavy-chain specific anti-human IgA antiserum and then examined with a fluorescent microscope. It was demonstrated that the IgA antibodies in sera obtained from patients with IgA nephropathy or HSP nephritis were bound with the nuclear regions of such fibroblasts. It was suggested that IgA antibodies in sera could be bound with some antigenic substances which were transferred from pharyngeal cells of patients with IgA nephropathy to fibroblasts in vitro.

Discrepancy of serum IgA levels determined by single radial immunodiffusion and laser nephelometry in patients with IgA nephropathy.

The levels of IgA in sera were quantitated by single radial immunodiffusion (SRID) and laser nephelometer (LN) to evaluate a precise amount of serum IgA in patients with IgA nephropathy, other glomerular diseases and healthy adults. Thirty patients with IgA nephropathy, twenty patients with other glomerular diseases and eighteen healthy adults were examined. It was shown that the levels of IgA in sera measured by LN were significantly higher than those in sera measured by SRID in patients with IgA nephropathy associated with increased levels of serum IgA. This suggests that the levels of serum IgA measured by SRID in patients with IgA nephropathy were affected by the physicochemical characteristics of circulating IgA in patients with IgA nephropathy. It is postulated that an increase of serum IgA in patients with IgA nephropathy may be mainly due to an increase of polymers rather than a monomer of IgA.

Evaluation of the staining findings of immunofluorescence in unfixed or fixed renal biopsy specimens from patients with IgA nephropathy and membranous nephropathy.

A study on the evaluation of staining findings of immunofluorescence in unfixed or fixed renal biopsy specimens is described. Renal biopsy specimens obtained from ten patients with IgA nephropathy and membranous nephropathy were embedded in gelatin or paraffin matrix. Renal biopsy specimens embedded in paraffin matrix were digested with 0.05% protease. The specimens were stained with FITC-conjugated anti-human IgA, IgG, IgM or C3 antisera at 4 degrees C overnight. IgA, IgG or IgM were markedly observed in glomeruli using unfixed materials embedded in gelatin matrix or 10% neutral buffered formalin fixed materials embedded in paraffin matrix from patients with IgA nephropathy and membranous nephropathy. There was no significant difference in the intensity or distribution of IgA, IgG or IgM deposition among the two different conditions of immunofluorescence in patients with such diseases. Although the deposition of IgA using unfixed materials embedded in gelatin matrix was prominently coarse granular or lumpy in glomeruli from patients with IgA nephropathy, that of IgA using 10% formalin fixed materials embedded in paraffin matrix was fine granular and/or interrupted linear in glomeruli. It was suggested that the immunofluorescence in renal biopsy specimens embedded in paraffin matrix after digestion with protease is useful for the evaluation of immunoglobulins in glomeruli from patients with IgA nephropathy or membranous nephropathy.

Immunofluorescence staining in unfixed or fixed renal biopsy specimens from patients with diabetic nephropathy.

Immunofluorescence staining in unfixed or fixed renal biopsy specimens were evaluated in nine patients with diabetic nephropathy in order to elucidate if immunofluorescence staining is applicable in fixed renal tissues in such patients. Renal biopsy specimens were embedded in gelatin or paraffin matrix. Renal biopsy specimens embedded in paraffin matrix were digested with 0.05% protease. Immunofluorescent studies of kidney tissues were performed by staining with FITC-labeled heavy chain specific anti-human IgG, IgA, IgM, acute phase reactant (APR) proteins such as alpha 1-anti-trypsin (alpha 1-AT), haptoglobin (Hpt) and beta-lipoprotein (beta-Lp) antisera, and then examined with a fluorescent microscope. Linear and nodular deposition of IgG, IgA, IgM, alpha 1-AT, Hpt, and beta-Lp were observed in the glomerular capillary walls of the renal specimens embedded in paraffin matrix. The staining patterns in specimens embedded in paraffin matrix was similar to that embedded in gelatin matrix. There was no significant difference in the intensity or distribution of IgG, IgM, alpha 1-AT, and beta-Lp deposition among the two different conditions of immunofluorescence in patients with diabetic nephropathy. It was suggested that immunofluorescence staining in renal biopsy specimens embedded in paraffin matrix after digestion with protease is useful for the evaluation of IgG, IgM, APR proteins, and beta-Lp in glomeruli from patients with diabetic nephropathy.

Immunofluorescent studies on alpha 2-plasmin inhibitor (alpha 2-PI) in glomeruli from patients with IgA nephropathy.

Detection of alpha 2-plasmin inhibitor (alpha 2-PI) and/or fibrinogen in glomeruli by immunofluorescence in 26 patients with IgA nephropathy was described. The present study showed that glomerular injuries such as glomerular adhesion to Bowman's capsule and the cellular and/or fibrous crescent were predominantly observed in glomeruli with alpha 2-PI and/or fibrinogen deposits in patients with IgA nephropathy. Alpha 2-PI coexisted with fibrinogen in glomeruli from patients with IgA nephropathy. It was postulated that the deposition of alpha 2-PI in vivo might lead to the accumulation of glomerular fibrinogen deposits in patients with IgA nephropathy. It was suggested that the depositions of alpha 2-PI and/or fibrinogen in glomeruli may be one of the exacerbative factors in glomeruli from patients with IgA nephropathy.

A case of primary systemic amyloidosis.

An adult patient with primary systemic amyloidosis associated with nephrotic syndrome and complete atrial-ventricular (A-V) block is described. This case was considered to be a slowly progressive primary amyloidosis because of a long time episode of poor appetite and nephrotic syndrome. Temporal pace making of the heart was not effective presumably due to heart muscle damage, pér sé. Autopsy specimens revealed typical features of primary amyloidosis in light microscopy and immunofluorescence. It is suggested that early detection of amyloid fibers in the heart muscles may improve prognosis in patients with primary amyloidosis.

An autoradiographic analysis of experimental non-oliguric acute renal failure in the mouse.

Non-oliguric acute renal failure is an increasing disease entity although management of this disease is yet to be established. Poisoning with the sublethal doses of HgCl2 provides an experimental model for the study of non-oliguric acute renal failure. The purpose of this study was to determine the mechanisms of regeneration of renal cortex in HgCl2 treated mice using whole body autoradiograms to evaluate the changes of DNA synthesis in the renal cortex as well as whole body organs in the mice. The whole body autoradiograms showed considerable uptake of thymidine in the renal cortex of the mice after 1.5 h, the highest uptake after 48 h and negligible uptake 8 days after the injection of HgCl2. On the other hand, autoradiograms showed no remarkable changes in the uptake of thymidine in the brain, lungs, heart and liver in HgCl2 treated mice. It was suggested that the initial treatment of non-oliguric acute renal failure within one week may play an important role in its prognosis.

Cold-reacting antinuclear factor in sera from patients with primary glomerular diseases.

Antinuclear factor (ANF) was determined in sera from patients with primary glomerular diseases to evaluate the autoimmune nature of this disorder. Sera from 61 patients with various types of glomerular disorders were assayed for ANF by the fluorescent antinuclear antibody (FANA) technique at 37 degrees C and 4 degrees C. Thirty healthy adults served as controls. Cold-reacting ANF, which was detected predominantly in the class of IgM, was observed in 54 out of 61 patients with various types of primary glomerular disorders. The nuclear staining pattern was "speckled". Sera from healthy adults did not show any positive signs of ANF. It is suggested that some autoimmune mechanisms may be involved in the development of primary glomerular diseases.

Effect of danazol on solubilization of immune deposits in patients with IgA nephropathy.

A study on the solubilization of glomerular immune deposits by administration of danazol in patients with IgA nephropathy is described. A clinical trial on danazol was performed in seven patient with IgA nephropathy. Administration of danazol was effective in improving proteinuria in patients with IgA nephropathy. In vitro effects of patients' sera on solubilization of glomerular immune deposits were examined in parallel studies. Renal biopsy specimens obtained from IgA nephropathy were incubated with fresh patients' sera before and after administration of danazol at 37 degrees C for one hour in plastic test tubes. The sections were stained with FITC-labeled heavy chain-specific anti-human IgA antiserum and then examined with a fluorescent microscope. It was shown that the solubilization of glomerular immune deposits by sera after administration of danazol from patients with IgA nephropathy was significantly higher than that by sera before such treatment.

IgA nephropathy associated with HLA-DR4 antigen.

This report describes 2 siblings with IgA nephropathy. Patient No. 1 was a 38-year-old woman with hematuria and proteinuria of 19 years duration. Her blood ABO type was A and Rh positive. She was found to have HLA-A2,Aw24; Bw54 , Bw48 ;Cwl,C-;DR1,DR4. Her renal specimen was diagnosed as the advanced stage of IgA nephropathy histologically. Patient No. 2 was a 41-year-old man who was a brother of patient No. 1. His blood ABO type was O and Rh positive. His serotype for the HLA was found to be HLA-Aw24,A-;Bw35, Bw54 ;Cw1,Cw3;DR4, DRw9 . His renal histology showed the advanced stage of IgA nephropathy. It is suggested that an abnormal immune response linked to gene coding for HLA-DR4 antigen might be involved in the development of IgA nephropathy.
