[Histopathological and clinical studies of acute necrotizing ulcerative gingivitis and evaluation of treatment].

We report the clinical data, light and electron microscopic findings, therapy and clinical course of three cases treated for acute necrotizing ulcerative gingivitis. The following results were obtained. 1. An ulcerous lesion was observed in the gingiva of all three cases, with case I also showing a depressed lesion in the ulcerous region. 2. In all three cases, the gingival sections displayed ulceration and showed fibrin deposition and neutrophil infiltration on the surface of the ulcerous region. Congested blood vessels and neutrophil infiltration also were observed in the connective tissue beneath the ulcerous region. 3. Many microorganisms, fibrin, cell debris, and enlarged intercellular spaces of the epithelium were seen on the surface on the ulcerous region. 4. Localized treatment, mainly plaque control, was found to be effective clinically and histopathologically.

Necrosis of the femoral head in growing rats. Occlusion of lateral epiphyseal vessels.

The cause of the vascular occlusion in necrosis of the femoral head of growing, spontaneously hypertensive rats was investigated histologically using serial sections. The lateral epiphyseal vessels, which supply the proximal femoral epiphysis, disappeared immediately before entry into the femoral heads. In fresh osteonecrosis the pathway of the vessels between the margin of the articular cartilage and the growth plate was replaced with granulation or scar tissue. We conclude that the vascular occlusion occurs in the layer of the epiphyseal cartilage where the lateral epiphyseal vessels penetrate, and that the abnormalities of the epiphyseal cartilage might play a part in the occurrence of osteonecrosis.

[Analyses of the effects of intravesical Bacillus Calmette-Guerin (Tokyo 172 strain) therapy of superficial bladder cancer].

In this paper, we summarize the results of a multicentric controlled study of induction treatment with intravesical BCG in patients with superficial bladder cancer (Ta, T1) or CIS. The BCG Tokyo 172 strain (Japan BCG K.K.) was given for 8 weeks, as a rule, in a dose of either 80 mg or 120 mg in 40 ml of saline instilled into the bladder. Sixty-six (90.5%) of the 73 patients registered in the study were evaluable for response. High response rates were obtained in the patients with Ta or T1 tumors, irrespective of the treatment dose; 65.4% CR, 27.3% PR and 7.3% NC for all. The number of CIS patients is too small to draw any conclusions, but 54.5% CR, 36.4% PR and 9.1% NC were obtained. Several factors were evaluated in relation to the tumor ablative effects. The CR rate was significantly higher for tumors less than 1 cm in size compared with tumors 1-3 or larger in size, although the response rates, including CR and PR, were very similar irrespective of the size. Other factors did not contribute to provoke a significant difference. Japanese elder people show a high positive rate of PPD reaction. Thus, in our study, 78% of the patients showed positive PPD reaction. In this situation, no significant relation between PPD reaction and BCG efficacy was obtained. In addition, OKT8, OKT7, OKT4, OKT4/OKT8 ratio, Leu7, ADCC activity and NK activity did not show any correlation with tumor ablative effect of BCG.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of gall bladder cancer with high level alpha-fetoprotein].

A case of primary gall bladder tumor with high level AFP were demonstrated, which showed hepatocellular carcinoma like pattern with trabecular proliferation histologically, positive AFP stain of cancer cells by a peroxidase conjugated antibody method immunohistochemically. This case was no metastasis with liver and lymph nodes.

HLA-linked immune suppression in humans.

There is no doubt that HLA-DR molecules are acting as the products of HLA-linked immune response genes (Ir-genes), because (i) HLA-DR molecules are the restriction elements in the interaction between CD4+ helper T cells and antigen-presenting cells (APC) to respond to many antigens such as streptococcal cell wall antigen (SCW) (Nishimura & Sasazuki, 1983; Sone et al., 1985; Hizayama et al., 1986), schistosomal antigen (Sj) (Hirayama et al., 1987), Mycobacterium leprae antigen (ML) (Kikuchi et al., 1986) and so on; and (ii) anti-HLA-DR monoclonal antibodies completely abolish the immune response to those antigens (Nishimura & Sasazuki, 1983; Sone et al., 1985). However, genetic analysis of the immune response to those antigens in families or populations revealed that responsiveness is recessive and non-responsiveness to those antigens is a dominant genetic trait that is tightly linked to HLA (Sasazuki et al., 1980a, 1983; Watanabe et al., 1988). This is completely opposite to the situation under the Ir-gene control where responsiveness is dominant and non-responsiveness is recessive. In this paper, we report evidence of how we came across the concept of HLA-linked immune suppression genes (Is-genes) besides Ir-genes, and show evidence for the epistatic interaction between HLA-DR and DQ to determine the immune response to several antigens in humans.

[The usefulness of motion artifact suppression technic (MAST) in the MRI diagnosis of liver tumors].

The usefulness of motion artifact suppression technique (MAST) in the MRI diagnosis of liver tumors was assessed. Spin-echo with MAST was superior in lesion detection and contrast-to-noise ratio to conventional spin-echo. Its usefulness was verified. As the portal vein is delineated frequently as a high signal area, the portal structure up to the third branches was distinctly visualized. It was found helpful for the segmental diagnosis of the liver. However, careful attention is needed in the diagnosis of portal vein thrombosis.

[Distribution of pulsed intra arterial infusion chemotherapy in hepatic carcinomas].

Evaluation of "Gianturco-Wallace chemotherapy pulser," which was developed to produce a more homogeneous drug distribution of the tumors in intra-arterial infusion chemotherapy, was assessed by comparative study of pulsed and nonpulsed arterial radionuclide infusion using Tc-99m pertechnetate for 18 cases of hepatic carcinomas (11 cases of hepatocellular carcinomas and 7 cases of metastatic hepatic carcinomas). Tc-99m pertechnetate, 740 MBq (20 mCi) diluted with saline (30 mL) was infused with or without pulse through the catheter into the hepatic artery at a rate of 1mL per minute. The intrahepatic dynamic radionuclide distribution was analyzed by the time activity curves of ROIs in the tumor and nontumor areas. Pulsed infusion interrupted laminar flow and produced more homogeneous radionuclide distribution in the liver, and combination of pulsed and nonpulsed infusion also produced better radionuclide distribution in the areas of the tumors. This method using Tc-99m pertechnetate was very useful as a simulation to determine the dynamic drug distribution of the tumor and non-tumor region in intraarterial infusion methods.

[Subacute myeloneuropathy after abuse of nitrous oxide: an electron microscopic study on the peripheral nerve].

We have experienced a case of myeloneuropathy following habitual abuse of nitrous oxide. We report clinical and pathological findings of this case with review of literatures. A 36-year-old dentist was first admitted to our hospital on August 17, 1983 because of numbness of both lower legs and unsteady gait. He had recreationally inhaled nitrous oxide 30 to 60 minutes everyday since a year ago. Neurological examination showed ataxic broad-based gait, moderate loss of pain and touch sensation in the lower limbs up to patella. Position and vibratory senses were more severely impaired. Deep tendon reflexes increased in the upper extremities and at the knees, but diminished at ankle jerks. Muscle strength was normal. Prominent Lhermitte's sign was present. Except for reduced serum vitamin B12 level, laboratory results were normal. Needle electromyography showed high amplitude and long duration NMU in the right quadriceps and anterior tibial muscles. Motor nerve conduction velocity was 34 m/sec at the right posterior tibial nerve and could not be detected at the left. Sural nerve biopsy was performed. The density of the myelinated fibre measured on transverse section was in normal range but degenerated fibres were occasionally recognized. Single teased nerve fibre showed various degree of myelin ovoid along the fibre. Myelin loss was shown to occur in some parts of the fibre. Electron microscopy showed myelin splitting and formation of intramyelinic vacuoles containing myelin debris. Axon was almost normal at least in the early stage of degeneration. Later, axon disappeared with destruction of myelin sheath. These nerve changes largely demonstrated demyelination but it was occasionally accompanied with axonal degeneration.(ABSTRACT TRUNCATED AT 250 WORDS)

[Hemorrhagic infarction following cerebral vasospasm].

Among 528 cases with ruptured aneurysm, 10 cases (1.9%) developed hemorrhagic infarction following vasospasm. There was no obvious relationship between the occurrence and location of aneurysm and the neurological grade on admission. Hemorrhagic infarction occurred from day 9 to 25 (mean day 16) after aneurysmal rupture, and the major neurological symptoms were aggravation of consciousness level, which appeared in 6 cases. On the CT scans of the hemorrhagic infarction following vasospasm, nine cases revealed heterogeneous hemorrhage as assembled of spotty or linear hemorrhages within the ischemic infarction, and 5 cases had massive hemorrhagic infarction in size with mass effect. Although surgical therapy for 2 cases and conservative therapy for 8 cases were performed, the results were unfavorable; ie, 2 cases were good, 5 fair or poor, and 3 died. Especially, 5 cases with massive hemorrhagic infarction obviously resulted in poor prognosis. In our series, induced hypertension therapy for vasospasm was considered as a risk factor. In conclusion, it is necessary to avoid induced hypertension therapy in the remission stage of vasospasm and serial SPECT study might be recommended as a useful prospective method estimating the vasospasm.

[Multicystic encephalomalacia in an adult--a case report].

A case of multicystic encephalomalacia found in adult life was described. A 35-year-old man was admitted to our hospital with a chief complaint of unsteadiness. He had developed normally until he fell into the shock state induced by mismatch blood transfusion at the age of 15 months. Since then he has been mentally retarded moderately and had clumsiness of the skillful movement in the right hand. The other neurological abnormality was hyperreflexia only. Laboratory examination failed to disclose metabolic defect. Both CT scan and MRI demonstrated numerous cystic lesions of various size spreading over bilateral cerebral white matter partially involving the inner layer of the cortex. On the contrary basal ganglia, cerebellum and brainstem were completely spared. The diagnosis of MCE was made from (1) anoxic-ischemic episode in infancy, (2) static clinical picture and (3) characteristic distribution of cystic lesions. It is well known that MCE results from perinatal hypoxia, but it is a polyetiologic condition caused by various damages to immature brain of early infancy and usually results in severe psychomotor retardation. Nonetheless, it is intriguing in our case that marked discrepancy was found between morphological change and neurological deficit. It is probable that at the age of 15 months the myelination of major projecting fibers was almost completed, but sufficient plasticity was preserved in immature brain. As a result, the patient had the neurological deficit in the minimum degree in spite of severe morphological change.

Interconvertible hairpin structure found in the replication origin of phage G4 single-stranded DNA.

We found a synthetic GCGAAAGC fragment with a mobility greater than that of other oligodeoxyribonucleotides in gel electrophoresis to take on a stable hairpin structure possessing two terminal G-C base pairs. The GCGAAAGC sequence is also found in the replication origin of phage G4 single-stranded DNA, but the hairpin structure originally proposed differs from that of the GCGAAAGC fragment we have studied. Possibility of rearrangement of the secondary structure in the replication origin of phage G4 was examined in relation to its replication initiation mechanism.

[Determination of the MIC of cefotetan against freshly isolated gram-negative bacilli].

Antimicrobial activities (MICs) of cefotetan (CTT) against 575 strains of 16 spp. of Gram-negative bacilli isolated in 1988 were determined to investigate distribution of MICs in comparison with those of cefmetazole (CMZ), cefoxitin (CFX), latamoxef (LMOX) and cefazolin (CEZ). The change in frequencies of incidence of cephem-resistant strains in the latter half of the 1980 was also investigated. Distribution of MIC of CTT varied with test strains. No or very few MICs were at or higher than 12.5 micrograms/ml at an inoculation of 10(6) cfu/ml, thus rates of CTT-resistant strains were low among Escherichia coli, Citrobacter diversus, Klebsiella spp., Proteus spp., Providencia spp., and Haemophilus influenzae. High rates of resistance to CTT were observed, however, among Citrobacter freundii, Enterobacter spp., Serratia marcescens, Morganella morganii, and Bacteroides fragilis. 2. Most CTT-resistant strains of C. freundii, Enterobacter spp. and S. marcescens were also resistant to LMOX. These resistant strains were considered to be multi-resistant strains to antibiotics including oxime type cephalosporins and monobactams. 3. Cephem-resistant E. coli was confirmed to be resistant to 22% of CEZ, 14% of CFX, 10% of CMZ and 2% of CTT tested. The incidence of cephem-resistant E. coli unconditionally showed an increasing tendency. 4. The incidence of resistant strains against cephamycins including CTT is discussed with regard to the mechanism of resistance against all beta-lactam antibiotics, and the problem of the appearance of resistant strains is close and inseparable from social background.

An immunopathological study on the placenta in pre-eclampsia.

An investigation was done to clarify the pathological background of pre-eclampsia. Thirty four placentae were studied immunopathologically in comparison to control cases. The results showed slightly positive stains of hCG containing alpha- and beta-subunits in syncytiocytotrophoblastic layers especially in severe pre-eclampsia cases. The placentae in mild pre-eclampsia showed only weakly and partly positive by those stains. Other control cases were negative except earlier abortion ones. No any positive stains of hCG series could be seen in two placentas from mothers with spontaneous hypertension. Stains by hPL and SP 1 were all positive and increased gradually in their staining pattern and degree from early pregnancy toward term. It supports the findings that maternal serum hCG series increased slightly in pre-eclampsia and that those changes might occur before clinical onset of pre-eclampsia. Then, the placenta has an important role in developing pre-eclampsia.

[Ultrasonography-guided cobalt-60 brachytherapy of malignant glioma].

Brachytherapy with cobalt-60 source is reported. In this method it is characterized that the source is inserted interstitially with remote control system by after-loading method via outer catheter (using tandem tube), which was established in the center of residual tumor, using ultrasonography guide with trepanation, or intraoperatively put within the dead space after tumor resection. Six cases of deep-seated and recurrent malignant glioma, were treated with this method. A total dose of 20 to 45 Gy (10 to 15 Gy/day for 2 to 3 days) was delivered to the target. Additionally conventional external irradiation was followed. The effect of cobalt-60 brachytherapy on such tumors were favorable especially for well-circumscribed glioma less than 3 cm on CT scan.
