Determination of discriminatory power of prognostic factors for recurrence of breast cancer.

Vascular and lymphatic invasion of cancer cells, type of histology, histopathological TNM classification, level of tumor invasion, menopausal status, and age were reported as prognostic factors for breast cancer. We determined the discriminatory power of these prognostic factors for predicting breast cancer recurrence in 98 patients with the method of quantification theory type II, using a computer, model Hi-TAC M-150. The discriminatory rate of these combined seven factors for recurrence was 82.609%. We calculated the probability of breast cancer recurrence using the method of quantification theory type II. The probability of breast cancer recurrence (P) was given as follows. P = 1/1 + 7.333e5.468X. The score X is given by the sum of category weight of seven prognostic factors. Averages of P values of patients with recurrence and without recurrence were statistically different (p less than 0.01). The rate of having more than 30% of P values was 78% in 23 patients with breast cancer recurrence, and it was only 3% in 75 patients without breast cancer recurrence. The difference was statistically significant (X2 = 61.83, p less than 0.001). These results suggest that we can discriminate a high-risk patient with breast cancer recurrence using this method.

Bilateral persistent primitive hypoglossal arteries associated with a large ruptured aneurysm on one side.

Bilateral persistent primitive hypoglossal arteries associated with multiple aneurysms in a 59-year-old man are reported. Angiography revealed bilateral persistent primitive hypoglossal arteries with a large ruptured intracranial aneurysm on the left side. There was one additional small aneurysm at the trifurcation of the right middle cerebral artery. This seemed to be the first case of bilateral persistent primitive hypoglossal arteries associated with multiple aneurysms.

Synthesis and antibacterial activity of 2,2'-dithiobis(benzamide) derivatives against Mycobacterium species.

A series of compounds, which are analogues of 2,2'-dithiobis(benzamide), were synthesized and tested for in vitro antibacterial activity against Mycobacterium tuberculosis H37Rv including resistant strains against streptomycin, kanamycin, or isonicotinic acid hydrazide. MICs of these compounds against atypical mycobacteria, Mycobacterium kansasii and Mycobacterium intracellulare were also examined. Structure-activity relationships were found in a series of (acyloxy)alkyl ester derivatives depending upon the length of alkyl carbon chain. The MIC of the most potent compound, 2,2'-dithiobis[N-[3-(decanoyloxy)propyl]benzamide] [56] was superior or at least equivalent to streptomycin, kanamycin, and ethanbutol. All the compounds showed no cross-resistance between the current antitubercular agents.

[A clinicopathological study on hematogenous recurrence of colo-rectal cancer--with special reference to prediction and prevention of the recurrence].

Hematogenous recurrence was investigated in 325 cases of colo-rectal cancer. Hematogenous recurrence was confirmed in 34 cases including 25 liver, 5 lung, 2 bone, one brain and one skin recurrence. Most of the cases (28 cases, 82.4%) revealed a single or multiple hematogenous recurrences without showing additional other types of recurrence. Histological examination of cancer lesions indicated that patients with the findings consisting of moderate and/or high grade of vein invasion (v2-3), subserosal and extramural vein invasion with node metastasis showed high recurrent rate. One and two year survival rate of the patients treated with MF-MF' adjuvant chemotherapy was greater those the patients with F-F' chemotherapy or those without chemotherapy. However, 3-, 4- and 5- year survival rate did not show significant difference among 3 therapeutic groups. The analysis of survival curves also indicated no significant difference among 3 therapeutic groups. These results may suggest that hematogenous recurrence is derived from pre- and/or intra-operative micrometastasis or transplantation of cancer cells, that patients with the above mentioned histological evidence must be clinically treated as a high risk group for recurrence and that MF-MF' adjuvant chemotherapy can be effective in lowering the recurrence rate during the first one or two years after operation.

[Mechanism and classification of hormone therapy for human breast cancer studied by changes of SHBG binding capacity].

We determined SHBG titer before and during hormone therapy for breast cancer. The increased SHBG titer was 78 +/- 29 pmol/ml of plasma in DES responsive breast cancer. However that of DES non-responsive breast cancer was 31 +/- 22 pmol/ml of plasma (p less than 0.01). SHBG titer was increased during the administration of tamoxifen and was decreased during that of fluoxymesterone and after surgical hormone therapy. These results indicate that the mechanism of hormone therapy was due to a decreased free estradiol, which was caused by increased SHBG titer and resection of the ovarium or adrenal gland.

[Systemic chemotherapy of mammary carcinoma: plasma and tissue concentrations of 5-fluorouracil and adriamycin, and result of CAF and CMF therapy of breast cancer].

Plasma and tissue concentrations of 5-fluorouracil(5-FU) were determined with bioassay method in 8 primary breast cancers. After oral administration of 200 mg/M2 of 5-FU. peak plasma concentration was detected fifteen minutes after administration. Concentration of 5-FU in normal breast tissue was only detected in one case out of seven: the concentration was 0.080 microgram/g. However, concentration of 5-FU in breast cancer tissue was detected in three cases out of seven, ranged from 0.041 microgram/g to 1.300 microgram/g. On the other hand, plasma and tissue concentrations of adriamycin (ADR) were determined with fluorometric assay in 6 primary breast cancers. 6-20 mg/m2 of ADR was administered intravenously. Plasma concentrations of ADR ranged from 0.74 microgram/ml to 0.13 microgram/ml, and ADR disappeared within one hour after injection. ADR was detected in all cases of normal breast tissues and of breast cancer tissues. The concentrations were 0.67 +/- 0.36 microgram/g (average +/- SD) in normal breast tissue, and 1.26 +/- 0.26 microgram/g in breast cancer tissue. Twenty-eight patients with recurrent breast cancers were treated with a combination chemotherapy consisting of 5-FU (200 mg/m2/day P 0 days 1-28), ADR (27 mg/m2 IV day 1, 8), and Cyclophosphamide (67 mg/m2/day P 0 days 1-28). Response was assessed to the U.I.C.C. criteria. The response rate was 39% (PR: 39%, CR: 0%), and duration of response was 11 +/- 8 months (average +/- SD). Metastasis to lung, liver and brain responded very well to CAF therapy. On the other hand, fifteen patients with recurrent breast cancer were treated with a combination chemotherapy consisting of 5-FU (200 mg/m2/day P 0 days 1-28), cyclophosphamide (67 mg/m2/day P 0 days 1-28) and ADR (27 mg/m2 IV day 1, 8). Response rate was 33% (PR: 33%, CR: 0%). These results indicate that anticancer agents with high tissue concentration have higher antitumor activities.
