Two infants, aged 36 days old (Case 1) and 18 days old (Case 2) with interrupted aortic arch types B and A, respectively, and with severe aortic stenosis, were successfully operated on by use of pulsatile cardiopulmonary bypass. The great arteries were normally related in Case 1 and were transposed in Case 2. Repair involved the following procedure: ligation of the patent ductus arteriosus, restoration of aortic continuity with an 8 mm polytetrafluoroethylene graft, placement of an internal patch to tunnel all left ventricular blood from the left ventricle through the ventricular septal defect into the pulmonary artery in Case 1 and patch closure of the ventricular septal defect in Case 2, transection of the main pulmonary artery, anastomosis between the proximal pulmonary artery and the ascending aorta, and interposition of a valved conduit between the right ventricle and the distal pulmonary artery. The operative field could be approached easily through a median sternotomy. Postoperative cardiac catheterization revealed satisfactory anatomical and hemodynamic results in both cases.

Peripheral blood mononuclear cells from asymptomatic HBV carriers and normal individuals were incubated with 3H-estradiol and cold estradiol at 37 degrees C. The radioactivity bound to these cells was then counted, and the estradiol binding capacity estimated. The estradiol binding capacity of the mononuclear cells was lower in asymptomatic HBV carriers than in normal individuals. This suggested that the unresponsiveness of the mononuclear cells to estrogen in asymptomatic HBV carriers, which we have reported previously, may be due to a deficiency in the estradiol binding capacity.

Liver cell damages were inducible, when isolated liver cells coated with specific antibody against the liver cell membrane were cultured with peripheral blood mononuclear cells. Although this antibody-dependent cell-mediated cytotoxicity (ADCC) was induced by the closed contact of effector cells to targets via the specific antibody, a cytotoxic factor or factors causing the inhibition of protein synthesis in the liver cells was detected in the culture supernatant of the ADCC reaction. Similarly, peritoneal exudate macrophages activated by endotoxin lipopolysaccharide also exerted cytotoxic effects on the isolated liver cells by producing a cytotoxic substance or substances. These liver cell injuries induced by either the ADCC or activated macrophage culture supernatant were significantly reduced, when the isolated liver cells were pretreated with tritoqualine before adding the cytotoxic culture supernatants. These results suggest that tritoqualine may protect liver cells from immunological injuries.
We analyzed the transient response of oxygen consumption (VO2) after the sudden termination of exercise. The study population consisted of 20 male athletes (age 13 to 15.9 years) and 87 male patients, 59 of whom (age 4.8 to 17.8 years) were considered to be normal subjects, and 28 of whom (age 6 to 14.8 years) had restricted physical activity because of underlying heart diseases (restricted group). The exercise tests were performed according to Bruce's protocol and terminated at signs of exhaustion. As soon as exercise ceased, the examinee sat on a chair and took rested completely. VO2 was measured every 30 seconds throughout the test. We characterized the rate of the initial decay of the VO2 transient as the ratio of the 30 second VO2 right after the exercise to that at the end of the exercise (Irv). This index did not differ significantly among the groups. On the other hand, Exc, the Irv normalized by the end-exercise VO2, was a sensitive index for separating the restricted group from the control and athlete groups. Should the Exc remain sensitive enough to quantify exercise capacity regardless of the intensity of the exercise imposed, even the submaximal exercise test should enable us to safely evaluate the exercise capacity of patients with impaired cardiac function.
Effects of TJN-101, one of the components isolated from Schizandra fruits, on liver regeneration after partial hepatectomy, and on regional hepatic blood flow and fine structure of the liver were investigated in normal rats. TJN-101, which was administered orally at the doses of 10, 30 and 100 mg/kg/day for 4 days after partial hepatectomy, increased the regeneration rate of the liver and improved the serum retention rate of BSP which had been dose-dependently decreased after the operation. Elevation of serum protein to control levels, elevation of serum LCAT activity, decrease in plasma insulin and increase in plasma glucagon were all dose-dependent responses to TJN-101. The mitotic index on the 5th day after the operation was hardly influenced by TJN-101. Regional hepatic blood flow was increased after intraduodenal administration of TJN-101 (30 and 100 mg/kg). Ultrastructural studies of liver tissue using the transmission electron microscope revealed that TJN-101 stimulated an increase in rough and smooth endoplasmic reticulum in the groups receiving 100 and 300 mg/kg/day. These results suggest that TJN-101 accelerates both the proliferation of hepatocytes and the recovery of liver function after partial hepatectomy and increases hepatic blood flow. It is also thought that the liver enlargement caused by repeated administration of TJN-101 is associated with the proliferation of endoplasmic reticulum.
