[A statistical analysis of multiple primary malignant tumors].

The authors analyzed 32 patients with multiple primary malignant tumors treated in the Department of Otolaryngology, Mie Univ. from 1965 to 1984 and reviewed the literature. Of 1149 malignant head and neck tumors in our clinic in the same period, 32 cases (2.79%) were multiple primary malignant tumors. In this series, quadruple and triple malignancies were found in one case each and double ones in the others. The average age of these 32 patients was 67.5 years for the men and 52.7 for the women. The male: female ratio was 4.3:1. The average interval between the first and second tumor was 35 months, and in 17 cases (56.7%), they were synchronous. Laryngeal cancers were the most frequent double-primary lesions and cancers in the head and neck were most commonly associated with stomach cancers in this study.

Macromolecular permeability of the tight junction of the human nasal mucosa.

Using HRP as a cytochemical tracer, we investigated the tight junctional permeability of the human nasal mucosa in the early stage of its transportation. We found that the tight junction becomes "leaky" in the inflammatory and in the allergic conditions. Connection of goblet-goblet and goblet-ciliated cells were weaker than that of ciliated-ciliated cells. All epithelial tight junctions seemed to be loose in cases of allergic rhinitis.

Cilia injury during virus infection in chickens.

The reduction of mucociliary clearance in the Newcastle disease virus-B infected nasal turbinate in chickens is mainly due to ciliary pathology, decrease of the number of cilia and of the ciliary beat frequency.

[Morbidity and treatment in the anomalous arrangement of the pancreaticobiliary ductal system].

A nationwide survey of cases of congenital dilatation of the biliary duct was conducted to obtain the data on cases of congenital dilation of the biliary duct in Japan between 1972 and 1982. A total of 645 cases were surveyed. An anomalous arrangement of the pancreaticobiliary ductal system (AAPABIDS) was associated with congenital dilatation of the biliary duct in 90.2% of the cases examined extensively. These cases and 62 cases that were collected in our hospital during the period from 1963 to 1984 were chosen as the subject of this study. The results may be summarized as follows: Various factors which affect the sphincter of Oddi are needed for the clinical manifestations of AAPABIDS. The incidence of biliary carcinoma increases with age, in parallel with that of intestinal metaplasia in cases of congenital dilatation of the biliary duct. These findings suggest that intestinal metaplasia may be one of the important factors in the development of biliary carcinoma. From the view points of timing and indication of surgery, it is necessary to cut off reflux of pancreatic juice into the biliary duct and remove the portion in which bile is stagnant prior to the development of serious complications.

Antibodies to lymphocyte membrane preparations in systemic lupus erythematosus.

IgG and IgM antilymphocyte antibodies were determined in sera from patients with systemic lupus erythematosus (SLE) by an ELISA method using isolated lymphocyte plasma membranes as antigens. Lymphocyte membranes prepared from T or B cell lines or normal peripheral blood showed a high degree of purity by enzyme assay and electron microscopic monitoring. Sera from patients with active SLE showed high IgG and IgM ELISA reactivity against T and B cell plasma membranes compared with normal sera. IgM antibodies to lymphocyte membranes showed a correlation with microcytotoxicity results. Serial studies showed a relationship of antimembrane antibody to disease activity in some patients, but not in others. The ELISA method for antilymphocyte antibody assay provides a sensitive technique for monitoring reactivity against antigens retained in isolated lymphocyte membrane preparations.

The effect of protein malnutrition on the susceptibility of the chicken nose to Newcastle disease virus.

The effect of protein malnutrition on the susceptibility of the chicken nose to Newcastle disease virus (NDV) was studied. Chicks that were rendered acutely protein-deficient for 1 or 3 weeks from the time of hatching yielded approximately 10 times more NDV from their throats than did normally fed animals 7 days after inoculation with the virus. Serum antibody responses in the protein-deficient animals 14 days after inoculation of NDV was statistically lower than in the normally fed animals. These results were supported by the mucociliary transport time in the turbinate and sinus, as well as by the histological changes in the turbinate. This difference may thus be due to the fact that normally fed maturing chicks develop an increased resistance to virus which is depressed in protein-deficient animals.

Serum bactericidal effect on capsulated and non-capsulated Haemophilus influenzae in chronic sinusitis.

Study on the incidence of bactericidal (BC) antibodies to a Haemophilus influenzae type b and a non-capsulated H. influenzae was carried out on sera from 50 healthy subjects, and from 30 patients with chronic sinusitis. Of the 50 sera from normal subjects, only one (2.0%) lacked BC antibodies to the H. influenzae type b, whereas 13 (26.0%) lacked BC antibodies to the non-capsulated H. influenzae. All of the 30 sera from patients with chronic sinusitis were bactericidal for the H. influenzae type b, whereas only three (10.0%) lacked BC antibodies to the non-capsulated H. influenzae. Specific IgG, IgM and IgA antibodies to the non-capsulated H. influenzae in serum were determined by the indirect fluorescent antibody assay. Of the 22 sera tested in this study, 15 (68.0%) had antibodies of at least the IgG, IgM, or IgA classes; with IgG occurring most often. These data suggest that a lack of circulating BC antibodies to both H. influenzae isolates is not associated with the chronic carriage of these organisms by patients with chronic sinusitis.

Enhanced antitumor effects of bleomycin and moderate hyperthermia by additional use of ethanol.

The combined effects of moderate local hyperthermia, bleomycin and ethanol were investigated with Lewis lung carcinoma tumors in female C57BL/6 mice. Different combinations of treatments were performed on days 4 and 7 after tumor implantation. Combined treatment of 41 degrees C hyperthermia and bleomycin resulted in mild reductions of tumor growth. Hyperthermia plus bleomycin led to marked reduction in tumor growth under the condition of a 43 degrees C temperature. The antitumor effects of 41 degrees C hyperthermia combined with bleomycin were enhanced by the additional use of ethanol, and these effects were more remarkable than those of 43 degrees C hyperthermia and bleomycin. A possible application of these findings to clinical therapy for advanced esophageal cancer was discussed.

Sensation elicited by mechanical stimuli to the oropharyngeal mucosa.

An inaccurate localization of the site of stimulus, a higher two-point threshold for touch than those at lower lip or soft palate mucosae, and reference of touch sensation to the submaxillary cervical region following mechanical stimuli to the posterior oropharyngeal wall or lateral pharyngeal band were observed in the present study on otolaryngologists, medical students and patients with abnormal sensations in the throat. These results indicated that not only the somatic but also the visceral nervous system might contribute to the elicitation of sensation from the oropharyngeal structures behind the soft palate.

Interrelationship between drug resistance and bacteriocinogeny of Clostridium perfringens.

One hundred and eight strains of Clostridium perfringens were collected from polluted waters and tested for their drug resistance and bacteriocinogeny . Thirty of the strains were tetracycline resistant and 36 were bacteriocinogenic. Only one strain possessed both tetracycline resistance and bacteriocinogen . Transfer experiments on tetracycline resistance and bacteriocinogeny were carried out with several selected strains among these isolates. Both tetracycline resistance and bacteriocinogeny were shown to be transferable by conjugation-like procedures. Transfer experiments on these two properties revealed that there were two types of recipient strains among the isolates in regard to the acceptance of tetracycline resistance and bacteriocinogeny . In one type of the recipient strain, these two properties seemed to be incompatible although in the other, both tetracycline resistance and bacteriocinogeny were transferred by two-step mating. Transcipients of this two-step mating retransferred their tetracycline resistance but not bacteriocinogeny to another recipient strain. The existence of an incompatibility-like relationship between tetracycline resistance and bacteriocinogeny in one recipient strain, and the inability of these two properties to be co-transferred from the two-step transcipients , suggest a certain interrelationship between the factors responsible for tetracycline resistance and bacteriocinogeny .

Transferability of tetracycline resistance to Clostridium perfringens isolated from human feces.

118 strains of Clostridium perfringens isolated from human fecal specimens were examined for sensitivity to tetracycline. Minimum inhibitory concentrations of tetracycline were more than 50 micrograms/ml for 18 strains and less than 3.1 micrograms/ml for 54 strains. Tetracycline resistance transfer experiments were carried out using 18 tetracycline-resistant strains as donors and 54 drug-sensitive strains as recipients. 14 of 18 tetracycline-resistant strains transferred their resistance to at least 1 recipient, and 48 of 54 tetracycline-sensitive strains accepted tetracycline resistance from at least 1 donor strain.

Antifibrinolytic activity in middle ear effusion.

No activity of the plasmin and plasminogen activators could be detected in middle ear effusion (MEE). A streptokinase proactivator, which was not plasminogen and markedly enhanced the activation of plasminogen by streptokinase, could be detected. MEE had an apparent inhibitory activity against both thiol protease (papain) and serine proteases (plasmin and trypsin). All these findings indicate that MEE contains unsaturated protease inhibitors and is in a state of predominance of inhibitors over fibrinolytic activity.

Nationwide survey of cases of choledochal cyst. Analysis of coexistent anomalies, complications and surgical treatment in 645 cases.

A nationwide survey of cases of choledochal cyst was conducted to obtain the data on cases of choledochal cyst in Japan between 1972 and 1982. A total of 645 cases in 73 institutions, including 52 cases from this hospital, were surveyed. Choledochal cyst was associated with an anomalous arrangement of the pancreaticobiliary ductal system in 90.2% of the cases which were examined extensively. The serum amylase level was elevated in 26.6% of the cases. Calculus disease of the biliary duct was recognized in 28.5% of the cases. Biliary carcinoma developed in 54 (8.4%) of 645 cases. The incidences of biliary carcinoma were 0.3% (1/306) in cases in children and 15.6% (53/339) in adult cases; carcinoma was found in the wall of the choledochus in 30, and in the gallbladder in 16 of these 54 cases. The nationwide survey showed that an anomalous arrangement of the pancreaticobiliary ductal system was closely associated with choledochal cyst and that cases of biliary carcinoma developed with the advance of age. Therefore, early resection of the cyst wall and reconstruction of the biliary duct is proposed as suitable surgical treatment for stopping reflux of pancreatic juice into the biliary duct.

Clinical results of sfericase (AI-794) in chronic parasinusitis.

Forty patients with chronic sinusitis were treated orally with sfericase tablets (each tablet = 10,000 units), a proteolytic enzyme preparation, at a daily dosage of six tablets for adults and three tablets for children in three divided doses after each main meal for 4 weeks. As regards subjective symptoms, very favorable results were obtained after 4-week treatment with sfericase as evidenced by an efficacy rate of 81.6% for postnasal rhinorrhea, 66.7% for rhinorrhea, and 43.6% for nasal obstruction. However, with respect to objective symptoms, the efficacy rate was 28.9% for redness and 30.8% for swelling of the nasal mucosa, with a somewhat lower rate than that obtained for subjective symptoms. According to the patient's and physician's global assessments, sfericase was effective in 65 and 57.5% of the cases. None of the cases showed undesirable reactions to sfericase.
