[Systemic tumor emboli from primary bronchogenic carcinoma].

A 59-year-old man underwent left pneumonectomy for bronchogenic carcinoma. During operation, his right arm pulse became suddenly unmeasurable. He was unconscious after operation and brain CT scan revealed cerebral infarction of the left hemisphere. Histopathological examination of the resected specimen showed a large cell carcinoma. Massive invasion into the inferior pulmonary vein of malignant tissue was recognized without nodal involvement. Twenty-six cases including ours in which systemic tumor emboli arise from primary bronchogenic carcinoma were reviewed. In 19 of these cases, tumor emboli occurred during or after pulmonary operation and in 7 cases spontaneously. The most common features of these cases were large tumor mass and poorly differentiated structures.

[A case of isolated anomalous return of bilateral superior pulmonary veins].

A 14-year-old girl was admitted for evaluation of heart murmur. Intravenous digital subtraction angiography (DSA) showed the right superior pulmonary vein drained into the superior vena cava and the left superior pulmonary vein drained into the innominate vein. At the operation, an anomalous bilateral superior pulmonary venous return with an intact atrial septum was confirmed. An atrial septal defect was created and the right superior pulmonary vein was baffled into the left atrium with a pericardial patch. The left superior pulmonary vein was divided and anastomosed to the left inferior pulmonary vein with a expanded polytetra-fluoro-ethylene graft. Post-operative DSA showed satisfactory long-term result of the operation.

[Isolation of cisplatin-resistant subline from human ovarian cancer cell line and analysis of its cell-biological characteristics].

The cisplatin (CDDP)-resistant subline TYK-nu (R) was developed by culturing TYK-nu (human ovarian cancer cell line in vitro) with exposure to CDDP in stepwise increasing concentrations. The characteristics of both cell lines were compared and the results were as follows; 1. Both cell lines formed a monolayer in a pavement-like arrangement and large cells were occasionally present in TYK-nu (R) rather than TYK-nu. The population doubling time of TYK-nu and TYK-nu (R) was 43 hr and 48 hr, respectively. 2. IC50 (microgram/ml) in 96 hr treatment with CDDP and carboplatin (CBDCA) was 0.035 and 0.5 in TYK-nu and 0.62 and 2.0 in TYK-nu (R), respectively. 3. In the intracellular CDDP concentration, there was no marked difference between TYK-nu and TYK-nu (R) after CDDP (2.0 micrograms/ml) treatment in 2hr. 4. After treatment with CDDP (0.2 microgram/ml) or CBDCA (2.0 micrograms/ml), a decrease in S and G2 + M compartments was observed in the pattern of the DNA histogram of TYK-nu, but not in that of TYK-nu (R). 5. The majority of chromosomes in both cell lines were in hyperdiploid areas and the mode of TYK-nu and TYK-nu (R) was 56 and 51, respectively. The karyotype of TYK-nu (R) showed deletion of chromosome 7q. Thus, compared to TYK-nu, TYK-nu (R) was 17.7 times more resistant to CDDP and 4 times more resistant to CBDCA. The results suggest that the mechanism of resistance to CDDP may be due to genetic changes at the cellular level.

[Antitumor effect of UFT on human ovarian cancer grafted to nude mice and 5-FU concentration in tumor and normal tissues].

Antitumor effects of UFT, tegafur (FT-207), cisplatin (CDDP) and the combination of UFT with CDDP on a human ovarian cancer xenograft in nude mice and the concentration of 5-FU in the tumor tissue and major organs were studied. UFT (48.6mg/kg/day X 20) or tegafur (15.0mg/kg/day X 20) was daily administrated orally, and CDDP (5mg/kg/day X 3) was administrated intraperitoneally at an 7-day interval. The inhibition rates of the tumor growths were 49.6% with UFT, -2.3% with tegafur and 17.7% with CDDP, respectively. In the combination of UFT with CDDP, severe side effect were observed. The concentration of 5-FU in UFT-treated group was higher than tegafur group: about 2 times in the tumor, 5 times in the liver, 9 times in the kidney and 4 times in the spleen, respectively. In the combination of UFT with CDDP, the concentration of 5-FU in major organs, especially in the kidney, in nude mice that died at 10 day after drug administration were higher than in those of UFT. These findings indicate that UFT increases the intratumoral concentration of 5-FU to elicit better antitumor effect and also the concentration of 5-FU in various normal organs after long time administration.

[Establishment and characterization of three endometrial cancer cell lines from the same patient].

In order to determine some important cytological behavioral characteristics of endometrial cancer, I tried to establish endometrial cancer cell lines. Three endometrial cancer cell lines (KKNS-1, KKNS-2, KKNS-3) derived from the same patient have been established and successfully maintained in vitro for more than one year. The cells formed a monolayer in a mosaic fashion and pile up. Pathological findings for the tumor induced in athymic nude mice were: KKNS-1 was undifferentiated, KKNS-2 was differentiated and KKNS-3 was undifferentiated. Population doubling time was calculated to be about 35(KKNS-1), 60(KKNS-2), and 28(KKNS-3) hours. The modal chromosomal number for the cells fell in the diploid range. Estrogen receptor was demonstrated only in KKNS-2 by the ER-EIA and ER-ICA methods. HLA-expression was demonstrated, HLA-ABC was positive in all three lines and HLA-DR was positive only in KKNS-2. Anticancer drug sensitivity was demonstrated only in KKNS-3. Hormone sensitivity was demonstrated only in KKNS-2. We must therefore carefully treat endometrial cancer patients with anticancer drug or hormonal therapy whose pathological findings are heterogeneous.

[Anti-tumor and adverse effect of etoposide and cisplatin on human choriocarcinoma transplanted to nude mice--a correlation between effect and tissue distribution of the drugs].

The study dealt with anti-tumor and adverse effects of etoposide and cisplatin on human choriocarcinoma cell line (GCH-1) transplantable in nude mice in relation to the rate of their uptake into tumor tissue. Nude mice were divided into 3 groups (etoposide, cisplatin and MTX-group), 7 to 9 per group and received drug treatment which started when their tumor grew to 100 approximately 300 mm3 in volume. All drugs were intraperitoneally administrated to nude mice (each drug: 1/4 mouse LD50 dose X3, weekly). The inhibiting action on the tumor was observed to be etoposide greater than cisplatin greater than MTX. The body weight loss of nude mice was observed to at its maximum in the cisplatin-treated group. The serum beta-HCG level did not rise in the drug-treated groups but rose in the non-treated control group. No histopathological changes characteristic of each drug were found. A concentration of etoposide and cisplatin in various tissues was serially measured after one shot intraperitoneal injection of etoposide 25 mg/kg or cisplatin 5 mg/kg. Etoposide reached its peak concentration after 30 minutes in tumor, liver and kidney and, after 4 hours, was left with 37.1%, 10.4% and 2.3% concentrations after 30 minutes in tumor, liver and kidney, respectively. Cisplatin showed similar kinetics, but was found to remain at a comparatively high concentration in both liver and kidney. Thus, it was demonstrated that etoposide and cisplatin were active against the human choriocarcinoma cell line. Furthermore, the less adverse effect of etoposide was felt to be partially due to its lower residue in liver and kidney.

Two simple methods for the evaluation of topically active anti-inflammatory steroidal ointments.

Simple laboratory methods for quantitating the topical anti-inflammatory activity of steroidal ointments are described. One is of croton oil ear edema in rats and the other is a new method using homologous passive cutaneous anaphylaxis (PCA) in rats. In order to avoid problems such as the animals' licking and/or rubbing the ointment at the applied sites, which might result in oral uptake, each rat was housed individually and fitted with a plastic collar in the croton oil experiment. The sites of ointment application in the PCA experiment were covered with adhesive plaster. Optimal experimental conditions were as follows. In the former method, ointments were applied to the inside surface of the ear 5 min after the irritant treatment and anti-edematous activity was determined after 6 h. In the latter, ointments were applied 3 h before the antigenic challenge to the dorsal area of animals which had been passively sensitized by anti-serum, and inhibition of the increased permeability was determined 45 min after the challenge. These methods were found to be reliable with respect to sensitivity and reproducibility of data. Ointments of halcinonide, betamethasone-17-valerate, hydrocortisone-17-butyrate, fluocinonide, flumethasone-21-pivalate and beclomethasone-17,21-dipropionate were evaluated by these methods.

Development of a control algorithm for a rotary blood pump: prediction of recovery from cardiac failure.

Under extracorporeal membrane oxygenator (ECMO) support, it is difficult to predict recovery of cardiac function. The authors examined the rotary blood pump function curve and developed a new index of dynamic vascular impedance (DVI) for controlling the ECMO system. The cases of nine patients assisted by ECMO after cardiac surgery were analyzed. The index of DVI was calculated from the pump flow and the pressure difference (delta pressure) between the inlet and outlet of the pump. Seven patients were weaned from support. In the patients with recovery of cardiac function, the index of DVI was high and the delta pressure converged on the specific value. Analysis of the rotary blood pump function curve during ECMO support reflects the pump characteristics and provides data for the management of pump speed. The index of DVI can be used to predict the timing for weaning patients from ECMO support. The authors are developing an automatic control system that uses the index of DVI.

Cytokine mediated endothelial activation during and after normothermic cardiopulmonary bypass: heparin-bonded versus non heparin-bonded circuits.

Studies evaluating cytokine production under normothermic cardiopulmonary bypass (CPB) are limited. We evaluated cytokine production, levels of thrombomodulin (TM), and soluble endothelium-derived adhesion molecules (ICAM-1) under normothermic CPB with and without heparin-bonded circuits. Nine patients treated with non heparin-bonded circuits (control group), and seven patients treated with heparin-bonded circuits (heparin group) were the subjects. Granulocyte elastase (G-E), and interleukin (IL) -6 and IL-8 were chosen as proinflammatory mediators, and TM and ICAM-1 served as indicators for endothelial damage. Blood samples were obtained before CPB, 30 minutes after initiation of CPB, at the termination of CPB, and 2 and 24 hours after CPB. G-E values in the heparin group were lower than those in the control group after 30 minutes of CPB. A G-E surge occurred at the end of CPB, and IL-6 and IL-8 surges were observed 2 hours after CPB in both groups. TM and ICAM-1 values, which were reduced at the initiation of CPB, returned to initial levels 2 hours after CPB, and exceeded them 24 hours after CPB compared with preCPB levels. Both groups showed similar changes. We conclude that there are no significant differences in serial G-E, IL-6, IL-8, TM, or ICAM-1 levels between the heparin and control groups during or after normothermic CPB for 2 to 3 hours.

Pitfalls in the development of a rotary blood pump controller.

The controller presents a major obstacle in the development of the rotary blood pump as a left ventricular assist device (LVAD). Clinically, LVAD flow is a good indicator in the regulation of circulatory conditions and pump flow changes, depending on pump preload and afterload. Many investigators have tried estimating pump flow by referencing the motor current. There have been pitfalls in in vitro experimental settings, however. Using a test loop with a pneumatically driven LV chamber and a centrifugal pump as an LVAD, we monitored pump flow and pressure head to evaluate the pump performance curve (H-Q curve). Under pulsatile LV conditions, the H-Q curve was a loop that changed, depending on LV contractility. The pneumatically driven LV chamber cannot mimic the Starling phenomenon, so the developed LV pressure does not change according to the LV preload. Rotary pump flow estimation is the most effective control method. In pulsatile conditions, however, the H-Q curve is a loop that changes under various LV contractility conditions, complicating determination of linear equation for calculating flow. In addition, the LV chamber in the test loop cannot mimic native heart contractility as described by Starling's law. This finding can lead to a misanalysis of the H-Q curve under pulsatile conditions.

An unusual complication associated with the Carpentier-Edwards porcine bioprosthesis.

Primary tissue failure of bioprosthetic heart valves refers primarily to calcification of the leaflets of the bioprosthesis. A 75 year old patient underwent reoperation 15 years after mitral valve replacement with a Carpentier-Edwards porcine bioprosthesis. The extracted bioprosthetic valve was found to have one prolapsed leaflet and a small amount of calcification on all three leaflets without tear or perforation. The two commissures suspending the prolapsed leaflet were detached, causing mitral valve regurgitation.

Fourteen year experience with the omnicarbon prosthetic heart valve.

The Omnicarbon prosthetic valve was implanted in 168 patients (mean age, 53 years); 84 had aortic valve replacement (AVR), 57 had mitral valve replacement (MVR), and 27 had both aortic and mitral valve replacement (DVR). The mean follow-up period was 6.8 years, with a maximum of 13.7 years. Three patients (1.8%) were lost to follow-up. There were 4 (2.4%) early deaths and 26 late deaths. Survival at 10 years was 76% in the AVR and MVR groups, and 85% in the DVR group. Freedom from thromboembolism at 10 years was 94% in the AVR group, 80% in the MVR group, and 92% in the DVR group. Freedom from hemorrhagic complications at 10 years was 86% in the AVR group and 92% in the MVR group. At 10 years, 97% in the AVR group and 96% in the MVR group were free from endocarditis. One patient in the DVR group suffered a paravalvular leak. At 10 years, 97% in the AVR group and 95% in the MVR group had not needed reoperation. Elevation of the postoperative serum lactate dehydrogenase levels were acceptable in the three groups. In conclusion, the Omnicarbon prosthetic valve has shown excellent long-term clinical results.

Radical modulation activity of pine cone extracts of Pinus elliottii var. Elliottii.

The radical modulation activity of lignins prepared from the cone of Pinus elliottii var. Elliottii was investigated, using ESR spectroscopy. These lignins produced radical(s) under alkaline conditions, and the radical intensity was increased with increasing pH. Lower concentrations of lignins slightly reduced the radical intensity of sodium ascorbate, whereas higher concentrations of lignins enhanced both the radical intensity and cytotoxic activity of sodium ascorbate. Lignins effectively scavenged superoxide anion, produced by hypoxanthine-xanthine oxidase reaction. Elliottii lignins significantly inhibited the human immunodeficiency virus (HIV)-induced cytopathic effect, in similar fashions to other natural, commercial and synthetic lignins. Pretreatment of mice with lignins significantly protected them from the lethal infection with E. coli. Crude alkaline extracts of Elliottii pine cone displayed similar magnitude of activity with lignins. These data further supports the medicinal efficacy of plant extracts.

Stimulation of granulocytic cell iodination by tannins and related compounds.

Tannic acid stimulated the iodination (incorporation of radioactive iodine into an acid-insoluble fraction) of human peripheral blood polymorphonuclear cells (PMN) and human promyelocytic leukemic HL-60 cells, without affecting the iodination of 9 other cultured cell lines. The stimulation of both PMN and HL-60 cells depended on incubation time and temperature, and was significantly suppressed by myeloperoxidase inhibitors. Among chemically defined natural polyphenols, condensed tannins (epicatechin gallate oligomers) and monomeric and oligomeric hydrolyzable tannins potently stimulated PMN iodination, whereas polyphenols of lower molecular weight (gallic acid, alkyl gallates, epicatechin, epicatechin gallate, epigallocatechin, caffeic acid derivatives and licorice flavonoids) had much less activity. Various synthetic polyphenolic compounds structurally unrelated to tannins also stimulated PMN iodination depending upon their molecular weight, but to a slightly lesser extent. The results suggest that the stimulation activity of tannins and related polyphenols might depend more on their molecular weights than the number of hydroxyl groups on each benzene ring in the molecule, or the presence of sugars or hexahydroxydiphenoyl groups.

Pulmonary microcirculation during pulsatile and non pulsatile perfusion.

During development of a rotary blood pump as an assist device, the efficacy of non pulsatile perfusion to the end organ has to be verified. However, there are few evaluations of two different perfusion mechanisms through the tissue microcirculation. In this study, the pulmonary microcirculation was analyzed by vital microscopic observation. Wistar rats weighing 400-500 g were anesthetized and ventilated by a respirator. After establishing right heart bypass from the right atrium to the pulmonary artery using a roller pump, the pulmonary microcirculation was observed during intravenous infusion of bovine albumin tagged with fluorescein isothiocyanate. The images were recorded on a videotape through an ultra-sensitive SIT TV camera. Initially, the pulmonary circulation was pulsatile, produced by the native heart, and the pulmonary capillary network was evenly perfused by the blood. After starting the pump, the flow became non pulsatile and the distribution of capillary perfusion was displaced to the short circuit connecting the pulmonary arterioles and venules. Flow distribution during non pulsatile perfusion was heterogeneous compared with pulsatile perfusion. This result suggests that non pulsatile flow may lead to the deterioration of function. Further investigation is necessary to evaluate the relationship between the microcirculation and organ function.
