Tumor DNA ploidy and prognosis of patients with serous cystadenocarcinoma of the ovary.

BACKGROUND: Although it is important to determine any relationship between tumor DNA ploidy and its biologic behavior, the correlation between DNA ploidy and the prognosis of patients with ovarian cancer is not conclusive. Accordingly, the authors evaluated the clinical application of DNA ploidy in ovarian cancer. METHODS: Flow cytometric measurements were performed in 45 selected patients with well-differentiated serous cystadenocarcinoma of the ovary, Stages Ic-IV. All of them had the same surgical procedure, with retroperitoneal lymphadenectomy including paraaortic nodes, followed by the same postoperative chemotherapeutic regimen. RESULTS: Of the 45 ovarian cancers, 28 were diploid and 17 were aneuploid. The 2-year survival rate and the estimated 5-year survival rate for patients with diploid tumors were significantly greater than those for patients with aneuploid tumors (73.2% versus 46.7% and 29.1% versus 22.4%, respectively). The 2-year survival rate in patients with advanced disease (Stage III or IV) was also significantly higher for those with diploid tumors (53.3% versus 37.4%, respectively), but the estimated 5-year survival rate was similar in both groups (8.9% versus 9.1%, respectively). Patients with advanced disease had aneuploid tumors more frequently than those with early-stage disease. A significantly higher incidence of retroperitoneal lymph node metastasis was observed in aneuploid tumors than in diploid tumors (43.8% in diploid tumors versus 86.7% in aneuploid tumors). The authors found no difference in the response to chemotherapy between diploid and aneuploid tumors. CONCLUSIONS: Although tumor DNA ploidy was not as reliable as conventional parameters such as surgical stage in establishing prognosis, it may provide an indicator of lymph node involvement.

Involvement of the local immune response in a case of advanced cervical cancer in a patient with systemic lupus erythematosus.

Although patients with systemic lupus erythematosus (SLE) have frequently accompanying malignancies, there are few documented cases in the literature of advanced cervical cancer patients with SLE. This report describes a case of advanced cervical cancer in a 39-year-old patient with a 20-year history of SLE (FIGO stage IIIb) and evaluates the immunological response of the tumor. The clinical course of this patient with SLE, with rapid growth of cervical cancer and a poor prognosis, may suggest that T cells play a key role in the growth of cancer.

Tumor cell kinetics in elderly patients with cervical cancer.

OBJECTIVE: To determine whether cell kinetics in cervical cancer differ by patient age. METHODS: We analyzed specimens from 118 women who had been treated for cervical squamous cell carcinoma. In a preliminary study, labeling indices were determined in 16 patients using bromodeoxyuridine in vitro. Adjacent portions of the same specimens were analyzed by flow cytometry. Proliferation indices were calculated and compared with labeling indices for 12 diploid tumors. For the remaining 102 tumors, paraffin-embedded specimens were analyzed by flow cytometry to determine DNA ploidy and proliferation index. Using these findings, we examined the relationship between tumor cell kinetics and age. RESULTS: There was a significant correlation between the labeling index and age (r = 0.86, P < .001) and a weak but statistically significant correlation between the proliferation index and the labeling index (r = 0.58, P < .05). The proportion of aneuploid tumors increased with age. The cutoff point of highest statistical significance was 65 years (P = .025). The proliferation index showed an age-dependent shift, and the cutoff point of highest statistical significance was also 65 years. Aneuploid tumors were more common in elderly patients aged 65 or more years. Proliferation indices were also significantly larger in the elderly patients (P < .005). Although stage III tumors were more common in the elderly, stage was not related to ploidy pattern or to proliferation index. CONCLUSION: Tumor cell kinetics of cervical squamous cell carcinomas differ between elderly and younger women.

Evaluation of cytoreductive surgery with lymphadenectomy including para-aortic nodes for advanced ovarian cancer.

The prognosis of 84 patients with advanced ovarian cancer who received the same regimen of postoperative chemotherapy but who received different surgical procedures, was evaluated retrospectively. Of those patients, 41 underwent cytoreductive surgery with lymphadenectomy, including removal of the para-aortic nodes (group A). The other 43 patients received hysterectomy, bilateral salpingoophorectomy and omentectomy (group B). Overall the patients in group A showed a significantly greater survival than those in group B. The performance of lymphadenectomy in patients with positive nodes improves the prognosis to equal that of patients with negative nodes. It is suggested that cytoreductive surgery with lymphadenectomy may be useful for the improvement of the prognosis of patients with advanced ovarian cancer.

Response rate and cell-cycle changes due to intra-arterial infusion chemotherapy with cisplatin and bleomycin for locally recurrent uterine cervical cancer.

Intra-arterial infusion with cisplatin (CDDP) and bleomycin (BLM) was carried out in 21 patients with locally recurrent uterine cervical cancer who were previously treated with irradiation alone. Patients were treated with a bolus infusion into both internal iliac arteries of 50 mg/m2 of CDDP and 30 mg/m2 of BLM. Two to four courses of the infusion therapy were given to each patient, and the response rate, the tumor and serum drug concentrations, and the cell kinetics in tumor tissue were evaluated. The response rate (CR+PR) was 71.4% according to the WHO criteria. There was no difference, in the tumor tissue concentrations of CDDP and BLM between responders and nonresponders. Although the DNA ploidy of tumor cells was not significantly different between the two groups before treatment, both the labeling index with BrdU and the proliferation index with flow cytometry significantly increased 24 hours after treatment in responding tumors but not in nonresponding tumors. These results show that intra-arterial infusion with CDDP and BLM improves the prognosis of recurrent cervical cancer and that labeling and proliferation indices may be useful for determining the response of cervical cancer to intra-arterial chemotherapy.

Solitary metastatic ovarian carcinoma of the spleen: a case report.

Carcinomatous metastatic involvement of the spleen usually indicates a widespread malignant disease. Solitary metastatic lesions in the spleen are exceedingly rare. The literature contains fewer than 16 cases. In this paper we report a case of a solitary metastatic lesion of the spleen arising from a serous cystadenocarcinoma of the ovary 5 years after the initial operation. A splenectomy was performed followed by smooth postoperative course.

The influence of intra-arterial infusion of cisplatinum and bleomycin on the cell cycle in uterine cervical carcinoma.

We examine the influence of CDDP and BLM on the cell cycle in the patients with uterine cervical cancer. Specimens were obtained from 8 cases of local recurrent uterine cervical cancer which were treated with CDDP and BLM bolus infusions into bilateral internal iliac arteries. Except for two patients who showed no response according to the criteria of the National Cancer Institute (NCI), the other patients exhibited partial response (PR). The concentrations of BLM and CDDP were 0.83 +/- 0.05 microgram/g and 2.9 +/- 0.54 microgram/g in the tumor tissues 24 hours after treatment. The peak values of the drug concentrations in serum, which were observed at 30 min for both drugs, were 0.92 +/- 0.12 microgram/ml BLM and 1.02 +/- 0.65 microgram/ml CDDP and the concentrations of the drugs decreased rapidly in the serum. The higher levels of CDDP and BLM were retained in the tumor tissue 24 hours after treatment, than in serum. Changes in DNA content determined by flow cytometry, revealed that proliferation indices (PI) increased after treatment only in PR cases. The mean increase in the ratio was 0.10 +/- 0.015. The BrdU staining showed that cells in S phase increased in tumor tissues. Labeling indices increased from 0.10 +/- 0.04 to 0.18 +/- 0.05. These results suggested that cell synchronization was induced by CDDP and BLM in vivo. PI seemed a useful parameter for measuring drug sensitivity.

[A carcinosarcoma of the ovary: a case report and a review].

A carcinosarcoma of the ovary, which is a combined epithelial and mesenchymal tumor, is exceeding rare and usually has poor prognosis. Only four known cases, including this case, have been reported in Japan. In our case, after resection a combined chemotherapy consisting of cisplatinum, adriamycin cyclophosphamide and etoposide was administered and the patient had no uneventful postsurgical course.

[The survey on the perinatal variables and the incidence of cerebral palsy for 12 years before and after the application of the fetal monitoring systems].

The effects of electrical fetal monitoring such as cardiotocogram (CTG) and nonstress test (NST) were studied during the years 1975-86 in 6,981 cases (6,893 singletons, 41 twins and 2 triplets). These cases were divided into three groups according to the mode of fetal monitoring. The first group was managed without electrical fetal monitoring (control group). The second was managed mainly with intrapartum fetal monitoring (transitional group) and the third was managed with antepartum and intrapartum fetal monitoring (fully monitored group). The incidence of cerebral palsy (CP) was investigated at the social health center during the same period. The introduction of intrapartum fetal monitoring resulted in a reduction in neonatal asphyxia from 7.0% in the control group to 5.2% and 4.3% in the transitional group and fully monitored group respectively. The perinatal mortality rate showed a significant reduction from 0.61% in the period without antepartum fetal monitoring to 0.31% in the fully monitored group, and this was attributed to the significant reduction in the number of stillbirths. The incidence of CP in the district decreased from 2.2 to 0.2 per 1,000 children during the period studied. In these circumstances, the caesarean section rate increased from 7.0% in the control group to 11.8% in the transitional group but decreased to 9.8% in the fully monitored group. These results showed the clinical and social benefits of electrical fetal monitoring.

Microscopic observation of leukocyte kinesis in the vascular bed during hemodialysis using the rabbit ear chamber technique.

Leukocyte kinesis in the capillary vascular bed during hemodialysis (HD) was investigated to elucidate the mechanism of transient leukopenia. Leukocyte movement was observed microscopically during HD using the rabbit ear chamber (REC) technique, which permits visualization of the movement of blood corpuscles in capillaries. Blood was drawn from the femoral artery and returned into the auricular and/or carotid artery so that the blood passing through the hollow fiber artificial kidney (HFAK) flowed into capillaries in the REC. Leukocyte counts of blood samples taken from the afferent and efferent limbs of the HD circuit, the right jugular vein and the right atrium were determined consecutively during HD. The difference in the leukocyte count was observed between the afferent and efferent limbs for the first 15 minutes and thereafter between the efferent limb and the jugular vein. The "transpulmonary" difference in the leukocyte count was not noticed throughout HD. Between 15 and 90 minutes after the start of HD, scarcely any circulating leukocytes were found in capillaries in the REC and some leukocytes were attached to the endothelial surface. Thereafter circulating leukocytes were seen again and detachment of leukocytes from the endothelial surface was observed. No leukocyte aggregation or embolization of aggregating leukocytes was noticed. This evidence suggests that leukopenia may be attributed to the transient shift of leukocytes to the marginal pool of the vessel lumen and this process may not be specific for the pulmonary vasculature, but may occur in the first capillary bed into which the blood passing through the HFAK flows.(ABSTRACT TRUNCATED AT 250 WORDS)

The ultrasonic Doppler fetal actocardiogram and its computer processing.

Gross fetal movement was detected using a lower frequency ultrasonic Doppler shift than that used in the study of fetal heart action. The movement signal was changed into deflection which was recorded on CTG chart simultaneously with the fetal heart rate (FHR), and was called the fetal actocardiogram. Visual analysis of the actocardiogram showed bursts of active fetal movements in the active fetal state which were concordant with acceleration of FHR. The analysis was useful in the study of fetal behavioural states. Imminent fetal distress produced a loss of FHR acceleration which was accompanied by a burst of fetal movement, i.e. a true non-reactive state of the FHR. The output of the actocardiograph was analysed using a PC98XA computer with a simple statistics of deflection amplitude and interval, displayed as 3-dimensional histograms of the number and frequency, and the formation of the envelope curve of the deflections. The study demonstrated that it is possible to recognize fetal behavioural states automatically. This includes active and resting fetal states as well as fetal breathing movements.

[Identification and characterization of a human sperm coating antigen isolated from sperm cell membrane].

An antigenic spermatozoal polypeptide was identified from nonionic detergent solubilized membrane material of sperm cells by using a human seminal plasma specific rabbit antiserum. The radiolabeled sperm cell membrane preparation was subjected to gel filtration and gel fractions were assayed for the binding activity with antiseminal plasma antiserum. The immune complex of the peak fraction of the binding was applied on SDS-PAGE, and the antigen was estimated to be a single polypeptide of approximately 12,000 daltons. The inhibition assay of the binding revealed that the antigen is present in sperm, seminal plasma, and milk, but is absent from any other organs and tissues except male genital tracts. Seminal plasma showed the largest amount of the activity, suggesting that this antigen is a sperm coating antigen. In seminal plasma, the antigen molecule present in a large form with a molecular weight of approximately 50,000 daltons. The antigenicity was stable when treated with heat, acid, alkali and various protein denaturants. The characteristics of the present antigen are very similar to those of lactoferrin and ferrisplan, the other sperm coating antigens common to milk. However, they are quite different in molecular weight, and no cross reactivity was found between the present antigen and lactoferrin or ferrisplan.

Evaluation of fetal development and well-being with ultrasonic Doppler fetal actocardiogram.

A new ultrasonic technique for objective, continuous and simple recordings method of fetal movements on CTG chart has been introduced. Fetal activity and heart-rate will be correlated. This allows a differentiation of fetal active and resting states. The duration of the first one was 24 to 150 minutes, that of the last one 10 to 36 minutes. Fetal actogram is useful in the prediction of imminent fetal distress.

[ELISA for antisperm antibodies in unexplained infertile women].

It is commonly believed that autoimmune or isoimmune responses to human sperm antigens are associated with human infertility. We examined an enzyme linked immunosorbent assay (ELISA) kit used in the detection of antisperm antibodies in the sera of women with unexplained infertility. 1) For the ELISA assay, an absorbance at 405nm greater than 0.300 was considered positive. Of the 83 sera of infertile women studied, 20(24.1%) were positive. This value was significantly higher than that of the control group: 10% of pregnant women, 11.1% of unmarried women and 10% of healthy men (p less than 0.05). 2) The results obtained with ELISA were compared with those of the sperm immobilization test. Comparison of the results of ELISA with those of the sperm immobilization test indicated that these methods detect a different, though often overlapping, spectrum of antibody activity. 3) The results obtained with ELISA did not always correlate with those of the Huhner test. However the ELISA method may be useful in the clinical screening of antisperm antibodies because of its simplicity and rapidity.
