[Effects of anteroseptal myocardial infarction on systolic and diastolic filling function of the right ventricle].

To assess the effects of wall motion abnormalities of the anterior left ventricle and interventricular septum on right ventricular (RV) systolic and diastolic filling function, multiple-gated radionuclide angiography was performed at rest in 53 patients with previous anteroseptal myocardial infarction and 14 normal subjects. There were 26 patients with left ventricular ejection fraction (LVEF) greater than or equal to 45% (group 1) and 27 with LVEF less than 45% (group 2). Six measurements were obtained simultaneously in both ventricles: 1) EF, 2) peak ejection rate, 3) duration of systole, 4) peak filling rate, 5) time to the peak filling rate, and 6) ratio of the peak filling rate to the peak ejection rate. The mean RV ejection fraction and the peak ejection rate in groups 1 and 2 were similar to those in the normal subjects. The ratio of the peak RV filling rate to the peak RV ejection rate, a sensitive indicator of diastolic filling function, decreased significantly in group 1 (0.61 +/- 0.15, p less than 0.02) and group 2 (0.59 +/- 0.15, p less than 0.02) as compared with that in the normal subjects (0.74 +/- 0.14). This indicates an impairment of early diastolic RV filling compared with RV systolic function. Thus, RV systolic function appears to be normally maintained even when anteroseptal wall motion is severely impaired; however, even in the presence of mild anteroseptal wall motion abnormality, RV filling may be impaired without reduced RV systolic function.

[An experimental study of the effect of food on osteoporosis especially on the mandible].

Three-week-old male rats of the Wistar strain given standard formed food (MF formed food) were grouped as controls. Those given calcium-deficient food for three weeks and then standard formed food after experimental osteoporosis induced in the rats were grouped as the experimental group. Experimental periods were for 4 and 6 weeks and the effect of the food on osteoporosis was investigated. The results were as follows: 1. Bone density of the alveolar bone showed significantly lower values for the experimental group than for the controls. 2. In comparison of relative Ca ratios (Ca/[Ca]c) and of P ratios (P/[P]c) on an X-ray microanalyzer, the controls showed higher values each week than the experimental group. 3. Compressive strength of the mandible in the experimental group decreased compared with the controls.

[A five-year follow-up study of recipient rates for welfare allowance for the disabled elderly in a metropolitan area].

Improvement of the domiciliary care system for the disabled elderly is one of the most urgent issues of welfare for the elderly. In order to understand more clearly the utilization of domiciliary care service in a metropolitan area, we studied recipient rates for welfare allowance for disabled elderly which was the most frequently used welfare service. Between October 1984 and March 1985 a baseline survey was performed on a random sample of elderly living at home, aged from 65 to 89, in Shinagawa-ku, Tokyo. A follow-up survey of this sample of 873 persons was performed in November 1989 where survival and their recipience of welfare allowance were reviewed. The main results were as follows: 1. Recipient proportion of welfare allowance increased rapidly with disability of ADL. Of the severely disabled elderly at the baseline the recipient proportion was 46%. During the following five years 27% of the severely disabled were accepted as new recipients. 2. Recipient proportion of elderly who died at age 75 or greater for the following five years was more than 30%. 3. The recipient incidence rate of those who were non-recipient at baseline increased significantly with age after controlling for ADL. This indicates that the risk of ADL disability increases with age. 4. Mean recipient duration varied according to age. Mean durations for those aged over 80 and those under 80 were 2.1 years and 3.9 years respectively.

[Localization of glycylproline dipeptidyl aminopeptidase in the liver tissue and possible mechanism of its release into blood flow].

We examined the localization of glycylproline dipeptidyl aminopeptidase (GPDAP) in the liver tissue and the mechanism of its release into blood flow. An immunohistochemical study using rabbit polyclonal antibody against the purified GPDAP from human liver obtained at autopsy was performed in liver biopsy samples with the peroxidase-antiperoxidase stain technique. GPDAP staining was detected on the liver cell membrane around bile canaliculi. After treatment with deoxycholic acid (DOC) or Triton X-100, this enzyme disappeared. GPDAP was solubilized rapidly from microsome of human liver by treatment with either DOC or Triton X-100. DOC or Triton X-100 solubilized GPDAP coincided with the isozyme that was the specific isozyme in sera of patients with acute hepatitis or obstructive jaundice. These results suggest that GPDAP localizes on the liver cell membrane around bile canaliculi and that this enzyme is released by the detergent action of bile acids.

[A case report of right ventricular infarction clearly detected by transesophageal echocardiography].

We reported a case of a 70 year-old woman who suffered from right ventricular infarction with cardiogenic shock, detected clearly by transesophageal echocardiography. On admission, her pulse rate was 31 bpm and her blood pressure was unobtainable. Conscious level was III-1-2 and she was cold and clammy. The ECG showed complete AV block with junctional escape rhythm at a rate of 31 bpm which required temporary pacing and ST elevation in leads II, III, a VF, V4R, V3R, V1. An echocardiogram showed akinesis of RV free wall and paradoxical septal motion. Transesophageal echocardiography was performed safely on the 5th hospital day and detected RV wall motion abnormality clearly. A Swan-Ganz catheter was inserted. Mean PCW was 12 mmHg. PA pressure was 19/11 mmHg. Mean RA pressure was 13 mmHg. Cardiac index was 1.33 l/min/m2. SvO2 was 54%. Volume loading, administration of dopamine, dobutamine and nitroprusside were started. Cardiac index increased to 1.88 l/min/m2, and SvO2 increased to 59%. On the 4th hospital day, mean RA pressure increased to 29 mmHg and PA pressure increased to 47/31 mmHg acutely. Endotracheal intubation was done and PEEP 6 cmH2O was used and mean RA pressure and PA pressure decreased. On the 6th hospital day, cardiac index increased 4.08 l/min/m2. Cardiac catheterization done two months after acute myocardial infarction showed 75% stenosis of the proximal right coronary artery.

[Allergenicity of the osmophilic fungus Aspergillus restrictus evaluated by skin prick test and radioallergosorbent test].

Recently large amounts of Aspergillus restrictus, a species of osmophilic fungi, have been detected in house dust using culture media with low water activity. But little attention has been paid to this fungus as an allergen. In the present study, the authors examined the allergenic activity of A. restrictus by skin prick tests and radioallergosorbent tests (RAST) on 94 asthmatic patients (mean age 12.0, range 3-18). Aspergillus fumigatus, Alternaria alternata and house dust were used for comparison. In the skin prick tests, A. restrictus, A. fumigatus, A. alternata and house dust elicited positive reactions in 8 (8.5%), 8 (8.5%), 15 (16.0%) and 69 (73.4%) patients, respectively. RAST showed positive reactions in 27 (28.7%) subjects for A. restrictus, 22 (23.4%) for A. fumigatus, 35 (37.2%) for A. alternata, and 75 (79.8%) for house dust. These results indicated that some asthmatic individuals showed immediate-type hypersensitivity to A. restrictus, and the prevalence of hypersensitivity of A. restrictus determined by skin prick tests and RAST was comparable with that of A. fumigatus but lower than that of A. alternata or house dust. This indicates that this fungal species may be of importance as a causative agent in atopic diseases.

[Clinicopathological study of endometrial-type adenocarcinoma of uterus].

Seventy-one resected cases of endometrial-type adenocarcinoma of uterus were clinicopathologically investigated. As to the gross findings, the incidence of localized type and diffuse type was 47.9% and 52.1%, while that of extroversive type and introversive type was 59.1% and 40.9%, respectively. The incidence of the lesions less than 3 cm in diameter was 36.7% and that beyond 5 cm was 28.1%. The majority (88.7%) of the endometrial-type adenocarcinoma were histologically well-differentiated. Concerning depth of invasion, the incidence of the cases of intramucosal invasion, invasion within 1/3 of muscle layer, invasion between 1/3 and 2/3 of muscle layer, and invasion beyond 2/3 of muscle layer was 19.7%, 46.5%, 16.9%, and 16.9%, respectively. Lymphatic and venous invasion were more frequently found in diffuse, introversive, moderately-differentiated, and deep invasive types than the other types. Lymph nodal metastasis was found in 9.8% of all cases (16.2% in diffuse type, 20.8% in introversive type, 33.3% in moderately-differentiated type and 41.7% in the cases beyond 2/3 of muscle layer).

[An approach to the control of the so-called vagrant patients with tuberculosis].

In recent 3 years, we experienced 46 cases of so-called vagrants, who were admitted for active tuberculosis to our hospital at the time of detection, their disease was in advanced and severe state, and 35 out of 46 patients were admitted for emergency and 7 cases died of tuberculosis soon after admission. In the remaining cases, anti-tuberculosis agents were effective, however, the incidence of dropout cases was much higher. How to improve compliance of these cases to treatment is one of the most difficult but important problem in present tuberculosis control.

Autogenous oscillatory potentials in neurons of the guinea pig substantia nigra pars compacta in vitro.

In spontaneously firing neurons of the guinea pig substantia nigra pars compacta (SNC) maintained in slices, blockade of the fast spikes by tetrodotoxin (TTX) revealed a slow oscillatory potential change, which was depressed by Cd2+, a Ca2+-channel blocker. The spontaneous firing was suppressed by the application of Ca2+-free saline, Cd2+ or Co2+, but not by nifedipine. These findings lend support to the view that the spontaneous firing of SNC neurons is produced by an intrinsic, Ca2+-dependent pacemaking process affected by Cd2+ but not by nifedipine.

Human liver serine dehydratase. cDNA cloning and sequence homology with hydroxyamino acid dehydratases from other sources.

Rat liver serine dehydratase cDNA was used to screen a human liver cDNA library in lambda gt11. One positive clone occurred in every 5,000 clones. Fifteen positive clones were plaque purified. The largest cDNA obtained contained an open reading frame of 987 base pairs, and 5' and 3' noncoding regions of 89 and 317 base pairs, respectively. The deduced amino acid sequence, with a calculated Mr of 34,615, was similar to that of rat liver serine dehydratase except for the absence of a segment consisting of 36 amino acid residues. In vitro transcription/translation with the cDNA resulted in the formation of a polypeptide with an Mr of approximately 35,000, which cross-reacted with the anti-rat serine dehydratase antibody. These results suggest that the human serine dehydratase is structurally cognate with the rat enzyme. Moreover, portions of the sequence postulated to be essential for activity in microbial threonine dehydratases are found in the mammalian serine dehydratases, suggesting that hydroxyamino and dehydratases may have originated from a common ancestor.

Effects of sublingual nitrate in patients receiving sustained therapy of isosorbide dinitrate for coronary artery disease.

To examine the effects of sublingual isosorbide dinitrate (ISDN) in patients receiving sustained ISDN therapy, 24 patients with coronary artery disease were divided into 2 groups. Group C comprised 12 patients without sustained ISDN therapy and group N included 12 patients with sustained ISDN therapy. Before and during administration of sublingual ISDN in both groups, aortic systolic pressure, left ventricular end-diastolic pressure and coronary artery diameter were examined at cardiac catheterization. During sublingual ISDN, the aortic systolic pressure decreased by 20 +/- 6% (138 +/- 26 to 112 +/- 27 mm Hg, p less than 0.01) in group C and 10 +/- 6% (127 +/- 26 to 113 +/- 23 mm Hg, p less than 0.01) in group N (p less than 0.01, group C vs group N). The left ventricular end-diastolic pressure decreased by 65 +/- 16% (11 +/- 5 to 4 +/- 3 mm Hg, p less than 0.01) in group C and 43 +/- 14% (12 +/- 5 to 7 +/- 3 mm Hg, p less than 0.01) in group N (p less than 0.01, group C vs group N). During sublingual ISDN, the diameters of the proximal and distal segments of the left anterior descending and circumflex coronary arteries increased more significantly in group C than in group N (p less than 0.01, group C vs group N). Thus, sublingual ISDN produced less reduction of aortic systolic pressure and left ventricular end-diastolic pressure, and less dilation of coronary artery diameter in patients receiving sustained therapy with ISDN than in those without sustained therapy.(ABSTRACT TRUNCATED AT 250 WORDS)

[Continuous hyperthermic peritoneal perfusion with cisplatin and mitomycin C for peritoneal dissemination in gastric cancer].

Thirty-four patients with advanced gastric cancers had received continuous hyperthermic peritoneal perfusion (CHPP) for prevention or treatment of peritoneal dissemination (PD). These patients received intra-abdominal perfusion 30-40 minutes by about 10-liter saline heated to 50-52 degrees C dissolving 200-300 mg cisplatin (CDDP) and 20-30 mg mitomycin C (MMC). The perfusate was infused in the peritoneal cavity through the peritoneal cavity expander (PCE Nihon Kayaku Co. Ltd.) newly developed for sufficient irrigation in our clinic. Eleven patients with macroscopic serosal invasion without PD (P0) or with mild PD (P1) underwent prophylactic CHPP for prevention of peritoneal recurrence. (Pn means degree of PD according to The General Rules for the Gastric Cancer Study.) Twenty-three patients with moderate PD (P2) or severe PD (P3) underwent therapeutic CHPP. The prognosis of patients having undergone prophylactic or therapeutic CHPP (CHPP (+) group) was compared with those not having done CHPP (CHPP (-) group) in the historical control study. In the prophylactic study two-year-survival rates of CHPP (+) group was 90% significantly higher than 63% in CHPP (-) group. There was no significant difference between two-year-survival rates of CHPP (+) (11%) and CHPP (-) (0%) in the therapeutic CHPP. But ascites was observed to disappear in five of twelve (41.7%). Surprisingly second look operation disclosed macroscopic and microscopic disappearance of PD in three of nine (33.3%). The side-effects in those patients had consisted of leakage, bone marrow suppression, perforation of small bowel and so on. But there was no mortality after introduction of PCE. About laboratory data, rises in WBC, BUN, creatinine and LDH and drops in total lymphocytes counts and platelets were all normalized within four weeks. The maximal concentrations of total and free, which emerged at five minutes after CHPP, were 2.19 and 1.29 micrograms/ml. These results suggested that CHPP with CDDP and MMC was well tolerated and effective for prevention of peritoneal recurrence and treatment of peritoneal dissemination in the gastric cancer.

Alpha-fetoprotein antibody-lectin enzyme immunoassay to characterize sugar chains for the study of liver diseases.

An antibody-lectin enzyme immunoassay (EIA) technique was developed for the analysis of sugar chains of serum alpha-fetoprotein in various liver diseases. The anti-'alpha-fetoprotein'-IgG was coated on a microtiter plate and then treated with periodic acid. A serum sample was added to the plate and then a 'peroxidase'-conjugated lectin was added. The amount of lectin bound to the sugar chain of the 'alpha-fetoprotein' was estimated from the 'peroxidase' activity. The 'peroxidase' activities of 4 different lectins, LCA, Con A, LCA and EPHA, were compared. The LCA/'wheat germ agglutinin' activity ratio and LCA/EPHA activity ratio were increased in liver diseases and LCA/'wheat germ agglutinin' ratio showed a statistically significant difference between the chronic hepatitis and the liver cirrhosis groups (p less than 0.05). Furthermore, when serum samples were pretreated with sialidase, a statistically significant difference was observed in the LCA/EPHA and LCA/Con A ratios between the chronic hepatitis and the hepatoma groups (p less than 0.05). These results indicated that low sialylation at the non-reduced end of the sugar chains of 'alpha-fetoprotein' occurs in liver cirrhosis and that high fucosylation at the reduced end of N-acetylglucosamine residue of 'alpha-fetoprotein' occurs in hepatomas.

Difficult deflation of Probe balloon due to twisting the system.

While using the balloon-on-a wire device, we experienced a case of difficult deflation of the Probe balloon system. Blockage of the balloon system was possibly caused by twisting the system to reach and pass the lesion in the branch of left circumflex coronary artery. The duration of inflation was 20 min, causing prolonged ischemia.

Left atrial conduit function for left ventricular filling dynamics in patient with myocardial infarction.

This study observed the left function in determining filling dynamics of the left ventricle in patients with myocardial infarction. The study consisted of eight control subjects and ten patients with myocardial infarction. The left ventricular filling volume is considered to be composed of the left atrial passive emptying, active emptying, and conduit volumes. The change of left ventricular filling volume was correlated with that of conduit volume (r = .87, P less than .01). However, the change of left ventricular filling volume did not have any correlation to those of left atrial passive emptying and active emptying volumes. These results suggested that the left atrial conduit function was important in determining filling dynamics of the left ventricle.
