[Experimental and clinical evaluation of cefpiramide in pediatrics].

Fundamental and clinical studies on cefpiramide (CPM), a new semisynthetic cephalosporin were performed and the following results were obtained. Antibacterial activity The antibacterial activity of CPM was investigated in comparison with those of CTT, CPZ, CEZ, LMOX and CFS. Against clinical isolates of S. aureus, CPM was superior to CTT and LMOX, but almost similar to CPZ and inferior to CEZ. Against E. coli, K. pneumoniae, P. mirabilis and S. marcescens, CPM showed the activity almost similar to that of CEZ, but inferior to those of the others. On the contrary, the activity of CPM against P. aeruginosa was satisfactory and was superior to those of CTT, CPZ and LMOX, but slightly inferior to that of CFS. Blood level and urinary recovery Twenty mg/kg of CPM was given intravenously at one shot to 3 patients. The mean serum levels of CPM were 116.9 micrograms/ml at 30 minutes, 90.5 micrograms/ml at 1 hour, 71.1 micrograms/ml at 2 hours, 55.8 micrograms/ml at 4 hours, 24.9 micrograms/ml at 6 hours, 19.3 micrograms/ml at 9 hours and 12.1 micrograms/ml at 12 hours after administration, respectively. The mean half-life was very long and the value was 3.85 hours. The urinary recovery rates in 2 cases were 18.31 and 21.47% respectively up to 12 hours after administration. Clinical results and side effects CPM was given intravenously to 30 diseases including 11 cases of bronchopneumonia, 3 cases of bronchopneumonia and pleurisy, 2 cases of bronchitis, 4 cases of purulent tonsillitis, 5 cases of pyelonephritis and each one case of pyothorax, parotitis, cellulitis, otitis media and salmonellosis. CPM was effective in 29 out of 30 cases, and the effective rate was 96.7%. As side effects, 2 cases of fever and 1 case of cough were observed, but no abnormality in clinical laboratory findings was observed.

[Experimental and clinical evaluation latamoxef in newborn and premature infants].

Basic and clinical studies of latamoxef (LMOX) were carried out in neonates. In basic study, these neonates consisted of 16 mature babies and 12 premature babies. LMOX was administered at dose of 10 and 20 mg/kg, either as a single intravenous injection or as a 60 minutes intravenous drip infusion. Both the mature babies and the premature babies were divided into 3 subgroups as a function of the number of days after birth (0--3, 4--7 and 8--25 days). A clinical study of LMOX was performed in 12 neonates aged between 0--35 days, consisting 6 males and 6 females. (Purulent meningitis 4 cases, septicemia 1 case, bronchopneumonia 5 cases, pertussis pneumonia 1 case, urinary tract infection 1 case). Serum concentration and urinary excretion 10 mg/kg, one shot intravenous injection In the 3 subgroups of neonates the peak serum concentrations of LMOX were found to range from 14.6 to 28.9 micrograms/ml. Although there was no significant difference, the half-life of the drug became shorter as the age of the neonates increased, these values were 4.46, 3.85 and 3.30 hours, respectively. 10 mg/kg, 60 minutes intravenous drip infusion. As above, the peak LMOX serum concentrations were found to range from 23.7 to 38.9 micrograms/ml, the half-lives of the 3 subgroups were 4.83, 2.48 and 3.01 hours, respectively. And urinary excretions were ranged from 46.0 to 56.5% for 6 hours. 20 mg/kg, one shot intravenous injection The peak serum concentrations were found to range from 31.0--82.5 micrograms/ml, and it was found out 3.29--15.9 micrograms/ml at 8 hours after the injection. There was a tendency for the half-life to be shorter in more mature subjects in 3 subgroups. 20 mg/kg, 60 minutes intravenous drip infusion In the 3 subgroups, the peak concentration was the level existing at the end of the intravenous drip infusion, and that showed a range of 41.8--58.6 micrograms/ml. Half-lives were found out the significant difference to their age, these showed 4.08, 2.31 and 2.52 hours. Cerebrospinal fluid concentrations of LMOX Cerebrospinal fluid concentrations of LMOX were studied in 2 cases at the dose about 50 mg/kg. In 1 case, that's meningitis estimated E. coli organism, the cerebrospinal fluid concentrations of LMOX were found to range from 29.0 to 49.9 micrograms/ml in that acute state. In another case from N. meningitidis, that values were found to range 12.1 to 21.3 micrograms/ml. These cerebrospinal levels were superior value at it's penetration ratio. [Pharmacokinetic studies on parenteral infusion of tobramycin in newborn and premature infants].

The fundamental trials on intravenous administration of tobramycin (TOB) were studied in a total number of 29 neonates and premature babies who was divided into 3 groups by their ages; 0--3, 4--7 and 8 or more days after birth. Moreover, the results obtained in them were compared with those in a small number of infants and the school children reported previously to investigate the possibility of administration of the drug to neonates and premature babies. Analysis of the pharmacokinetics of TOB in neonates and premature babies demonstrated that its half-life (T 1/2) became shorter with age; it was 5.37, 4.28 and 3.03 hours at the age of of 0-- 3, 4--8 or more days, respectively when 1.5 mg of the drug per kg body weight is instilled into the vein for 30 minutes. A similar trend was observed at a dose of 3.0 mg per kg body weight. Although the number of subject was small, T 1/2 averaged 2.07 hours at the dose of 1.5 mg/kg in 2 infants and 1.40 hours at 3.0 mg/kg in 3 infants. These results indicate that administration of TOB based on the same conception as that in infants and school children should be avoided in neonates and premature babies. Especially in 0--3, 4--7 day-old neonates and premature babies the behavior of the drug in the body suggests that once or twice administration a day may be safer and better to avoid its accumulation. The peak serum concentrations in neonates and premature babies were slightly higher with decrease in number of days after birth, but it was not specially high as compared with those in infants and school children. It is, therefore considered that 1.5 mg/kg is an adequate single dose to moderate cases. On the basis of the above-mentioned results of the pharmacokinetic study we are confirming the clinical safety and utility of TOB in cases in which its administration is essential.

[Effect of splenic mononuclear cells on autochthonous peripheral blood lymphocyte responses to mitogens in gastric cancer patients].

Mononuclear cells and their fractions which were prepared from spleens removed at the time of surgery for gastric cancer were treated with mitomycin C and their effect on autochthonous peripheral blood lymphocyte responses to PHA-P and Con A was assessed in coculture experiments. Splenic mitomycin C treated mononuclear cells from certain gastric cancer patients were found to have suppressor activity on peripheral blood lymphocyte responses to PHA-P and Con A and the cells with the activity were either plastic dish adherent cells or T-enriched cells alone or both of them. The suppressor activity were more frequently detected in patients with stage IV disease and the patients with the activity tended to exhibit more decreased cell mediated immunocompetence than those without it. Splenic mitomycin C treated mononuclear cells from certain gastric cancer patients were also found to have augmenting activity on peripheral blood lymphocyte responses to PHA-P and the cells with the activity were either plastic dish adherent cells or T-enriched cells or all of plastic dish adherent cells and T- and B-enriched cells.

Study of tumor-associated blastogenic reactivity of peripheral blood lymphocytes and regional lymph node lymphocytes in human malignancy.

The in vitro blastogenic responses of peripheral blood lymphocytes (PBL) and the regional lymph node lymphocytes (LNL) from cancer patients to autochthonous tumor cells (TC) were investigated. A positive lymphocyte response to autochthonous TC was observed in 23% (6/26) for PBL and 34% (9/26) for LNL. There seemed to be an inverse relationship between PBL and LNL responses to autochthonous TC. There was no significant difference between PBL and LNL responses to autochthonous TC according to the primary site of the tumor. Analysis of PBL and LNL responses to autochthonous TC according to the extent of the disease revealed that LNL were definitely more reactive than PBL in patients with a primary tumor that had not spread to the regional lymph nodes.

[Studies on phage type, beta-lactamase activity and sensitivity of multiple drug-resistant Staphylococcus aureus isolated from clinical specimens to cefmetazole].

The 868 strains of S. aureus were isolated at the Department of Pediatrics, Third Hospital and Aoto Hospital, The Jikei University, School of Medicine and the Kanagawa Prefectural Nursing and Hygienic School Hospital during 6 months from May to October in 1981. From them 66 strains not sensitive to CEZ were selected by a 3-concentration disk method. A total number of 54 strains except for 12 isolated from the same infant patient was examined for their MIC's for 6 drugs, CMZ, CEZ, CTM, CXM, MCIPC and GM. Moreover, phage typing and beta-lactamase activity determination were carried out in them. 1. Antibacterial activity Sixty-six (7.6) of 868 strains were not sensitive to CEZ. The MIC's of CMZ against these resistant strains were between 1.56 and 50 micrograms/ml with a peak between 3.13 and 6.25 micrograms/ml when the 10(5) cells/ml bacterial suspension were inoculated. CMZ was superior in antibacterial activity to CEZ and CXM by about 4 degrees and to CTM by about 3 degrees. MCIPC and GM has higher antibacterial activity against a few strains than CMZ. However, the number of strains with MIC higher than or equal to 50 micrograms/ml was 17 for MCIPC and 40 for GM, but only 2 for CMZ. Thus, the former 2 drugs were far inferior to the latter one. 2. Phage type (1) Nineteen strains (35.2%) had MIC's for CEZ greater than 50 micrograms/ml and CMZ less than 6.25 micrograms/ml. Seventeen of them belonged to the nontypable. (2) Fourteen (25.9%) had MIC's for CEZ greater than 100 micrograms/ml. Of them 9 were allocated to the group III, 3 to the mixed (I + II + III, I + III) group and 2 to the nontypable. (3) Of 20 strains (37.0%) which had MIC's for CEZ greater than 100 micrograms/ml and CMZ less than 6.25 micrograms/ml 5 belonged to the group I, 3 to the group III and 12 to the nontypable. (4) Five strains classified into the group I were all isolated at the Kanagawa Perfectural Nursing and Hygienic School Hospital. (5) Eleven of 15 strains belonging to the group III were isolated at the Third Hospital, The Jikei University, School of Medicine. (6) Seventeen of 21 strains isolated at the Aoto Hospital, The Jikei University, School of Medicine belonged to the nontypable. (7) Phage type was considered to be influenced by difference in areas, infection within hospitals, etc., 3. beta-Lactamase activity beta-Lactamase activity was demonstrated at levels between 0.07 and 3.26 mumol/min/mg in all 54 strains. There was no correlation between MIC for CEZ or CEZ and beta-lactamase activity. It was suggested that beta-lactamase might not contribute to mechanism of resistance of S. aureus to each drug examined.

Cefmetazole↗

[Laboratory and clinical studies on ceftizoxime in the field of pediatrics (author's transl)].

Ceftizoxime, a new cephalosporin preparation, was evaluated for its antibacterial activity, absorption, excretion and clinical effectiveness, and the following results were obtained. The minimum inhibitory concentrations (MICs) of ceftizoxime against 211 clinical isolates were determined in comparison with those of cefazolin, cefmetazole, cefotiam and 6059 S. Against S. pyogenes (50 strains), ceftizoxime was 1 tube inferior to cefazolin inoculum size of 10(8) cells/ml, but was 2--3 tubes superior to cefmetazole and 6059-S. Against E. coli (50 strains), ceftizoxime and 6059-S were significantly more active than the other drugs. The susceptibility pattern of Klebsiella sp. (50 strains) to ceftizoxime was similar to that to cefotiam and 6059-S. Against Proteus sp. (50 strains), cefotiam and 6059-S were more active than the other drugs. Ceftizoxime was intermediate in activity, and cefazolin was the least active. Against H. influenzae (11 strains), ceftizoxime was the most active, with concentrations of 0.1 mcg/ml required to inhibit 100% of strains with an inoculum size of 10(8) cells/ml and 10(6) cells/ml. A dose of ceftizoxime 10 mg/kg or 20 mg/kg was administered to 15 patients aged from 5 years to 12 years, and serum levels and urinary excretion of the drug were measured. Intravenous bolus injection of the drug in dose of 10 mg/kg and 20 mg/kg yielded mean serum levels of 26.6 mcg/ml and 55.7 mcg/ml at 30 minutes, respectively. The serum levels of the drug, thereafter, declined gradually but still remained 1.3 mcg/ml and 2.7 mcg/ml at 6 hours. The serum half-lives (T 1/2) were estimated to be 1.17 hours in dose of 10 mg/kg and 1.31 hours in dose of 20 mg/kg. When a dose of 20 mg/kg was infused over a period of 30 minutes, the serum levels attained the peak of 72.4 mcg/ml to 82.4 mcg/ml (mean 79.4 mcg/ml) at the end of infusion. The levels, thereafter, tapered to mean levels of 45.3 mcg/ml at 30 minutes, 24.7 mcg/ml at 1 1/2 hours, and 3.6 mcg/ml at 5 1/2 hours, with a T 1/2 of 1.22 hours. Meanwhile, when the same dose was infused over 1 hour, the serum levels attained the peak of 59.4 mcg/ml to 68.5 mcg/ml (mean 64.2 mcg/ml). The mean serum levels after the end of infusion were 41.3 mcg/ml at 30 minutes, 21.6 mcg/ml at 1 hour and 1.9 mcg/ml at 5 hours, with a T 1/2 of 0.97 hours. Urinary recovery of the drug was 69.2% to 79.9% after intravenous injection and 62.3% to 79.9% after drip infusion, most of the given drug was excreted in the first 2 hours after administration. In our clinical study, 27 children with moderate or severe infections (12 cases of bronchopneumonia or bronchitis, 5 of pyelonephritis, 3 of purulent meningitis, etc.) were treated with ceftizoxime at the daily dose of 30--309 mg/kg for 3--23 days. Clinical response was excellent in 10, good in 9, fair in 5 and poor in 3. The drug was proved to be very effective against infections due to H. influenzae K. pneumoniae, E. coli and S. aureus. No serious side effects were observed in any case.

Effect of spleen-derived cells on autochthonous peripheral blood lymphocyte responses to mitogens in gastric cancer patients.

Lymphocyte-rich mononuclear cells and their fractions which were prepared from spleens removed at the time of total or proximal gastrectomy for gastric cancer were treated with mitomycin C, and their effect on autochthonous peripheral blood lymphocyte responses to phytohemagglutinin P and concanavalin A was assessed in coculture experiments. It was found that splenic mitomycin C-treated lymphocyte-rich mononuclear cells from certain gastric cancer patients had suppressor cell activity on peripheral blood lymphocyte responses to phytohemagglutinin P and concanavalin A and that the cells with the activity were either plastic dish-adherent cells or T-enriched cells alone or both of them. It was also found that splenic lymphocyte-rich mononuclear cells from certain gastric cancer patients had augmenting activity on peripheral blood lymphocyte responses to phytohemagglutinin P and that the cells which exerted the activity were either plastic dish-adherent cells or T-enriched cells or all of plastic dish-adherent cells and T- and B-enriched cells.

[Basic and clinical studies of cefotiam in the field of pediatrics (author's transl)].

A new synthetic cephalosporin, cefotiam (CTM) was studied from the basic and clinical aspects, and the following results were obtained: 1. Bacteriological study: The bacterial activities of CTM against the clinical isolates of S. aureus, S. pyogenes, E. coli, Klebsiella sp. and Proteus sp. were compared with those of CEZ, CER, ABPC and GM. (1) As for S. aureus and S. pyogenes, the antibacterial actions of the conventional cephalosporins were slightly more potent than those of CTM. (2) However, CTM had the antibacterial actions which were most potent Proteus sp. among the above 4 antibiotics and more potent against E. coli and Klebsiella sp. than the above 2 conventional cephalosporins. 2. Pharmacokinetic study: The peak serum levels of CTM were comparatively low with 19.30 +/- 1.66 mcg/ml 30 minutes after a bolus injection of 20 mg/kg and 25.85 +/- 4.32 mcg/ml just after a drip infusion of 20 mg/kg. The half-life of the serum levels was 0.98 hrs. and 0.87 hrs., respectively. The 4 hours urinary excretions in six patients ranged from 38.6 to 64.9%, showing a slightly wide variance. 3. Clinical study: The clinical response was good in 23 out of total 25 cases, i.e. 14 cases with respiratory tract infection, 7 with urinary tract infection and 4 with skin and soft tissue infection. The response rate was 92%. Also, neither side effects nor abnormal laboratory findings was observed.
