[Surgical resection in pulmonary metastases of genitourinary cancers].

Four patients (2 with renal cell carcinoma, 1 with testicular embryonal carcinoma, 1 with testicular seminoma) underwent surgical resection for pulmonary metastases. Three patients except one with renal cell carcinoma are still alive without disease at 1, 2.3 and 2.3 years, respectively. Surgical resection of metastatic pulmonary sarcoma or carcinoma is now the accepted method of treatment. As chemo- and radio-therapy are often ineffective for metastatic genitourinary cancers, our current policy is to employ surgical treatment provided that the operability is assessed carefully.

Hepatocellular carcinoma arising in noncirrhotic and highly cirrhotic livers: a comparative study of histopathology and frequency of hepatitis B markers.

Hepatocellular carcinoma (HCC) associated with cirrhosis and HCC developing in a noncirrhotic liver may have differing pathogeneses. To study this possibility, 425 autopsied cases of HCC were investigated. Of these, 45 livers were not cirrhotic, 50 were highly cirrhotic (liver weight less than 99 g), and the remaining 331 were cirrhotic but not so highly. The average age was significantly older in the highly cirrhotic group, suggesting a longer premalignant period of chronic liver disease. The liver weight in the noncirrhotic group was about 3.5 times that in the highly cirrhotic group. Hepatitis B surface antigen was positive in serum in only 9.3% and in liver tissue in 10% in the noncirrhotic cases, the positivity rate being much lower compared with other groups (P less than 0.005--0.01), yet antibody to HB core was positive in 90%. The antibody titers were low, however, indicating that these noncirrhotic patients had in the past had HB virus (HBV) infection with no residual chronic B hepatitis. Analysis of the grades of anaplasia of cancer tissue demonstrated an inverse correlation between the degree of fibrosis and grade of anaplasia, i.e., the more advanced the fibrosis, the less anaplastic the cancer. These data suggest that HCC arising in highly cirrhotic liver and in noncirrhotic livers have different pathogenetic backgrounds, and that HBV infection, even though transient, has a certain role in hepatocarcinogenesis. The generally held conjecture that HCC in a noncirrhotic liver is caused by nonviral carcinogens and HCC arising on the ground of cirrhosis is due to HBV seems untenable in such a simple concept.

Syntheses of 15 alpha- and 15 beta-carboxymethyltestosterone bovine serum albumin conjugates: characteristics of the antisera to testosterone.

Syntheses of 15 alpha- and 15 beta-carboxymethyltestosterone (15 alpha- and 15 beta-CMT) were investigated in order to prepare testosterone-bovine serum albumin conjugates for radioimmunoassays of testosterone. A mixture of 15 alpha- and 15 beta-bis(ethoxycarbonyl)methyl-3 beta-hydroxy-5-androsten-17-one (IIa and IIb) obtained by a reaction of 3 beta-hydroxy-5,15-androstadien-17-one (I) and sodium diethyl malonate was oxidized to afford a mixture of 15 alpha- and 15 beta-bis(ethoxycarbonyl)methyl-4-androstene-3, 17-dione (Va and Vb). After the separation by silica gel chromatography, each epimer obtained was hydrolyzed by acid, followed by decarboxylation, and selective reduction of the 17-ketone to give 15 alpha- and 15 beta-CMT. The antisera, generated in rabbits by immunization with the bovine serum albumin (BSA) conjugates of 15 alpha- and 15 beta-CMT, respectively, exhibited high specificity for testosterone.

A new staining method for hepatitis B surface antigen by horseradish peroxidase.

An affinity of horseradish peroxidase for hepatitis B surface antigen (HBsAg), irrespective of antigen-antibody conjugation, was disclosed incidentally, and a new staining method of HBsAg was established. The new technique is simple, inexpensive and available for formalin-fixed, paraffin embedded sections, and does not require fresh liver tissue. Compared with three other methods, involving Shikata's orcein staining, the conventional immunoperoxidase method, and the peroxidase-anti-peroxidase method, it showed high specificity and sensitivity, which were not lower than the other three methods. Conjugation of horseradish peroxidase with HBsAg was confirmed by affinity chromatography. Attention should be paid to the fact that there is a possibility of obtaining false positive results by immunoperoxidase methods, staining antigens other than HBsAg in HBsAg positive liver tissue because of antigen-antibody independent affinity of peroxidase for HBsAg.
