The uptake of horseradish peroxidase in monkey temporomandibular joint synovium after occlusal alteration.

We conducted an electron microscopic study on the alterations in the temporomandibular joint (TMJ) synovium of the monkey after it was subjected to unilateral bite-raising, by means of a maxillary occlusal splint. The uptake of intravenously injected horseradish peroxidase (HRP) by type A cells was demonstrated in the disturbed TMJ. The uptake of HRP decreased three to six wk after occlusal alteration, and was the same as in normal type A synovial cells after 16 wk.

Successful removal of air gun bullets from the third ventricle.

A patient with air gun bullets in the 3rd ventricle associated with delayed ventricular hemorrhage is presented. Through an anterior transcallosal approach, the surgeon successfully removed the bullets without any significant permanent sequelae.

New additional operation for the prevention of the gastric stasis after selective gastric vagotomy with antrectomy.

In order to prevent postoperative gastric stasis, we devised and tested the following improvements in the selective gastric vagotomy with antrectomy. (1) The gastroduodenostomy was made so as to have an acute angle to the longitudinal axis of the remnant stomach, and (2) the downward traction being exerted on the remnant stomach by the transverse colon was eliminated by dissecting the greater omentum, and then (3) the posterior wall of the corpus on the lesser curvature side was fixed to the stump of the hepatogastric ligament, and the posterior wall of the corpus on the greater curvature side was fixed to the retroperitoneum inferior to the pancreas. By these procedures, the corpus was maintained in a position superior to the anastomosis. These operative procedures resulted in preventing the gastric stasis after the start of oral feeding. The two patients on whom this operation was performed have been followed up for 3 to 4 months since the operation, and neither of them has had any complaint of gastric stasis, such as anorexia or a feeling of heaviness of the stomach.

[Current status of diagnosis and treatment of metastatic liver cancer].

Clinical usefulness of liver function tests, tumor markers including CA 19-9, CEA and serum ferritin and image diagnosis was discussed in regard to early diagnosis. Liver function tests were not useful, and tumor markers were highly positive, i.e., CA 19-9 was positive in 70.7% and CEA in 66.7%. However, these were low positive rate, in the early stage. Imaging techniques were very useful in detecting liver metastasis; US positive in 92.5%, CT 87.2% and angiography 93.1%. In the treatment of metastatic liver cancer, arterial chemotherapy and transarterial embolization are the major modality.

Automatic radiologic reporting system using speech recognition.

A radiograph report is usually made from an oral dictation by a radiologist, which is then typed. Typing Japanese is rather inconvenient and consumes many hours. In this paper we introduce a computer-assisted reporting system for radiologic images using speech recognition. The hardware of the reporting system consists of a speech recognizer DP-200(NEC) and a personal computer PC-8801 or PC-9801. The DP-200 has the capability of storing 500 different words spoken by a radiologist. At present, three application programs have been designed. These are for the interpretation of a liver scintigram, a bone scintigram and a chest radiograph. Data entry is done by the radiologist at a CRT display terminal in a conversational manner with predefined and predetermined branching. The time required to make a normal report using the liver or bone scintigram system was within one minute. The reporting time was several minutes in the case of an abnormality report. It is suggested that the system is useful for making an imaging report, for constructing the data base for the interpretation of medical images and for the picture archiving and communication system.

Spinal haemangiolipoma. Three case reports.

Three cases of haemangiolipoma are described. Two of them were in the thoracic region and one in the lumbar region. All of them were subtotally removed, positively diagnosed histologically and good recovery was obtained. In reviewing 16 previous cases, the clinical characteristics, neurological features and neuroradiological findings were noted. Spinal extradural haemangiolipomas show a high incidence in middle-aged people and at the mid-thoracic level. Usually they show the signs of an extradural space-occupying lesion. Sometimes, worsening and improving are found particularly in female cases. Reviewing the existing literature, our diagnosis and procedures stress the importance of technical aspects in performing myelography and the findings of CT scanning.
