[Infected solitary renal cyst extending to subphrenic abscess--a case study and review of the literature].

A 54-year-old female, who had been mediated for simple type schizophrenia for the past eleven years, was hospitalized with left lumbar discomfort or pain. After admission, the following abnormalities were apparent serologically; leucocytosis of 15700/mm, erythrocyte sedimentation rate of 147 mm/h, hyper alpha 2-globulinemia (675 ng/dl of haptoglobulin). Urine cultures were negative. Left renal simple cyst with infection was evaluated by intravenous urography, ultrasonogram, computed tomographic (CT) scan., and renal angiogram. In addition, left subphrenic abscess behind the spleen, which had to be penetrated through retroperitoneum, could be diagnosed. So, in our case, cystic wall resection (unroofing) and/or conservatively percutaneous cystotomy for pus drainage were all not indicated instead of radical operation for abscess removal completely. CT scan is mostly available to make differential diagnosis whether the infectious renal cyst influence to the neighbouring organs beyond the renal capsule, or not. Here, we discussed the pathogenesis on the infection of solitary renal cyst, and some problems reported in the literature.

[Dopamine infusion test as a gastric secretion test].

The secretion of gastric acid in the peptic ulcer patients were examined by the dopamine infusion test (DIT). Production of gastric acid was investigated when the patients were treated by 30-minute dopamine infusion of 2 micrograms/kg/min. According to the difference of production of gastric acid, 132 patients were categorized into three groups: 59 Cata-responders, 28 Ana-responders and 45 Non-responders. Cata-responders were the patients whose production was less than 0.2mEq/30 min, Ana-responders, greater than 0.2mEq/30 min and Non-responder less than or equal to 0.2mEq/30 min. Cata- and Ana-responders showed higher gastric acid secretion preoperatively, and higher gastric acid reduction rate after selective proximal vagotomy (SPV) than Non-responder. Cata-responder showed decreased acid output preoperatively and unchanged after SPV with DIT. Ana-responder showed increased acid output preoperatively and decreased after SPV with DIT. Non-responder showed unchanged acid output preoperatively and decreased after SPV. Cata-responder may be depending more on vagus-mediated factor and parietal cell mass. Ana-responder may be depending more on vagus-mediated factor, parietal cell mass and dopaminergic factor. Non-responder may be depending more on gastrin cell mass and less cholinergic influence. From these findings dopamine receptor may exist in the stomach, and DIT seems to be useful in studying pathogenesis of the peptic ulcer and determining treatment for patients with peptic ulcer.

[A case of chemotherapy by the transarterial catheter with reservoir against malignant lymphoma of the colon].

We had a case of malignant lymphoma of the colon with two lesions, one in the transverse colon and the other in the ascending colon. The tumor of the transverse colon was resected, but that of the ascending colon was unresectable. Chemotherapy was performed through the catheter with reservoir via the iliac artery into the right colic artery. Three months later, the tumor disappeared completely, and the ascending colon was resected. On histopathological examination, there was no lymphoma cell in the ascending colon. Chemotherapy by a transarterial catheter with reservoir is a new method of treatment against malignant lymphoma.

[A case of bronchopulmonary aspergillosis recurring in a residual tuberculous cavity].

A 52-year-old man, who had undergone right upper lobectomy because of active tuberculosis 29 years before, was admitted with complaints of severe cough and expectation. Two years ago, he had pulmonary aspergillosis and was successfully treated with some anti-mycotic agents. This time his chest X-P showed fungus ball in a residual tuberculous cavity in the right upper field and he was diagnosed as pulmonary aspergilloma from the results of radiological findings, sputum culture, and serologic test. By bronchofiberscopy fungus ball was observed. With transbronchial infusion of Amphotericin B, intravenous administration of Miconazole and oral administration of Flucytosine, clinical symptoms have improved and lysis of fungus ball was observed. Sputum culture revealed Aspergillus flavipes group. Bronchopulmonary aspergillosis, which was incurable by surgical treatment because of underlying disease, was successfully treated with transbronchial infusion of Amphotericin B and administration of some anti-mycotic agents.

The effect of ulinastatin on reduced nerve conduction velocity and blood pressure.

Application of a pneumatic tourniquet in orthopedic surgery is sometimes followed by hypotension and paralysis. Seventy-five patients scheduled for knee joint surgery were examined to evaluate the effects of ulinastatin on changes in blood pressure, venous pH and motor nerve conduction by pneumatic tourniquet application above the knee joint. In fifty-five of the patients, the femoral vein of the operating side was cannulated with a catheter to obtain venous blood samples before and after tourniquet application. 300,000 Unit of ulinastatin (UST) was administered intravenously before inflation of the tourniquet in 10 patients. In the other sixteen patients, the motor nerve conduction velocity (MNCV) of the peroneal nerve was measured before inflation and after release of the tourniquet. In the UST-free group, the reduced blood pressure and pH persisted for more than 15 min, while in the UST-treated group, the reduced blood pressure returned to the normal leven in 15 min. In the control group, femoral venous P(O)(2) continued to increase after 10 min, but that in the UST-treated group returned to the normal range. Tourniquet application significantly reduced peroneal MNCV. Pre-and post-treatment with UST significantly lessened the reduction of MNCV induced by the tourniquet. It is concluded that UST may have protective and therapeutic effects on ischemic nerve injury, induced by the application of a turniquet.

Effect of stilbenedisulfonate binding on the state of association of the membrane-spanning domain of band 3 from bovine erythrocyte membrane.

The membrane-spanning domain of bovine band 3, the anion transport protein of erythrocyte membrane, was purified in the presence of nonaethyleneglycol lauryl ether (C12E9) and the effect of a covalent attachment of 4,4'-diisothiocyanostilbene-2,2'-disulfonate (DIDS), a potent transport inhibitor, on the state of association of the domain isolated (the 58 kDa fragment) was studied via gel filtration, gel electrophoresis and sedimentation velocity experiments. It was indicated that the DIDS-unlabeled fragment in C12E9 solution forms heterogeneous aggregates which are larger in size than the dimer. This contrasted with the behavior that bovine band 3 is present as dimers or tetramers in the same medium (Nakashima and Makino (1980) J. Biochem. 88, 933-947). When DIDS was covalently attached, the fragment was present as a single molecular species which was indicated to be a dimer by molecular weight determination. The secondary structure of the fragment was not affected by DIDS. The change in the state of association caused by the DIDS-binding was also found in the presence of sucrose monolaurate (SE12), which was a more potent detergent for extraction of the 58 kDa fragment from membranes than C12E9. However, the complex with SE12 was extremely unstable.

Hepatocellular carcinoma with rhabdomyoblastic differentiation.

An extremely unique case of a liver tumor occurring in a 70-year-old man is documented in this article. The primary tumor was well encapsulated by a thick, connective capsule and was histologically composed of two distinct elements, i.e., a common hepatocellular carcinoma (HCC) and a rhabdomyosarcoma. Metastasis of HCC was only seen in the left adrenal gland, whereas intrahepatic metastatic foci as well as tumor thrombi occluding the portal vein branches were composed exclusively of rhabdomyosarcoma. The possibility that the rhabdomyoblastic component might have come from the preexisting HCC by way of metaplastic proliferation is discussed.

Effects of phosphorylation, MgATP, and ionic strength on the rates of papain degradation of heavy and light chains of smooth muscle heavy meromyosin at the S1-S2 junction.

The effects of ionic strength, MgATP, and phosphorylation on the degradation rates of heavy meromyosin (HMM) by papain have been compared to their effects on the sedimentation coefficient (s20,w) to determine the relationship of the degradation rate to the equilibrium between the flexed and the extended forms (Suzuki, H., Stafford, W. F., Slayter, H. S., and Seidel, J. C. (1985) J. Biol. Chem. 260, 14810-14817). At 0.025 M NaCl, where HMM is predominantly in the flexed form, MgATP, Mg-adenylyl imidodiphosphate or MgADP reduce kH by 80-90%. MgATP exerts its optimal effect at this ionic strength, where at least 70% of HMM is flexed in the presence or absence of MgATP, suggesting that nucleotides reduce kH by decreasing the proteolytic susceptibility of the flexed form. At 0.5 M NaCl, where HMM is in the extended form, MgATP has no effect on kH. At low ionic strengths phosphorylation decreases kH but increases it in the presence of MgATP. Plots of kH against s20,w determined at various ionic strengths are linear, the data for phosphorylated and dephosphorylated HMM falling on the same line. Thus, raising the ionic strength or phosphorylating the 20-kDa light chain appears to alter kH by increasing the fraction of HMM in the extended form. The degradation rate of the 20-kDa light chain (kL) of dephosphorylated HMM responds to changes in ionic strength in essentially the same way as does kH, suggesting that the response of kL to changes in ionic strength can also be attributed to conversion of HMM to the extended form. However, kL for phosphorylated HMM measured in the presence of MgATP exhibits very little dependence on ionic strength.

Erythromycin and its derivatives with motilin-like biological activities inhibit the specific binding of 125I-motilin to duodenal muscle.

Erythromycin, one of the macrolide antibiotics, and its derivatives had been found to mimic actions of exogenous motilin, a gastrointestinal peptide hormone. We found that some of the macrolide compounds inhibited the specific binding of 125I-motilin to rabbit duodenum muscle at 15 C in a dose-dependent fashion. The inhibitory activity of several macrolides examined did not relate to their antibacterial activity but to their motilin-like activity. A 50% inhibition by EM536, a non-antibacterial erythromycin derivative with the highest motilin-like activity, was obtained at 3-40 nM and little higher than that of non-radioactive motilin (5-6 nM) under the present conditions. The results suggest that erythromycin and its derivatives mimic physiological actions of motilin by acting as agonists for a motilin receptor.

Mechanisms of basolateral base transport in the renal proximal tubule.

Renal proximal tubules absorb HCO3- by secretion of H+ into the tubular lumen. This paper focuses on the mechanisms of HCO3- exit across the basolateral cell membrane. The major exit pathway is rheogenic sodium bicarbonate co-transport. This system transports Na+ and HCO3-, but not Cl-, in obligatory coupling at a fixed overall stoichiometry of three HCO3- to one Na+. The fact that HCO3- flux is reduced after inhibition of cytoplasmic and/or membrane-bound peritubular carbonic anhydrase indicates that HCO3- is not transported as such but is split during permeation into its buffer subspecies from which it is regenerated on the other side of the membrane. Since flow of OH- or of H+ (in opposite directions) can be excluded, it appears most likely that one HCO3- and one CO3(2-) move together with one Na+. Besides carbonic anhydrase inhibitors, disulphonic stilbenes and harmaline are known to block the co-transporter. In addition to rheogenic Na+ (HCO3-)3 co-transport, Na+-dependent and Na+-independent electroneutral Cl-/HCO-3 exchange have been identified. The latter mechanisms are particularly important in S3 segments of proximal tubule where Na+ (HCO3-)3 co-transport is missing. Further mechanisms which operate in parallel, but at lower rates, are electroneutral SO4(2-)/HCO3- exchange and, in some species, lactate/HCO3- exchange. Moreover, there may be some uncoupled OH- flux and it is reasonable to assume that OH- (or H+) flux is involved in the transport of dicarboxylic acids across the basolateral cell membrane.

Dual effect of N-ethylmaleimide on Cl- transport across the thin ascending limb of Henle's loop.

Effects of SH reagents on Cl- transport were studied in the isolated hamster thin ascending limb of Henle's loop (TAL) perfused in vitro. Parachloromercuribenzene sulfonate (PCMBS) at 10(-4) M in the bath decreased the relative permeability for Cl-/Na+ (PCl/PNa), as determined by the transmural diffusion voltage (VT) generated under a NaCl concentration gradient, from 2.71 +/- 0.16 to 1.11 +/- 0.09 (P less than 0.001). The effect of PCMBS was prevented by the pretreatment with 10(-3) M dithiothreitol (DTT). N-Ethylmaleimide (NEM) at 10(-3) M in the bath exhibited a dual action on Cl- permeability of the TAL: It inhibited the Cl- permeability in fresh preparations, whereas it stimulated the Cl- permeability in the preparations pretreated with SH reagents including NEM, maleimide and PCMBS. The inhibitory effect was irreversible but the stimulatory effect was reversible. Both responses were prevented by DTT. Since dextran-maleimide did not show any inhibitory effect on PCl/PNa, the SH site responsible for the inhibition may be located inside of the cell. The stimulatory effect of NEM on PCl/PNa was markedly reduced when bath pH was reduced to 5.8. On the other hand, when the bathing fluid was made nominally Ca2+ free, the stimulatory effect of NEM was unaffected, although the basal level of PCl/PNa was reduced These observations suggest that the conductive Cl- pathway in the TAL is either stimulated or inhibited by modifying two distinct SH sites. The site of modulation by proton binding may exist distally to these SH sites. The regulatory mechanism involving Ca2+ may be independent of the SH regulatory sites.

Morphometric analysis of hepatocellular carcinoma.

In order to characterize the cytological features of highly differentiated hepatocellular carcinoma (HCC), a comparative morphometric study was made by observing 30 cases of HCCs and controls (normal, cirrhotic, and atrophic livers). Among trabecular HCCs, normotrabecular subtype (1-2 cell thick cell plate) usually showed minimal cytological atypism and was categorized as well or highly differentiated HCC. Using an image analyzer, the following 4 parameters were applied to quantitate the hepatocyte changes: mean cell size (C), mean nuclear size (N), nucleocytoplasmic (N/C) ratio and a coefficient of variance (CV = index of anisokaryosis). In normotrabecular HCCs, C was slightly but significantly reduced when compared with normal and cirrhotic livers (t-test: p less than 0.005). The value was further reduced in mid- and macrotrabecular HCCs. Normotrabecular HCCs showed almost the same N value as normal and cirrhotic livers but displayed significantly a higher N/C value than those of controls (t-test: p less than 0.001). The N/C ratio became even greater in other types of HCCs. While CV was relatively constant in other HCC groups and controls, it was extremely high in the pleomorphic type of HCC and liver cell dysplasia. The results indicated that a reduction in C and increase in N/C ratio, which appear as "nuclear crowding" in histological specimens, actually occurs in well differentiated HCC. For the histologic diagnosis of well differentiated HCC, it would be very important to examine liver specimens with these observations in mind.

Morphometric analysis of cytological atypia in colonic adenomas.

A total of 29 colorectal polyps were classified into those with mild, moderate, and severe dysplasia, and carcinoma. Morphometric analyses were performed on each group, determining; mean nuclear area (N-area), nuclear cytoplasmic (N/C) ratio, nuclear crowding index (Cx) and mean cellular area (C-area). N-area and N/C ratio increased in accordance with the progress of the atypical changes. Cx showed a gradual increase up to moderate dysplasia and a decline from severe dysplasia to carcinoma. N-area in severe dysplasia and carcinoma was distinctively larger than that in mild and moderate dysplasia. As for C-area, the value from normal to moderate dysplasia remained about the same, in contrast to the much higher levels in severe dysplasia and carcinoma. These results clearly indicated that the rise in N/C ratio from normal to moderate dysplasia depends mainly on nuclear crowding, but that from severe dysplasia to carcinoma on nuclear enlargement.

Genetic polymorphism of tear proteins in the rat.

Polymorphism of tear proteins was found by agarose gel electrophoresis among inbred strains of rats. The proteins (RTP-1) are inherited as a single autosomoal trait. The locus was designated Rtp-1 (rat tear protein-1) and it had two codominant alleles (Rtp-1a, Rtp-1b). Although we did not find any recombinant between the Rtp-1 and the Mup-1 loci among 67 backcross progeny, we found 3 strains with the recombinant type between them in 33 inbred strains tested. The results suggest that the Rtp-1 locus is very closely linked with the Mup-1 locus, which belongs to rat linkage group II. RTP-1 proteins strongly reacted with anti-MUP-1 A serum on agarose gel electrophoretograms.

Colorimetry of dehydroalanine residues preserved as 'lost side chains' in thyroglobulin.

We developed a new assay method for dehydroalanine residues in thyroglobulin, which had been proposed to be the 'lost side chains' during thyroid hormonogenesis. Thyroglobulin preparations were labeled with 4-aminothiophenol at 30 degrees C for 10 days. Under the conditions, the reagent reacted only with dehydroalanine and cysteine residues. The 4-aminothiophenol bound to cysteine was eliminated by reductive cleavage. The 4-aminothiophenol-labeled dehydroalanine (4-aminophenylcysteine) residues were liberated by acidic hydrolysis, converted to a colored derivative by the Bratton-Marshall reaction and quantified colorimetrically. The number of dehydroalanine residues was the same as that of hormone residues in each thyroglobulin preparation. The results indicate that when one hormone residue is produced by the coupling of two iodotyrosine residues, the 'lost side chain' is preserved as one dehydroalanine residue in the thyroglobulin molecule.
