Extracellular adenosine triphosphate completely reverses the thyrotropin-induced morphological change in FRTL-5 cells.

We quantified the TSH-induced morphological change in FRTL-5 thyroid cells according to a morphological index corresponding to the mean cell area measured from microscopic photographs. Within 15 min, TSH induced, at 10 pM and higher concentrations, a decrease in morphological index together with a rise in cAMP levels in a TSH dose-dependent manner. Forskolin, 3-isobutyl-1-methylxanthine, and RO 20-1724, the latter two being phosphodiesterase inhibitors, mimicked these TSH effects, indicating that the rise in cAMP levels is responsible for the TSH effect. Extracellular ATP and its derivatives, known as purinergic receptor agonists, decreased cAMP levels and caused a complete reversal of the TSH morphological effect. Prior exposure of the cells to islet-activating protein (pertussis toxin), the depletion of extracellular Ca2+, or the addition of low doses of protein kinase-C inhibitors completely abolished the inhibitory action of ATP on the TSH effect, whereas phorbol 12-myristate 13-acetate, which activates protein kinase-C, mimicked the ATP action to some extent. Thus, although the TSH-induced change in cell morphology seems to be dependent on cAMP levels, the inhibition of TSH action by ATP seems to be mediated by at least two signal transduction pathways involving islet-activating protein substrate G-proteins: one inhibiting adenylate cyclase and the other involving Ca2+ and protein kinase-C.

Second genetic polymorphism of tear proteins in the rat (Rtp-2).

Electrophoresis of tear proteins on agarose gel showed polymorphism in the fastest migrating protein among 32 inbred strains of rats. In 8 strains, the protein was missing (RTP-2 B), while the other strains expressed the protein (RTP-2 A). The trait was found to be controlled by a single autosomal locus. The designation Rtp-2 locus, with two alleles (Rtp-2a, Rtp-2b), is tentatively proposed. The Rtp-2 locus is loosely linked to c locus, with a recombination frequency of 36.7 +/- 5.4 percent.

[Microbeading resin-bonded retainers. 1. Tensile bond strengths and durability].

With the development of adhesive luting resins that adheres both dental alloys and tooth enamel has been widely used the resin-bonded fixed partial denture recently. Since we experienced, however, that the metal retainers without mechanical retention such as channels were often dislodged with metal interface failure, the more durable bonding between a luting resin and a metal has been desired. This study was undertaken to investigate the effect of microbeading onto the adherent metal surface on the tensile adhesive strength between an adhesive luting resin and a dental alloy. Maxgold equivalent to Type IV gold and Panavia EX were used as an adherent metal and a luting resin, respectively. The following conclusions were drawn: 1. Microbeading on the adherent metal surface made no contribution to an increase of the tensile adhesive strength, unless the adherent surface treatment was achieved. However, once 50 microns alumina blasting and tin plating were achieved on the microbeaded metal surface, the tensile adhesive strength was significantly increased. 2. Taking into account the fabrication technique and the tensile adhesive strength, the optimum diameter of microbeads seems to be 150 microns. 3. In the result of various accelerative durability tests (invasion tests in 70 and 100 degrees C water, thermal cycling between 4 and 60 degrees C water), the durability of 150 microns microbeaded metal surface on which 50 microns alumina blasting and tin plating were achieved was much better than that of the smooth metal surface.

[Adherent surface treatment that improve the shear bond strength of calcium metaphosphate glass-ceramic (CMP)].

In this study we examined the shearing adhesive strength between a calcium metaphosphate glass-ceramic (CMP) and various luting materials. Two usual luting cement-zinc phosphate cement (Elite Cement 100), glass-ionomer cement (Fuji ionomer cement Type I) and three adhesive luting resin-Super Bond C & B, Panavia EX and a tentative luting resin were used as the material. We also examined the effect of three adherent surface treatments--50 microns Almina blasting, hydrofluoride etching, and sodium hydroxide etching--on the adhesive strength of the two adhesive resin, Panavia EX and the tentative luting resin, to CMP. The following conclusion were drawn: 1. The adhesive strength to CMP surface polished with #600 emery paper was higher in order of Super Bond C & B, a tentative luting resin, Panavia EX, Fuji ionomer cement Type I and Elite Cement 100. 2. Sodium hydroxide etching was the most effective for the adhesion between the adhesive luting resin and CMP among the above four adherent surface treatments. 3. Irrespective of the etching time, the stimulation of sodium hydroxide bath by supersonic waves gave an additional increase to the adhesive strength. 4. Under the stimulation by supersonic waves, increasing sodium hydroxide concentration tends to increase the adhesive strength, whereas it is little affected by the etching time within three minutes. The maximum was obtained with concentration in four or five normal (4 or 5 N) sodium hydroxide.

[Evaluation of the effectiveness of ion exchange on strengthening of dental porcelain].

This study evaluated the effectiveness of ion exchange on strengthening of porcelain used in metal ceramic. The paste for ion exchange was painted directly on the porcelain surface, dried, and heated at 450 degrees C in the porcelain furnace. The bending strength of porcelain test samples (6.5 x 1.5 x 20 mm), the compressive strength of porcelain-fused-to-metal (PFM) crowns, which were cemented on the metal dies of upper central incisor by an adhesive luting resin 'Panavia EX', and the color change of porcelain test samples (phi 10 x 4 mm) were measured before and after ion exchange. The conclusions were: 1. It was confirmed by X-ray analysis that the compressive stress layer generated by ion exchange for 30 minutes was 150-200 microns in thickness. However, further elongation of time of ion exchange elicited no significant argumentation in the strength. 2. 34 or 57 percent increase of the mean compressive strength of PFM crowns was observed after ion exchange for 30 minutes when the load was applied in the direction of 45 degrees lingually to the tooth long axis or of the tooth long axis, respectively. 3. No significant decrease was found when parts of the incisal edge or the lingual surface of PFM crowns which were strengthened by ion exchange for 30 minutes were reduced by 200 microns. 4. There was so slight difference in the color of dental porcelain before and after ion exchange that the influence seemed to be negligible in its clinical application.

[Effect of zinc iontophoresis on sealing of dentinal tubules by a new handy electolytic applicator].

The authors devised a new handy apparatus for iontophoresis in order to prevent pulp irritation caused by tooth reduction. This study was intended to observe the ability of the device. 8% zinc chloride solution was iontophoresed in the dentin of extracted human and bovine teeth by the device. Then, the dye and microbial invasion tests were carried out to investigate how effectively it could seal the dentinal tubules. The following conclusions were drawn: 1. By the zinc iontophoresis, the zinc briefly penetrated into the deep portion of the dentin. 2. The zinc iontophoresis by using the new device showed enough ability to decrease the coloring and bacterial invasion into the dentin, although all of the dentinal tubules were not sealed. 3. The tensile strengths of an adhesive luting resin 'Panavia EX' to the bovine dentin decreased significantly by the zinc iontophoresis. This phenomenon could be partly due to the lack of resin tag formation into the dentinal tubules.

[Influence of luting materials on marginal fitness and tensile strength of full veneer crowns. Comparison between conventional dental cements and adhesive luting resins].

The ability of luting materials to minimize the marginal opening during the cementing process is an important factor in preventing recurrent caries and gingival inflammatory response. The newly developed adhesive resins may have the possibility to achieve better retention and to minimize the microleakage at the tooth/restoration interface since they have the ability to adhere both dental alloy and tooth structure. The purpose of this study is to compare the marginal fit and tensile strength of full veneer crowns cemented by two adhesive resins--Panavia EX and Super-Bond C&B--with those cemented by three usual luting materials-zinc phosphate cement, polycarboxylate cement, and glass-ionomer cement--. The results were summarized as follows: 1. Panavia EX provided the most excellent marginal fit, whereas Super bond C&B the worst. 2. There was no significant difference in marginal fit between the shoulder and the chamfer configuration in every luting material examined. 3. The tensile strength of full veneer crowns cemented by the two resins was approximately 4-5 times as large as that cemented by other three luting materials. 4. The results show that Panavia EX was superior to other four materials in that it had the highest tensile strength and the least marginal opening at the crown/abutment interface.

Effects of the yeast extract components pyrroloquinoline quinone and aspartic acid on vitamin B12 production in Klebsiella pneumoniae IFO 13541.

The production of vitamin B12 from carbohydrates, peptone, casamino acid, etc., by intestinal bacteria was investigated. Klebsiella pneumoniae IFO 13541 was the most efficient strain for vitamin B12 production, which depended exclusively on the concentration of yeast extract added to the medium. A concentrated solution of yeast extract (1 ml) was chromatographed on a Sephadex G-25 column (1 x 180 cm) and eluted with H2O (eighty fractions of 3 ml each were collected). It was found that fractions in which bacterial growth was most prevalent also exhibited the highest amount of vitamin production. The effectiveness of yeast extract was shown by the participation of pyrroloquinoline quinone and aspartic acid in the growth stimulation and in the vitamin B12 production in this strain.

[Study on quantification of HBs-antibody by immunoradiometric assay].

Quantification of HBs-antibody assay was carried out using a commercial assay kit and standard solutions of HBs-antibody recognised as 1st reference preparation of hepatitis B immunoglobulin by WHO. Standard curve of HBs-antibody was drawn with the function of 3D-spline and the correlation factor was obtained as r = 0.999. Coefficient of intra-assay variance was 3.8% and that of inter-assay variance was 7.8%. Dilution tests showed satisfactory results in the range of 2-16 times. Correlation between value of cut-off indices and concentration of HBs-antibody was obtained as the formula of y = 2.599 x-3.894 (r = 0.992) and 2.1 of cut-off index corresponded to about 5 mIU/ml of HBs-antibody concentration.

[A case of thymoma associated with prostatic cancer].

A case of prostatic cancer treated with an LH-RH analogue depo-preparation was found to have a mediastinal thymoma which was later removed by surgery. The association of thymoma and prostatic cancer has not been documented in the world literature, and it remains to be elucidated whether the occurrence of the both tumors is a coincidence or etiologically inter-related.

[Clinical application of Sonolith 2000 type B on extracorporeal shock wave lithotripsy for upper urinary tract calculi].

This report describes the results of clinical trials of the second generation extracorporeal shock wave lithotriptor (Sonolith 2000 Type B) in patients with upper urinary tract stones. The studies were carried out on 101 cases at the Departments of Urology, Juntendo University School of Medicine, Kanto Teishin Hospital and General Daiyukai Hospital from Nov. 1987 to Jun. 1988. The location of stones were renal calyx and pelvis in 84 cases, ureteropelvic junction in 7 cases and upper ureter in 12 cases (2 of them had multiple stones at different levels). The average number of treatment per a patient was 1.25, and that of shock waves delivered per treatment was 1798. Ultrasound localization has been effective in all cases. The rate of destruction of the stones was 100% in the kidney, 66.7% in the upper ureter, with an overall average of 95.0%. On the X-ray film obtained six weeks after ESWL treatment, the stone free rate was 53.5%, and the effectiveness rate was 89.1%, including the cases of stone free and cases with fragments smaller than 5 mm. No serious adverse effect was observed, although there were mild transient hematuria in all cases and pyrexia (more than 38.0 degrees C) in 7 cases (6.9%). The procedure was performed safely in the majority of patients without anesthesia. In 10 cases, we applied anesthesia (epidural anesthesia in 3 cases, and local anesthesia in 7 cases) for the prevention of pain. It is concluded that ESWL treatment using Sonolith 2000 Type B is as effective as other types of shock wave lithotriptor previously applied to urolithiasis without serious clinical complication.

[Testicular endocrine function in patients with prostatic cancer receiving medical castration by long-term administration of LH-RH agonists].

Six patients with advanced prostatic cancer who had been treated by long-term administration of LH-RH agonistic preparations (Buserelin or Leupron) were tested for their pituitary-testicular endocrine functions. Serum levels of luteinizing hormone (LH), follicle-stimulating hormone (FSH), testosterone (T), prolactin (PRL), estradiol (E2) and dihydrotestosterone (DHT) were measured consecutively. In all medically castrated patients, serum levels of LH, FSH, T, DHT and E2 were suppressed and particularly serum T levels were below the castration level of 1.0 ng/ml. On the other hand, serum PRL levels were unchanged after the long-term treatment with the agonists. Serum LH and FSH levels failed to respond to LH-RH stimulation after the treatment, whereas serum T responded to stimulation by human chorionic gonadotropin (hCG) to various degrees. It was remarkable that, in 4 out of 6 medically castrated patients treated up to more than 3 years, serum T response levels above 1.0 ng/ml were noted. It is suggested that testicular endocrine function to secrete T and DHT in patients under treatment with long-term LH-RH agonist administration are still preserved in response to hCG stimulation.

[Studies on the pre-treatment of dental alloy for adhesive restorations. 4. Adhesive durability of adhesive resin to various dental alloys treated with composite plating].

In this study, the durability of adhesion between an adhesive resin (Panavia EX) and dental alloys (gold or Ni-Cr) were examined in regard to thermal cycling, immersion, either in water (70 degrees C or 100 degrees C) or in sodium chloride solutions (pH was 3, 7 and 9). An favourable adhesive strength, such as 450-500 kgf/cm2, was obtained even after 24 hours immersion in 37 degrees C water, when the surface pre-treatment of the alloy was done with either Sn- or composite (TMSAC/Sn or PVC/Sn)-plating. However, during the durability test, the adhesive strength has decreased to such on extent, that about 60% of early strength with Sn-plating and 80% with TMSAC/Sn composite plating. But, with PVC/Sn composite-plating, more than 90% of the early strength was maintained. In regard to the pH of the corrosive solution, no apparent difference was observed regarding the above mentioned adhesive characteristics.

[Gastric acid secretion and cellular immunity in patients with gastric cancer].

Three hundred and one gastric cancer patients have been examined preoperatively to investigate their gastric acid secretions after stimulation by tetragastrin, and serum gastrin stimulation by a test meal, as well as for skin reactions and an evaluation of their serum glycoproteins. The results have indicated that their gastric secretions and serum gastrin response were found to be reduced, according to the advancement of their cancer, and that the gastric acid secretion of patients with signet ring cell carcinoma was higher than that of patients with other histological carcinomas. Gastric acid secretions of patients with an ulcerated type of cancer, that is, type IIc and type III in an early cancer stage and type IIc of an advanced Borrmann V type, was higher than in patients with other types, and there were significant correlationships between gastric secretions and PHA skin test and gastric secretions and the IAP and the sialic acid.

Well-differentiated cholangiocarcinoma: diagnostic significance of morphologic and immunohistochemical parameters.

In order to provide useful criteria for the histologic diagnosis of well-differentiated cholangiocarcinoma, we reviewed 62 cases of this type. We assessed the incidences of various cytological as well as structural atypia, positive staining for carcinoembryonic antigen, secretory component, or carbohydrate antigen 19-9. We also assessed alterations in mucin production in comparison with noncancerous bile duct epithelium (48 cases). Using a computer-assisted multivariate analysis, three indicators--including nuclear size variation, formation of a second gland (distended intracytoplasmic lumina or focal cribriform pattern), and positive reaction in carcinoembryonic antigen--were shown to be the most important histologic parameters for the diagnosis of cholangiocarcinoma. One of the three indicators was occasionally positive in noncancerous bile ducts, but two or three indicators were only found in cholangiocarcinoma (p less than 0.001). The majority of the cases of cholangiocarcinoma were indeed positive for two or three of these criteria (54/62; 87%). Among other atypical changes, irregular nuclear configurations, mitosis, and prominent nucleoli showed a high specificity for cholangiocarcinoma. In addition to the major criteria, combined application of these factors can also be helpful for the actual histologic assessment.

[Pre-treatment of dental alloy for adhesive restorations. Part 3. Alkyl ammonium/Sn composite plating to various dental alloys].

The effect of alkyl ammonium salt/Sn composite plating on the adhesive characteristics of dental alloys was examined. The surface of dental alloys, on which such a composite plating was made, was analyzed by ESCA. The effect of the length of alkyl chain and its terminal atomic groups on the tensile adhesive strength to an adhesive resin was also examined. ESCA analysis revealed that both C and N atoms from the alkyl ammonium salts were certified on the surface of the dental alloys. Stable tensile adhesive strength, as high as 500 kg/cm2, was obtained when the number of C atoms of alkyl chain was 10 to 16. The alkyl ammonium salts containing such groups as hydroxyl, amino and vinyl at the terminal of the alkyl chain, were effective for increasing the tensile adhesive strength.

[Increased neutrophils in bronchoalveolar lavage fluid from a patient with developing adult respiratory distress syndrome].

A 16-year-old, 28-week pregnant woman was admitted to our hospital with multiple bone fractures caused by a traffic accident. She had massive blood transfusion because of anemia in her laboratory findings and ritodrine hydrochloride was administered because of the fear of threatened abortion. She developed a cough with bloody sputum on the 4th day after admission, and developed pulmonary insufficiency with PaO2 41.0 torr and presented bilateral diffuse infiltrates on chest roentgenograms on the next day. Swan-Ganz catheterization revealed normal pulmonary capillary wedge pressure and analysis of the lavage fluid from the patient showed an increase in the percentage of neutrophils (40.0%) and the existence of leukotriene B4 which is known to be the most potent chemokinetic and chemotactic agent for neutrophils. Her condition was considered to be permeability edema developing adult respiratory distress syndrome (ARDS) and 1 g/day of methylprednisolone was administered intravenously for 3 days, which brought about remarkable improvement of her respiratory failure. This report suggests that analysis of the lavage fluid may provide useful information for the early diagnosis of ARDS and the indications of corticosteroid treatment.

[Anesthetic management of combined kidney and pancreas transplantation].

Combined kidney and pancreas transplantation was performed for the first time in Japan in a 29-year-old diabetic male with end stage renal nephropathy. He previously required injections of 44 IU insulin daily, and the fasting plasma glucose concentration before the transplant was 722 mg/dl. Anesthesia was maintained with fentanyl, nitrous oxide (70%) and halothane (0 approximately 1.0%). A continuous infusion of intravenous regular insulin (2 approximately 8 IU/hr) was started after induction of anesthesia. After revascularization of the pancreatic graft, his plasma glucose concentration fell from 350 mg/dl to 130 mg/dl in one hour and a half. Blood glucose level was maintained between 200 to 300 mg/dl without insulin during anesthesia and thereafter. The grafted kidney and pancreas showed good function during this period. In this report, anesthetic problems in combined kidney and pancreas transplantation were discussed.
