Endophthalmitis following staphylococcal sepsis in renal failure patients.

Endophthalmitis occurred three months following completion of therapy for documented staphylococcal septicemia in two patients on long-term hemodialysis. The indolent course of the endophthalmitis, and its excellent response to systemic and subconjunctival antibiotics and subconjunctival and topical corticosteroid therapy, suggest the possibility that the acute fulminating clinical course of metastatic bacterial endophthalmitis may be modified in this population of patients. The reason for this modified clinical picture is probably the immune incompetence associated with uremia, which favors both the development of metastatic endophthalmitis as well as altering its clinical presentation. While funduscopic examination is suggested in all dialysis patients with eye complaints, this procedure becomes mandatory following episodes of sepsis.

C3b receptors on human glomeruli in vitro. Loss in culture.

C3b receptors, easily demonstrated on isolated human infant glomeruli, are lost as cells grow in culture. This suggests that epithelial cells are dedifferentiated or that the C3b receptor is not intrinsic to the epithelial cell.

Neuropsychological correlates of hypertension.

Twenty newly diagnosed, untreated hypertensive men (diastolic blood pressure greater than 105 mm Hg) and 20 normotensive controls were given a neuropsychological battery, including tests of generalized, more global functions (eg, reaction time and full-scale IQ) and of specific functions (eg, language and visual-spatial abilities) sensitive to focal damage. Tests of specific abilities yielded no differences between the two groups. In contrast, tests of general neuropsychological functioning revealed a deficit among hypertensives, who were significantly slower on the reaction time test and had a shorter span for digits in forward order. Results suggest that arterial hypertension is associated with impairment of vigilance and attention span. Future research will determine whether this impairment is associated with the diffuse pathological changes seen in the brain of hypertensive subjects, or with a more "functional" change (eg, reduced cerebral blood flow).

The influence of egg consumption on the serum cholesterol level in human subjects.

The influence of whole fresh eggs on the serum cholesterol level in men and women was studied independently in hospitalized patients in Sofia, Prague and Urbana-Champaign. The patients were fed two eggs or the equivalent of two eggs in a custard base or milk shake in addition to the foods that were consumed in their diet pattern. The serum cholesterol level was determined before and at periods varying from 5 hr to 54 days after the consumption of the eggs. The mixed fatty acid composition of the total lipids in the serum and the erythrocytes was also determined. In the majority of patients, the serum cholesterol level did not change significantly 5 hr after the consumption of 465 mg of cholesterol in an egg custard base or milk shake or after up to 54 days of continued consumption of two whole eggs per day. The serum cholesterol level of some subjects increased and others decreased at all three experimental sites. A comparison of the mixed fatty acid composition of the total serum lipids obtained from men and women who had received treatment for other reasons than cardiovascular disease with those that had been treated for cardiovascular disease indicated that the serum from both groups contained a substantial amount of polyunsaturated fatty acids. The lipids extracted from the red blood cells obtained from patients in Urbana-Champaign and Sofia did not differ significantly in linoleic and arachidonic acid content.

Polyanion and complement receptor of the glomerular epithelium. Relationship to pH.

Two markers on the surface of glomerular epithelial cells were studied on human and rat tissue. The glomerular polyanion is known to be a sialoglycoprotein present profusely on glomerular epithelial cell surfaces, and it is detected by binding with cationic dyes. This staining is lost after incubation in the pH range 3.8 to 4.4, yet retained at a more acidic or physiologic pH. A receptor for complement is also present on human glomerular epithelial cells. This receptor is also lost as a function of varying pH incubation and this loss parallels the pattern observed for glomerular polyanion. Neuraminidase treatment of renal tissue removes the sialic acid-dependent glomerular polyanion staining but preserves and stabilizes the complement receptor. Both functions are markers of the cell surface of glomerular epithelial cells and behave as peripheral membrane proteins.

Opsonic requirements for staphylococcal phagocytosis. Heterogeneity among strains.

Efficient phagocytosis of staphylococci by human neutrophilis is dependent on bacterial opsonization by serum factors. These factors include specific antibodies as well as components of the classical and alternative complement systems. In this study the opsonic requirements of three strains of S. aureus and three strains of S. epidermidis were investigated by incubating [3H]thymidine-labelled bacteria in sera with different opsonic activities and measuring rates of phagocytosis by human neutrophils. Opsonization of S. aureus Cowan I and 502 A depended primarily on activation of the classical complement pathway. Effective opsonization occurred in the absence of immunoglobulin but not in the absence of complement. A protein A deficient mutant of S. aureus Cowan I was poorly opsonized in the absence of IgG, however. S. aureus Wood 46 and two strains of S. epidermidis were opsonized primarily through the alternative complement pathway and depended on the presence of serum IgG. A third S. epidermidis strain was efficiently opsonized in heat-inactivated serum without complement activity. Thus, a heterogeneity of opsonic requirements was found among staphylococcal strains. It is proposed that cell wall protein A may be an important determinant of this heterogeneity.

Breakdown products of C3 and factor B in hemolytic-uremic syndrome.

Serum concentrations of Clq, C4, C3, and Factor B but not properdin were significantly decreased in patients with HUS compared to values in normal control subjects (p value less than 0.01). Sera from 13 HUS patients obtained early after the onset of the disease showed breakdown products of Factor B (Ba, Bb) by immunoelectrophoresis; 12 of these sera showed C3 breakdown products (C3c, C3d). Sera from seven patients studied 1 month to 3 years later no longer demonstrated any breakdown products of Factor B or C3. These data suggest that the complemented system is activate in HUS. The concept that immunological mechanisms play a major role in this disease is additionally supported by the occurrence of IgM, C3, and fibrin in glomeruli and renal vessels.

Response to delayed auditory feedback in patients with hemispheric lesions.

The effect of delayed auditory feedback (DAF) on the performance of verbal and nonverbal tasks at two levels of difficulty by patients with left or right hemispheric lesions and non brain-damaged controls was compared. All subjects showed DAF effects, as measured by increases in response duration and intensity. While controls showed an increase in DAF effects as a function of increased task difficulty, the brain-damaged patients failed to show such an increase on tasks on which they were presumably impaired (verbal tasks for lefts, nonverbal for rights). Findings suggest a clear dissociation between DAF effects in unilateraly lesioned patients due to the nature of the stimulus material and its difficulty level.
