There is an unmet need for a reliable method of evaluating disorders of mood and affect in developmentally disabled children and adolescents. Such a measure is required for both accurate diagnosis and treatment monitoring in this population. An extensive review of existing assessment techniques confirms that: (a) current techniques for the evaluation of emotional disorders in cognitively normal individuals are inappropriate for most children with developmental disabilities; and (b) current instruments designed for the assessment of developmentally disabled children pay inadequate attention to affective symptoms. In this paper, the preliminary version of a new instrument, the "Emotional Disorders Rating Scale for Developmental Disabilities" (EDRS-DD), designed to evaluate mood and affect in children and adolescents with developmental disabilities, is presented. A pilot study indicates that interrater agreement is good.
Bulimia was diagnosed in a pair of monozygotic twins and was manifested in both sisters concurrently through their adolescent years. After a later separation only one continued to manifest the disorder with additional associated features of the bulimic syndrome. The case is presented and suggestions regarding the roles of environmental and genetic factors in the development and course of the disorder are discussed.
Although high rates of psychopathology in children and adolescents suffering from temporal lobe epilepsy (TLE) have been reported in the literature, the subject remains controversial. In this investigation we made a psychiatric study of 26 adolescents with TLE using a detailed structure interview and rigorous diagnostic criteria. These were compared to 26 matched controls suffering from chronic bronchial asthma (BA). In neither group was there a high rate of specific psychiatric illness, although many adolescents in both samples showed moderate to severe depression. We conclude that TLE may not be a specific cause of psychopathology in adolescence, and that the depression might result from the suffering involved in chronic illness.

Two cases of attempted suicide by insulin overdose in adolescent girls with insulin-dependent diabetes mellitus were observed. It is possible that suicidal insulin overdose in adolescents is more common than reports suggest and that it may often be unrecognized.
Bulimia nervosa is an eating disorder characterized by secretive binge eating and purging with induced vomiting, laxatives, and diuretics. The disorder primarily afflicts young white women between 18-35 years of age. We report the case of a 30-year-old pregnant woman with a 17-year history of bulimia that involved up to six episodes of binging and purging daily. A multidisciplinary approach enabled outpatient management throughout gestation. The pregnancy resulted in the delivery of a normal 3000-g female infant at term. The ramifications for maternal and fetal well-being and the goals of therapeutic management are discussed.

A combination of techniques, based mainly on the stress inoculation training approach, was used to treat severe self-mutilating behavior in a 32-year-old woman who for 15 years had manifested different kinds of this type of behavior. In 18 therapy sessions over 6 weeks in a psychiatric inpatient setting she achieved complete control of the self-mutilating behavior. After a year of follow-up she was completely symptom free and had made additional social and personal gains.

A 26-year-old male was admitted to the emergency ward, presenting what seemed to be an acute dystonic crisis. The patient was found to be an oral and parenteral abuser of Valium (diazepam). He used to feign the dystonic syndrome in order to get an i.v. shot of the drug.
