Observational measurement of symptoms responsive to treatment of major depressive disorder in children and adolescents.

Observational methods were used to determine whether depressive symptoms in 20 inpatients with major depressive disorder could be observed to change during the course of treatment with antidepressant medication. Four consecutive weekly observation sessions were done using the Emotional Disorders Rating Scale, which collects information about symptoms of depression, mania, anxiety, hostility, and irritability. Only those scales indexing depressive symptoms evidenced change. Significant change was observed between the third and fourth weeks of hospitalization, thereby replicating the findings reported in the adult literature regarding depressive symptoms' responsiveness to antidepressants.

Behavioral profile and social competence in temporal lobe epilepsy of adolescence.

A group of 26 adolescents with temporal lobe epilepsy was compared with a matched control group of 26 adolescents with chronic bronchial asthma as well as with a group of 90 healthy adolescents, using the social competence measure and the behavior profile of the Child Behavior Checklist. Both chronically ill groups exhibited more social and behavioral problems than the healthy adolescents but were similar to each other on the social and behavioral profiles. The one area in which the temporal lobe epilepsy and asthmatic groups differed from one another was that of "schizoid" psychopathology in male subjects.

The Teen-Addiction Severity Index: rationale and reliability.

There is an urgent need for a reliable method of evaluating the severity of adolescent chemical abuse and problems related to chemical abuse. The lack of an appropriate rating scale to fill this objective hampers the design and the assessment of objective treatment outcome and follow-up of adolescent chemical abusers. The Teen-Addiction Severity Index (T-ASI) is a structured interview which was developed to assess the seven following domains: chemical use, school status, employment-support status, family relationships, peer-social relationships, legal status, and psychiatric status. This paper discusses the rationale for the design of the T-ASI and presents a preliminary study indicating satisfactory interrater reliability of the rating scale.

An observationally based rating scale for affective symptomatology in child psychiatry.

There is growing dissatisfaction with current methods for rating affective symptoms in children. We report findings from a preliminary psychometric study of an alternative approach, that of direct observational ratings. The Emotional Disorders Rating Scale (EDRS) is an observation-based instrument containing 59 items divided into eight subscales. The results of this study indicate that measurement of nonverbal components of affective symptoms in children is feasible. Interrater reliability and internal consistency of the EDRS subscales were high. The EDRS also has potential as a measurement of state-related changes in affective behavior and as a technique for examining treatment response.

Drug abuse severity in adolescents is associated with magnitude of deviation in temperament traits.

Substance abusing adolescents were discriminated from normal controls on seven of the nine scales comprising the revised Dimensions of Temperament Scales. However, upon aggregating the scales into orthogonal dimensions using factor analysis, it was found that only activity level was associated with drug use severity. This behavioural trait was correlated with seven of 10 scales comprising the Drug Use Screening Inventory, including severity of substance use, behavior problems, psychiatric disorder, work, peer relationships and leisure activities.

Insulin misuse: a review of an overlooked psychiatric problem.

A number of forms of insulin misuse, other than the usual noncompliance, have been reported in the literature on the treatment of diabetes mellitus. These include attempted and completed suicide, factitious hypoglycemia, Munchausen syndrome by proxy, and the use of insulin by substance abusers. Such misuse has involved diabetic patients, their family members and medical staff, as well as others. The reports of suicide attempts reveal an equal distribution of misuse between the sexes (rather than the expected preponderance of females), underrepresentation of adolescents, and a high rate of recurrence. Recommendations for treatment are discussed for clinicians and investigators regarding this overlooked problem.

Adolescent chemical use and dependence: current issues in epidemiology, treatment and prevention.

This review provides a pragmatic and clinical orientation to current issues in adolescent chemical dependence in addition to exploring legitimate and relevant unanswered questions about chemical dependence. The lack of research on adolescent chemical dependence is addressed in light of the updated epidemiology. Patterns of use, abuse and dependence among adolescents are reviewed. The article examines treatment issues as well as prevention strategies among children and adolescents. Future clinically oriented research directions are suggested to promote further investigation into these areas.

Comorbidity of substance abuse and other psychiatric disorders in adolescents.

Psychopathology coexisting with substance abuse in adolescents is often encountered in a variety of clinical settings. Research findings suggest a major role for substance use in the etiology and prognosis of psychiatric disorders such as affective disorders, conduct disorder and antisocial personality disorder, attention-deficit hyperactivity disorder, and anxiety disorders. Psychiatric disorders also appear to have an important role in the etiology of and vulnerability to substance use problems in adolescents. Although the comorbidity of substance abuse and other psychiatric disorders in adolescents is recognized as an important factor in the treatment of adolescents, further research is needed to establish its prevalence, genetics, and clinical implications.
