[Radiation therapy of esophageal cancer--radical radiotherapy and radiotherapy in combination with surgery].

The role of radical and palliative radiotherapy, and radiotherapy combined with surgery are discussed. Surgical resection and radiation therapy have been applied to curative treatment of carcinoma of esophagus. Despite of recent advances of surgery, the results are still unfavorable. Radical irradiation was applied to attempt at long term control of the local and regional disease despite dismal cure rate: doses in the range of 6000 to 7000 rad were required. Palliative irradiation is also achieved significant improvement, and dose range of 4000 to 5000 rad were required. The lesions of cervical or upper one third of the esophagus showed better prognosis by radiotherapy than the lower lesions. Prognosis was related to the size of the lesion and the depth of penetration of the esophageal wall. To improve the treated result, pre- or post operative radiotherapy was tried in combination with surgery. Preoperative irradiation has improved the surgical resectability and treatment result, which was contributed by the recent advances of surgical technique which made extended operation possible. Therefore, the role of preoperative irradiation should be re-evaluated. Postoperative prophylactic irradiation was used to control the possible improvements of mediastimun and/or neck nodes. The treatment results were better in the group of postoperative irradiation than in the group without irradiation.

Metabolic fate of 2,2-dimethylbutyryl moiety of simvastatin in rats: identification of metabolites by gas chromatography/mass spectrometry.

Metabolic pathways of simvastatin (MK-733), a lactone prodrug of an inhibitor of HMG-CoA reductase, were elucidated in male rats, using the [14C]-labelled compound. Evidence has been obtained for hydrolysis of simvastatin and its metabolites at their 2,2-dimethylbutyryl moieties. Metabolites identified in plasma were 2,2-dimethylbutyric acid (DMB), 2,2-dimethyl-3-hydroxybutyric acid (DMHB) and an open chain hydroxy acid of simvastatin: metabolites identified in urine were DMHB, a glucuronide and the glycine conjugate of DMB. They were characterized by gas chromatography/electron impact and chemical ionization mass spectrometry as phenacyl or pertrimethylsilylated derivatives. The structures of the metabolites and the aglycone of the glucuronide were confirmed as phenacyl esters by comparison of their chromatographic data and mass spectra with those of the phenacyl derivatives of authentic compounds.

Brain involvement in Hodgkin's disease: case reports and review of the literature.

Hodgkin's disease involving in the central nervous system is extremely rare. It usually spreads contiguously, as visceral involvement is generally thought to occur secondary to involved adjacent lymph nodes. We report three such cases found in our institution in the last two decades. Based upon our limited experience together with reported data, whole brain irradiation combined with systemic chemotherapy remains the treatment of choice for these lesions.

A case report of Paget's disease of the breast treated with radiotherapy alone.

Paget's disease of the breast is a rare form of breast cancer. Patients with nipple-areolar disease alone and no palpable mass have an excellent prognosis. Although there is some argument about the treatment of this type of Paget's disease, only two reports have been published on the role of conservative treatment. This report discusses a case of limited Paget's disease of the nipple and areola of the breast which was treated by radiation alone without limited surgery.

Interstitial irradiation for carcinoma of tongue complicated by chronic renal failure undergoing hemodialysis--a case report.

There has been some controversy concerning dose-time correction in continuous irradiation at low dose rates. We present the results of carcinoma of the tongue in a patient complicated by chronic renal failure, for which he was undergoing hemodialysis three times a week. This patient was treated with a single implant but with a doubling of 137Cs needles for double strength. The actual dose was given in a shorter treatment time than recommended, with the dose-time adjustment following the Paterson-Parker system. The patient has been alive and well for eight years.
