[Acute appendicitis. Experimental model in rabbits].

The evolving phases of acute appendicitis were studied experimentally. Sixty female rabbits (Oryctogalus cuniculus) of New Zealand lineage weighing about 2510 to 3040 g were divided in two groups: a control group and experimental group. The experimental group was divided into three subgroups for observation after 12, 24 and 48 hours of the operation, that consisted on a 4-0 polypropylene circular suture at 8 cm from the distal part of the cecal appendix. The control group was sham operated. The macroscopic exam (increase of the appendix volume, necrosis, perfuration, adherence and secretion in the abdominal cavity) and the microscopic finding showed a progression in the anatomopathological alterations. There was a close relationship between the histopathological findings and time after the appendiceal obstruction. We conclude that the method causes acute appendicitis and that the anathomo pathological alterations depends on the time elapsed between the operation and the postoperation findings.

[Study of mechanisms and factors related to sexual abuse in female children and adolescents]

OBJECTIVE: To study the frequency of sexual abuse in children and adolescents and its related factors. METHODS: Retrospective analysis of 617 cases of sexual abuse, assisted between July 1994 and August 1999 at the Womeńs Health Reference Center, divided into two groups: 71 children (age <10 years), and 546 adolescents (age > or =10 and <20 years). The analyzed variables were: sexual crime; embarrassment; presumption of violence; characteristics and number of abusers; the victimacute;s situation at the moment of the crime; and occurrence of physical traumas. Epi Info 6 software was utilized to gather data, and the results were analyzed by the chi-square test. RESULTS: Significant statistic results were found. 90.8% of the adolescents were victims of rape, and 46.5% of the children suffered sexual assault. The presupposed violence (PV) was more frequent in the group of children (63.4%) and a serious threat in the group of adolescents (63.2%). Innocentia consilli was exclusive PV among 100% of children and 59.5% of adolescents. 84.5% of children were molested by identifiable abusers, most frequently family members. Perpetrators were unknown in 72.3% of the cases of adolescent abuse. 42.3% of sexual abuse of children occurred in their homes, and 28.2% at the abusers. Adolescents were approached during daily activities (34.8%) and on their way to work or school (28.4%). Most patients did not have genital or extragenital trauma. CONCLUSIONS: Children suffered sexual assault, perpetrated by a known abuser through presupposed violence in private places. Adolescents were raped by unknown individuals, under serious threat, in nondomestic places.

Sex variation at the time of sonographic appearance of the proximal tibial epiphyseal ossification center.

The time of sonographic appearance of the proximal tibial epiphyseal ossification center and its sexual variation was assessed in 659 normal pregnancies. Proximal tibial center appears between 34-37 weeks gestation. In female fetuses, it appears between 34-35 weeks and in males, between 35-36 weeks (P < 0.001). There was a tendency of female proximal tibial center appearance to precede male appearance, by approximately one week. This difference, however, didn't show statistical significance.

Iron, copper and alpha 1-antitrypsin deficiency in the liver of cirrhotic and schistosomotic patients.

While a number of studies investigated iron and copper storage or alpha 1-antitrypsin (A1AT) deficiency in the liver of patients with cirrhosis, we did not find any similar study in schistosomotic patients reported in literature. We investigated the storage of both metals and the A1AT deficiency in the liver of 72 cirrhotic and 27 schistosomotic patients (5 with the hepatointestinal and 22 with the hepatoesplenic form of the disease). Forty-four patients with cirrhosis were also alcoholic, and 28 were not. Iron storage was detected in 23 patients with cirrhosis (31.9%); among these 16 (36.3%) were alcoholic and 7 (25.0%) non-alcoholic (the difference was not statistically significant). Thirteen (56.5%), 5 (21.7%) and 5 (21.7%) patients presented I-grade, II-grade, and III-grade iron storage, respectively. Copperstorage was detected in 24 cirrhotic patients (33.3%), 15 being alcoholic (34.0%) in contrast with 9 non-alcoholic patients (32.1%), a statistically non-significant difference. A1AT deficiency was observed in 2 patients with cirrhosis (2.8%), one with history of alcoholism. HBsAg and HBcAg in hepatic tissue were detected in 5 cirrhotic patients (6.9%), three of them with a history of alcoholism. Iron and copper storage and A1AT deficiency were observed in 3 patients with cirrhosis (12.5%), while iron storage and A1AT deficiency were found in 2 additional patients with cirrhosis (2.8%). The authors underline that neither iron nor copper storage nor A1AT deficiency was found in any schistosomotic patient. The authors discuss the possible importance of these data.

[Influence of exposure time to silica and smoking on pulmonary function of workers in the ceramic and stone quarry industry].

The aim of this study was to analyze the influence of the numbers of years a worker is exposed to silica dust and the smoking habit upon his lung function. In order to be included in this study the worker should have been exposed to silica dust for a minimum of ten years and give his oral consent to have his lung function studied and a chest X-ray done. The study was done with individuals who claimed for compensation for silicosis to the Brazilian National Institute of Social Security during the period from 1973 to 1981, in Jundiaí city, São Paulo state. The study groups were made up of 137 workers: Group I, exposure between 10 and 20 years (44 workers) and Group II, exposure over 20 years (93 workers); 59 (42.4%) workers were cigarette smokers, 53 (38.1%) non-smokers and 27 (19.4%) former smokers. Lung function was accomplished through spirometry (FVC, FEV1, FEV1/FVC and FEF 25-75%) flow volume curves breathing air and helium-oxygen mixture (Vmax 50%, Vmax 25% and VisoV). No difference was found between the two groups concerning radiological chest nodules, spirometry, flow volume curves and smoked packyears; smokers had a worse lung function when compared to the non smokers of the same group. These alterations in Group I smokers were predominantly in the peripheral airways (Vmax 25%--p less than 0.05) whereas in those smokers of Group II large airways were also involved (VisoV, FEV1, FEV1/FVC--p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

[Diagnosis of cervicovaginal infection by human papillomavirus. Value of colposcopy, cytology and histopathology as methods of diagnosis].

The authors evaluated 476 patients to determine the accuracy of colposcopy, cytology, and histopathology as diagnostic methods for cervical and vaginal HPV infections. Results were treated by statistical analysis which showed significant disagreement among the methods tested. Colposcopy was positive in 87.8% of the cases. Abnormal transformation zone was the most frequent finding in the cervix, acetowhite epithelium being its main representative. Condylomatous vaginitis was the most frequent feature in the vaginal surface. Cytology was positive in 76.9% of the cases. To increase sensitivity of this method, the authors suggest that not only koilocytosis is important, but other major aspects are also important, such as minimal koilocytosis, dyskeratosis, karyopyknosis, binucleation, and multinucleation. Histopathology was positive in 81.9% of the cases. In conclusion, the three diagnostic methods studied presented statistical disagreement, however, they are supplementary and efficient.

[Behavior of cystic tumor of the ovary after wait-and-see management and hormonal treatment].

The authors have analyzed the results of the treatment of benign ovarian cysts with 30-49mm (group I) or more than 50mm in diameter (group II). 40 patients were treated with progestogen, 41 with contraceptive pill, and another 48 patients were only observed without any treatment (control group). Ovarian cyst regression was observed in 84.6% of patients in group I, and in 83.3% of patients in group II; 85.4% of patients in group I and 88.9% of patients in group II were treated with contraceptive pill; 92.5% of patients in group I and 93.1% of patients in group II were treated with progestogen. The authors have concluded that hormone treatment is not necessary to induce functional regression of ovarian cysts.

[Morphologic and morphometric aspects of the uterine tube epithelium during the menstrual cycle].

PURPOSE: Under light microscopy, the authors observed the morphologic and morphometric features of the human uterine tube during the menstrual cycle. TYPE OF STUDY: Experimental, prospective study. SITE: Escola Paulista de Medicina. PATIENTS: Specimens obtained from ten eumenorrheic, multigesta patients, ages ranging from 22 to 35 years. INTERVENTION: They were all submitted to salpingectomy because they showed a high reproductive risk. They were operated during the proliferating and secretory phase of the menstrual cycle. The isthmus, ampullae and infundibulum were studied under light microscopy. MEASUREMENTS AND RESULTS: The authors observed that the epithelium of the human tube has a single layer of cylindrical and ciliated cells. Three kinds of cells were present: ciliated, secretory, and non-differentiated. Height of the epithelium was uniform in the secretory phase, but not in the isthmus and ampullae. Images of mitosis were rarely seen. In the isthmus, the number of secretory cells predominated in both phases of the cycle; in the ampullae, the number of ciliated cells was increased in the proliferation phase, while secretory cells were proeminent in the lutheal phase. In the infundibulum and fimbrae, ciliated cells were increased. Morphometric features of human uterine epithelium were studied during the menstrual cycle and showed: 1) the height of the epithelium in the ampullae was significantly higher in the proliferating phase; 2) the number of secretory cells was significantly higher in the isthmus, while in the infundibulum ciliated cells were increased in number in both phases of the cycle. The number of ciliated cells in the ampullae was significantly higher in the proliferating phase while secretory cells predominated in the secretory phase; 3) The nuclear volume of ciliated and secretory cells is not statistically significant in all regions during the two phases of the cycle. CONCLUSIONS: The uterine tube epithelium shows cyclic modifications which are probably associated to its function in reproductive physiology.

Serum type III procollagen peptides in patients with hepatointestinal and compensated hepatoesplenic schistosomiasis forms.

The Serum Type III Procollagen Peptides (SIIIPP) were determined in 35 individuals: 25 untreated schistosomotics: 16 with hepatointestinal (HI) and 9 with the compensated hepatosplenic (CHE) forms and a control group (C) consisted of 10 healthy volunteers. Kits of radioimmunoassay were performed for SIIIPP dosage. It was searched whether there was relationship between the SIIIPP and the serum levels of alanine aminotransferase (ALAT), aspartate aminotransferase (ASAT), alkaline phosphatase (AP) and gamma glutamyltranspeptidase (GGTP). The mean values of SIIIPP in the forms of HI (13.0 ng/ml) and CHE (17.0 ng/ml) were significantly higher than controls (9.0 ng/ml) (p less than 0.05). No significant difference was observed in SIIIP values between the HI and CHE patient groups, and between SIIIPP and ALAT, ASAT, AP and GGTP serum levels.

Delayed type hypersensitivity induced by LPF from Bordetella pertussis.

Lymphocytosis Promoting Factor (LPF) obtained from Bordetella pertussis, has the capacity to induce peripheral lymphocytosis in mice and blastogenesis of human lymphocytes in vitro. When this Factor is injected intracutaneously in animals and in humans, it was shown to induce local inflammatory reactions with histological, morphological and chronological characteristics of delayed hypersensitivity. LPF and conventional microbial antigens were tested intracutaneously in 35 patients with carcinoma undergoing immunotherapy and in 15 normal individuals. The results showed that the cutaneous reactions induced by LPF were similar to those of the conventional antigens routinely used to asses cell-mediated immunity. LPF via intracutaneous route did not show any adverse reactions. Histological studies were carried out in mice receiving intracutaneous injections of LPF. The immunological mechanisms involved in the induction of delayed cutaneous hypersensitivity by LPF are discussed.

Quantitation of immunoglobulins in patients with schistosomiasis mansoni under chemotherapy.

The humoral immune response of patients with intestinal and hepato-intestinal form of Mansoni's schistosomiasis treated with a single dose of Oxamniquine was evaluated by quantitation of IgG, IgM, IgA and IgE. A constant and significant increase of IgE was observed in the majority of patients after treatment. Augmentation of serum IgE after treatment was variable, but a 3 fold increase was observed. IgG increased slightly after treatment in 6 patients, and IgM increased in 5 patients under the same conditions. However, the difference between the levels of IgG and IgM before and after treatment was not significant. IgA levels were within the normal range, before and after treatment.

[Comparison between CSF samples from AIDS and non AIDS patients with neurocryptococcosis].

Neurocryptococcosis was a rare nervous system infection. With the rising number of patients with AIDS it became a very frequent disease. This infection is supposed to infect patients with some kind of immunodeficiency and the CSF alterations often simulate tuberculous meningitis. The purpose of this research was to compare the CSF changes in AIDS and non-AIDS patients with meningoencephalitis caused by Cr. neoformans. There were analysed 41 CSF samples from non-AIDS patients with neurocryptococcosis and 23 CSF samples from AIDS patients with neurocryptococcosis. The results of this research allowed to conclude that the inflammatory changes in the CSF from AIDS patients showed a lower intensity compared to those non-AIDS patients. These results showed as well, that the CSF samples from non-AIDS patients always revealed some changes besides the yeast cells. In some samples of AIDS patients, however the unique change was the presence of the yeast. It was demonstrated also, that the presence of Cr. neoformans in CSF, not accompanied by any other change, may suggest that is a patient with AIDS. In non-AIDS patients CSF alterations often simulates tuberculous meningitis. However these alterations were rare in AIDS patients. The yeast cells were more numerous in CSF samples from AIDS patients than in those from non-AIDS patients.

[Surgical treatment of stress urinary incontinence: analysis of the results by abdominal or vaginal pathways].

BACKGROUND: The postoperative results of 367 surgical interventions for the correction of urinary stress incontinence were analyzed in terms of time of onset and occurrence of relapses. MATERIAL AND METHOD: The cases were assigned to four groups: A = 206 cases of Kelly-Kennedy surgery (56.1%); B = 37 cases of Kelly-Kennedy surgery associated with vaginal hysterectomy (10.1%); C = 95 cases of Burch surgery (25.9%), and finally, D = 29 cases of Burch surgery associated with total abdominal hysterectomy (7.9%). RESULTS: Postoperative follow-up ranged from 1 to 194 months, with a mean of 31 months. The incidence of relapses was 20.7% in group D, and 10.8% in group B. In 68.7% of the surgical interventions by vaginal route and in 82.3% of interventions by abdominal route, the relapses occurred within less than three years. CONCLUSION: The results were not different in the groups. The relapses occurred within less than three years in most cases.

[A comparative study of the sodium, potassium, urea, creatinine, and uric acid concentrations in the human amniotic fluid between weeks 15-20 and 38-42].

UNLABELLED: The amniotic fluid physiology is a dynamic process involving maternal and fetal compartments and depends on gestational age. PURPOSE: To analyze the concentration of sodium, potassium, urea, creatinine, and uric acid in the amniotic fluid of normal pregnant women in the second and third trimester. Also, to evaluate the influence of maternal age, race, parity, and fetal sex on those elements. METHOD: Fifty samples obtained by genetic amniocentesis (15-20 weeks, group I) and fifty obtained by elective cesarean section (38-42 weeks, group II) were analyzed. According to the variables we used the following statistical tests: Analysis of variance; Test t Student: Chi-square test; Mann-Whitney test (a 0.05 pounds). RESULTS: In group II, urea, creatinine and uric acid levels were significantly higher than in group I. The sodium level was significantly lower in group II compared with group I. The potassium concentration did not show any significant difference in both groups. There was no significant interference of maternal age, race, parity and fetal sex in any of the five studied variables. CONCLUSION: These findings were emphasized for prenatal diagnosis purposes and analysis of renal function. The authors suggest future comparisons of obtained "normal" data with pathological situations.
