Pathological considerations of reoperative vascular patients.

Since vascular disease is always progressive and a perfect vascular prosthesis has yet to be developed, postoperative complications are almost inevitable. In this paper, case histories of those who had to have a second operation or, in other words, re-operated patients, have been examined to ascertain the current problems in vascular surgery. Of 176 vascular reconstructive operations performed between January 1, 1980, and December 31, 1986, 29 re-operations were performed on 19 patients (mean age: 64 years; 15: male). The incidence of late graft failures was 8.4% and, of these, anastomotic aneurysms seemed to be the most serious complication (3.1% incidence rate). Late graft failures included intimal hyperplasia, occurring within two years in five cases, and four cases of progressing atherosclerosis, which appeared three years after the initial operation. In all cases of anastomotic aneurysm, arterial wall failure, possibly combined with the changing of implanted grafts, was considered to be related to the false aneurysmal formation. Knitted Dacron demonstrated susceptibility to atherosclerotic progression, whereas the major fault of polytetrafluoroethylene (PTFE) grafts was the insufficiency of the anastomotic diameter. In conclusion, it was revealed that many factors can provoke late graft failure. Improvement of long-term patency seems to be achieved by a more increased understanding of the pathological meaning of these factors, along with the proper application of medical techniques suited to the causes of vascular obstruction.

[Determination of various tumor markers, with special reference to neuron-specific enolase in lung cancer].

Serum neuron-specific enolase (NSE) was measured in 23 patients with small cell lung cancer (SCLC) and 184 patients with non-small cell lung cancer (non-SCLC), both of which were untreated. Increased levels of serum NSE were observed in 82.6% (19/23) of SCLC, whereas 9.8% (18/184) of non-SCLC had positive results showing an overall positive rate of 17.9% (37/207) in lung cancer cases. In addition, the elevation of serum NSE levels in non-SCLC patients seemed to suggest poor prognosis. Elevated serum NSE levels returned to normal with either surgical resection of the tumor or response to chemotherapy, after which serum NSE levels were again raised to levels higher than the previous ones in cases of relapse or progression. The evaluation of serum NSE may be a useful marker for both diagnosis and monitoring of responsiveness to therapy as well as for recognition of relapse and progression in SCLC. Identification of NSE as assessed by immunohistochemical procedure employing the ABC method on formalin-fixed paraffin-embedded tissue sections in lung cancer cases of each histological type, showed that some materials from non-SCLC cases were positively stained despite the presence of normal serum NSE levels, and did not always parallel the serum levels. Among other various tumor markers determined, serum CA 19-9 had a relatively high positive rate of 38.2% (42/110) in adenocarcinoma of the lung.

[Clinical evaluation of serum carbohydrate antigen 19-9 in carcinoma of the lung--a comparison with carcinoembryonic antigen].

Serum levels of carbohydrate antigen 19-9 (CA 19-9) were measured in 235 untreated patients with lung cancer, 20 patients with benign pulmonary disease and 39 healthy controls. In almost all these patients, carcinoembryonic antigen (CEA) was determined at the same time. The positivity of CA 19-9 in lung cancer patients was significantly higher than in those with benign pulmonary disease (30.2% vs. 5%: P less than 0.05). With regard to histological types of lung cancer cases, the positivity of CA 19-9 in adenocarcinoma cases was higher than that in cases of squamous cell carcinoma (39.6% vs. 16.4% : P less than 0.01). With regard to clinical stages of lung cancer positivity of CA 19-9 in stage IV (37.9% vs. 10.3% : P less than 0.01) or stage III (30.3% vs. 10.3% : P less than 0.05) was significantly higher than in stage I. Sensitivity, specificity and accuracy in the detection of lung cancer were 31%, 95%, and 36% for CA 19-9, and 46%, 80% and 49% for CEA, respectively. Combined evaluation with CA 19-9 and CEA gave the highest specificity (98%) when both were measured, and the highest sensitivity (55%) when at least one was measured. Combined assessment of CA 19-9 and CEA improved the measurement of each one in the detection of lung cancer.

[Magnetic resonance imaging (MRI) of pituitary adenomas].

MRI of 20 patients, 17-82 years old, with pituitary adenomas confirmed histopathologically, and 30 normal patients without pituitary dysfunction were reviewed. The studies were performed with a 0.5 Tesla MR scanner with a slice thickness of 10 mm. Inversion recovery sequences were employed as T 1-weighted examination, with repetition time (TR) of 2100-2500 msec, an inversion time (TI) of 600 msec and an echo time (TE) of 40 msec. The T 2-weighted examination had a TR of 1800-2500 msec and a TE of 120 msec. T 1-weighted images were obtained in all cases and T 2-weighted images in 14 cases. Spin echo images with a TR of 600-1000 msec and a TE of 40 msec (SE 40/600-1000) were also obtained in 14 cases. On T 1-weighted images, 20 adenomas were classified into six groups, according to their signal intensities; marked low intensity (1), low intensity (1), isointensity (11), high intensity (3), marked high intensity (1) and mixed intensity (3). On T 2-weighted images, 14 adenomas were classified into five groups; low intensity (1), isointensity (2), high intensity (4), marked high intensity (6) and mixed intensity (1). On SE 40/600-1000 images, 14 adenomas were also classified into four groups; low intensity (8), high intensity (2) and mixed intensity (3). Two adenomas with recent intratumoral hemorrhage had marked high intensity on both T1 and T2-weighted images. SE 40/600-1000 images were useful in evaluating the size and the extent of the visual pathway was best appreciated on IR images. Comparison between normal and involved cavernous sinus by adenomas was made.

Qualitative and quantitative analyses of Müller's muscle in dysthyroid ophthalmopathy.

A portion of the Müller's muscle of the superior eyelid was obtained from 3 men and 6 women with dysthyroid ophthalmopathy, aged from 19 to 58 years, at the time of surgery to treat excessive lid retraction. Disturbance of ocular motility was seen in 7 out of the 9 cases. Electron microscopy of the excised portion of Müller's muscle showed that the muscles were classifiable into three types, 1) cells with normal appearance, 2) dark cells and 3) clear cells: the percentages of these cells averaged 35.7% (30.9-42.1%), 39.4% (24.8-56.3%) and 24.9% (12.8-40.0%), respectively. A quantitative analysis of the cell shape revealed that the dark cells were more irregular and the clear cells more rounded than the normal cells. The dark cells had a high electron density with a dense distribution of actin-like filaments, and the clear cells showed a low electron density with irregular cytoplasmic distribution. It was thought that the dark cells represented contractile changes and the clear cells represented degenerative changes. The dark cells appeared to decrease and the clear cells appeared to increase, as the exophthalmos increased. In 2 cases with no ocular motility disturbance, the percentage of the dark cells was higher and that of the clear cells lower than in the other cases with ocular motility disturbance. It is suggested that the contraction of the Müller's muscle contributes to lid retraction in dysthyroid ophthalmopathy and the muscles undergo degenerative changes as the ophthalmopathy progresses.

[Relationship between clinical efficiency of cefoperazone and the value of area under the time concentration curve in infectious diseases].

Clinical efficiency and side effects of cefoperazone (CPZ) against 40 infectious diseases in the field of internal medicine were studied. In addition, relationship between clinical efficiency and the value of area under the time concentration curve (AUC) was evaluated. It was possible to evaluate clinical responses in 36 cases of 40 infections; Responses were excellent in 15, good in 16, fair in 3 and poor in 2. An overall efficacy rate was 86.1%. A high dose of CPZ (2 g X 2/day) produced a higher efficacy rate than a low dose of the drug (1 g X 2/day) [95.8% (23/24 cases) vs. 66.7% (8/12 cases)] but the difference was not statistically significant. It was possible to evaluate bacteriological efficacy in 32 cases. Bacteria were eradicated in 29 (90.6%) and decreased in 3. The value of AUC in the high dose group was significantly higher than that in the low dose group. Values of AUC in cases of excellent or good clinical responses tended to be higher than values in cases of fair or poor responses. Side effects were noted in 4 cases, but they were not serious and improvements were observed without withdrawal of the drug except 1 case.
