[The effect of local injection of interferon against human malignant glioma transplanted into nude mice and the mechanism of its effect].

In order to clarify the mechanism of action of recombinant human leucocyte interferon, the effect of local injection of it to the human malignant gliomas (one oligodendroglioma, the other glioblastoma) transplanted into nude mice were evaluated. The volume of the tumors were calculated as 1/2 (short diameter [cm])2 x (long diameter[cm]). And the ratio of tumor volume (T)/original size (C) were calculated in terms of experimental day. Among the groups of control (vehicle of IFN injected), 1 million units of IFN locally injected group, 3 million units injected group, and 9 million units injected group, the effects of the treatment were statistically evaluated in terms of T/C. At the end of the experiment, each animal was injected 4 mg of BrdU intraperitoneally, and the labeling indices of the tumor tissue were measured and compared among the groups above mentioned. Local injection of IFN to the tumor was effective even at the dose of 1 million units every other day for 16 days for glioblastoma and 50 days for oligodendroglioma. The labeling index of the treated groups was significantly reduced when compared to that of control group in both tumors. And the experiment was performed to evaluate the variation of NK activity and ADCC activity of mouse spleen cells among the experimental groups. For the oligodendroglioma, NK activities were significantly increased in the 9 million units of IFN injected group when compared to those of control group. For glioblastoma, there was no definite variation of NK and ADCC activities among the groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[MR measurement of normal brainstem cerebellum and corpus callosum on midsagittal section].

The dimensions of the brainstem, cerebellum and corpus callosum were measured on magnetic resonance (MR) images with sagittal spin-echo sequence. Eighty-two normal adults (average 49.6 years old) were measured. The mesencephalic, pontine or cerebellar diameters and lengths could be measured more accurately and reproducibly than medullary diameter and length. The anterio-posterior diameter of the pons and the cerebellum was 23.2 +/- 1.4 mm and 26.4 +/- 2.5 mm respectively. The length of the pons and the cerebellum was 27.8 +/- 2 mm and 45.8 +/- 3.5 mm respectively. We have observed focal thinning at the body of corpus callosum in 73%. This narrowing is almost unquestionably a normal variant.

[Preoperative evaluation of digital subtraction pulmonary angiography in primary lung cancer].

The preoperative evaluation of the resectability for primary lung cancer was studied by using digital subtraction pulmonary angiography (DSA-PAG). Thirty operative cases with primary lung cancer performed DSA-PAG as preoperative test at random for 3 years from June 1985 were subjected in this study. The apparatus used is DSA device (Angiotoron) of a product of Siemens Co., Ltd. The findings obtained by DSA-PAG were retrospectively studied according to the surgical findings, operative modes and postoperative histopathological findings. Moreover, at the same time, usefulness of DSA-PAG was evaluated comparing its findings with preoperative enhanced CT interpretation. The useful findings were obtained in 10 cases of 30 cases (33%). Its contents were 2 cases with invasion of the left atrium, 2 cases with invasion of superior and inferior pulmonary vein, 5 cases with invasion of truncus pulmonary artery and 1 case with interlobar invasion. The findings in all cases were superior to those obtained by the enhanced CT. The decision of operative modes confirming invasion of tumor or metastatic lymph node is one of the important aims of the preoperative test. DSA-PAG can be carried out simply and safely on out-patient basis, additionally, favorable result can be especially obtained in invasion lesions of blood vessels of hilus regions. From the result, informations of regions unknown by preoperative CT were accurately obtained. As a rule, DSA-PAG should be performed by the preoperative routine test of primary lung cancer.

[Aspects of exophthalmos in relation to treatment for dysthyroid ophthalmopathy].

257 eyes of 135 patients with dysthyroid ophthalmopathy were treated with steroids and/or radiation. Steroid therapy consisted of 100-150 mg betamethasone given intravenously and orally. Radiation therapy was conducted with 1.5 Gy Lineac delivered to the retrobulbar tissue from two directions once a day for a period of 10 days. In combination therapy, the above two therapies were performed simultaneously. Exophthalmos and the new index of the c/o ratio (c/o ratio; defined as the content of the orbit divided by the capacity of the bony orbit) were measured on CT films and followed for 1 or 2 years in order to evaluate the effectiveness of these therapeutic modalities. Combination therapy yielded the best results among these therapies. Steroid therapy yielded good results for 3 or 6 months, but rebound phenomenon was recognized after a course of 1-2 year.

[Extraocular muscle aspects of the treatment in dysthyroid ophthalmopathy].

The Maximum width of four rectus muscles (MR, LR, IR, SR) was measured on CT films and followed for 1-2 years in 304 eyes of 160 patients with dysthyroid ophthalmopathy. Based on these parameters, steroid therapy with 100-150 mg betamethasone, intravenously and orally, 1.5 Gy Lineac radiation therapy was given to the retrobulbar tissue from two directions repeated for 10 days and combination therapy consisting of the above two therapies given at the same time were evaluated. Combination therapy yielded the best results. Steroid therapy showed little effect on MR, but remarkable effect on SR. It was speculated that different portions of the rectus muscles may be related to different responses of rectus muscles to different treatment regimen.

A practical method for estimating total oxygen uptake during exercise in elderly men.

In the present study, after a total of 51 observations of a 30-min cycle exercise performed by 17 men ranging in age from 60 to 65 years, the following formula was finally obtained for evaluating total O2 uptake (TVO2) during exercise: TVO2 (ml.kg-1) = SR125 X (49.5 X mean HR + 3760) X THB X 10(-4), where mean HR and THB are mean heart rate (beats.min-1) and total heart beats in exercise, respectively, and SR125 is the slope of the regression line of accumulative O2 uptake on accumulative heart beats during exercise at a mean HR of 125 beats.min-1. SR125 was significantly correlated not only to predicted VO2max but also score (X) in the step test for 2 min (25 steps.min-1 on 35-cm stool), yielding a formula, SR125 = -0.00131X + 0.3660. Consequently, both formulae indicate that total O2 uptake of any exercising elderly man can be estimated from total heart beats and mean HR during exercise, regardless of intensity of exercise when SR125 was determined by the step test. The discrepancy between total O2 uptake evaluated by the estimation method for elderly men and that determined by the Douglas bag method was 10.2 +/- 7.3%.

[Limb salvage procedures in malignant bone tumors of the extremities, with special reference to the various reconstructive procedures of bone defects].

The outcomes of 31 cases of limb salvage procedures for malignant bone tumors performed between 1974 and August 1988 were investigated. The age at operation ranged from 7 to 76. Thirteen cases were growing children under 15. The site of lesions were humerus in 6 cases, femur in 20, tibia in 3 and fibula in 2. Pathological diagnoses were osteosarcoma in 19, chondrosarcoma in 5 and others in 7. Reconstructive procedures were performed in 28 cases, consisting of endoprosthetic replacements in 18, vascularized fibula grafts in 6 and free autogenous bone grafts in 4. Two cases of osteosarcoma recurred, and amputations were performed. Six cases died of pulmonary metastases, while 22 cases (71%) are alive and free of disease. Functional results depended mainly on the size and site of resection rather than on the reconstructive procedures, and were generally good in the proximal femur and fibula, and poor in the distal femur and proximal tibia. Discrepancy in the length of lower limbs occurred in 8 cases of growing children; 5 of 6 vascularized fibula grafts showed thickening of the grafted bones, and one of them grew by 2.5 cm in length. Endoprosthetic replacements are adequate for hip and shoulder regions, and for low grade sarcoma which does not require chemotherapy or irradiation. Free autogenous bone grafts are good for narrow defect of bone, and cases of arthrodesis of joint. Vascularized fibula grafts are adequate for the upper limb, and lower limb of growing child.

[Clinical experience of chemotherapy with cefmetazole for severe infections accompanying malignant hematological disorders].

We made an attempt to treat with cefmetazole (CMZ) 25 patients who developed severe infectious diseases while suffering with granulocytopenia associated with the treatment of malignant hematological disorders. 1. Determination of bacteriological efficacy While 20 strains were isolated and identified from 15 patients, no significant bacteria were detected in 9 patients. Isolates obtained were: 5 strains of Enterococcus faecalis, 3 strains of Haemophilus influenzae, 2 strains of Staphylococcus epidermidis, 2 strains of Klebsiella oxytoca, 2 strains of Staphylococcus aureus, and 1 strain each of Neisseria sp., Pseudomonas maltophilia, Enterobacter sp., alpha-Streptococcus, beta-Streptococcus and Gram-positive cocci. Causative organisms were eradicated or markedly in 7 of the 15 patients from whom bacteria were isolated. Clinical findings, including fever, revealed that none of the patients, in whom bacteriological efficacy was determined to be poor, exhibited sufficient clinical response. E. faecalis was isolated from 4 of 6 patients bacteriologically determined to have no response. 2. As for 23 patients, who were found to be evaluable among the 25 patients, 8 (34.8%), 4 (17.4%), 4 (17.4%), and 7 (30.4%) demonstrated excellent, good, fair and poor responses, respectively, showing a 69.6% efficacy rate which indicates a sum of percentages of patients with excellent, good and fair responses. 3. While an efficacy rate of 100% was obtained for 3 patients with number of peripheral neutrophils less than 500/mm3 before the beginning of CMZ administration, only an efficacy rate of 66.7% was obtained for 15 patients with neutrophils more than 500/mm3.(ABSTRACT TRUNCATED AT 250 WORDS)

[Adenocarcinoma of the uterine cervix].

The prognosis of 589 patients with adenocarcinoma, 387 with mixed type of adenocarcinoma and squamous cell carcinoma of the uterine cervix from 18 hospitals in Japan were evaluated. Stage 0, I a patients with adenocarcinoma or mixed type had good survival. Stage I b, II, III patients had smaller five-year survival (p less than 0.01) than squamous cell carcinoma. Lymph nodes metastasis was related to poorer survival. Radiation therapy or chemotherapy was not sufficient for patients with stage I b, II, III, IV diseases.

[Adhesion of dental resin to tooth structure--syntheses and adhesion to tooth structure of various aliphatic methacrylates with a carboxylic group].

Twelve aliphatic methacrylates with a carboxylic group [succinate (2 MES, 5 MPeS and 10 MDS), methylsuccinate (2 MEMS, 5 MPeMS and 10 MDMS), maleate (2 MEM, 5 MPeM and 10 MDM) and citraconate (2 MEC, 5 MPeC and 10 MDC)] were synthesized by the addition of four dicarboxylic acid anhydrides to each of three different alkylene chain length hydroxy methacrylates to investigate the relationship between the methacrylate structure and its bonding to tooth. The bond strength of methacrylates to polished tooth surface decreased with increasing alkylene chain lengths under dry conditions, but water immersion reduced this change. The bond strength of succinate and maleate to polished tooth surface was higher than that of methylsuccinate and citraconate under dry conditions. All methacrylates showed high bond strength to etched enamel, with maleate showing the highest bond strength. On the other hand, the bond strength of 2 MEM and 5 MPeM to etched dentin was markedly high, and about 5 microns thick resin reinforced dentin at the interface between etched dentin and resin (2 MEM or 5 MPeM) was observed by SEM and EPMA analysis.

[New quantitative parameters for CT evaluation of orbitopathy in dysthyroid ophthalmopathy].

The new index (c/o ratio) is defined as the content of the orbit divided by the capacity of the bony orbit on the mid-orbital axial CT slice showing the medial and the lateral rectus muscles and optic nerve. Other quantitative parameters such as for exophthalmos and the capacity of bony orbit were also obtained on CT films. These parameters were measured in 474 eyes of patients with dysthyroid ophthalmopathy. The capacity of bony orbit showed large variations among all patients. The average for males was 17.8cm3 and that for females was 16.1cm3. Exophthalmos measurement on CT is more reliable than that by the Hertel exophthalmometer. The new index (c/o ratio) is a very reliable parameter for evaluating orbitopathy in dysthyroid ophthalmopathy. A large c/o ratio (greater than 1.6) was of clinical significance, because approximately 70% of the enlargement of the extraocular muscle observed in older patients (greater than 40 yrs old) was associated with a large c/o ratio (greater than 1.6).

[Intra-hepato-arterial chemotherapy combined with hyperthermic treatment: clinical results of metastatic cancer of the liver and effects on correct (but not at all necessary) hepatic blood flow].

In treating cancerous metastases to the liver, we combined hyperthermic treatment with chemotherapy via the intra-hepato-arterial injection (IHAI) of cis-diamminedichloroplatinum (II) plus 5-fluorouracil. The subjects were 14 patients having metastases to the liver: 3 from gastric cancer and 11 from colorectal cancer. In metastases of colorectal origin, the response rate (partial response) was 55%; the 1-year survival rate was 80%; the 50%-survival period was 23 months. The response rate and the 50%-survival period in metastases of gastric origin were 67% and 11 months, respectively. When IHAI chemotherapy was combined with hyperthermic treatment, antitumor effects were generated in 3 of 6 metastatic patients from colorectal cancer, who had received no benefit from the IHAI chemotherapy alone. Better results were produced by thermochemotherapy than by IHAI chemotherapy alone. Hepatic blood flow, which influences the antitumor effect of hyperthermic treatment or chemotherapy, was measured using the 133Xe clearance method. Over the long term, the hepatic blood flow, especially that of the portal-venous route, showed a decreasing tendency after repeated sessions of the present regimen. This fact suggests the increased retainability of arterial-injected carcinostatics in cancerous tissues.

[Cerebral blood flow in patients with thalamic hemorrhage (Part 2): Etiological analysis of decreased cerebral blood flow in acute stage].

In twenty-nine patients with thalamic hemorrhage, single photon emission CT (SPECT) and CT were performed in the acute stage. Measurement of cerebral blood flow (CBF) was performed by the 133-Xe inhalation method using SPECT (Tomomatic 64). CT findings such as hematoma volume, involvement of internal capsule, ventricular hematoma and topographical localization of hematoma were investigated. We studied etiological analysis of decreased CBF in the acute stage. CBF values in the group of large-volume hematoma (greater than or equal to 10 ml) decreased moderately on the hematoma side and mildly on the nonhematoma side. CBF values in the group of small-volume hematoma (less than 10 ml) decreased mildly on the hematoma side but didn't decrease on the nonhematoma side. CBF values of the former on the hematoma side decreased significantly compared with the latter. Linear correlation between hematoma volume and CBF was significant. As to topographical localization, CBF values of the group which involved medial thalamus decreased significantly compared with the other group. Factors of involvement of internal capsule and ventricular hematoma didn't affect CBF values. In conclusion, major factors which affected decreased CBF in the acute stage were hematoma volume and tomographical localization.

[Reversibility of glaucomatous defects of the central visual field].

We analyzed more than 4 consecutive measurements of the central area of the visual field in 1,004 eyes of 533 subjects with various types of glaucoma as diagnosed by the Octopus 201 and its examination program No. 31. We used a non-repeated analysis of variance to determine the statistical relationship between the mean sensitivity measurements for individual quadrants of the central visual field. We made the series of measurements used in this statistical analysis during repeated examinations that took place over a follow-up period of 1 year. Out of 1,004 eyes, 53 eyes (5.3%) were shown to have an improved mean sensitivity of the central 30 degrees of the visual field. We adopted the degree of sensitivity improvement [(final mean sensitivity - initial mean sensitivity)/follow-up period] as the indicator of the degree of reversibility in the central visual field to determine which factors might influence changes in the visual field. In patients under 50 years of age, who were at an early stage of the disease, as opposed to those over 50 years of age, who were at an intermediate or advanced stage of this disease or who had undergone reduction of intraocular pressure, a relatively high degree of sensitivity improvement was more frequently observed. In the cases with primary open angle glaucoma which includes low tension glaucoma, we think treatment for abnormality in the aqueous humor dynamics is very important. We concluded, that the reversibility of the central visual field defect in glaucomatous eyes is possible, though it can only be clearly detected by careful follow-up examinations employing computerized perimetry.

[Factor analysis of bone mineral dynamics in patients with renal osteodystrophy].

Remarkably high accumulation of Tc-99m-MDP is seen in the skull on bone scintigraphy of patients with renal osteodystrophy (ROD), especially with secondary hyperparathyroidism. For the quantitative evaluation, the Factor Analysis (FA) was used for the early phase of bone scan. Tc-99m-MDP (14.8 MBq/body weight kg) was injected as a bolus through the medial antecubital vein. Dynamic acquisition of 40 x 30 sec frames were performed in a 64 x 64 matrix. For pre-processing, nine points smoothing was carried out, and then the region of interest was set on the frontal image of the head for the FA. The FA was performed with an 8 x 8 sampling corresponding to 64 dixels from 4,096 dixels. Bone factor was clearly extracted by the FA. Then, two original parameters were calculated. One is the bone radionuclide uptake counts (Bu) which is the product of the total radionuclide counts of skull and the contribution ratio, the other is the uptake ratio (Br) derived by the time activity curve (physiological component) of the FA. These parameters of ROD were significantly different compared to those of controls. The FA seems to be useful in quantitative evaluation of bone mineral dynamics.

[Effect of morphine in cancer pain].

The usefulness of oral morphine to alleviate pain has been tested in 70 patients with cancer. The efficacy was found to be 87% (61/70). The starting dose was 10 mg/d-12 mg/d (mean 36 mg/d), and the maximum dose was 10 mg/d-3,600 mg/d. Twenty four of these patients were able to stay at home or go to the hospital for dose treatment. In 27 patients, oral morphine dose were able to administrated until death. Although vomiting and constipation were frequent side effects, the administration of adjuvant drugs relieved these symptoms. It was found that these oral morphine doses did not shorten a patient's life span. Thus we have concluded that oral morphine proved a useful, safe, and convenient drug for the control of cancer pain.
