Herpes simplex virus latency in human cornea.

The authors attempted to demonstrate the latency of herpes simplex virus type 1 (HSV-1) in corneas obtained during penetrating keratoplasty from patients with herpetic stromal keratitis in the nonactive (subsided) stage. The subjects comprised 20 patients (9 men, 11 women; average age 43.5 years). Infectious virus was not detected in the supernatants following corneal homogenization and centrifugation. Latent virus was detected in the cultured supernatants of the corneal sections from 8 patients. Although the ganglion trigger theory by Hill et al is conventionally supported as the mechanism of recurrence of herpetic keratitis, the present results suggest a ganglion and skin trigger theory, in which proliferation from latent HSV-1 in the cornea (peripheral tissue) might stimulate the ganglion.

Membranous cataract in association with aniridia.

A case of aniridia associated with a membranous cataract and glaucoma in a 52-year-old woman was reported. Bilateral ocular findings included marked hypoplasia of the iris, goniodysgenesis, corneal opacification with superficial vascularization, macular hypoplasia and glaucoma. A membranous cataract was found in the right eye. The intraocular pressure was abnormally high. Since trabeculectomy on the right eye was ineffective, a seton procedure was carried out. During the follow-up period, the membranous cataract in the right eye spontaneously moved from its original position and floated in the vitreous, maintaining its shape.

Lipid hydroperoxide-resistance gene in Saccharomyces cerevisiae: utilization as a selectable marker gene for yeast transformation.

Two kinds of DNA fragments concerned with resistance against lipid hydroperoxide were cloned from the yeast Saccharomyces cerevisiae. When the recombinant plasmid carrying the DNA fragment was introduced into the cells of S. cerevisiae which do not have an appropriate auxotrophic marker, the transformants could be selected by their resistance to t-butyl hydroperoxide. By Southern hybridization analysis, such transformants were found to contain the recombinant plasmid, suggesting that the DNA fragment could be used as a selection marker for the transformation of S. cerevisiae.

[Long-term follow-up of a case of Landau-Kleffner syndrome].

Long-term investigation of a male case of Landau-Kleffner syndrome with auditory, verbal and non-verbal agnosia was performed clinico-electroencephalographically and neuropsychologically. Atypical absences and secondarily generalized tonic-clonic seizures were observed. The spike-wave index of slow wave sleep from all-night EEG recordings was 96% in this case. It was impossible to differentiate this case from epilepsy with "continuous spikes and waves during slow wave sleep (CSWS)" except the presence of auditory agnosia. Auditory agnosia of this case disappeared after antiepileptic therapy. It is suggested that Landau-Kleffner syndrome showing CSWS may have a good prognosis about agnosia from that of this case and others previously reported.

[A study of plasma atrial natriuretic peptide, antidiuretic hormone and cerebral vasospasms in patients with aneurysmal subarachnoid hemorrhage].

The relationship between plasma atrial natriuretic peptide (ANP) and antidiuretic hormone (ADH) both of which show high values after subarachnoid hemorrhage and cerebral vasospasm was studied. The subjects were 23 patients who were admitted because of aneurysmal subarachnoid hemorrhage during three years from March, 1989 to March, 1992 and in whom plasma ANP and ADH levels could be determined over time. Cerebral vasospasm was evaluated by the finding of cerebral angiography, clinical symptoms, and presence or not of low density areas on CT. Hyponatremia was defined as the serum sodium level of 130 mEq/l or less for two days or more. Angiographical vasospasm was found in 17 patients (85%), symptomatic vasospasm in 15 patients (65.2%), low density areas on CT in 9 patients (40.9%) and hyponatremia in 8 patients (34.8%). Symptomatic vasospasm was noted in 7 of the 8 patients (87.5%) with hyponatremia, low density areas on CT in 4 patients (50%), the detection rate being high. The plasma ANP and ADH levels were 76.7 +/- 32.1 pg/ml and 2.2 +/- 0.7 pg/ml respectively in the patients with symptomatic vasospasm against 38.3 +/- 21.3 pg/ml and 2.4 +/- 0.6 pg/ml respectively without symptomatic vasospasm, the plasma ANP level being significantly high in the patients with symptomatic vasospasm (p < 0.01). The plasma ANP and ADH were 71.2 +/- 33.8 pg/ml and 2.0 +/- 1.1 pg/ml respectively in the patients with low density areas on CT against 51.2 +/- 31.3 pg/ml and 1.8 +/- 0.5 pg/ml respectively without low density areas on CT.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical application of 11C-NMSP to the patients with pituitary adenoma other than prolactinoma].

The existence of dopamine D2 receptors in the pituitary adenoma is an important factor for bromocriptine therapy. In order to evaluate the D2 receptors in the pituitary adenoma, 11C-N-methylspiperone (NMSP) and positron emission tomography was performed in a variety of pituitary adenomas other than prolactinoma. Eight patients with pituitary adenoma including four non-functioning adenomas, two growth hormone (GH) secreting adenomas, one ACTH secreting adenoma and one TSH secreting adenoma and ten normal healthy volunteers were examined. 740 MBq of 11C-NMSP was injected and dynamic scan was performed from the time of injection to 80 min postinjection. The time dependent regional radioactivity in the striatum (st) and in the tumor tissue (tm) was measured and the binding rate, alpha, was calculated according to the following equation: Cst, tm/Ccbl = alpha x integral of t0 Ccbl dt/Ccbl + 1. The ratio of total radioactivity in the striatum or tumor over total activity in the cerebellum was plotted versus "normalized time", which was obtained by dividing integrated cerebellar radioactivity by actual cerebellar radioactivity for each timepoint. In three patients with pituitary adenoma, two scans were performed before and three hours after oral intake of 2.5 mg of bromocriptine. The alpha values for pituitary adenoma and normal striatum were 0.031 +/- 0.023 (0.017-0.088) and 0.042 +/- 0.006 (0.034-0.049), respectively. Bromocriptine reduced alpha value to various degree in pituitary adenoma (13-56%) but not in striatum (-3.7-3.3%). Our data suggests that in some of pituitary adenoma other than prolactinoma, dopamine D2 receptors existed but that nonspecific binding also contributed to the radioactivity in the tumor tissue.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical and pathological efficacies of intra-arterial infusion chemotherapy and radiotherapy combined with hormone therapy in prostate cancer].

Eleven patients with prostate cancer were treated by intra-arterial infusion chemotherapy and radiotherapy combined with hormone therapy. CDDP was given one patient, THP and CDDP to eight patients, and THP and CBDCA to two others. All patients were treated by whole pelvis with prostate boost radiation. Seven patients underwent castration and four were administered LH-RH agonist in hormone therapy. Clinically, CR was observed in two patients and PR in nine. Histologically, G3 was observed in six patients, G2 in four patients and G1 in one. The main side effects were myelosuppression and rectal disturbances, but treatment could be completed in all patients. These results suggest that this treatment is a useful regimen for prostate cancer.

[Dual-energy data acquisition for simultaneous 81mKr ventilation/99mTc-MAA perfusion SPECT: ventilation/perfusion ratio mapping in cross section].

Methods of simultaneous pulmonary ventilation/perfusion (V/Q) tomogram (SPECT) for V/Q ratio mapping were reported. Crosstalk was corrected by subtraction assuming crosstalk/signal ratio is constant for an individual examination. Crosstalk was reduced by 85% for that of 99mTc-to-81mKr window (R12), and by 83% for that of 81mKr-to-99mTc window (R21) in a phantom experiment. The R12 and R21 were 1.0-1.9% (mean 1.3%) and 34.6-55.3% (mean 41.9%), respectively in 9 patients (mean age, 56.1 years; range, 38-82 years). V/Q SPECT was undertaken in 7 of the 9 patients. 99mTc-macroaggregated albumin (185 MBq) was injected intravenously in supine position, and 81mKr gas was administered continuously during the SPECT data acquisition. Sixty-four 64 x 64-matrix (size = 8 mm/pixel) images, 20-second acquisition time for each, were collected by a rotating gamma camera in a step-and-shoot fashion during a 360-degree rotation with the two photopeaks, one at 140 keV for 99mTc and another at 190 keV for 81mKr. Examination time was about 40 minutes, and the V/Q ratio maps in cross section were obtained in all of the examinations.

[Hepatic metastases from jejunal leiomyosarcoma treated effectively by repeated transarterial embolization with carboplatin].

A 67-year-old man complained of abdominal mass, abdominal pain and tarry stool. He was diagnosed as having jejunal leiomyosarcoma and multiple liver metastases after examination. The jejunal leiomyosarcoma was resected by operation. Unrectable liver metastases were repeatedly treated by transarterial embolization with carboplatin and Lipiodol, and a significant reduction was achieved. TAE with carboplatin and Lipiodol was considered evaluating efficiency therapy without side effect. He has remained well presently for 1 years 10 months after operation.

The role of natural killer cells in murine cytomegalovirus eye infection.

PURPOSE: To study the role of natural killer cells in the dissemination of virus to the eye and virus growth as well as the production of lesions in the eye during primary infection of murine cytomegalovirus. METHODS: Virus activity and lesion production by murine cytomegalovirus in various organs of natural killer cell-depleted BALB/c strain of inbred mice were compared with those of immunocompetent normal BALB/c mice. RESULTS: In mice injected intraperitoneally with murine cytomegalovirus, virus could be isolated from 50% of eyes in the natural killer cell-depleted group whereas no virus was detected from any eye in the control group. In natural killer cell-depleted mice with positive virus isolation from eyes, no murine cytomegalovirus was isolated from either optic nerves or trigeminal ganglia whereas virus was detected from blood lymphocytes, macrophages, granulocytes, and plasma. After intravitreal injection of murine cytomegalovirus, virus titers in eyes of natural killer cell-depleted mice were significantly higher than those in the control group. Derangement of retinal cell layer and inflammatory cells as well as cytomegalic cells in iris and ciliary body were noted in natural killer cell-depleted group, whereas no such changes were observed in the eyes of the control mice. CONCLUSION: Natural killer cell depletion enhances the dissemination of murine cytomegalovirus to the eye through the hematogenous route, and increases virus multiplication as well as lesion production in the eye.

[Radionuclide studies of atherosclerosis].

New Tc-99m-labeled brain perfusion imaging agents, such as Tc-99m-HMPAO, and Tc-99m-ECD, are developed and expected to improve the clinical study of cerebral perfusion disturbance due to arteriosclerosis. They enable us better images than I-123-IMP and more accurate diagnosis. Similarly, in the field of cardiovascular nuclear medicine, new Tc-99m-labeled compounds, Tc-99m-MIBI, Teboroxime and Tetrofosmin are available. Furthermore, I-123-MIBG, a tracer taken up in presynaptic adrenergic vesicles, and I-123-BMIPP, a new agent developed for the evaluation myocardial fatty acid metabolism, will mark a new epoch in the study of coronary sclerosis. In order to image atherosclerosis directly, In-111-polyclonal immunoglobulin G, Tc-99m-low density lipoprotein etc., are under investigation.

[Comparison between 180 degrees and 360 degrees data collection in 99mTc-Tetrofosmin SPECT of the myocardium].

We compared 360 degrees data collection technique with 180 degrees technique in SPECT using a new myocardial perfusion imaging agent, 99mTc-Tetrofosmin. Eight patients were examined at stress and at rest on separate days. Following the injection of 555 MBq 99mTc-Tetrofosmin, 64 views of 15 sec each were collected during a 360 degrees rotation, and 32 views of 30 sec each during a 180 degrees rotation. The 180 degrees data collection provided superior images in sharpness in 14 of 16 scans. The interpretation was concordant in 123 of 128 regions (96.1%), and hypoperfusion was considered to be underestimated in 5 regions using the 360 degrees technique. It was suggested that the 180 degrees data collection technique is more effective in myocardial SPECT employing 99mTc-Tetrofosmin than the 360 degrees technique.

[Clearance of 99mTc-Tetrofosmin from the myocardium and the adjacent organs].

We studied clearance of a new myocardial perfusion imaging agent, 99mTc-Tetrofosmin, from the myocardium and the adjacent organs in nine patients. 99mTc-Tetrofosmin was injected intravenously at stress and at rest, and SPECT images were obtained 25 min and 120 min later. Anterior view images were also obtained 10 min and 150 min after the injection to compare the myocardial clearance with those from the liver and lung. Clearance from the liver was faster than those from the myocardium and lung. Washout rate calculated with SPECT was higher for the inferior wall than for the anterior wall and lateral wall (p < 0.05 at rest, N.S. at stress). The value was higher at stress in any region than at rest (the anterior wall and lateral wall, p < 0.01; the septum, p < 0.05; the inferior wall, N.S.). Washout rate for the anterior wall was 3.4 +/- 14.0% at rest and 18.0 +/- 10.6% at stress. The rate for the lateral wall was 2.7 +/- 22.5% at rest and 15.8 +/- 7.9% at stress. It was suggested that rapid hepatic clearance and high myocardial washout at stress may be important in determining the period between the injection of 99mTc-Tetrofosmin and the data acquisition for SPECT imaging.

A bradycardic agent, UL-FS 49, increases atrial force and decreases ventricular force in isolated, perfused heart preparations of dogs.

We investigated the effects of a bradycardic agent, UL-FS 49 (1,3,4,5-tetrahydro-7,8-dimethoxy-3[3-[[2-(3,4-dimethoxyphenyl) ethyl]methylimino]propyl]-2H-3-benzazepin-2-on hydrochloride), on the sinus rate and atrial contractile force and the ventricular force in the isolated, blood-perfused right atrial and left ventricular preparations of the dog and the inhibition by UL-FS 49 of the negative cardiac responses to acetylcholine, adenosine and cromakalim. UL-FS 49 (0.003-3 mumol) decreased sinus rate, increased atrial force and decreased ventricular force in a dose-dependent manner. The threshold doses of UL-FS 49 (0.03 mumol) for atrial responses were smaller than that (1 mumol) for a ventricular response. The duration of bradycardia induced by UL-FS 49 was longer than the intropic one. Propranolol and atropine did not block the positive inotropic and negative chronotropic responses to UL-FS 49, respectively, in isolated atria. UL-FS 49 (0.1-3 mumol) dose-dependently inhibited the negative chronotropic and inotropic responses to parasympathetic nerve stimulation and acetylcholine exogenously administered. UL-FS 49 at higher doses (1 and 3 mumol) attenuated the negative inotropic but not chronotropic responses to adenosine and cromakalim.(ABSTRACT TRUNCATED AT 250 WORDS)

[Chemotherapy with imipenem/cilastatin for severe infections accompanied by malignant hematological disorders].

The clinical and bacteriological efficacy of imipenem/cilastatin (IPM/CS) was evaluated in 30 cases of serious infections associated with hematological malignancies. 1. Among 28 evaluable cases, excellent efficacy was obtained in 6 cases and good effectiveness in 10 cases, resulting in a high clinical efficacy rate (57.1%). The clinical effectiveness of IPM/CS was not dependent on neutrophil count in peripheral blood. A 53.8% efficacy rate was observed in 26 cases which had received pretreatment with other antibiotics. 2. Antibacterial activities of IPM/CS have so far been evaluated against organisms isolated in 20 of 28 cases: 2 strains of coagulase-negative Staphylococcus, 2 strains of methicillin-resistant Staphylococcus aureus, 2 strains of Enterococcus faecalis, 9 strains of Enterobacter cloacae, and 8 other strains. 3. Among 3 evaluable cases treated with IPM/CS alone, response was good in 1 case. Among 25 patients receiving IPM/CS in combination with an aminoglycoside or a penicillin, the efficacy rate was 60%. 4. Five patients had IPM/CS-related adverse events; nausea and vomiting in 2 cases, seizures in 1 case, small increases in GOT and GPT in 2 cases, and the appearance of casts in urine sediment in 1 case. These patients, however, tolerated the complete course of therapy with IPM/CS except the 2 cases with nausea and vomiting. These results indicate that chemotherapy with IPM/CS is effective for the treatment of severe infectious diseases accompanied by hematological disorders.

Orthognathic surgical approach and partial glossectomy to a skeletal 3 adult open bite.

Combined orthodontic treatment with orthognathic surgery and partial glossectomy were simultaneously performed in an adult male patient with mandibular prognathism and open bite from the lower right to the left first molars. Total active treatment period was 15 months. During the retention period, a tooth positioner was given to obtain proper interdigitation from set-up models. The post-surgical occlusion has been stable without any complication of the mandible and the tongue.

Thermoluminescence and flash-induced oxygen yield in herbicide resistant mutants of the D1 protein in Synechococcus PCC7942.

Several strains of Synechococcus PCC7942 carrying point mutations in the gene psbA were studied by thermoluminescence and polarographic measurement of flash-induced oxygen yield. The following results were obtained: (a) Replacement of Ser-264 in D1 by Ala (mutant Di1) or Gly (mutant G264) resulting in DCMU and atrazine resistance leads to a downshift of the thermoluminescence (TL) B-band peak temperature from 40 degrees C in wild-type thylakoids to about 30 degrees C. In dark adapted samples of both mutants the TL and oxygen yield pattern induced by a train of single turnover flashes were strongly damped indicative of a high miss factor. (b) In contrast to Ser-264 mutants, replacement of Phe-255 in D1 by Tyr (mutant Tyr5) induced strong resistance to atrazine but not to DCMU and did not affect the peak termperature of the B-band and the flash-induced TL and oxygen yield patterns. In this respect mutant Tyr5 resembles the wild type. (c) No significant differences have been found between strains with single site mutations in psbAI and normal psbAII/psbAIII genes, and strains with same mutations in psbAI but additional deletion of psbAII and psbAIII. Obviously in strains were psbAI is present, PS II complexes containing gene products of psbAII and psbAIII are not assembled in detectable amounts. (d) Strains with double mutations at positions 264 and 255 display a downshift of the B-band peak temperature. Their oscillatory patterns of B-band intensity and oxygen yield are highly damped. This behaviour is similar to strains D1 and G264 which are modified at position 264 only. We extend reports on additivity of mutation effects on herbicide binding to binding of QB. (e) Mutations at the QB site not only influence the binding of QB and herbicides but also change the thermoluminescence quantum yield and the lifetimes of the redox states S2 and S3 of the water oxidase. This finding might indicate long ranging effects on Photosystem II exerted by structural modifications of the QB site. From these data we conclude that Ser-264 is essential for binding of atrazine, DCMU and QB, whereas Phe-255 is involved in atrazine binding and its substitution by Tyr does not markedly affect QB or DCMU binding in Synechococcus PCC7942.
