Effects of prostaglandin E1 in experimental autoimmune myasthenia gravis.

The effects of prostaglandin E1 (PGE1), a potent inhibitor of lymphocyte functions, were studied in rats immunized with acetylcholine receptor (AChR) to induce experimental autoimmune myasthenia gravis (EAMG). Daily injections of PGE1, 400 micrograms per day, prevented the development of acute EAMG, which is attributed to antibody-dependent, complement-mediated cytolysis. This was associated with suppression of delayed-type cutaneous hypersensitivity response to AChR. PGE1 did not prevent the subsequent onset of chronic EAMG, which reflects accelerated degradation of AChR by antibody and complement-mediated cell lysis in the postsynaptic membrane.

[Basic and clinical evaluations of cefmetazole in postoperative wound infections].

The time course of the concentration of cefmetazole (CMZ) in the serum and in skin and intestinal tissues was determined after a single intravenous injection of 2 g of the drug. CMZ moved into them well. Furthermore, 41 patients with postoperative wound infection (superficial in 29 and deep in 12) were treated with CMZ 2-4 g daily. Bacteriological examination of the lesions with simultaneously carried out. As a result, 101 strains of bacteria were isolated and identified. Mixed infection was found in 27 cases (65.9%). Fifteen strains (14.9%) of E. coli, 15 (14.9%) of B. fragilis, 7 (6.9%) of Klebsiella sp. and 7 (6.9%) of Proteus sp., were the main bacteria isolated. Eight cases (19.5%) had mixed infection of E. coli and B. fragilis. The committee (3 members) evaluated CMZ to be effective in 75.6% (31 of 41 cases) and bacteria disappeared in 60.5% (23 of 38 cases). The side effects observed were pyrosis and feeling of gastric malaise in 1 case. The results suggest that CMZ is useful, which exerts an excellent effect on postoperative wound infections.

Pharmacophysiological study on muscle treated with calcium ionophore A23187.

Muscle treated with A23187, which raises intracellular free Ca2+ concentration, was studied to elucidate effects on membrane properties. The mean resting membrane potential was reduced; it was normally depolarized by ouabain but not significantly hyperpolarized by epinephrine and insulin. In the presence of ouabain, membrane depolarization was not provoked by insulin. The results imply a defect of active Na+-transport of the membrane after exposure to a calcium ionophore.

Contractile and chemosensitive properties of muscle treated with calcium ionophore A23187. Comparison with dystrophic muscle.

Rat diaphragms were treated with calcium ionophore, A23187, for 60 min at 37 degrees C, and twitch, tetanus and their derivatives were studied on the basis of the active state concept. Results were than compared with those of human dystrophic muscle. Like dystrophic muscle, maximum tetanic force and maximum velocity of tetanus development were markedly reduced. Changes in twitch parameters, which tend to appear in an early stage of muscular dystrophy, were not noted after the treatment with this ionophore, except alterations relating to prolongation of the active state. Twitch/tetanus ratio was thus increased, while it was decreased in dystrophic muscle. Isoproterenol-induced change of twitch was normally seen in the A23187-treated muscle, while dystrophic muscle responded to catecholamine in a manner different from control. Therefore, the muscle treated with calcium ionophore is not wholly similar to dystrophic muscle.

[The Tolosa-Hunt syndrome: report of a case with recurrent (9 times) painful ophthalmoplegia (author's transl)].

A 48-year-old woman was referred to the First Dept. of Int. Med., Nagasaki Univ. Sch. Med., in August, 1979, with a six-month history of recurrent episodes of right-sided painful ophthalmoplegia and diplopia. An epidode affected the right eye, lasted one to two weeks, and relapsed every month. On examination she had a complete ptosis on the right side and pain on the right eye. All extraocular muscle supplied by the 3rd nerve were paralysed. The pupils were equal in size both sides, reacting to light completely. Visual acuity was normal except myopia. All the other cranial nerves and the remainder of central nervous system was normal. Results of thyroid function tests and of lumbar puncture were normal. The glucose tolerance test showed a mild diabetic pattern. Blood and CSF cultures for bacteria, fungi, and acid-fast bacillus were negative. The skull, brain CT scan, and carotid angiogram were within normal limits. A tentative diagnosis of Tolosa-Hunt syndrome was made after an unproductive search for a cause for this woman's painful ophthalmoplegia and unsuccessful treatment of ophthalmoplegia with antibiotics or diet therapy for mild hyperglycemia. The patient was given prednisolone 30 mg daily orally when she had the 9th attack of painful ophthalmoplegia Pain, ptosis, and diplopia disappeared in 5 days and she did not show any recurrence of symptoms over the next 7 months.

Ultrastructural localization of acetylcholine receptor at the motor endplate: myasthenia gravis and other neuromuscular diseases.

Peroxidase-conjugated alpha-bungarotoxin (P-BGT) was used for the ultrastructural localization of the acetylcholine receptor in motor endplates. Brachial biceps muscle specimens were obtained from six patients with myasthenia gravis (MG) (two ocular and four generalized), five other patients with neuromuscular diseases (limb-girdle dystrophy, polymyositis, and amyotrophic lateral sclerosis) and two controls. In all patients with generalized MG, most of the endplates showed a marked decrease in P-BGT binding. In one of two patients with ocular MG, the amount and distribution of P-BGT binding appeared normal, whereas the other patient showed a slight decrease in P-BGT binding. There was a loose correlation between clinical severity of MG and acetylcholine receptor index or antiacetylcholine receptor antibodies. On the other hand, the amount and distribution of acetylcholine receptor in other neuromuscular diseases was well preserved, even at the endplates denuded of their nerve terminals in amyotrophic lateral sclerosis (ALS) cases.

Effects of epinephrine, ouabain, and insulin in experimental autoimmune myasthenia gravis.

To gain a clue to the target of anti-AChR antibody, rats with acute and chronic experimental autoimmune myasthenia gravis (EAMG) that was induced by immunization with Narke anti-acetylcholine receptor (AChR) were studied using agents acting on active Na-k transport. Postsynaptic response to epinephrine was defective in chronic EAMG with high titers of antibody, suggesting that active Na-K transport system modulated by cyclic adenosine monophosphate (AMP) may be affected primarily by antibody. Sensitivity to ouabain was less than normal in acute EAMG and became close to normal when treated with anticomplementary factor. Findings suggest that acute EAMG is a case of functional denervation. Normal response to insulin occurred in all phases of EAMG.

Critical evaluation of atrial presystolic murmur.

A crescendo-decrescendo pattern of atrial presystolic murmur has long been considered highly specific for atrial septal defect. Atrial presystolic murmur was observed in 67 or 460 consecutive cases. Of these 67 cases, there were only six with atrial septal defect and four with mitral valvular diseases. The occurrence of atrial presystolic murmur was observed over a broad spectrum of clinical entities. Although the precise pathogeneis of atrial presystolic murmur is not clearly defined, certain conclusions can be drawn from the present study, as follows: (1) atrial presystolic murmur is observed in a variety of clinical situations; (2) atrial presystolic murmur is not infrequently observed in normal subjects; (3) atrial presystolic murmur is not a specific phonocardiographic finding for atrial septal defect; and (4) atrial presystolic murmur may be caused by a multiplicity of factors.
