Genetic control of nickel sulfate delayed-type hypersensitivity.

Genetic control of delayed-type hypersensitivity (DTH) to nickel sulfate antigen was studied using various strains on inbred mice. Mice with class II Ad,k,s haplotypes showed a high magnitude of response, whereas those with class II Ab,f were low responders. These results indicate that nickel sulfate DTH may be influenced by the I-A region of H-2. Transfer experiments revealed that these responses might be mediated by L3T4+, Lyt-2- T cells.

[Microtiter enzyme-linked immunosorbent assay for diphtheria antitoxin in human sera--application of parallel line assay method].

The antibodies against Diphtheria toxin in human sera were measured by the ELISA method and the antitoxin titer was calculated using the parallel line assay method which is a quantitative bioassay method. The reproducibility of this method and the comparison of the titer between this method and cell culture method were proven. The dose response curve of standard antiserum and test serum calculated by the parallel line assay method showed linearity, and the regression line of the test serum was parallel to the standard serum. The coefficients of variance (CV) of the antitoxin titers obtained from triplicate measurements of 3 serum samples ranged from 9.1 to 36.0%. The serum at low titer gave a higher CV level. A good correlation was observed between the Diphtheria antitoxin titer by the neutralizing test in cultured cells and that measured by ELISA by the parallel line assay method. The coefficient of regression was 0.996 and the coefficient of the correlation was 0.899.

[Treatment of nausea and vomiting induced by a 24-hour i.v. infusion of cisplatin].

Nine advanced gastric cancer patients were given 17 courses of cisplatin administrations by means of a 24-hour intravenous infusion at a dose of 100 mg/m2. For an anti-emetic, 40 mg of metoclopramide was administered 5 times at 6-hour intervals, along with a 500 mg hydrocortisone administration at the start of the cisplatin infusion. Despite this preventative treatment, nausea and/or vomiting occurred in over one-third of all the courses. Thus, to combat this nausea and/or vomiting, a combination of lorazepam (1.5 mg/day, divided into 3 p.o.), dexamethasone (20, 10 and 10 mg by i.v. at 3, 8, and 24 hours, respectively, after start of the cisplatin infusion), and a 60 mg intravenous administration of metoclopramide (5 times at 6-hour intervals) was given, and it was found that this new method (Method IV) prevented both nausea and vomiting.

[A case of malignant mesenchymoma of the maxilla].

The case of a 48-year-old male with a malignant mesenchymoma of the maxilla is reported. The patient had complained of bloody rhinorrhea and cheek pain, and a total maxillectomy was performed. The tumor consisted of a rhabdomyosarcoma, an osteosarcoma and a chondrosarcoma, which were relatively differentiated. Later, recurrence of a tumor and a lung metastasis occurred, however the tumor revealed undifferentiated mesenchymal cells. This led us to suspect the origin of the malignant mesenchymoma had been a primitive mesenchymal cell. To our a knowledge, the present case is the first reported case of a malignant mesenchymoma with a maxillary origin in the world.

O-carboxymethyl-O-ethylcyclomaltoheptaose as a delayed-release-type drug carrier: improvement of the oral bioavailability of diltiazem in the dog.

The utility of O-carboxymethyl-O-ethylcyclomaltoheptose (carboxymethyl-ethyl-beta-cyclodextrin, CME-beta CD) as a delayed-release-type drug carrier was investigated in vitro and in vivo, using diltiazem hydrochloride as a model drug. The aqueous solubility of CME-beta CD showed a marked dependency on pH, because of the ionization of the carboxyl group (pKa 3.75). The formation of an inclusion complex between diltiazem and CME-beta CD in aqueous solution and in the solid state was assessed by a solubility method and by X-ray diffractometry, respectively. The rate of release of the drug from the compressed tablet containing the complex was significantly retarded in solutions at low pH and increased with increase in pH, and this was reflected in the blood levels in the dog after the oral administration. The results suggested that the use of CME-beta CD could improve the oral bioavailability of diltiazem and release the drug preferentially in the intestinal fluid but only slightly in the gastric fluid.

Operation for the stiff elbow.

Forty two cases of stiff elbow were treated by operation. Thirty three were subsequent to trauma, seven were due to ectopic ossification after quadriplegia, and two were associated with massive ectopic ossification after long term coma following head injury. Two separate skin incisions, postero-medial or medial and anterior, were used in most cases. In the post-traumatic cases, scarred, thickened posterior oblique fibres of the medial collateral ligament were usually found. Ectopic ossification was frequently situated on the postero-medial aspect of the stiff elbows. Resection of the thickened fibres or the ectopic ossification was the most effective procedure to improve elbow flexion. The average improvement in range of motion was 59 degrees in post-traumatic cases, 72 degrees in ectopic ossification after quadriplegia and 127 degrees in ectopic ossification following coma.

Dorsal dislocation of the D.I.P. joint with fracture of the volar base of the distal phalanx.

Between 1978 and 1985, we treated 12 cases of dorsally displaced fracture-dislocation of the I.P. joint of the thumb or of the D.I.P. joint of the finger, with a volar bony fragment. All were treated surgically, active exercise being started three to five weeks later. At the last follow-up, averaging 6.4 years (range 4.3-10 years) after operation, none of the joints was painful but the range of motion was somewhat limited.

Cutaneous protothecosis--a case report.

The present report describes a case of cutaneous protothecosis, as confirmed by skin biopsy and culture, in a 62-year-old man with an erythematous plaque lesion on his back. The disease was initially detected after return from a trip to Taiwan. The characteristic sporangia and biochemical sugar assimilation tests indicated the organism to be Prototheca wickerhamii. Oral administration of (5-FC) failed to have any effect and so finally, the lesion had to be removed by surgical excision.

Senile erythroderma with serum hyper IgE.

Eight patients with senile erythroderma with serum hyper IgE were studied. Characteristic laboratory findings were persistently elevated IgE levels, high peripheral-blood eosinophils, and high lactic dehydrogenase (LDH). Immunologic analysis using immunoglobulin antibodies showed frozen sections of inguinal lymph nodes to stain with anti-IgE antibody, particularly in the germinal center. Five of the patients had an atopic history, suggesting hereditary factors.

[The role of high-resolution CT in evaluating disease of the posterior tympanum].

The posterior tympanum consists of several irregular eminences, ridges, and sinuses. These sinuses do not communicate with the mastoid air cell system, and cholesteatoma and/or granulation tissue prefer to fill them. This area cannot be visualized with ease by the usual surgical approach, and is thus quite important for surgeons. Recent developments in high-resolution computed tomography (HRCT) allow more than ever precise identification of subtle changes in the posterior tympanum. Axial HRCT sections provide essential information for the preoperative evaluation of the posterior tympanum. The performance of surgeons has been promoted both in evaluation and treatment planning of patient with suspected posterior tympanic lesions. HRCT findings were compared with the operative findings in 52 patients (54 ears) who had the operation for chronic otitis media. Important findings by HRCT diagnosis of posterior tympanum are as follows: (1) Bone destruction in the posterosuperior part of the tympanic anulus and in the lateral wall of the facial sinus are the important findings for the diagnosis of invasion of cholesteatoma in the posterior tympanum. (2) Similarly important finding is the bone destruction of the pyramidal eminence. This was found in 6 of 9 cases with cholesteatomas extending into the sinus tympani. (3) The soft tissue density in the posterior tympanum does not necessarily indicate pathological processes. Effusion in the posterior tympanum is imaged as soft tissue density, and can not be differentiated from cholesteatoma or granulation by present HRCT.

[Frequency specificity of ABR evaluated by tuning curves].

A tone on tone simultaneous masking paradigm was used to determine tuning curves of ABR both from the normal and hearing-impaired subjects. ABR tuning curves were constructed to define masker intensity that resulted in a 50% reduction in probe elicited wave V amplitude. The frequency specificity of each probe stimulus was evaluated by Q10, low cut-off slope, high cut-off slope and the maximum masker frequency calculated for the tuning curves. The results were as follows; 1) Q10, low cut-off slope and high cut-off slope increased gradually with the increase in rise time. However, prolongations of the rise time beyond 3 cycles of probe frequency yielded little improvement in Q10, low cut-off slope and high cut-off slope. 2) Q10, low cut-off slope and high cut-off slope for normal-hearing subjects increased gradually with the increase in stimulus frequency or the decrease in stimulus pressure. Maximum masker frequency of the tuning curves was not always equal to the frequency of probe without the 2-kHz. For the 0.5, 1kHz probe, the maximum masker frequency of the tuning curves showed higher values than the frequency of probe. For the 4kHz probe, the maximum masker frequency of the tuning curves showed lower values than the frequency of probe. The results indicate that the tone pip stimuli will allow to assess certain conditions of auditory function at different frequencies, and they show wider frequencies' spread in the cochlea area near stimulus frequencies. 3) For subject with abrupt high-frequency hearing loss (30dB/oct), a pronounced down-ward shift of maximum masker frequency, down-ward shift of high cut-off slope and up-ward shift of low cut-off slope were found when the probe was placed in the region of elevated threshold. For subject with low-frequency hearing loss (25dB/oct), a pronounced up-ward shift of maximum masker frequency, down-ward shift of low cut-off slope were found. Maximum masker frequency, low and high cut-off slope of hearing-impaired subjects were not always equal to those of normal subjects for same probe. Especially the value of the maximum masker frequency shifted to the direction in which the most sensitive frequency was observed in audiogram. The threshold of ABR reflected the cochlea function of the most sensitive area near stimulus frequencies. Greatest predictive error was observed in steeply sloping audiograms.

[Superstructure of stapes: an analysis by high-resolution computed tomography].

High-resolution Computed Tomography (HRCT) of the temporal bone has now become a routine test for the diagnosis of various temporal bone lesions. Correct assessment of such minute structures as ossicles, especially stapes, is important in the pre-operative HRCT evaluation. On the other hand, analysis for the reliability of HRCT findings has not been done, including the superstructure of the stapes. A retrospectively study was done in order to assess the availability of HRCT findings in axial overlapping scans in 226 ears, with respect to the superstructures of stapes. The study was concerned with the analysis of HRCTs of 148 normal ears, 71 ears of chronic otitis media and 7 ears of ossicular abnormalities. HRCT findings were compared to those of surgeries in pathological cases. The present study revealed that the superstructures of stapes are noted in 70% when the stapes surrounded by air. The major limitations in the proper analysis of stapes by HRCT are partial volume averaging and effects of soft tissue silhouetting, this is especially so when the stapes is surrounded by soft tissue density.

[UFT/CDDP treatment in advanced gastric cancer--case report].

In treating advanced gastric cancer cases, 100 mg/m2 of cisplatin (CDDP) was given to such patients by means of a 24 hr continuous iv infusion once a month. This was in addition to daily UFT chemotherapy with an oral administration of UFT at a dose of 200 mg/m2 twice a day before meals. In this paper, two patients who achieved an objective tumor response to this UFT/CDDP chemotherapy are discussed. It was felt that this treatment was not likely to induce either leukocytopenia or thrombocytopenia. With regard to this drug combination, it has been reported that a remarkable, synergistic, antitumoral activity of combined 5-fluorouracil and cisplatin was demonstrated against L-1210 leukemia in BDF1 mice.

Determination of the epinephrine-refractory hypoglycaemic activity of whole-cell pertussis vaccine in mice.

A new assay method has been developed for the quantitative estimation of the inhibitory effect of pertussis vaccine on epinephrine-induced hyperglycaemia in mice. The statistical analysis of the assay was based on logarithm-transformed estimates of the blood glucose levels. The method was sufficiently sensitive to detect the activity of 0.004 millilitre of commercial combined diphtheria-tetanus-whole cell pertussis vaccine. The estimated common variance was as small as 0.0034 and the assay was highly reproducible. Among commercial vaccines there was a significant difference in activity. The activity of a stock pertussis vaccine was inactivated by 5 mM glutaraldehyde at 37 degrees C for 30 min, but resisted treatment with 40 mM formaldehyde at 37 degrees C for 5 days. The extent of inactivation with the chemicals was calculated by a parallel line assay as the activity relative to that of untreated control pertussis vaccine.
