Metastatic tumor of the penis is uncommon. A 62-year-old man was admitted with the complaint of diffuse induration of the penile shaft. Biopsy material of the corpora cavernosa and supraclavicular lymphnode showed metastatic seminoma histopathologically, but no abnormal finding of bilateral testes was seen. Chest X-ray showed a mass at the upper anterior mediastinum. We considered the primary site of this malignant disease to be the mediastinum. The patient died of liver cirrhosis but permission of autopsy could not be obtained.
MRI of bladder cancer was accomplished in 28 patients. All of the 28 patients were simultaneously studied by CT, and 22 were also studied by transurethral US before operation. MRI is not as useful as US for local staging of bladder cancer. However, MRI is more useful for diagnosing extravesical extension and covers the deficit of the overdiagnosing tendency of US.

Myelolipoma of the adrenal gland is a benign oligosymptomatic tumor composed of various proportions of fat and bone marrow elements. We report two operated cases found incidentally in a 76-year-old man and 49-year-old man.
The prognosis of patients with advanced neuroblastoma remains poor despite recent progress in chemo/radiotherapy. Therapeutic trials on the induction of differentiation of neuroblastoma by chemical and biological agents have been attempted to improve patients' prognosis. Recently a new synthetic polyprenoic acid, E5166, having retinoic acid properties, has been described. In this study two human neuroblastoma cell lines, KP-N-RT(LN) and SK-N-DZ, were treated in vitro by E5166. Morphological differentiation of KP-N-RT(LN) and SK-N-DZ cells could be induced by E5166 in the presence of 1.7 X 10(-5) M E5166 for 10 days in culture. Levels of catecholamines (dopamine, adrenaline, and noradrenaline) were not elevated in the E5166-differentiated cells. E5166-induced differentiation may not be cyclic AMP dependent, since levels of cyclic AMP did not increase after exposure of cells to this agent. No significant increase in neuron-specific enolase levels could be demonstrated in E5166-treated neuroblastoma as compared to control untreated cells. E5166 treatment of KP-N-RT(LN) and SK-N-DZ cells was found to inhibit colony formation in soft agar in a dose-dependent manner. Colonies of KP-N-RT(LN) cells in the presence of E5166 showed morphological differentiation as defined by the expression of long neurite processes. E5166 is a less toxic reagent than the retinoic acids used for the induction of differentiation, it can be administered to patients p.o., and the concentration of E5166 which induces the morphological differentiation in vitro can be achievable in vivo. Therefore our study suggests that E5166 could be a useful therapeutic agent in advanced neuroblastoma to differentiate residual anaplastic tumor cells to a benign form (ganglioneuroma) after surgery and chemotherapy.
The annual incidence of neuroblastoma in Japan, a common malignancy of young children which has a very poor prognosis in progressive cases, was previously estimated to be 8.2 cases/million children under 15 years of age. Based on the Japanese National Census of the Population for 1982, the annual number of cases of this tumor is predicted to be 220. (Sawada et al: Med Pediatr Oncol 12:101-103, 1984). Before the mass screening program was started, one-fourth of all neuroblastoma cases were detected before the age of one by clinical symptoms and other findings. Since 1974, a neuroblastoma mass screening program for 6-month-old infants by means of a qualitative Vanilmandelic Acid (VMA) test has been in effect in Kyoto, Japan, for the prognostic improvement of neuroblastoma patients (Sawada et al: Am J Dis Child 136:710-712, 1982). A Neuroblastoma Mass Screening Study Group (NBMSSG) was organized in 1981 (Sawada et al: Lancet ii:271-272, 1984), and the mass screening program has grown to encompass eight selected areas. This group discovered 15 cases of neuroblastoma among 247,500 6-month-old infants, 1 of every 16,500, until the end of 1983, and 23 cases among 434,970, 1 of every 18,900, until the end of 1984. All cases were asymptomatic. As the incidence of infantile neuroblastoma detected by this program is projected to be 52.9-60.6 cases/million 6-month-old infants, it could be predicted that 80-91 neuroblastoma cases will be discovered annually in Japan by mass screening. The cure rate of 25 cases discovered by the mass screening and followed up over 20 months was 92% (Sawada: Lancet i:377,1986). In Japan, it should be possible to detect 135-146 cases--[symptomatic 55 = 220 X 1/4] + [80-91 cases detected by mass screening]--(61.4-66.4% of the total neuroblastoma cases) in infants under 1 year of age annually, and the cure rate of all neuroblastoma cases in Japan is expected to reach 60% or more.
The maximum theoretical quantization noise (shimmer and jitter) for digital recording systems is estimated and compared with normal vocal shimmer and jitter. Nine bits of resolution and 500 samples per cycle are needed to minimize the contaminating sampling noise without interpolation between samples. With interpolation, however, fewer than 100 samples per cycle can resolve jitter down to 0.1%. Low-pass filtering is not harmful unless peak-picking strategies are used, and the peaks are severely broadened by the low-pass filter. A window of at least 20 cycles is suggested, and multiple tokens of an utterance are necessary to obtain a stable mean for perturbation measures.
Two allergenic components, termed J1 and J2, were isolated from a soluble egg antigen preparation (SEA) of Schistosoma japonicum by anion-exchange chromatography on DE52 and gel chromatography on Sephacryl S-200. The apparent molecular weights of J1 and J2 were 260,000 and 46,000, respectively, by gel chromatography on Sephadex G-150. By SDS-polyacrylamide gel electrophoresis, both J1 and J2 showed apparent homogenicity and their estimated molecular weights were 135,000 and 45,000, respectively. The isoelectric point of J1 (pI 4.9) was similar to that of J2 (pI 4.8). Both J1 and J2 bound to Con A-Sepharose 4B, indicating their glycoprotein nature. The amino acid compositions of J1 and J2 have some similarities. However, phenylalanine and leucine, which contain large hydrophobic groups, were dominant in J1, whereas serine and threonine, which contain a hydroxyl group, were dominant in J2. J1 was sensitive to heating or pronase treatment, whereas J2 was rather stable to these treatments. Both J1 and J2 were sensitive to 0.1 M periodate treatment. When mice were immunized with either J1 or J2 with A1(OH)3 as an adjuvant, anti-J1 or anti-J2 IgE antibody was highly specific to the respective antigens. Since S. japonicum-infected mouse serum has high PCA titer to J1 and J2, these two components are the major allergens of S. japonicum eggs.
Antigenicity and specificity of two major allergens termed J1 and J2, both of which had been purified from soluble egg antigen (SEA) preparation of Schistosoma japonicum, were examined against serum from schistosomiasis japonica patients. By using enzyme-linked immunosorbent assay, specific IgE or IgG levels to J1 or J2 in the patients' sera correlated well with those to crude SEA, indicating that J1 and J2 are the major antigens in SEA binding human IgE and IgG antibodies. Binding inhibition studies using 125I-J1 and 125I-J2 clearly showed that cross-reactivity was not present between J1 and J2 in terms of their capacity to bind IgG or IgE antibody. When antibody levels of an individual patient to these two purified antigens were compared, a high degree of correlation was observed between them, regardless of the class of antibody. However, specific IgE antibody levels of individual serum to J1 or J2 were not correlated with IgG antibody levels to the respective antigen.
In the previous study, it was shown that the treatment of human neuroblastoma cells with human interferon-gamma (HuIFN-gamma) induced the morphological changes. However, the treatment with human interferon-alpha (HuIFN-alpha) or -beta (HuIFN-beta) did not induce them. In the present study, the effect of HuIFNs on the overexpression of N-myc of the human neuroblastoma cells (GOTO strain) is examined. The treatment of GOTO cells with rHuIFN-gamma inhibits the overexpression of N-myc, and its degree is dependent on the duration of the treatment. However, HuIFN-alpha and HuIFN-beta did not inhibit the overexpression of N-myc. This suggests that the oncogene N-myc may have relation to the morphological differentiation of human neuroblastoma cells because only the HuIFN-gamma, which induces the morphological differentiation, inhibits the overexpression of N-myc.
Eighty-three cases of renal cell carcinomas admitted to Nara Medical University and its related hospitals from August, 1962 through July, 1984 were reviewed. We have been using computed tomography (CT) and ultrasonography (US) for early detection of renal cell carcinomas since 1980. Thereafter the number of patients with low stage renal cell carcinoma was significantly increased. Furthermore 6 carcinomas were incidentally detected by CT and/or US examination for checkup of other diseases. We believe that CT and US may be valuable as a screening modality for early detection of renal cell carcinoma.
The retrospective analysis of 64 patients between 1 month and 14 years old with 87 primary vesicoureteral units with reflux was reviewed. They were followed-up for a period of 3 to 60 months. Thirty one patients were managed conservatively by chemotherapy (group A) and 28 patients were treated surgically. As the anti-reflux operation the Politano-Leadbetter procedure was used in 27 patients and the modified Politano-Leadbetter procedure in 1 patient (group B). Five patients were not treated (group C). In 13 of the 30 renal units of group A, the vesico-ureteral reflux disappeared spontaneously and in group B the post-operative cystourethrography revealed cure of reflux in all the renal units. In 2 of the 21 renal units of group A excretory urography (IVP) revealed improvement and in 3 deterioration. Post-operative IVP revealed improvement in 13 of 39 renal units of group B and in 2 deterioration. In 3 of the 22 renal units of group A the renogram revealed improvement and in 3 deterioration. The post-operative renogram revealed improvement in 8 of the 28 renal units of group B and deterioration in renal units. Analysis was also made according to the vesicoureteral reflux grade.

An inaudible voice variable, known to be influenced by stress-arousal in adults, was recently discovered to differ significantly among four situationally defined types of infant vocalizations. Other research suggests that jitter in adults also may be altered by stress-arousal. Further, there is a lack of normative data on jitter and shimmer in infant vocalizations. These findings prompted this study comparing F0, jitter, and shimmer among four situationally defined vocalizations ("pain," "hunger," "fussy," and "cooing") of 2-6-month-old infants. Data showed no significant difference in either jitter or shimmer among the four vocalizations; no significant difference in F0, jitter, or shimmer due to the sex or age of infant; a significant difference in F0 among "hunger," "cooing," and either "pain" or "fussy" vocalizations; and no significant difference in F0 between "pain" or "fussy" cries.
In contrast to the amount of information available on acoustic characteristics of adults' effective speech, little data are available on children's vocal effectiveness. A study, therefore, has been initiated to examine the relationship between some acoustic characteristics and listener judgments of children's vocal effectiveness. This article is a sequel to the first one, which reported some acoustic characteristics of oral reading by 36 10- to 12-year-old children. It describes results of perceptual analyses of oral reading proficiency on a five-point scale, of reading error analysis, and results of multiple regression analysis for testing predictability of the oral reading proficiency from the perceptual and acoustical measures. Some sex differences in oral reading proficiency are also discussed.
Eosinophil chemotactic activity was detected in the serum obtained at an acute stage of murine schistosomiasis japonica. Gel filtration of the dialyzable fraction of the serum on Sephadex G25 showed that the chemotactic component had an apparent molecular weight of less than 1,000. It was stable to heating, but was sensitive to pronase or carboxypeptidase A digestions, indicating its peptide nature. Eosinophil chemotactic activity of the dialysate of the serum from mast cell-deficient W/Wv mice infected with Schistosoma japonicum was far less than that from normal littermate +/+ mice, although the titers of specific IgE antibody to soluble egg antigen in the serum measured by passive cutaneous anaphylaxis was comparable between them. These results suggest that at least some part of low molecular weight ECF in the circulation seems to be a ECF-A derived from mast cells. Possible biological significance of circulating ECF in schistosomiasis has been discussed.
