Yolk sac tumor of the testicle: is retroperitoneal lymph node dissection necessary?

We report our experience with 11 cases of yolk sac tumors, which were treated between 1960 and 1982 at the Hôpital Sainte-Justine and the Montreal Children's Hospital. The role of lymphadenectomy in yolk sac tumors is analyzed critically and put into perspective with the use of serum alpha-fetoprotein. A flow sheet is presented for the management of yolk sac tumors. It seems that the indication for lymphadenectomy in yolk sac tumor should be restricted to patients with persistently elevated serum alpha-fetoprotein after inguinal orchiectomy and chemotherapy.

Urolithiasis in pregnancy.

A retrospective analysis of pregnancies complicated by urolithiasis in a major referral center for high-risk pregnancies indicated an incidence of 1:2,534. In contrast to previous series, the majority of patients were primiparas. The judicious use of hydration, analgesics and antibiotic therapy resulted in a high rate of spontaneous passage of stones initially localized below the renal pelvis and a favorable outcome for all pregnancies.

Dermoid cyst of testis in five-year-old-child.

We report the case of a five-year-old child who had a hard painless left testis discovered on a routine physical examination. He underwent a left inguinal orchiectomy, and the pathology examination revealed a dermoid cyst of the testis. This is a rare tumor, considering the low incidence of testicular tumors in childhood. Histologic criteria and surgical management are also presented. We believe this report is the first well-documented case.

Urolithiasis in children.

Between 1971 and 1980, 55 children with urolithiasis were assessed and treated. Pediatric urolithiasis accounted for 1:4090 hospital admissions. Forty percent of patients had no identifiable predisposition to urolithiasis. Eight patients presented with hematuria in the absence of renal colic suggesting that pediatric urolithiasis, although uncommon, is an important cause of painless hematuria.

Pyeloplasty: to divert or not to divert?

The use of diversion in conjunction with pyeloplasty has been a matter of continuous debate for a number of years. In consequence the matter has been left to the urologist's individual preferences with no objective data to support these preferences. Herein the authors present an analysis of 105 pyeloplasties performed by four surgeons over a period of eighteen years with a view to determine the place and type of diversion in relation to the surgical technique used and the long-term effects of such diversion or abstinence thereof.

Cystine calcium bladder calculus in a 2-year-old child.

We report on a 2-year-old child who presented with urinary tract infection and a large solitary bladder calculus. Dietary history indicated an intake of 2 gm. calcium with supplemental vitamin D daily. Urinary amino acid analysis revealed cystinuria. The high dietary intake of calcium and vitamin D may have precipitated early cystine calcium stone formation.

[Renal and perinephritic abscesses in children].

One child with a pure perinephric abscess and three with renal abscesses, one of which had perinephric extension, are described. All presented with a long course of subacute infection leading to localizing symptoms or signs in the flank. The diagnosis was confirmed by radiologic examination. All the abscesses were surgically drained at various intervals after diagnosis, while the patients were receiving antibiotic therapy. Salvage of renal function was possible in all cases. A rational approach to the diagnosis and management of such abscesses is emphasized.

Avulsion injury to the renal pedicle: successful autotransplantation after "bench surgery".

Avulsion injury to the renal pedicle from blunt trauma is a common injury in children. We report a patient who survived this injury with successful reimplantation of the kidney after bench surgery to trim the vessels and the ureter. The function of this kidney is somewhat impaired and shows posttraumatic scarring 19 months post-surgery. This appears to be the first report of such a patient in the literature.

Long-term assessment of genital reconstruction in female pseudohermaphrodites.

The cases involving 32 women who had undergone early revision of ambiguous genitalia were reviewed 5 to 22 years postoperatively. Seven clitoridectomies, 25 clitoral recessions, 17 Y-V introitoplasties and 10 midline episiotomies were performed. There were 16 patients available for followup. The results of clitoral recessions were cosmetically and functionally satisfactory. Introital revisions done before puberty all required further revision and suggested that vaginoplasty should be deferred until after puberty.

[Role of urinary diversion in pyeloplasty (author's transl)].

A retrospective study was made of 105 pyeloplasties in 103 patients over a period of 18 years with the aim of precisely determining the role of urinary diversion in the post-operative period by nephrostomy tube and/or ureteral splint. A group of patients undergoing the same type of procedure, but without diversion, served as controls. Short-term results are evaluated clinically and radiologically and long-term results are evaluated by review of X-ray charts. These results were submitted to statistical analysis that have helped to demystify the role played aby diversion in pyeloplasties.
