B-cell malignancy and monoclonal gammopathy, and idiotype of cell surface and serum immunoglobulin.

We studied cell surface membrane immunoglobulins (SmIg), intracytoplasmic Ig (CIg) and monoclonal Ig (M protein) in patients with the B-cell series malignancies; malignant lymphoma, chronic lymphocytic leukemia (CLL), Waldenström's macroglobulinemia (WM), multiple myeloma (MM), and monoclonal gammopathy (MG) associated with Sjögren's syndrome (SS). The patients were examined for idyotype (Id) determinants on the cell surface and in the cytoplasm of malignant cells as well as serum M proteins. Twenty-two patients had malignant lymphoma with monoclonal SmIg. The malignant cells of 11 patients contained CIg; four of these 11 patients had a small amount of serum M protein. SmIg of the IgM class was most frequent (12/22) and the lambda type light chain was predominant (kappa/lambda ratio = 4/8). By means of rabbit anti-Id antisera prepared against M proteins, Id determinants were demonstrated on the cell surface and in the cytoplasm of malignant cells as well as in the serum M proteins, indicating that the malignant cells proliferated monoclonally producing monoclonal IgM on the cell surface and in the cytoplasm and secreting it to the serum. Six CLL patients were included in our series; one manifested monoclonal IgM in the serum. The Id determinants of M protein was detected on the cell surface and in the cytoplasm of pathologic cells in the peripheral blood. Eight of ten WM patients were examined for SmIg; all had monoclonal SmIg on the surface of cells from peripheral blood or bone marrow. Id determinants were detected on the cell surface and in the cytoplasm of malignant cells in four WM patients. There were 42 patients with MM; 10 of them were examined for SmIg and three of these had monoclonal SmIg on plasma cells derived from the bone marrow. By immunofluorescent and mixed rosette methods, Id-bearing B (0-20%) and T cells (0-2%) were identified in the peripheral blood of MM patients who had no plasma cells in the peripheral blood. Our findings suggest that a monoclonal change may occur at the B-cell level and that Id-bearing T cells may exist as regulatory T cells. Ten SS patients had M proteins. Non-IgM class M proteins (2 IgG and 4 IgA) were more frequent than the IgM class (3 IgM). Four M proteins with rheumatoid factor (RF) activity were found among these 10 M proteins.(ABSTRACT TRUNCATED AT 400 WORDS)

Cefroxadine (CXD) capsules and dry syrup, an oral cephem antibiotic, were administered into 120 cases with ocular infections and the following results were obtained: The daily dose of CXD capsule was ranged from 500 to 1,500 mg and that of CXD dry syrup from 17.9 to 85.7 mg/kg, and the duration of CXD administration was from 2 days to 14 days. Clinical response rates classified by diagnosis The clinical response rates were 77.8% (14/18) in blepharitis, 86.7% (26/30) in hordeolum, 62.5% (5/8) in meibomianitis, 74.6% (44/59) in conjunctivitis, 100% (2/2) in corneal infiltration, 100% (1/1) in cellulitis of the lid, in dacryocystitis and in corneal ulcer, respectively. Clinical response classified by isolated organisms The response rates on S. aureus were 80.0% (20/25), on S. epidermidis 75.8% (47/62) and on S. pneumoniae 66.7% (2/3), respectively. The overall clinical response rate on Gram-positive bacteria was 78.3% (94/120). The response rates on H. influenzae, Acinetobacter spp., P. mirabilis, E. coli and Moraxella spp. were ranged from 42.9 to 100%. The sensitivity distributions of clinically isolated S. aureus and S. epidermidis to CXD were ranged from 1.56 to greater than 100 micrograms/ml and from 0.39 to 12.5 micrograms/ml, respectively. The former showed a peak at 3.13 micrograms/ml and the latter in 1.56 micrograms/ml. Side effects in 3 cases (2.3%) out of 129 were observed. That is; glossitis, thirst feeling and palpitation in each case, respectively.

Protection by nicotinamide against streptozotocin-induced reduction in pancreatic TRH.

The concentration of thyrotrophin-releasing hormone (TRH) immunoreactivity was determined in pancreatic islets and acini in the rat. In addition, time-course changes in TRH in response to an iv injection of streptozotocin (65 mg/kg body weight) with or without nicotinamide (500 mg/kg body weight) were examined in the whole pancreas. Furthermore, pancreatic TRH was measured in diabetic rats treated with insulin for 3 weeks. The TRH concentration in rat islets was 42-fold higher than in exocrine glands, indicating that the majority of pancreatic TRH is of islet origin. The mean concentration of pancreatic TRH decreased to 60 and 65% of the respective control values at 4 and 7 h after administration of streptozotocin, respectively. AT 24 h, it fell to 10% of control values without significant changes in TRH levels in the hypothalamus and gastrointestinal tract. In contrast, no significant change in pancreatic TRH was noted in rats given combined treatment with streptozotocin and nicotinamide. The injection of streptozotocin alone resulted in severe hypoglycaemia at 7 h and hyperglycaemia at 24 h, whereas neither resulted from the combined treatment. Insulin therapy had no influence on the decreased TRH concentrations in the diabetic pancreas. These results suggest that TRH may be localized to the B cells of pancreatic islets, and that the marked reduction in TRH in diabetic pancreases is not a metabolic consequence of insulin deficiency.

Laryngeal narrowing measured with low frequency sound.

We measured laryngeal resistance with 800 Hz sound in human subjects. 800 Hz sound was forced into the mouth and the intensities of sound above and below vocal cord were detected by two microphones on the anterior neck. Respiratory resistance (Rrs) was simultaneously measured by an oscillation method. When the subject voluntarily controlled the glottis aperture at functional residual capacity, the percent increase of difference of intensity of sound between two microphones (Y %) was curvilinearly proportional to the increase of Rrs (X) from the control state; Y = 20.8 X X 1.24. Since Rrs below the larynx was constant at a given lung volume, we could estimate the increase of laryngeal resistance from the control state by knowing the percent increase of difference of intensity of sound. The present method would be clinically applicable to measure Rrs independent from laryngeal artifact.

[Study of nickel-chromium alloys containing titanium for porcelain bonding (author's transl)].

In order to analyze the effects of titanium and chromium and to determine the best composition of nickel-chromium alloys containing titanium for porcelain bonding, the mechanical properties, the tarnish resistance, and the porcelain bond strength of each of sixteen experimental alloys which were composed of nickel (54 to 90 wt%), chromium (10 to 40 wt%), titanium (0 to 6 wt%) were studied. The values of tensile strength, proportional limit, and Vickers hardness number for the alloys ranged from 33.4 to 68.1 kgf/mm2, 9.2 to 67.6 kgf/mm2, and 109 to 504 respectively. The increasing amount of titanium and/or chromium tended to the increase of those values. The elongation was higher than 24% for the alloys containing no titanium, but decreased to 0.3% with the increase of titanium and/or chromium content. The tarnish resistance for the alloys which contained only 10% chromium was not good. The porcelain bond strength of the alloys increased very significantly as the titanium content and/or the chromium content increased. In this study, the compositions of 58 to 76% Ni - 20 to 40%Cr - 2 to 4%Ti were considered to be optimum for porcelain bonding.

Somatostatin concentration responds to arginine in portal plasma: effects of fasting, streptozotocin diabetes, and insulin administration in diabetic rats.

Changes of somatostatin concentration in response to a single i.v. injection of arginine (400 mg/kg body weight) were examined in extracted portal plasma of normal and diabetic rats in the fully fed state and after 24 h or fasting, as well as in diabetic rats treated with insulin for one week. In both normal and diabetic animals fasted for 24 h, the basal level of somatostatin declined but the magnitude of the arginine-induced elevation of somatostatin was not affected, suggesting a physiologic role of the tetradecapeptide in nutrient homeostasis. When compared with intact rats, diabetic animals were shown to have increased levels of somatostatin before and after arginine administration, both of which were attenuated by insulin replacement therapy. These findings suggest that alterations of D cell function in streptozotocin diabetes may be related to either insulin deficiency or its metabolic consequences.

Isolation of a new peptide antibiotic, permetin A, from Bacillus circulans.

Permetin A was purified from the culture filtrate of Bacillus circulans AJ 3902 by extraction with n-butanol, precipitation with sodium helianthate, CM-cellulose column chromatography and Sephadex LH-20 column chromatography. The compound was found to be a new peptide antibiotic containing 2,4-diaminobutyric acid (Dab), leucine, isoleucine, phenylalanine, valine, serine (in a molar ratio of 3:2:1:1:1:1) and a fatty acid. This antibiotic showed activity in vitro against Gram-negative, Gram-positive and some anaerobic bacteria.

Determination of redox potential levels critical for cell respiration and suitable for L-leucine production.

The effect of oxygen supply on L-leucine fermentation was investigated employing a leucine-producing mutant of Brevibacterium lactofermentum. Since it was not possible to measure oxygen tension below 0.01 atm by a Teflon-coated oxygen electrode, the degree of satisfaction of the cells' oxygen demand (cells' respiration rate/maximum oxygen demand of cells, rab/KrM) and the redox potential of the culture medium (E, mV) were used as indices to oxygen supply in cultures under low oxygen tension. When the oxygen demand of the cells was satisfied (rab/KrM = 1.0) and the E value was between --90 and --110mV, L-leucine formation was 26.5 mg/ml. When the oxygen demand of the cells was not satisfied (rab/KrM = 0.85) and the E value was between --200 and --220 mV, L-leucine accumulation was 29.7 mg/ml. When the oxygen supply was extremely limited (rab/KrM = 0.27) and the E value was --280 mV, L-leucine formation was 12.9 mg/ml. A new method which simultaneously measures the redox potential and dissolved oxygen was applied to the determination of the critical dissolved oxygen level for cell respiration (PL crit), whick was too small to be detected by conventional oxygen electrodes. The value of PL crit of the leucine producer was estimated as 0.0002 atm.

A comparative study of the contraceptive mechanism of the Cu-intrauterine device and the plastic-intrauterine device in rats.

After inserting the Cu-IUD and plastic-IUD into the horn of the uterus of rats, vaginal smears were daily observed for twelve estrus cycles, but no abnormal changes were noted. The female rats with the IUD were exposed to impregnation placing together with male rats. Neither the Cu-IUD nor the plastic-IUD affected copulation, and a complete contraceptive result was obtained in these rats. There were no significant differences in any effect examined between Cu-IUD and plastic-IUD groups. In order to investigate the influence of copper on the penetration of sperm, we placed the rats with the IUD under direct observation and then exposed them to natural copulation. By examining various sites of the reproductive organs 30 min after copulation, the hindrance of sperm migration by the Cu-IUD was confirmed.
