Developmental and behavioral effects of maternal alcohol exposure in rats.

Fostered rat offspring whose mothers had been administered 7.5% alcohol solution (4.85 g/kg/day) as drinking fluid during 0-17 days of gestation were compared with equivalent groups whose dams had given ad libitum and restricted volume of tap water in the following behavioral tests: reflex test, swimming behavior test, open-field test, radial-maze learning and two-way avoidance learning. The treatment with alcohol resulted in developmental delay in ear-flaps uncurling, placing reflex, swimming behavior, walking in addition to reduction in body weight at 1 and 7 days of age. The treated group also exhibited decreased open-field activity and deficit in the avoidance learning. The results show that maternal alcohol consumption produces developmental and behavioral deficits in offspring, which are comparable to the fetal alcohol syndrome (FAS) in man.

Post-column derivatization of guanidino compounds in high-performance liquid chromatography using ninhydrin.

A new method for the high-performance liquid chromatography of guanidino compounds using ninhydrin as the fluorescence reagent is described. Use of organic solvent is not required since ninhydrin is highly soluble in aqueous media, and the problem of precipitation formation occurring in the phenanthrenequinone method was avoided. Creatine was also assayed in the present procedure. Separation of ten guanidino compounds was completed within 30 min using a small-size (38 X 4.2 mm I.D.) strong cation-exchange column.

[Clinical studies on tobramycin by intravenous drip infusion for Pseudomonas aeruginosa infection in chronic respiratory diseases (author's transl)].

Tobramycin (TOB) was administrated by intravenous drip infusion for 1 hour, to 15 patients with intractable chronic respiratory tract infections due to P. aeruginosa, and the following results were obtained. 1. Criteria of clinical effects We have made new criteria for the evaluation of the therapeutic effects on respiratory tract infections (Tables 1, 2). Clinical items were divided into 4 grades and radiographic ones into 6 to 11 grades. Each item was separately scored as followed; excellent was scored as 1 point when 3 or more grade upward-shifts were seen after treatment, good as 2 points in case of 2 grade upward-shifts, fair as 3 points in case of 1 grade upward-shift, unchanged as 4 points in case of no shift, impairment as 5 points in case of downward-shift. The integrated evaluation of the items was made according to the average points for the evaluated items; overall excellent was 1.9 or less, overall good 2.0 to 2.9, overall fair 3.0 to 3.5 and overall poor as 3.6 or more. 2. Clinical effects Overall clinical effects were evaluated as good in 7 cases, fair in 4 and poor in 4 out of 15 patients, and rate of good score was 46.7%. According to the dosage groups, rate of good score was 50% in the dose of 60 mg three times a day, 0% in the dose of 90 mg twice a day and 62.5% in the dose of 120 mg twice a day. Eradication rate of P. aeruginosa was 33.3%. Neither adverse side effects nor abnormal laboratory findings were observed.

Complications of augmentation rhinoplasty in the Japanese.

Augmentation rhinoplasties in our clinic were surveyed for complications during the five years 1974 to 1978. Our finding of three main complications--malposition, a too-large or too-high prosthesis, and perforation--is reported here. Prevention of such complications is also discussed.

[Clinical study of cefoxitin in chronic respiratory tract infections: evaluation in poorly responding cases to other antibiotics (author's transl)].

The therapeutic efficacy of cefoxitin (CFX) in chronic respiratory tract infection was evaluated in patients who poorly responded to other antibiotics. To 20 patients, CFX was administered 2 g b.i.d. intravenously by drip infusion. Clinical efficacy was judged based on the criteria by score. 1. Bacterial elimination rate with CFX was 73.7%. 2. A clinical cure rate was 80.0% was obtained by doctors in charge. 3. According to the score assessment, the overall clinical effectiveness rate was 60.0%, clinical symptom cure rate was 85.0% and improvement rate of X-ray findings was 55.0%. 4. Usefulness rate which was assessed by clinical effect and side effect was 70.0%. 5. No side effects and abnormal laboratory findings were observed in this study. We used the new antibiotic CFX in patients with chronic respiratory tract infections who responded poorly to other antibiotics and obtained satisfactory results. Especially CFX indicated more effective possibility in cases from whom Gram-negative bacilli was isolated.

Diagnostic routes in cases with single involvement in eyes suspected of sarcoidosis.

The ocular involvement of sarcoidosis is particularly important in Japan, because 1. the frequency of the eye involvement is as high as 20 to 40%, higher than in other countries- 2. the presence of this involvement proved to be an influential factor relating to the whole sarcoidosis lesions of a patient, and 3. the patients with this involvement will become blind unless adequately treated. In diagnostic routines of ocular sarcoidosis, chest X-rays will give the most useful information of the disease. If the chest X-ray findings are normal, it is not easy to make a diagnosis of a case whose ocular findings are very suspicious of sarcoidosis. This paper will give several case experiences in which we could successfully make a diagnosis, using mediastinoscopical biopsies though the X-rays had no findings.

A local outbreak of sarcoidosis in Northern Japan.

The annual chest X-ray surveys conducted in Hokkaido during a 5 year period from 1972 to 1976 disclosed that the Furano basin located in a central mountainous district had an extremely high discovery rate of the cases with sarcoidosis. Miniature films sized 70 x 70 mm were ready by two or more sarcoidologists. The cases suspicious of sarcoidosis were requested to attent nearby general hospitals for histological examinations. The number of the cases diagnoses as sarcoidosis were 16 out of about 30000 examinees in the Furano basin, 2 out of about 37000 in Sapporo City, the capital of Hokkaido, and one out of about 100000 in other 7 areas. The Furano basin had a significantly higher rate of discovery than Sapporo City or the other 7 areas. Due to the rural localisation, 6 of the 16 Furano cases were confirmed by biopsies, but the remaining 10 were carefully diagnosed with clinical and laboratory findings as they could not come up to general hospitals. Three of the Furano cases belonged to 3 generations in a family. Even if only the 6 histologically confirmed cases were observed, the rate in Furano is still higher than in the other two districts. Thus an intensive epidemiological investigation has been conducted in this district.

Parenteral treatment of sarcoidosis with triamcinolone.

The parenteral use of triamcinolone acetonide (Kenacort A) is said to have the following advantages: to keep the effect for two weeks by one injection, to be able to reduce the total treatment dosage and to secure the dosage regardless of patients cooperation. From these reasons, a trial of the Kenacort A treatment was made for 40 cases with sarcoidosis by 7 times of intramuscular injection at two-week intervals: 80 mg each at the 1st and 2nd and 40 mg at the 3rd to 7th. The results were compared with the already reported double blind trial using the oral initial dosage of 30 mg of prednisolone followed by a a reduced dosage for a period of 6 months. This paper will give an interim report of the 4 month follow-up after the ceasation of treatment. The results showed: 1. the disappearance rate of chest findings, either BHL or parenchymal pulmonary lesions, was higher in this trial than the above-mentioned oral treatment, 2. the high rate of improvement was found in ocular lesion, and 3. side effects were less in this trial than the oral treatment.
