Submucosal hematoma producing mechanical obstruction in the sigmoid colon, complicated by systemic amyloidosis.

A case of systemic amyloidosis involving the gastrointestinal tract is presented. The initial manifestation of this case was mechanical obstruction. On laparotomy, a submucosal hematoma of the sigmoid colon which completely obliterated the lumen, was found. With intense medical treatment, the obstructed lumen became patent, but segmental ischemic colitis ensued. The terminal course of this case was complicated by chronic renal failure, upper gastrointestinal bleeding and coagulopathy. Pathological examination of the stomal specimen revealed massive amyloid deposits in the wall of the large intestine as well as other vital organs.

A clinical study of long-term administration of dextran sulfate (MDS Kowa) in patients with retinopathy.

We administered MDS at a daily dose of 1,800 mg to 25 patients with diabetic retinopathy for periods ranging from 12 to 48 months (mean: 28 months) and achieved the following results: 1) During the administration period total cholesterol, triglyceride and fibrinogen levels were all maintained at significantly lower levels. 2) Eye fundus findings showed an improvement in 38.9% no change in 47. 2% and a deterioration in 13.9% of the cases. 3) No side-effects or abnormalities in clinical findings were observed in any of the cases even though clinical tests of hepatic function, renal function, blood chemistry, and urinalysis were conducted. The above results demonstrate that MDS has a marked effect on diabetic retinopathy, especially in cases with moderate to severe symptoms during the non-proliferative stage.

Significance of abnormal gastrointestinal gas in systemic amyloidosis.

Nine cases of amyloidosis involving the gastrointestinal tract were reviewed for the presence of abnormal gastrointestinal gas by taking plain roentgenograms of the abdomen (PRA) as a diagnostic adjunct. Findings were compared to data obtained by conventional barium study or endoscopic examination. PRA revealed marked gaseous dilatation of the gastrointestinal tract simulating intestinal obstruction in three cases and a moderate or mild increase of gas in two cases each. Abnormality was thus found in seven of the nine cases by PRA. Conventional barium study by means of upper gastrointestinal series, small bowel series and barium enema showed abnormal findings in five of seven cases. Endoscopic examination of the stomach and distal colon disclosed mucosal abnormality in three of seven cases. The rate of abnormal findings detected by the three methods cannot easily be compared to each other because of the differences in symptoms and treatment of individual patients. Nevertheless, PRA seem to be valuable in demonstrating a high rate of abnormalities in amyloidosis.

Treatment of postoperative respiratory distress syndrome.

We have studied 45 patients with postoperative adult respiratory distress syndrome (ARDS) who were treated by mechanical ventilation during the last four years. This period was divided into two periods, and the mortality and progress after treatment were analysed. The overall, mortality was 56%. In the first period this rate was as high as 76%, while in the second period this rate improved to 43%. This improvement in the second period was thought to have resulted from aggressive cardiorespiratory treatment and the diminution of infection. According to the course of ARDS after treatment, four types could be classified. Type 1 showed rapid improvement in respiratory function. Type 2 showed gradual improvement. Type 3 showed relapse of respiratory failure. Type 4 resisted mechanical ventilation. Patients of types 3 and 4 had extremely poor prognoses. Stricter management to avoid infection, specific treatment of multiple organ failure (which was seen frequently) seemed advantageous. High frequency positive pressure ventilation (HFPPV) may have some role in improving the respiratory function of the patients with ARDS.

Gastric cancer in a general hospital in Japan.

During 20 years from 1971 to 1990, 605 patients with gastric cancer were treated by gastric resection at a medium-size general hospital by one surgeon. The overall 5-year survival rate was 61.9%. The 5-year survival rates in the first 10 years (n = 261) and the second 10 years (n = 344) were 53.7% and 67.9%, respectively (P < 0.01). In the second 10 years there was a significant increase in the patients with early gastric cancer (P < 0.01). Meanwhile, operative procedure or R2 or R3 lymph node dissection did not show any improvement of the 5-year survival rates. These results indicate that the prognosis for gastric cancer was relatively good at a general hospital which does not specialize in cancer research, and that much of the improvement in the prognosis could be attributed to the increase in the incidence of patients with early gastric cancer.
