Heterogeneous phenotypes of mitochondrial encephalomyopathy in a single kindred.

Five patients with mitochondrial disorders in a single family showed marked heterogeneity of clinical signs and symptoms. Two patients had the syndrome of mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes; one had blepharoptosis, seizures, and diabetes insipidus; and two had a nonspecific encephalomyopathic disorder. This family supports the concept of a "mitochondrial cytopathy."

Immunoreactive neuromedin B and neuromedin C: distribution and molecular heterogeneity in rat and human tissue extracts.

Immunoreactive neuromedin B and neuromedin C were characterized and measured in rat and human tissue extracts, using radioimmunoassay combined with gel filtration and high performance liquid chromatography in order to clarify tissue distribution and molecular structure. There are two distinct systems of bombesin-like peptide found in rat and human tissue. The neuromedin B family, including neuromedin B and big neuromedin B, is a major bombesin-like peptide in the brain; the neuromedin C gastrin-releasing peptide family is found mainly in the alimentary tract. Neuromedin B is the major and big neuromedin B the minor constituent of central nervous neuromedin B immunoreactivity. Gastrointestinal neuromedin C immunoreactivity is composed chiefly of neuromedin C in rats, and two N-terminally extended forms of neuromedin C including gastrin-releasing peptide in man. It is probable that molecular heterogeneity reflects species variation.

Quantification of disease activity in patients with ulcerative colitis.

To develop an efficient index for evaluating ulcerative colitis, we quantitatively assessed disease activity by multiple regression analysis. Thirty-two consecutive cases with ulcerative colitis were studied. These cases were divided by degree of severity into three groups: mild, moderate, and severe; this tripartite grouping was used as a dependent variable. As independent variables, 11 predictors were selected from among physical findings and laboratory data. By multiple regression analysis with stepwise deletion, six factors indispensable to the disease activity were selected: temperature, pulse rate, erythrocyte sedimentation rate, red blood cell count, alpha 2-globulin, and number of bowel movements per day. Disease activity is predicted by using the following equation: Y = -12.168 + 0.333X1 + 0.022X2 + 0.018X3 - 0.337X4 + 0.056X5 + 0.033X6, where X1, X2, X3, X4, X5, and X6 denote temperature, pulse rate, erythrocyte sedimentation rate, red blood cell count, alpha 2-globulin, and number of bowel movements per day, respectively. By applying this equation to each case, the range of activity indices is expressed as follows: mild, less than 1.52; moderate, between 1.52 and 2.44; and severe, more than 2.44.

Inhibition of IgE antibody formation by n-pentyl alpha-L-sorbopyranoside.

Effect of n-pentyl glycosides and alkyl alpha-L-sorbopyranosides on IgE antibody formation in rats and mice were investigated. When n-pentyl alpha-L-sorbopyranoside was given subcutaneously or orally, the IgE antibody formation in rats and mice was suppressed, while no suppression of hemagglutinin formation was observed. The study on timing of administration indicated that n-pentyl alpha-L-sorbopyranoside significantly suppressed the secondary IgE antibody response when administered before the secondary immunization.

[Gastric diverticulum invaded by carcinoma].

Diverticulum of the gastric body is uncommon and the coexistence of this lesion and gastric carcinoma much more. The authors report a case of diverticulum and carcinoma of the gastric body in a 66-year-old woman. The partially resected stomach exhibited a diverticulum, 3 X 3 X 2 cm in size, on the lesser curvature and a carcinoma of the depressed type on the posterior wall. The latter was IIc + III-like advanced carcinoma showing poor differentiation histologically and partly invading the diverticular wall, which had no muscular layer (i.e., pseudo-diverticulum). As for pseudodiverticulum, carcinoma cells invading the submucosa will reach the serosa easily. It is important not to overlook carcinomas in or around the diverticulum.

A case of short bowel syndrome in which remarkable amelioration of mental deterioration was achieved by administration of trace elements.

A patient with short bowel syndrome manifesting psychiatric deterioration was demonstrated. Four years after extensive small bowel resection the patient developed various psychological manifestations. Laboratory data did not indicate the specific causes but showed extensive hyponutritional state. After 2 weeks therapy by total parenteral nutrition (TPN), laboratory data returned to normal, but her psychiatric condition remained almost unchanged. When trace elements (copper, cobalt, manganese and zinc) were added to TPN, her psychiatric impairment rapidly improved. Although the mechanism involved in this situation may be multifactorial, the trace elements were primarily responsible for recovering from abnormal psychiatric condition.

Remarkable improvement of clinical status in a patient with multiple lymphomatous polyposis of the gastrointestinal tract after repetitive chemotherapy.

The effect of repeated courses of chemotherapy on gastrointestinal tumors seen in a 43 year-old male patient with multiple lymphomatous polyposis involving the entire gastrointestinal tract was presented. Numerous polypoid lesions from the stomach to the rectum were the characteristic finding in this case. The biopsied specimens from polyps either of the stomach or large intestine showed diffuse lymphocytic accumulation in the submucosa, which is consistent with multiple lymphomatous polyposis of the gastrointestinal tract. Remarkable regression of the large masses in the gastrointestinal tract was obtained by two courses of VEMP therapy, along with improvement of hypoalbuminemia and a positive CRP. Recurrence of gastrointestinal masses in the cecum and rectum was also eliminated by six courses of CHOP therapy, and a total of nine courses of CHOP therapy led to complete disappearance of masses in the gastrointestinal tract. The present case is different from the others in terms of the following viewpoints that first the ensuing large masses favorably responded to repetitive chemotherapy, secondly the histopathological findings remained benign despite the fact that large nodular masses had recurred in the cecum and rectum, and thirdly the pathological changes were still confined to the gastrointestinal tract without developing systemic malignant manifestations.

[Evaluation of pancreatic ductal decompression procedure for patients with advanced chronic pancreatitis, mainly from the viewpoint of pancreatic ductal changes].

Several pancreatic ductal decompression procedures have been applied for chronic pancreatitis. Unresolved problems, however, remain, i.e., to select the appropriate procedure among several different decompression procedures, to investigate the changes of pancreatic endocrine and exocrine functions resulting from such procedures, etc. In this paper we evaluated these problems, mainly by comparing conditions of the pancreatic duct as seen with pre- and post-operative endoscopic retrograde pancreatography (ERP). We classified the pancreatic duct of patients with chronic pancreatitis into seven types. This classification was devised so that the selection of the suitable operative procedure could be made. The severity of the abnormal ductal change was also evaluated. Eighteen patients were studied with reference to the post-operative change in the pancreatic duct and the patency of the pancreaticoenteral anastomosis in each procedure. Postoperative ERP showed good patency in the patients with lateral pancreaticojejunostomy and modified bilateral pancreaticojejunostomy, both having long anastomotic stoma. Among the findings of postoperative pancreatic ductal changes, the dilatation of the main duct was reduced in most of the patients, while the stenosis of the main duct remained unchanged in nearly all the patients. The duct in the tail area showed the best results, which suggested the importance of preservation of the pancreatic tail.
