[Antihypertensive effect of arotinolol (S-596), a new adrenergic beta blocking agent, on experimental hypertension].

Effects of a new adrenergic beta-blocking agent, arotinolol (S-596), on the blood pressure and heart rate were assessed in comparison with those of other beta-blocking agents in deoxycorticosterone acetate (DOCA)-saline induced and spontaneously hypertensive rats (SHR). The relationship between the antihypertensive effect and the beta- or alpha-adrenoceptor blocking action of S-596 was also investigated in normotensive conscious rats. In the rat, a cannula was implanted chronically in a femoral artery, from which blood pressure was recorded. The test drugs were administered orally once a day for 14 days at several dose levels. The development of hypertension in DOCA-saline treated rats was clearly retarded with the consecutive oral administration of propranolol (100 mg/kg/day) and hydrochlorothiazide (10 mg/kg/day), but not with S-596 (20, 50 and 100 mg/kg/day) or pindolol (10 mg/kg/day). On the other hand, in SHR, S-596 (more than 10 mg/kg/day) propranolol (50 mg/kg/day), pindolol (10 mg/kg/day), labetalol (100 mg/kg/day) and hydrochlorothiazide (10 mg/kg/day) produced definite antihypertensive effects after the chronic administration. In normotensive conscious rats, the vasodepressor responses induced by isoproterenol were reduced by the beta-blocking agents at lower dose levels than those required for development of antihypertensive effects. The acute effects on blood pressure were determined in hypertensive rats during the chronic treatment with the test drugs. In either type of hypertension, S-596, (10-50 mg/kg/day) showed a depressor effect at 4 and/or 8 hr after administration. In normotensive conscious rats, S-596 antagonized the pressor responses to phenylephrine at doses more than 30 mg/kg. It is therefore suggested that an adrenergic alpha-blocking property is at least partly involved in the hypotensive effect of S-596 as labetalol. In the experiment of acute effect in SHR, pindolol and labetalol showed prominent hypotensive effect after the 1st administration, but lesser effect after the 10th administration. Propranolol showed a marked rise in blood pressure in this experiment.

Adenoid cystic carcinoma of lacrimal gland.

Adenoid cystic carcinoma of the lacrimal gland is a relatively rare disease. Only two cases, a man of 61 and a woman of 54 were seen in our clinic from 1962-1981. They presented histopathology of the cribriform pattern characteristic of adenoid cystic carcinoma. The lesion in the male patient belonged to the solid type and was mainly composed of solid nests. It is emphasized that radiotherapy in addition to surgical excision of the tumor is recommended because of its infiltrative nature.

[Clinical study on preoperative chemotherapy of primary breast cancer. 2--A comparative study of CPA + FT-207 (5-FUDS) and CPA + FT-207 (5-FUDS) + MMC].

In 41 cases of primary breast cancer preoperative treatment was performed using 2 methods consisting CPA + FT-207 (5-FUDS) (for Group I) and CPA + FT-207 (5-FUDS) + MMC (for Group II) to determine clinical and histological efficacies. A daily dose of each anticancer drug was: CPA 50-200 mg, FT-207 200-600 mg, and 5-FUDS 200 mg orally, and MMC 4-20 mg intravenously. The mean total doses were 1.8 g, 6.3 g, 3.4 g and 26.3 mg, respectively. Reduction in tumor size was obtained in 11 cases (37.9%) in Group I and 6 cases (50.0%) in Group II. According to Ohboshi's criteria, histological efficacy as defined over Grade II a was seen in 5 cases (17.2%) in Group I and 6 cases (50.0%) in Group II, while the efficacy classified as Grade III was not seen in any of the cases. Although the clinical effect was not always consistent with the histological effect, there was a tendency of agreement between them in Group II. As to the dosages of anticancer drugs, more effective cases were seen when dosages of more than 25 mg/kg of CPA, 80 mg/kg of FT-207 (5-FUDS) or 0.5 mg/kg of MMC were used. Reduction in tumor size began to appear at 2 to 3 weeks after the initiation of treatment.

[Basic and clinical studies on ceftazidime in the field of obstetrics and gynecology].

Ceftazidime (CAZ), a new cephalosporin antibiotic, was studied in the field of obstetrics and gynecology, and the following results were obtained. The absorption and tissue penetration of CAZ into intrapelvic genital organs were good after a single drip infusion of 1.0 g for 30--60 minutes. The maximum level of 76.4 micrograms/ml was obtained in uterine artery serum at 8 minutes after administration. The high concentrations were also obtained in genital tissues; the maximum concentrations ranged from 46.8--62.1 micrograms/g at 20 minutes after administration and the levels were as high as 2.1--7.7 micrograms/g at 5 hours and 40 minutes after administration. The concentration curves in tissues were consistent with those of serum levels. The concentrations of CAZ in retroperitoneal dead space exudate were determined after intravenous drip infusion of 1 g. The peak levels ranged from 26 to 32 micrograms/ml after 30 minutes of administration and the level of 8.53 micrograms/ml was sustained even 6 hours later. Good response was obtained in cases of gyneco-obstetric infections such as intrauterine infection, intrapelvic infection and external genital infection with daily dose of 2--4 g. CAZ was effective in 13 out of 14 cases (the efficacy ratio; 92.9%). As to side effects, gastric discomfort and vomiting were observed in 1 case.

[Enzymatic conversion of tegafur in human tumor tissue].

Enzymatic conversion of tegafur was studied using a thymidine phosphorylase preparation purified from human liver cancer. Tegafur dissolved in water slowly decomposed to 5-FU upon storage at 37 degrees C and this conversion was further activated with an addition of a thymidine phosphorylase preparation. Analysis of Tegafur and 5-FU was performed on a high performance liquid chromatograph. The Km value was in agreement with Michaelis-Menten Kinetics and 2.44 X 10(-2) M without thymidine phosphorylase preparation. When the preparation was added, it was 2.53 X 10(-4) M and 1.75 X 10(-3) M, respectively. Determination of 5-FU concentrations in blood, normal and tumor tissues following a long term administration of Tegafur was performed. The highest concentration of 5-FU was detected in tumor tissue followed by normal tissue and blood, suggesting a human liver thymidine phosphorylase was capable of converting Tegafur to 5-FU more actively in tumor tissue than in normal tissue.

Case of primary orbital hemangiopericytoma.

A case of primary orbital hemangiopericytoma was reported. A 44-year-old woman was first seen at our clinic in June 1970, complaining of exophthalmos of the right eye. On examination, the right eye showed a 6-mm proptosis and an upward displacement. Cerebral angiography revealed a vascularized mass occupying the lower and mid orbit. The tumor was surgically removed and diagnosed histologically as hemangiopericytoma. After the initial surgery, recurrent tumors in the right orbit were resected twice, in June 1975 and December 1979. The purpose of this presentation is to discuss the clinicopathologic features of the tumor. In addition, it is emphasized that hemangiopericytomas originating from the orbit or brain may be histopathologically identical to angioblastic meningiomas.

Malignant tumor of the eyelid--clinical survey during 22-year period.

A total of 52 malignant tumors of the eyelid treated at Kyushu University Hospital during the 22-year period from 1960 through 1981 were analyzed. The peak incidence in this series was in patients 70-79 years of age. There were 26 each of men and women. Squamous cell carcinoma was the lesion most frequently seen, i.e. in 25 cases (48.1%). Basal cell carcinomas were detected in 17, sebaceous adenocarcinomas in 7 malignant melanomas in 2 and malignant lymphoma in 1. In 13 cases, there were squamous cell carcinomas in the upper eyelid, 10 with basal cell carcinomas in the lower eyelid, and all with sebaceous adenocarcinomas in the upper eyelid. Follow-up information was obtained on 42 of the 52 cases. Twenty-four out of these 42 patients were living and 18 had died. There were two recurrences of squamous cell carcinomas and two of basal cell carcinomas. In two, there was a metastasis to the lung and in one the regional lymph nodes were involved. Thus, squamous cell carcinoma was the most frequently encountered malignant tumor of the eyelid seen in this series. We wish to stress the importance of examining specimens histologically, to ensure a rapid diagnosis and pertinent treatment.

Orbital inflammatory pseudotumor: clinicopathologic study of 22 cases.

Clinicopathologic features of orbital inflammatory pseudotumors were examined in 22 cases at the Kyushu University Hospital during the period from 1965 through 1981. These cases were divided into three groups according to the location of the tumors. Group I included 13 cases with the lesions located in the anterior portion of the orbit. The lesions of this group were often seen as a palpable mass without proptosis or visual disturbance. Histologically, most of them showed reactive lymphoid hyperplasia. In some lesions, fibrous tissue, in which eosinophils were scattered, was found around the lymph follicles. Group II consisted of 4 cases with the lesions situated in the anterior and posterior portion of the orbit. The patients of this group had severe proptosis without loss of vision. Histologically, reactive lymphoid hyperplasia was seen in all cases. Group III comprised 5 cases in which the tumor existed in the posterior portion of the orbit. The patients with these lesions had proptosis and visual disturbance. Histologically, the lesions of this group showed infiltration of various inflammatory cells including lymphocytes, plasma cells, mast cells and eosinophils. Most tumors in Group I were stationary for a long time without steroid therapy. Steroid therapy was effective to the patients of Group III. However, the lesion infiltrated progressively after steroid therapy in some patients of Group II.

Origins of substance P-containing fibers in the lateral septal area of young rats: immunohistochemical analysis of experimental manipulations.

The majority of substance P-like immunoreactive (SPLI) fibers in the lateral septal area (LS) are supplied by SPLI cells in the area (BAL) between the anterior hypothalamic nucleus and the lateral hypothalamus, and by those in the nucleus latero-dorsalis tegmenti (TLD). These conclusions are based on following: (1) Unilateral destruction of the BAL resulted in an ipsilateral decrease in the septal SPLI fibers similar to that seen after the destruction of the TLD, and (2) simultaneous destruction of the BAL and TLD caused a marked reduction of SPLI fibers in the LS on the operated side. The possibility that the destruction of the BAL affected the ascending SPLI system from the TLD seems to be excluded, because (1) the destruction of the TLD resulted in a decrease in SPLI fibers in the ipsilateral medial forebrain bundle (MFB), but failed to reduce the number of SPLI fibers in the BAL, and (2) the destruction of the BAL caused a decrease in SPLI fibers in the perifornical area rostral to the lesion, but failed to reduce the number of SPLI fibers in the MFB. These facts further suggest that ascending SPLI fibers from the BAL travel in the perifornical area and those from the TLD pass through the MFB. It should be noted that a few SPLI fibers remained intact following the simultaneous destruction of the BAL and TLD. The present study suggests that these remaining SPLI fibers might be innervated by intrinsic SPLI cells. In support of this, several SPLI cells were detected in the septal area after colchicine pretreatment.

Ontogeny of the neurotensin-containing neuron system of the rat: immunohistochemical analysis. I. Forebrain and diencephalon.

The ontogeny of the neurotensin (NT) neuron system in the forebrain and diencephalon of the rat was investigated by means of indirect immunofluorescence. NT-positive structures first appear in the primordium of the olfactory bulb, anterior amygdaloid area, piriform cortex, amygdaloid complex, nucleus tractus diagonalis, lateral preoptic area, and lateral hypothalamus, etc., of the fetus of gestational day 16. From this day, NT-positive structures appear gradually in various areas of the forebrain and diencephalon and reach their maximum content on postnatal day 7. In contrast, NT-positive structures decrease slightly in number as the rat grows. However, even in adult rats, numerous NT-positive structures are still identifiable in various areas of the forebrain and diencephalon.

Ontogeny of the peptidergic system in the rat spinal cord: immunohistochemical analysis.

The ontogeny of neuropeptides, such as somatostatin (SRIF), substance P (SP), leucine-enkephalin (LE), and neurotensin (NT) in the spinal cord (including the spinal ganglion) of the rat, was examined by means of the indirect immunofluorescence method. SRIF and SP appear in the early fetal period before the establishment of the spinal synaptic transmission system, and their appearance precedes that of LE and NT, thus suggesting that SRIF and SP might have some important role in the development of the spinal cord. Furthermore, a number of SRIF-positive structures are found during the fetal period in the spinal cord; however, SRIF-positive fibers in the ventral horn, lamina V, VI, and X tend to decrease remarkably in number after birth, while those found in the dorsal horn maintain their immunoreactivity even in the adult rats. These facts suggest that SRIF in the latter area might function as a neurotransmitter, whereas in the former areas, SRIF might have another role in the development of the spinal cord. SP-positive structures also made their appearance during the fetal period. SP-positive fibers continue to increase in number after birth, and they can be seen throughout the entire spinal cord even in the adult rats. It becomes difficult to identify SP-positive neurons as the rats grow. Numerous SP-positive cells are demonstrated, however, by colchicine pretreatment, thus suggesting that this system is functioning actively in the adult rats. LE- and NT-positive structures appear at perinatal stages and they continue to increase in number after birth. These facts suggest that these peptides (SP, LE and NT) might act as neurotransmitters.

Evidence for the existence of an enkephalin-containing pathway from the area just ventrolateral to the anterior hypothalamic nucleus to the lateral septal area of the rat.

The afferent source of enkephalin-like immunoreactive (ELI) fibers in the lateral septal area of the rat was elucidated experimentally by means of an indirect immunofluorescence technique. These ELI fibers had almost completely disappeared on the operated side after the destruction of the area just ventrolateral to the anterior hypothalamic nucleus, where a number of ELI cells was detected. This fact strongly suggests that these ELI cells project ipsilaterally to the lateral septal area.
