[Aortocaval fistula secondary to rupture of abdominal aortic aneurysm; report of two cases].

Two cases of aortocaval fistula secondary to rupture of abdominal aortic aneurysm were presented. First case was 70 year old man who was admitted with pulsating abdominal mass. Prior to admission, he had been suffering from congestive heart failure. On physical examination, a pulsating mass, remarkable thrill and continuous bruit were recognized on his abdomen. Aortography showed abdominal aortic aneurysm and aortocaval fistula. The fistula (5mm X 3mm) was repaired within the aneurysm controlling the bleeding from the fistula by the direct digital compression. Aortic reconstruction was done with woven dacron bifurcation graft. Postoperative course was uneventful. Second case was 68 year old man who was admitted with the sudden onset of severe back pain. On admission, his physical status was already deteriorated. Diagnosis was made easily by the physical examination. Immediately after aortography, cardiac arrest occurred suddenly. After resuscitation, he was operated in the same manner of the first case. Large fistula (2 cm X 1cm) was noted. This patient died of renal failure and cerebral damage on 30th post operative day. Problems of pre- and post-operative patient management and surgical therapy for aortocaval fistula secondary to rupture of abdominal aortic aneurysm were discussed.

Relationship between estrogen receptors and risk factors of breast cancer in Japanese pre- and postmenopausal patients.

The relation between estrogen receptors (ER) in breast cancer and risk factors was studied in 456 Japanese patients. ER was shown to be positive in 55.3% (252/456) of patients. There was no difference in ER positivity between pre- and postmenopausal patients. In premenopausal patients, only the age at menarche and obesity showed some influence on the ER positivity of breast cancer. Among postmenopausal patients, on the contrary, ER(+) cancers were predominant in the patient groups that have been known to be higher in the risk of incidence of breast cancer. The factors included were the age at marriage, number of pregnancies, number of live children, and body weight. Of these, the body weight of patients was the strongest influence on the ER positivity in the postmenopausal patients. After excluding the effects of body weight, some of the reproductive factors such as number of pregnancies and number of live children were shown to be related to the ER status. These results may suggest the combination of lower incidence of breast cancer and lower percentage of ER(+) cancers in Japanese postmenopausal women as compared with the Western countries.

Coronary spasm as a cause of perioperative myocardial infarction.

The case of a patient with severe left main trunk disease is presented, in which spasm of the right coronary artery resulted in intraoperative myocardial ischemia and perioperative myocardial infarction. It is suggested that coronary spasm may be a cause of perioperative myocardial infarction, and has to be considered in case of unknown etiology.

[Intracranial benign chondroblastoma. A case report and review of the literature].

A case of benign chondroblastoma extending from the nasal cavity to the frontal region, was reported. A 1-year and 9-month old girl was admitted to our hospital in April 1981 because of generalized convulsion. On admission, she was intact neurologically and had It. nasal obstruction and It. eye discharge. Her laboratory examinations, including serum electrolytes and A1-P, were all in normal range. Plain skull X-P showed marked calcification from midfrontal region to the nasal cavity, and destruction of the frontal base especially in It. side. Plain CT showed isodensity mass with spotty calcification in the nasal cavity and marked high density mass in the frontal region. By contrast enhancement, nasal mass was markedly enhanced and new enhanced mass appeared around the intracranial calcification. Lt. ext. CAG showed the enlarged middle meningeal artery which is the feeder of the tumor, fine tumor vessels and slight tumor stain. Lt. int. CAG only showed the mass effect. After three-stage operation of bifrontal craniotomy, sublabial transnasal approach and rhinotomy, all the tumor was removed and she was discharged with no neurological deficits. The tumor was well demarcated and consisted from solid, cystic and calcified region. The solid region was grayish and easy-hemorrhagic. Histological examination revealed characteristic features of benign chondroblastoma, such as diffuse proliferation of polygonal or round cells, foci of chondroid matrices, scattered polynuclear giant cells and some mitosis.

A vehicle for DNA transfer and for recovery of transferred genes: lambda Charon phage-pBR322 hybrid.

Recombinant Charon 4A phages accommodating the Herpes simplex virus (HSV-1) thymidine kinase (tk) gene, the ampicillin-resistance (ApR) gene, and the replication origin of pBR322 were constructed. The phage DNA was introduced into mouse Ltk- cells by a free DNA transfer method or phage-mediated DNA transfer method [Ishiura et al., Mol. Cell. Biol. 2 (1982) 607]. Analyses of the physical state of the transferred DNA in the recipient cell genome showed that a DNA fragment as long as 12.7 kb was integrated intact into 67% and less than 40% of the Ltk- transformant cells by phage-mediated DNA transfer and by free DNA transfer, respectively. We also developed a new rapid method for recovery of the transferred gene from the Ltk+ cell into Escherichia coli; the method depends on the fact that the recombinant lambda phage carrying the ApR gene and replication origin of pBR322 transduces lambda-lysogenic bacteria to ApR and is maintained as a plasmid. Using this method the HSV-1 tk gene from one Ltk+ transformant was rapidly and successfully recovered without any rearrangement of the target sequence.
