Changes in arachidonic acid metabolite patterns in alloxan-induced diabetic rats.

The vasodepressor responses to intravenous injections of arachidonic acid, and the formation of its metabolites, were studied in rats made diabetic 1 or 2 weeks after a 1-dose alloxan treatment. Arachidonic acid dose-dependently decreased the diastolic blood pressure in normal animals, but this hypotensive effect was significantly weaker in 2-week postalloxan-treated rats. Indometacin abolished arachidonic-acid-induced depressor responses in both normal and diabetic animals. Hypotension induced by sodium nitroprusside was of the same magnitude in non-diabetic and insulin deficient rats. Plasma levels of thromboxane B2 were significantly increased in both the 1- and 2-week diabetic rats, being greater in the latter group; those of 6-keto-PGF1 alpha remained unchanged during the 2-week diabetic period. It is concluded that the attenuation by diabetes of depressor responses to arachidonic acid could be due to changes in the thromboxane/prostacyclin balance, with thromboxane formation being elevated whereas prostacyclin generation remains unaffected.

[Effects of hyperlipidemic sera and lipoproteins on prostacyclin synthesis by rabbit aortic smooth muscle cells].

Effects of hyperlipidemic sera(HLS), serum high density lipoprotein (HDL) and low density lipoprotein (LDL) were studied on synthesis of prostacyclin (PGI2) by rabbit aortic smooth muscle cells (SMC) in culture. The results indicated that HDL induced a dose-dependent increase in synthesis of PGI2 by SMC. Although LDL at comparable cholesterol concentrations of HDL also induced increase in PGI2 synthesis in a dose-dependent pattern, the extent was much less than that of HDL. SMC incubated with HDL and hyperlipid + aspirin treated serum synthesized less PGI2 as compared with that incubated with normal serum, while SMC incubated with hyperlipid + 8501 treated serum produced, an amount of PGI2 closed to that after incubation with normal serum, it suggested that 8501 may stimulate directly PGI2 synthetase in SMC.

Short-term test for the induction of lung tumor in mouse by chloroprene.

In a previous study by the authors, positive results from both a case-control study and a cohort study were reported. In the present study a short-term test for the induction of mouse lung tumor by chloroprene was conducted to confirm whether chloroprene monomer itself can induce tumors. Kunming albino mice weaned at 2 weeks were subjected to inhaling 0, 2.9 +/- 0.3, 19.2 +/- 1.9, and 189.0 +/- 13.3 mg/m3 chloroprene (GC purity, 99.8%) 4 h daily (except Sunday) for 7 months. All survivors were killed at the end of the 8th month or when moribund. No lung tumors were found before the 6th month. Thus, survivors at the 6th month were counted as effective animals. Most lung tumors observed were papilloadenomas (50/57), and a few were adenomas (7/57). The tumor incidence in the 2.9 mg/m3 group was 8.1% in comparison to 1.3% in the control group, with the significance level at P less than 0.05. The higher the concentration, the higher the incidence. Examination of the multiplicity of tumor induction also demonstrated a dose-response relationship, and the number of tumors per mouse in the 189 mg/m3 group was significant at P less than 0.01.

[Lung tumors induction short-term test of chloroprene in mice].

In a previous report by the authors, positive results in both case-control study mouse lung tumor induction short-term test of chloroprene for carcinogenicity was conducted to determine whether chloroprene monomer itself could induce tumor. Kuangming albino mice weaned 2 wk were subjected to inhale 0.0, 2.9 +/- 0.34, 19.18 +/- 1.89, 189.00 +/- 13.26 mg/m3, chloroprene (GC purity 99.69%) 4 h daily (except Sunday) for 7 month. All survivors were killed at the end of the 8th month or when moribund. No lung tumor was found before the 6th month. Thus survivors at the 6th month were counted as effective animals. Most lung tumors observed were papillo-adenoma (50/57), and a few were adenoma (7/57). Tumor incidence of the 2.9 mg/m3 group increased to 8.1% in comparison with that of the control group (1.3%) at a significant level of P less than 0.05; and the higher the concentration the higher the incidence. Examination of the multiplicity of tumors also demonstrated the dose-response relationship, and the number of tumors per mouse in the 189 mg/m3 group was significant at P less than 0.01.

[Follow-up study on prevalence rate of congenital defects in children at birth and age of two years].

To obtain prevalence rate of congenital defects during age of two years 16107 children were followed. Our data demonstrate that the prevalence rate of over all congenital defects at birth increased about 69% compared with at age of two years. The reason for prevalence rate increasing with age was that many children with internal organ defects were not diagnosed until three months or later. Prevalence rate of congenital heart disease at birth was approximately one-fifth of that at age of two years. For gastrointestinal and abdominal wall defects, the prevalence rate increased about 1.5 fold. This two sorts of defects above-mentioned were number eight and number five among over all defects at birth respectively. Three months after birth they became number one and number two.

Long-term survival of 1035 cases of nasopharyngeal carcinoma.

Long-term results of 1035 previously untreated patients with nasopharyngeal carcinoma seen at Cancer Hospital, Chinese Academy of Medical Sciences, between March 14, 1958 and March 15, 1968 are presented. Of the 1035 patients, 715 were treated with radiation therapy; 281 patients did not receive radiation therapy; and 39 patients whose records were lost. The patients were further subdivided according to the dose of radiation therapy (greater than 4000 rad versus less than 4000 rad), the presence or absence of pathologic confirmation of diagnosis, whether or not follow-up was complete, and whether or not distant metastases developed during treatment. All patients are included in the analysis. The 5-year and 10-year absolute survival rates for the entire group of 1035 patients are 19.6% and 14.5%, respectively. The 5-year and 10-year relative survival rates for the 715 patients who received radiation therapy are 28.4% and 21.0%, respectively. The 5-year and 10-year relative survival rates of the 464 with a pathologically confirmed diagnosis, who were without evidence of distant metastases and who received at least 4000 rad were 34.7% and 25.4%, respectively. Details regarding our statistical methods as well as an exact breakdown of the patient population are presented.

Analysis of 1379 patients with nasopharyngeal carcinoma treated by radiation.

One thousand three hundred seventy-nine nasopharyngeal carcinoma (NPC) patients were treated from March 1958 to December 1978. Twenty-two percent had stage I or II and 78% Stage III or IV had lesions. Two hundred twenty-Kv radiographs were used before 1960; and telecobalt was used from 1961 to 1978. Factors influencing the 5-year survival rate favorably are youth of patient, being female, pathologic condition (poorly differentiated carcinoma, 45.1% versus adenocarcinoma, 13%), stage (Stage I, 86%, Stage II, 59.5%; Stage III, 45.8%; Stage IV, 29.2%), decade admitted for treatment in the past (31% in the 1950s, 48.6% in the 1970s), total dose delivered to the nasopharynx (40 to 49 Gy, 46%; 70 to 79 Gy, 54.1%; 90 Gy or more, 64%) and prophylactic radiation to the neck regions (with prophylactic irradiation, 53.8%, without prophylactic irradiation, 23%). This implies that prophylactic radiation of the neck is crucial even without positive clinical metastasis. For those who have a residual tumor in the primary site when 70 Gy has been delivered, the total dose may be boosted to more than 90 Gy with the cone-down technique or on basis of adding 20 Gy to the dose at which the primary lesion disappeared grossly. The common postirradiation complications are: radiation myelitis, trismus, and otitis media. Because disease recurred in some patients after the fifth year, NPC patients should be followed for at least 10 years.

Salvage surgery for nasopharyngeal carcinoma.

Nasopharyngeal carcinoma is not a disease to be treated primarily by surgery. Radiation is the first choice of treatment. But, once it recurs, a second course of radiation controls only a small portion of the patients, with a high risk of accumulated radiation injury. We discuss the outcome of salvage surgery in nine cases of nasopharyngeal recurrence and 69 cases of neck metastasis that was uncontrolled or had recurred after irradiation was evaluated. A five-year survival rate of 44% for the primary lesions and 49% for the neck node metastases justifies the rationale of surgery on selected cases of radiation failure.

Fluoride in cerebrospinal fluid of patients with fluorosis.

The CSF fluoride level of individuals drinking water with normal fluoride content and of patients with endemic fluorosis were studied. For the purpose of studying the relationship between the dynamic equilibrium of the CSF fluoride and other body fluids, urine and blood fluoride were examined simultaneously. Fluoride was revealed in every CSF sample of the control group and its mean value was lower than that of the blood. The CSF fluoride concentration of patients with fluorosis was slightly higher than that of the control group, although there was no statistically significant difference. The results suggests that fluoride is a normal component of CSF. In severe cases of fluorosis or breakdown of the blood-brain in some diseases of the central nervous system, the CSF fluoride value might be increased.

[Radiotherapy of nasopharyngeal carcinoma--analysis of 1379 patients].

This paper presents 1379 patients with nasopharyngeal carcinoma treated in our hospital from March 1958 to December 1978. The stage I and II lesions comprised 22% and stage III and IV lesions, 78%. 220 KV X-ray was used before 1960 and 60Co was predominant from 1961 through 1978. The overall 5 year survival rate was 41.4% (571/1379). Young adults and female patients gave better results. The 5 year survival rate was, according to pathological type, 45.1% for poorly differentiated squamous cell carcinoma and 13% for adenocarcinoma; according to clinical stage, 85.7% for stage I lesion, 59.5% for stage II, 45.8% and 29.2% for stages III and IV; according to period of treatment, 31.3% in the sixties and 48.6% in the seventies; according to tumor dose at the primary focus, 48.2% for 40-49 Gy, 55.6% for 60-69 Gy and 67.3% for 90 Gy or more; according to with or without prophylactic radiation in the neck region, 53.8% for the former and 23% for the latter. It implies that prophylactic radiation of the neck is necessary regardless of the presence of cervical lymph node metastasis or not. For those who had residual tumor at 70 Gy, the total dose may be boosted to more than 90 Gy with the cone down technic or the dose, at which the primary lesion disappears grossly, is added with 20 Gy. The common complications are radiation myelitis, trismus and otitis media. As in certain patients, the lesion would still recur 5 years after radiotherapy, the authors believe that the nasopharyngeal cancer patients should be followed for ten years at least.

[Postoperative radiation of retinoblastoma--report on 22 patients].

From July 1958 to December 1979, 22 patients with retinoblastoma were treated postoperatively by radiotherapy. All were proved by pathology. There were 12 stage I, 1 stage II, 8 stage III and 1 stage IV lesions. Radiation dose was more than 15 Gy and most of the patients received 40-50 Gy in 4-5 weeks. The overall cancer free 5 year survival rate was 55% (12/22). The 5 year survival rate of stage I patients was 75% (9/12), and that of stage III patients was 38% (3/8). None of the stage II and IV patients survived. Dose of 40-50 Gy/4-5 wk is considered to be optimal for control of the tumor. Dose higher than 60 Gy/6 wk could lead to retarded growth in the orbital and temporal bony structures if the patient survives for ten years or more.

Radiation therapy of giant cell tumor of bone: analysis of 35 patients.

This paper reports 35 patients treated for giant cell tumor of bone. Ten patients were treated by surgery followed by postoperative irradiation. All 10 were cured; eight have survived for more than 5 years and four for more than 10 years without recurrence. Twenty-five patients were treated by radiation alone. All patients were followed for 2 to 17 years. For those who received more than 3500 rad, the local control rate was 82% (14/17); for those who received more than 4000 rad, the local control rate was 85% (11/13). Thirteen have lived for more than 5 years and nine have lived for more than 10. Our experience reveals that surgery and postoperative radiation produce the best results, and that these results are better than those reported in the literature. The results of radiation alone are comparable to that of surgery. For lesions not amenable to surgery, or those which would become crippling if surgery is attempted, radiation should be used either alone or in combination with conservative surgery. The dose for radiation alone should be 5000 to 6000 rad and for postoperative radiation, 3000 to 4000 rad. Supervoltage X rays (60Co or 8 Mv X ray), but not orthovoltage X rays, are indicated. A single radiation course with higher dose is preferred to multiple courses with lower doses. In 62% (15/24) of patients, pain relief begins before treatment is completed. In 74% (14/19) of patients, the tumor mass will not resolve until the sixth month after radiation.

Cylindroma of the nasopharynx: a chronic disease.

From March 1958 to October 1984, 10 patients with cylindroma (cystic adenoid epithelioma) arising in the nasopharynx were treated in our hospital. The presenting symptoms of these patients ranged in duration for 2 weeks to 8 years, with an average of 25 months. Eighty percent of the patients had destruction of the base of skull and 60% had cranial nerve involvement. Of these 10 patients, two had Stage II disease, two had Stage III, and six had Stage IV. Response was seen in this tumor at tissue doses between 5000-8000 rad. Of the seven patients at risk for 5 years, 86% survived, yet 36% of the patients died of tumor between the fifth to tenth year. Fifty percent of the patients lived beyond 10 years. Based on this experience, a dose of more than 8000 rad is advised for the primary lesion. For the neck region, radiation is indicated only when there are lymph node metastases; prophylactic radiation of the neck is not necessary. Local recurrence or single distant metastasis is amenable to radiation therapy. Among the six patients who failed in the course of follow-up, three had local recurrences, two had pulmonary metastases, and one died of cerebral vascular accident.

[Double primary cancer in the larynx and lung--report of 12 cases].

12 patients with double primary cancers in the larynx and lung were treated from 1958 to 1984. The incidence is 1.2% (12/943) of laryngeal carcinomas, 1.3% (12/904) of multiple primary cancers, 1.4% (12/873) of double primary cancers and 39% (12/31) of double primary cancers related to laryngeal cancers. There were 9 male and 3 female. 11 of the first primary cancers occurred in the larynx and only one in the lung. All were proved to be squamous cell carcinoma. In the 11 patients whose second primaries occurred in the lung, 4 were proved to be squamous cell carcinoma, one adenocarcinoma, one oat cell carcinoma, one poorly differentiated carcinoma and one cancer unclassified. Of 10 patients in whom both the first and the second primary cancer were treated, 6 survived for more than 2 years, 4 for 3 years and one for 5 years after the second treatment. It seems that double primary cancers of the larynx and lung could yield favorable results.
