Exit block in myxedema, treated effectively by thyroid hormone therapy.

A case of a 71-year-old female who developed exit block one year after implantation of a pervenous pacemaker is reported. The pacemaker had been implanted for sick sinus syndrome; the exit block was probably due to the development of severe myxedema which caused high pacing threshold. Normal pacing threshold was obtained on the seventh day of adequate thyroid hormone therapy and a follow-up one year later revealed effective control of the myxedema and no abnormalities in the function of the pacemaker system.

Familial thyroid ectopy and hemiagenesis.

Two siblings had sublingual thyroid glands and hypothyroidism. A third sibling had a left lobe agenesis of the thyroid, but normal function of the gland. This is the second such family to be described.

Metabolic effects of intravenous ritodrine infusion during pregnancy.

The effect of intravenous administration of ritodrine on blood glucose, insulin, free fatty acids and electrolytes in 17 third-trimester gravidas was investigated. A highly significant increase in blood glucose, insulin and free fatty acids and a marked decrease in serum potassium levels were registered. No significant changes during ritodrine infusion were recorded in serum levels of sodium, chloride, calcium, phosphorus and magnesium. No cardiac arrhythmias or electrocardiographic deteriorations occurred. While intravenous ritodrine administration seems to be safe in normal pregnancy, there may be a risk in diabetic patients, digitalized cases and those patients being treated with diuretics.

The effect of glucocorticosteroid administration on the survival of patients with acute myocardial infarction.

Because of conflicting reports in the literature, we decided to test the effect of the administration of glucocorticosteroids on the survival of patients with A.M.I. admitted to hospital within 24 hours from the beginning of symptoms. When tested by sex, age or as a whole, there was no significant difference between the steroid treated group and controls. However, recent reports show that steroids reduce the size of the infarction and improve the chances for survival and less late complications in experimental animals. We feel that renewed efforts in this direction should be made, possibly with larger doses of steroids and for longer periods.

Lymphocutaneous nocardiosis due to nocardia brasiliensis.

A farmer had chaining lymphocutaneous granuloma on the right hand and arm. Clinically the syndrome seemed typical for sporotrichosis. Bacteriological examination revealed Nocardia brasiliensis as the only etiologic agent. Since 1920, eight similar cases of sporotrichoid nocardiosis have been reported from various parts of the world. To our knowledge, isolation of N brasiliensis has not been reported previously in this geographical area.

Typical Turner's syndrome with 45 XO karyotype and normal menstruation. Cytogenetic and histological findings.

The clinical, laboratory, histological and cytogenic findings in a patient with Turner's syndrome with normal menstruation are presented. Although the phenotype and karyotype were those of Turner's syndrome, normal sexual development and periods occurred at puberty. The endocrine studies revealed onyl a mild diabetic oral glucose tolerance test. At laparotomy, normal uterus, Fallopian tubes and ovaries were seen. Histological examination of the ovaries showed several primordial follicles and a follicle cyst. Tissue cultures of lymphocytes, skin fibroblasts and ovary revealed a single line of cells with the 45 XO karyotype. Although 25 cases of Turner's syndrome with normal sexual development have been reported, most of them were mosaics and only a small number showed the pure 45 XO karyotype.
