Mitral valve prolapse in hyperthyroidism of two different origins.

The prevalence of mitral valve prolapse was investigated in 126 patients with hyperthyroidism due to Graves' disease or toxic nodular goitre and that of hyperthyroidism in 64 patients with mitral valve prolapse. One hundred and eleven asymptomatic healthy subjects comprised a control group. The patients with hyperthyroidism were divided into those with Graves' disease and those with toxic nodular goitre. Of the group as whole, 12 (9.5%) patients had mitral valve prolapse compared with six (5.4%) in the control group, but the difference was not statistically significant. The prevalence of mitral valve prolapse in the patients with toxic goitre was also not significantly different from that in the controls. When the prevalence in the group with Graves' disease was compared with that in the control group (16.3% vs 5.4%) the difference was significant. Only one patient with mitral valve prolapse had hyperthyroidism.

Primary fibroxanthosarcoma of the kidney presenting as fever of unknown origin.

Fibroxanthosarcoma is a tumor of soft tissues, mainly effecting the extremities, mediastinum and retroperitoneum. Primary fibroxanthosarcoma of the kidney is extremely rare and has been described in only 4 cases in the past. We present an additional patient, presenting with continuous spiking fever, with rapid deterioration of her condition due to widespread metastases and death within 4 months.

Coumadin-induced skin necrosis.

Two women developed coumadin-induced skin necrosis. A 16-year-old woman sustained necrosis on the leg after a single dose of the drug, and a 59-year-old woman developed gangrene of most of her left breast.

Pseudo-precocious puberty in a young boy due to interstitial cell adenomas of the testis.

A case of pseudo-precocious puberty in a 2 4/12-year-old boy, due to Leydig cell adenomas of the testicle, is described. The special endocrine investigations revealed high levels of plasma testosterone and other androgens together with suppressed plasma gonadotropins unresponsive to stimulation. After operation the hormones returned to normal. The growth velocity and bone age were only gradually decreased, during a period of one year, while the sexual manifestations were the last to disappear, in the third year.

Metabolic effects of intramuscular and oral administration of ritodrine in pregnancy.

The metabolic effects of ritodrine, 80 mg/day, administered intramuscularly in the third trimester of pregnancy to 14 patients for 5 days and per os to 10 patients for 10 days were investigated. Each patient was considered at high risk for premature labor. Intravenous glucose tolerance and insulin, triglyceride, and electrolyte levels were assessed before and at the end of treatment with ritodrine. Intramuscularly or orally administered ritodrine in 10-mg doses 8 times a day did not influence carbohydrate homeostasis. Triglyceride levels fell significantly after intramuscular administration, whereas serum potassium levels increased during treatment. This study suggests that ritodrine administered intramuscularly or orally is metabolically safe in nondiabetic pregnant women.

Epidural angiolipoma producing compression of the cauda equina.

Epidural angiolipomas are rare tumours and are histologically benign, but their myelographic and macroscopic appearance is malignant. Surgical results are satisfactory. A case is reported in a 48-year-old woman with a slowly progressive cauda equina compression syndrome in whom an epidural angiolipoma was successfully removed. The special feature of this case was a generalized severe spinal osteoporosis with a pathological fracture of a vertebral body at the level of the tumour.
