Autopsy findings of the coronary arteries of variant angina with Raynaud's phenomenon of the tongue.

A 42 year old man with variant angina occasionally associated with syncopal attacks died of acute myocardial infarction 17 months after the onset of angina. Prior to the onset of variant angina, he had Raynaud's phenomenon of the tongue for 2 years. Both Valsalva maneuver and hyperventilation could repeatedly provoke chest pain and ST segment elevation in leads II, III and aVF. The infusion of prostaglandin E1 at a rate of 0.05 microgram/kg/min, was able to prevent the attack of variant angina induced by these maneuvers. Although coronary angiography performed 15 months prior to death revealed no organic lesions except for complete spastic occlusion at segment 1 following intravenous ergonovine, autopsy revealed marked intimal proliferation and accumulation of abundant glycosaminoglycans in three coronary vessels, as well as in small and muscular arteries of other organs. This suggests that a rapid systemic progression of narrowing due to proliferation of the intima might occur in some cases of variant angina.

[Coronary thrombolysis in acute myocardial infarction].

The effect of early perfusion after intracoronary infusion of urokinase (PTCR) in patients with acute myocardial infarction was assessed. Coronary perfusion was reestablished in 7 of 10 patients receiving intracoronary urokinase. We compared the recanalized group after intracoronary urokinase (n = 7) with the standard therapy group (n = 32). Left ventricular end-diastolic pressure, cardiac index, angiographic ejection fraction, radionuclide ejection fraction, defect score by Thallium-201 scintiscan and regional ejection fraction by the left ventriculogram, measured at about one month after the onset of acute myocardial infarction, were compared according to the site of the main coronary lesion (right coronary artery or left anterior descending coronary artery). No statistically significant differences between the recanalized group after the administration of intracoronary urokinase and the control group were found at each coronary lesion. On the other hand, the comparison between two patients with and without PTCR, who matched the clinical and coronary angiographic findings, showed that PTCR on acute myocardial infarction might be of value. However, evaluation of the long-term effects of coronary thrombolysis on survival and myocardial function will require controlled randomized trials of a large number of patients.

Physiological variations of warm and cool sense with shift of environmental temperature.

In order to obtain control data on the temperature sense (warm and cool threshold values) of fingertips, the relationships between room temperature and either skin temperature, warm threshold or cool threshold of the middle fingertips were investigated in healthy subjects (6 males), using our thermo-esthesiometer. The skin temperature changes in a sigmoidal response with the variation of room temperature. A point of inflection for this response was observed at the room temperature of 15 degrees C, at which the greatest standard deviation of skin temperature occurred. The warm and cool thresholds, on the other hand, were also affected by variations of room temperature. Warm threshold and skin temperature or cool threshold and skin temperature bore a linear relation to each other, and the correlation coefficient was 0.854 in the former, and 0.925 in the latter, respectively. The disorder of temperature sensitivity (warm and cool thresholds) must always be considered together with the room temperature or skin temperature. On the other hand, the width of the neutral zone between warm and cool thresholds was affected by neither the changes of room temperature nor the changes of skin temperature. Hence, the width of the neutral zone was approximately constant, especially, at the room temperatures in the vicinity of 15 degrees C to 25 degrees C.

Availability of temperature sense indices for diagnosis of vibration disease.

The correlation between stages of vibration disease and hypesthesia of temperature sense detected with our thermo-esthesiometer was analyzed. We measured the warm and cool thresholds among operators (644 males) in three occupational categories with vibration exposure from hand-held vibratory tools, such as grinder, drill, sander, impact-wrench, bush-cutter and chain-saw. Through the examination of the relationship between the temperature threshold and the stage of vibration disease, the following was revealed: The warm sense threshold elevated parallel with the aggravation of vibration disease, although the skin temperature had no change or reduction. The hypesthesia of the cool sense threshold appeared in serious cases of vibration disease. This abnormality of the cool sense indicated a more intensive disorder than that detected by the warm sense abnormality. The width of the neutral zone between the warm and cool thresholds of the intact subjects was 6.6 degrees C +/- 3.8 C degrees. This neutral zone was enlarged with the aggravation of the vibration disease. Data also showed that the measurements of temperature sense threshold should be carried out under standardized room temperature.

New echocardiographic observations in a patient with dissimilar atrial rhythms.

A patient with an impure flutter was found to have dissimilar atrial rhythms that were confirmed by direct ECG recordings. Echocardiographic studies disclosed that the a waves on the mitral echocardiogram occurred synchronously with the a waves on the esophageal ECG that, in turn, showed atrial flutter at a rate of 300 beats per minute. However, the a waves on the tricuspid echocardiogram coincided closely with the P waves on a surface ECG (lead V) that disclosed a chaotic atrial tachyarrhythmia. These results suggest that echocardiographic study, in conjunction with an esophageal ECG, may be a useful, noninvasive technique for the diagnosis of dissimilar atrial rhythms.

Nitroglycol concentrations in blood and urine of workers engaged in dynamite production.

Nitroglycol (Ng) concentrations in blood and urine of dynamite production workers were measured. Measurement of Ng concentration in blood of 16 workers revealed undetectable levels prior to work in all of them. The amounts of Ng detected after work were 0-145 ng/ml, high levels being noted in workers who had frequent exposure to skin absorption. Measurement of changes in blood Ng concentrations and the Ng excretion in urine in three workers during a week revealed no persistent trend in blood Ng concentrations but a tendency to higher Ng concentration in urine was found in the afternoon and in the morning with a higher concentration in the latter half of the week than during the first half. These results suggest the usefulness of the measurement of Ng urinary concentration excretion in the assessment of Ng absorption by the body. The relationship between the blood Ng concentrations in workers engaged in a dynamite factory and the experimental condition in dogs described in the previous report was assessed, and a valuable suggestion concerning further research in the study of chronic exposure to Ng at low concentrations was obtained.

[Vibration disorders in a factory producing concrete pipes and piles (author's transl)].

The investigations were carried out to estimate the actual conditions of exposure and health of workers using impact wrenches and vibrating long iron trowels (Fig. 1) in a factory producing concrete pipes and piles, and the following results were obtained: 1) Vibration acceleration levels of the impact wrenches and the trowels were in excess of the guideline of up to 30 minutes per day proposed by ISO (1979) (Figs. 3 and 4). 2) Daily amounts of time during which workers were exposed to vibration by using tools were roughly calculated to be about 60 minutes per worker. 3) As the results of the special health examinations for 35 workers, 5 cases (14.3%) were diagnosed as vibration disease, 8 cases (22.9%) were suspected of this disease, and white fingers were found in 14.3% of the workers. It was suggested that the protection from vibration and the prevention of vibration disorders are necessary for all factories such that concrete pipes or piles are manufactured by similar processes.

[Occupational impairment of the olfactory sense of chromate producing workers (author's transl)].

Using T & T Olfactometer which had been developed and standardized for clinical use in Japan recently, measurements of olfactory thresholds of odor detection and of odor recognition were carried out on 33 subjects (26 males and 7 females), who had worked in a chromate producing factory. The subjects had left the factory before 7 years or more. Five kinds of test odors were used; beta-phenyl ethyl alcohol, methyl cyclopentenolone, iso-valeric acid, gamma-undecalactone, and scatol. The degree of olfaction loss was expressed with the number of powers of 10 in series of multiple concentrations by 10 of each test odor with reference to the concentration of the detection threshold of young normal people. Seventeen of 33 subjects (51.4%) had a perforation of the nasal septum. Middle and high grade decrease of odor recognition faculty were found in 18 (54.5%) including 2 anosmia, one of whom complained also of the disorder of taste. Any specific pattern of olfactogram characterizing them were not found, namely, chromates impair olfactory sense for all kinds of odors, which means the impairment of olfactory epithelium itself. The degree of olfaction loss was related to the duration of employment in the chromate producing factory. The existence of septal perforation had no direct relation to the degree of olfaction loss.
