Rupture of the ascending aorta after surgical resection for lung cancer--a case report.

A rupture of the ascending aorta which occurred in a woman on the 13th postoperative day following a right upper lobectomy with mediastinal lymph node dissection for lung cancer is reported herein. Fortunately, the patient was rescued from a cardiac tamponade and hemothorax by emergency operation. The operative findings suggested a traumatic rupture of the aorta, however, lymph node dissection of the mediastinum could not be excluded as a possible cause. Therefore, careful mediastinal lymph node dissection should be carefully performed in operations for lung cancer.

A case of idiopathic vulvar calcinosis: the first in Japan.

Idiopathic calcinosis of the scrotum is a rare condition of unknown etiology. As counterparts of this male disorder, only two female cases have been reported. We report a third case, the first of its kind, in Japan. Histochemical stains revealed acid mucopolysaccharide (acid-MPS) within the calcinosis and many infiltrated mast cells near it; these might histogenese the calcium deposition. Immunohistochemically, polyclonal carcinoembryonic antigen (CEA) and keratin stains revealed no positive cells near the calcium depositions.

[Effect of KT-611 (Naftopidil) on the contraction of human prostatic tissue and its use in benign prostatic obstruction].

A new alpha 1-adrenergic receptor antagonist, KT-611 (naftopidil) antagonized dose dependently the contraction induced by agonists, noradrenaline and phenylephrine in the human prostatic tissue. KT-611 at a dosage of once or twice a day was evaluated for its effects on 49 patients with benign prostatic hypertrophy. The drug improved subjective and objective symptoms significantly. The residual urine was reduced in volume and percentage significantly. The average and maximum flow rates increased significantly. The optimal dosage was presumed to be in the range of 25 to 75 mg once a day or 25 to 100 mg twice a day. Adverse reactions and abnormal laboratory findings were all slight. KT-611 was concluded to be useful in the treatment of patients with benign prostatic hypertrophy.

[A case of surgical repair of aortic dissection associated with tear of the right coronary artery].

A 39-year-old female with dissecting aortic aneurysm of DeBakey type I, underwent replacement of the ascending aorta. We found that the right coronary artery originated just distally from the left sinus of Valsalva, run transversally in the aortic wall. Because the entry of dissection occurred at the right coronary artery, it was torn longitudinally. Aorto (graft)-rt. coronary bypass grafting was performed. She had no ischemic heart symptoms postoperatively. It is supposed that the dissecting aortic aneurysm was caused by the anomalous origin of the right coronary artery.

[Two cases of cardiac injuries due to nonpenetrating trauma of the chest].

Successful repair of nonpenetrating traumatic cardiac injury is uncommon. Two cases of nonpenetrating traumatic cardiac injuries from accident are reported. Case 1. Seventy eight year old man with shock was admitted to Iida City Hospital four hours after getting a bruise by a wood block on the anterior chest wall. Soon after his arrival ultrasonic cardiography was performed because of a doubt of cardiac trauma. And it revealed cardiac tamponade. Then the emergency operation was performed. After pericardial effusion was drained, rupture on the out-flow tract of the right ventricle was found and was successfully repaired without the aid of cardiopulmonary bypass. Case 2. Sixty five year old man with left heart failure was admitted to Iida City Hospital four months after getting a bruise on left chest and back from falling. Chest and back pain was reduced three weeks after the accident, however, shortness of breath was getting worse. Ultrasonic cardiography and aortography showed aortic regurgitation. Under cardiopulmonary bypass aortic valve replacement was performed. His aortic regurgitation was caused by perforation of non-coronary cuspid due to accident. Ultrasonic cardiography is useful to make a diagnosis of nonpenetrating traumatic cardiac injury. It is necessary to operate as soon as possible in cardiac rupture, and to select valve replacement for aortic regurgitation.

A study of systemic and topical effects of topical steroid application through the comparison of two application schedules.

We compared two application schedules of a topical steroid, BV (Betamethasone valerate). The schedules were once daily and twice a week on juvenile Wistar rats treated for four weeks. Steroid actions were assessed topically (skin thickness and epidermal Langerhans cell density) and systemically (body weight and internal organ wet weights). The results revealed some rather severe systemic effects caused by frequent steroid application. And we also found a uniquely weak BV effect with Langerhans cells compared to obvious systemic effects on rats. We came to the conclusion that frequent steroid application to the young should be avoided.

Expression of protein kinase C isozyme in human Langerhans' cells.

Protein kinase C is a key molecule controlling signal transduction into the cell. We recently reported that protein kinase C II isoenzyme, but neither I nor III isozyme, was expressed in epidermal Langerhans' cells of the adult mouse, and that none of these isozymes was detected in keratinocytes. In this study, we examined the expression of protein kinase C isozymes in human Langerhans' cells in vivo to see whether the expression of protein kinase C II isozyme in Langerhans' cells is a mouse-specific trait. Immunohistochemical studies revealed that protein kinase C II isozyme, but neither I nor III isozyme, was expressed in epidermal Langerhans' cells. None of these isozymes was detected in keratinocytes. These results suggest that the expression of protein kinase C II isozyme in epidermal Langerhans' cells in vivo is not a mouse-specific trait and that protein kinase C II isozyme is a novel phenotypic marker for epidermal Langerhans' cells in human as well as mouse skin.

[Thoracic duct cyst--a case report].

A case of thoracic duct cyst was reported. The patient, 72 year old man, was admitted with chief complaint of dysphagia. Chest X-ray film demonstrated an ovoid mass at the right upper mediastinum and the esophagus pressed by the tumor to the right. CT scan showed a round tumor with obscure margin located at the right upper mediastinum. Other laboratory data were almost within normal limits. Thoracotomy was performed on March 26, 1987 under diagnosis of leiomyoma of the esophagus. A fluid containing cyst covered with pleura was found at the upper mediastinum. The tumor was connected to the thoracic duct at its upper and lower portion. The cyst was isolated by sharp and blunt dissection without difficulties and removed. The cyst measured 7.5 X 4.5 X 4.5 cm and contained chyle. Pathological examination revealed no evidence of malignancy. Postoperative course of this patient was uneventful, and dysphagia was disappeared.

Serum succinyltrialanine p-nitroanilide-hydrolytic activity in workers occupationally exposed to lead.

The relationship between blood lead concentration and serum succinyltrialanine p-nitroanilide-hydrolytic (STN) activity was investigated in 74 workers occupationally exposed to lead and in 28 non-exposed workers. Exposure to lead was observed during transfer-printing processes in which paints or paint powders containing 20-50% lead were used. The mean lead concentration was 0.49 mg/m3 (0.12-1.43 mg/m3) in the working environment and 1.42 mg/m3 (0.21-4.34 mg/m3) in workers involved in printing processes. Serum STN activity became lower with increasing concentrations of blood lead (PbB) in non-exposed workers (controls). In lead-exposed workers, the activity decreased with increasing concentrations of PbB of 2.0 mumol/l and more, but increased when less than 1.9 mumol/l. Among lead-exposed workers with high PbB concentrations and/or with hepatic dysfunction, the STN/PbB ratio was distributed above the regression line obtained from the controls. The present investigation suggests that serum STN activity decreases in those highly exposed to lead. Enzymes such as elastase, which hydrolyzes succinyltrialanine p-nitroanilide and additionally is not inhibited by lead, may possibly be induced among lead workers when liver function is impaired.

[Phase II study with cisplatin in advanced stomach and colon carcinoma. Cooperative Study Group of Cisplatin for Stomach and Colon Carcinoma].

A multicenter phase II study was performed to evaluate cisplatin (CDDP) in patients with gastric carcinoma or colon carcinoma. Ninety-nine cases of gastric carcinoma and 25 cases of colon carcinoma were entered into the study. According to the eligibility criteria prepared for the study protocol, 28 and 5 patients were excluded from the stomach and colon group, respectively. CDDP, 70-100 mg/m2, was administered intravenously with sufficient hydration. The efficacy rates evaluated by the UICC tumor reduction criteria were 19.1% (13/68 cases) in gastric carcinoma and 5.0% (1/20 cases) in colon carcinoma. Lymph node metastasis and metastatic abdominal tumor were effective targets for CDDP treatment. As side-effects of CDDP, nausea/vomiting (69.2%) and general malaise (35.2%) were observed. CDDP was thus considered to be one of the effective chemotherapeutic agents for treatment of gastric carcinoma.

[A case report of an advanced and recurrent stomach cancer in a patient manifesting response to cisplatinum-based chemotherapy].

In an advanced stomach cancer with recurrence of regional lymph node metastasis, cis-diamminedichloro-platinum (CDDP) was administered intravenously (IV) according to the following doses and schedules. This treatment schedule consisted of CDDP at a dosage of 100 mg/body/time. This dosage was infused seven times for 24 months (total CDDP dose, 700 mg/body). The overall response rate to chemotherapy was determined as follows. Partial response (PR) was defined as a reduction of at least 50% in the sum of the products of the two greatest perpendicular diameters of all measurable lesions within 15 months of commencing therapy and the absence of disease progression elsewhere. The 56-year-old, male patient is being treated with an outpatient regimen at our Hospital.

[A long-surviving case of gastric cancer with peritoneal metastasis (P3) responding to short-term high-dose chemotherapy and long-term immunotherapy].

A 37-year-old female was admitted to our hospital for further examination of epigastralgia. She was diagnosed as having multiple metastases due to advanced gastric cancer (Borrmann type 3). The operative findings showed bilateral ovarian (Krukenberg), Schnitzler and widespread peritoneal metastases involving the appendix (P3H0N2S2). She underwent total gastrectomy, splenectomy, bilateral oophorectomy, and appendectomy with CDDP (100 mg intraperitoneal administration). After operation, CDDP (50 mg/body, twice i.p. and once i.v.) and PSK (3.0 g/day) were administered. She has been followed in our outpatient department for 3 years without any recurrence. The findings suggest that combination therapy using short-term high-dose chemotherapy and long-term immunotherapy can be effective for such cases.

Effect of freezing on lens epithelial cell growth.

The effect of freezing on the growth of rat lens epithelial cells was studied in vitro. We found that 80% of the lens epithelial cells died after freezing at -45 degrees C for two hours and that the surviving cells could grow with the addition of growth factors or when placed on a sheet of type 4 collagen, but not when placed on a plain plastic culture dish. These results suggest that the surviving cells are at the Go phase of the cell cycle and that type 4 collagen or growth factors can initiate cell division.

Light scattering caused by cells on the intraocular lens.

The anterior surface of a posterior chamber intraocular lens (IOL) was covered with cultured rat lens epithelial cells and light scattering was measured densitophotometrically. The light scattering intensity was maximum at a 30-degree scattering angle and was proportional to the number of cells per area on the IOL. Results suggests that the adhesion of lens epithelial cells on the IOL may influence its optical function.

Partial resection of atrial septal aneurysm with multiple fenestrations--a case report.

Aneurysms of the interatrial septum are uncommon lesions that are often accompanied by other cardiac anomalies or systemic thrombosis. The authors report 1 case of atrial septum aneurysm that was diagnosed by two-dimensional contrast echocardiography. At surgery a membrane-like, fenestrated aneurysmal protrusion to the right atrium without thrombosis was seen. The atrial septum was closed without using an artificial patch after some of the aneurysm with fenestrations was resected.
