[A case of adult type of anomalous origin of left coronary artery from pulmonary artery--with a referential consideration to the disease in Japan].

A case of a 33-year-old man with anomalous origin of the left coronary artery from the pulmonary artery (ALCP) was reported with a referential consideration to the adult type of ALCP previously reported in Japan. The patient visited our hospital for further examination of cardiac murmur detected on a mass physical checkup. He was found to have a continuous murmur, the loudest in the left sternal border of 3rd intercostal space, and developed evidence of ischemia during the exercise stress test in the ECG leads of L2, L3, aVF, and V2 through V6. In addition, studies using echocardiography and color Doppler-echocardiography detected a hypertrophied intraventricular septum and a pattern of turbulent blood flow in the proximal pulmonary artery. Cardiac catheterization and aortography proved the existence of ALCP with a markedly developed collateral circulation from the right coronary artery to the left coronary artery. After surgical restoration by ligating the anomalous opening of the left coronary artery, and by bypass grafting to the left coronary artery, it was clear that dilation and kinking of the right coronary artery was lessened and hypoperfusion in the anterior left ventricular wall was improved. As far as we know, this case is the 35th of adult type ALCP reported in our country. Results from this case and others suggest that early detection and therapy for adult type ALCP is important from the point of view preventing ischemic progression, and indicate that restoration by ligation and bypass grafting may be one of the effective surgical procedures for ALCP.

Adenine nucleotides metabolism of the canine pancreas during preservation by the two-layer cold storage method.

The relationship between tissue concentrations of adenine nucleotides (ANs) at the end of cold preservation period and organ viability after transplantation is controversial. The purpose of this study is to examine energy metabolism of the pancreas graft preserved by a two-layer cold storage method and the relationship with the viability of the pancreas graft following transplantation. After preservation by simple cold storage with Euro-Collins' solution (EC) (group 1) or University of Wisconsin solution (UW) (group 2), an original two-layer (EC/perfluorochemical(PFC)) method (group 3) and a modified two-layer (UW/PFC) method (group 4) for 24, 48, 72, 96 and 120 hours, tissue concentrations of ANs were determined using high-performance liquid chromatography (HPLC) and the viability of the pancreas graft was tested in the canine model of segmental pancreas autotransplantation. While EC alone (group 1) was available only for 24-hour preservation, UW alone (group 2) and the original two-layer method (group 3) were effective up to 72-hour preservation and the modified two-layer method (group 4) was successful up to 96-hour preservation. In simple cold storage with EC or UW, there was no significant difference between tissue concentrations of adenosine triphosphate (ATP) of viable grafts and nonviable grafts. But in groups 3 and 4, there was an excellent correlation between the posttransplant viability and ATP tissue concentration at the end of preservation, and ATP was useful to predict the viability of the canine pancreas during preservation by the two-layer method before transplantation.

[Coronary collateral function evaluated by coronary hemodynamics and myocardial lactate metabolism during rapid atrial pacing].

To elucidate coronary collateral function in the ischemic myocardium, we studied coronary hemodynamics and myocardial lactate metabolism of the collateral-dependent myocardium before and during rapid atrial pacing. Subjects consisted of 38 patients who were categorized into 3 groups according to their coronary and coronary collateral arteriographic findings: 14 patients with normal coronary arteriograms (Group A), 15 with significant stenosis in the left anterior descending coronary artery (LAD) without collaterals (Group B), and 9 with LAD stenosis and collaterals (Group C). Estimates of cross-sectional area (CSA) obtained from the orthogonal coronary arteriograms, and the great cardiac vein flow (GCVF) and myocardial lactate extraction ratio (MLER) before and during rapid atrial pacing were used as parameters to evaluate coronary stenosis. The results were as follows: 1. The cross-sectional areas in Groups B and C were 86% and 91% of that in Group A, respectively. 2. In Group B, there was a good linear relationship between cross-sectional area and % delta GCVF, as shown in % delta GCVF = 2.90 + 36.22 x CSA (r = 0.61, p < 0.05). This relationship was modified in Group C, increasing % delta GCVF against CSA. 3. Myocardial lactate extraction ratio in Groups B and C decreased significantly after rapid atrial pacing, while, it remained unchanged in Group A. This ratio after rapid atrial pacing did not differ significantly between the 2 groups. 4. In 3 of 4 patients with total LAD occlusions, GCVF and anterior coronary resistance before and during rapid atrial pacing were similar to those of Group A.(ABSTRACT TRUNCATED AT 250 WORDS)

The effect of prostaglandin E1 on local cerebral blood flow during cerebral-aneurysm clip ligation.

Prostaglandin E1(PGE1) was administered for deliberate hypotension during general anaesthesia in 27 patients undergoing cerebral-aneurysm clip ligation, and the effect of the drug on local cerebral blood flow was studied. Local cerebral blood flow measurements were made using a thermal-gradient blood flowmeter. Control measurements were made immediately before administration of the drug. Local cerebral blood flow was measured 10, 30, 60 and 120 min after starting the drug infusion and 10, 30 and 60 min after discontinuation. The mean blood pressure was reduced significantly by the administration of PGE1. Urine output was increased after commencement of the drug infusion. Local cerebral blood flow did not change. These results suggest that PGE1 may be an appropriate hypotensive drug for use during cerebral-aneurysm clip ligation, because the cerebral blood flow remains within the normal range and the urine output is increased.

[Coronary venous oxygen saturation dynamics during intracoronary infusion of ergonovine or acetylcholine in vasospastic angina].

We investigated changes in coronary venous oxygen saturation (CSO2-Sat) dynamics during intracoronary infusion of ergonovine (ERG) or acetylcholine (ACh) in patients with vasospastic angina. In 16 patients with suspected vasospasm in the left coronary artery, intracoronary ERG injection provoked vasospasm in 4 out of 6 patients, and intracoronary ACh in 6 out of 11 patients. A gradual decrease in CSO2-Sat, anteceding angina and ischemic ECG evidence, was characteristic in ERG-induced vasospastic patients, while no significant change of CSO2-Sat was observed in non ERG-induced patients. Intracoronary infusion of ACh was followed in all 11 patients by a transient increase in CSO2-Sat, suggesting decreased myocardial oxygen extraction due to increased coronary blood flow with ACh. However, earlier and sudden decreases in CSO2-Sat below the control levels were characteristic in 6 patients with ACh-induced vasospastic angina. These findings suggest that effects of ERG and ACh on coronary flow dynamics are essentially different although both drugs can provoke vasospasm.

Plasmid DNAs directly injected into mouse brain with lipofectin can be incorporated and expressed by brain cells.

In this study, we demonstrated that lipofectin-treated DNAs which were injected into mouse brain could be incorporated and expressed by brain cells. When L7RH-beta gal plasmid DNA harboring E. coli beta-galactosidase gene fused with the nuclear location signal of SV40 T-antigen gene was injected into brains of 1-week-old mice, cells whose nuclei appeared to be densely stained with the chromogenic substrate X-gal were detected in several portions of the brain till 9 days after injection. Injection of pMLV-CAT plasmid DNA which contains the E. coli chloramphenicol acetyltransferase (CAT) gene also resulted in cells immunoreactive to the anti-CAT antibody.

The long-term function and histology of segmental pancreatic autografts with pancreatic exocrine diversion to the esophagus in dogs.

The endocrine function and histology of segmental pancreatic autografts with pancreatic exocrine diversion to the esophagus were assessed in nine dogs which survived longer than three years. An original twelve dogs underwent total pancreatectomy followed by a segmental pancreatic graft autotransplanted with pancreatic duct to esophagus anastomosis in the neck. All twelve dogs immediately had normal fasting glucose, nine of which sustained it for more than three years. One of the twelve dogs died on the tenth day from a thrombosis and two others died of causes unrelated to the graft; one of pneumonia and the other of an unknown cause, within the first year of transplantation. An intravenous glucose tolerance test performed three years after the transplantation revealed K values (1.90 +/- 0.37) which were not significantly different from those tested before the transplantation (1.92 +/- 0.42). The patency of the anastomosis between the pancreatic duct and the esophagus was clearly identified in the specimen of a dog sacrificed three years after the transplantation. The mucosa of the esophagus was macroscopically and microscopically almost normal. Histological studies of the autografts done three years after the transplantation showed almost normal pancreatic architecture in the islets and exocrine tissues, while histochemical analysis with immunoperoxidase stains confirmed the presence of insulin, glucagon and somatostatin. It is therefore possible that this new technique could be used for clinical segmental pancreatic transplantation.

Retrovirus-mediated gene transfer into mouse cerebellar primary culture and its application to the neural transplantation.

To develop a procedure for the transplantation of genetically modified brain primary cells, we transplanted cultured mouse cerebellar cells infected with recombinant retroviruses into the cerebella of adult mice and examined the expression of introduced gene-products in the host cerebellum after transplantation. After infection of cultured cerebellar cells with recombinant retroviruses harboring chloramphenicol acetyltransferase (CAT) gene, we selected only virus-infected cells for transplantation by culturing the cells in medium containing G418 for 3 weeks. CAT was continuously expressed in the cultured cerebellar cells during the 3-week incubation, but by immunoblotting analysis with antiglial fibrillar acidic protein (GFAP) or antineurofilament protein (NFP) antiserum the population of cultured cerebellar cells was found to change during the incubation. Immunocytochemical analyses using anti-CAT antiserum demonstrated that the transplanted cell mass containing CAT-positive cells was detectable in the cerebellum up to 3 weeks, but not 3 months after the transplantation of G418-selected cells into the cerebella of 7-week-old mice.

Mechanism of oxygenation of pancreas during preservation by a two-layer (Euro-Collins' solution/perfluorochemical) cold-storage method.

To clarify the mechanism of oxygenation of the pancreas during preservation by two-layer (Euro-Collins' solution [EC]/perfluorochemical [PFC]) cold-storage method, the pancreas viability in the canine model of the pancreatic autotransplantation and tissue concentration of adenosine triphosphate were examined after 24-hr preservation by original and modified two-layer methods with respect to the position of the pancreas and oxygen bubbling into the PFC. Namely, the pancreas was in EC and on the surface of PFC with (group 1, original method) or without (group 2) oxygen bubbling into PFC. The pancreas was floated in EC with oxygen bubbling into PFC (group 3); compared with simple cold storage of the pancreas in EC (group 4); and nonpreserved pancreas (control, group 5). The preserved pancreas grafts by each method functioned immediately after transplantation and maintained normoglycemia for at least 5 days except that 1 of 5 dogs in group 4 died of a cause unrelated to the pancreas graft. The functional success rates of groups 1, 2, 3, 4, and 5 were 100%, 100%, 100%, 80%, and 100%, respectively. It was clear that mitochondrial function was well-preserved during 24-hr preservation regardless of the preservation method. In the condition that the mitochondrial function is well-preserved the tissue concentration of ATP was mostly dependent on the tissue oxygenation. The tissue concentration of ATP of group 1, 7.92 +/- 1.06 mumol/g dry weight, was significantly higher than that of nonpreserved pancreas (group 5), 4.44 +/- 0.49 mumol/g dry weight (P less than 0.01). It was apparent that the two-layer method was excellent to supply oxygen to the pancreas and maintain high ATP concentration of the pancreas during preservation. In contrast, ATP concentration of the pancreas of group 2 was 1.83 +/- 0.30 mumol/g dry weight, and there was no significant difference between group 2 and group 4, 1.19 +/- 0.33 mumol/g dry weight, thus meaning that PFC was biologically inert without oxygenation. In addition, when the pancreas was not contacted with oxygenated PFC and floated in EC (group 3) ATP concentration of the pancreas, 2.24 +/- 0.90 mumol/g dry weight, was significantly lower than group 1 (P less than 0.01), and no significant different was found as compared to groups 2 and 4. It was essential that the pancreas was contacted with oxygenated PFC to maintain high ATP tissue concentration during preservation by the two-layer method.(ABSTRACT TRUNCATED AT 400 WORDS)

A new method for induction of experimental autoimmune uveoretinitis (EAU) in mice.

Experimental autoimmune uveoretinitis (EAU) was induced in two strains of mice by repeated-immunization protocol. SMA mice (H-2 nondefined) and C57BL/6 mice (H-2b) were immunized with S-antigen mixed with Klebsiella 03 lipopolysaccharide (K03 LPS) repeatedly at intervals of 1 to 4 weeks. Following the tertiary immunization, the mice exhibited histopathological changes of EAU as well as significant immune responses to the antigen. The antigen doses required for successful EAU induction were 4 micrograms or more at each immunization time. The histopathology of EAU was characterized by mild infiltration of mononuclear cells in the retina and the choroid, particularly, at the retinal blood vessels and the photoreceptor cell layer. The anterior segment of the eye was not affected by inflammation, and therefore clinical signs of EAU were not detected even under an operating microscope. Since the mouse is a genetically and immunologically well-defined species, this model is useful for study of immunopathogenic mechanisms of EAU.

Antigen-specific suppressor cells induced by FK506 in experimental autoimmune uveoretinitis in the rat.

The authors previously reported that FK506 effectively suppressed the induction of experimental autoimmune uveoretinitis (EAU) in rats with much lower doses than cyclosporine A. This study was aimed at analyzing the immune status of the FK506-treated and EAU-suppressed rats and examining the hypothesis whether the agent could induce antigen-specific suppressor T (Ts) cells. It was found that spleens from S-antigen-immunized and FK506-treated rats contained a population of Ts cells inhibiting the proliferative responses of S-antigen-sensitized lymphocytes to S-antigen, yet these cells did not affect the proliferative responses of interphotoreceptor retinoid-binding protein (IRBP)-sensitized lymphocytes to IRBP. The helper T (Th) cells did not exhibit such suppressor activities. Furthermore, transfer of Ts cells from S-antigen-immunized and FK506-treated rats to naive syngenic rats induced partial inhibition of EAU induction or delay of EAU onset after immunizing the recipient rats with S-antigen. Lymphocytes from the EAU-suppressed recipients showed low proliferative response to S-antigen and low levels of antibody to S-antigen. These data thus indicate that FK506 treatment after S-antigen immunization induces an activation of Ts cells specific to S-antigen and that the Ts cells might contribute, at least in part, to the uniquely prolonged and intensive immunosuppression by FK506.

[Airbronchogram sign in small peripheral bronchogenic carcinomas].

18 bronchogenic carcinomas (16 adenocarcinomas, one squamous cell carcinoma, and one large cell carcinoma) in 17 patients and the control group (benign nodules) were examined with high resolution and thin section CT. Thin-section CT images showed airbronchogram or airbronchiologram in 63%, multiple small cavitations in 63%, and heterogeneous densities 50% of the adenocarcinomas with excellent spatial and contrast resolution. Airbronchogram or airbronchiologram was not seen in other types of bronchogenic carcinomas and benign nodules. Therefore, demonstration of airbronchogram or airbronchiologram in nodular lesions can make the CT diagnosis of small peripheral bronchogenic carcinomas, especially adenocarcinomas.
