[Flow cytometric DNA analysis of papillary carcinoma of the thyroid using paraffin-embedded specimens].

A flow cytometric DNA analysis of papillary thyroid carcinomas has been performed on paraffin-embedded specimens, in order to determine the clinical usefulness of this method by defining the degree of the biological malignancy. Fifty-eight patients with papillary carcinomas were followed from 2 to 8 years. They were classified according to their prognosis into three groups: non-recurrent (47 patients), recurrent (5 patients), and a deceased group (6 patients). Aneuploidy was found in 6 (10.3%) of the total 58 patients. It was more frequently detected in the recurrent (40%) and in the deceased (33.3%) groups than in the non-recurrent group (4.3%). Higher proliferating index values were observed in the deceased group than in the recurrent and non-recurrent groups. These results indicate that a DNA analysis, using routine paraffin-embedded materials, is a useful adjunct in the determination of the degree of the biological malignancy of a papillary carcinoma.

Identification of a dihydropyridine-sensitive calcium channel in chick brain by a monoclonal antibody.

A Balb/c mouse was immunized with chick synaptic plasma membranes and monoclonal antibodies were produced by fusion of spleen cells with NS-1 mouse myeloma cells. One antibody, MAC-L1, immunoprecipitated more than 90% of the [3H]PN200-110-labeled calcium channels but only 20% of the omega -conotoxin receptor solubilized from the chick brain membranes. Thus possibly, a certain portion of the omega -conotoxin receptor in the chick brain is a dihydropyridine-sensitive calcium channel. By the specific immunoprecipitation of 125I-labeled proteins, two large polypeptides of 193kDa and 130kDa under reducing conditions were identified as the major components of the calcium channel.

[Estrogen receptor status and expression of carbohydrate antigens in breast cancer].

Seventy-six cases of primary cancer of the breast were studied by clinicopathological examination and immunohistological examination using monoclonal antibodies. They were composed of 40 cases positive for estrogen receptor (ER) and 36 cases negative for ER, the ER-positive rate being 52.6%. ER was detected more frequently in the postmenopausal group than in the premenopausal group, though the ER-positive rate was not significantly different between the two groups. The disease was advanced with a large tumor (greater than t3) and high-grade lymph node metastasis in ER-negative cases compared with ER-positive cases. The carbohydrate antigens specific to SPan-1, CA 19-9 and SLEX were found in 34.2%, 39.5% and 68.4% of the cases studied, respectively. These antigens were detected more often, if not significantly, in cases with a large mass and high-grade metastasis than in those with a small mass and without metastasis or with low-grade metastasis. The SPan-1-positive rate was significantly higher in ER-negative cases graded t1 or t2 than in ER-positive cases falling under the same category of tumor size. Significant differences were also noticed between the ER-negative and ER-positive groups with respect to CA 19-9-positive rate (t2 category) and SLEX-positive rate (t3 category). Similar data were obtained when our cases were classified by the degree of lymph node metastasis. In consequence, the present observations suggested that the expression of carbohydrate antigens in primary breast cancer and disease progress might be correlated with ER status.

Aminopeptidases in human retroplacental sera: purification and characterization of two enzymes.

Ten different maternal serum samples were analyzed for the hydrolysis of S-Bz-Cys-pNA (substrate for CAP) and Ala-pNA. The results showed clear differences in the activities in individual sera. Similar S-Bz-Cys-pNA hydrolysis activity was detected for all sera. However, Ala-pNA hydrolysis activity differed remarkably. Serum exhibiting low Ala-pNA hydrolysis activity contained only CAP, and that exhibiting high Ala-pNA hydrolyzing activity contained CAP and AAP. The two aminopeptidases were independently purified to a homogeneous state through the same purification procedures and some of their biochemical properties were compared. The enzymes were quite different with respect to molecular mass, the substrate specificities for some aminoacyl-pNA substrates, and the effects of inhibitors. Among various natural peptides tested for hydrolysis, the enzymes hydrolyzed Met-enkephalin most rapidly, but their modes of action were different. Furthermore, only CAP degraded oxytocin and AAP exhibited a high kinin-converting activity.

Accumulation of abnormally high ploid nuclei in the liver of LEC rats developing spontaneous hepatitis.

Enlarged hepatocytes with huge nuclei were found in LEC rats with hereditary hepatitis. Flow cytometric analysis of the DNA content of nuclei from jaundiced LEC rats revealed the presence of very high polyploids, such as 32n and 64n. At the age of 12 weeks, before the onset of hepatitis, 8n polyploid nuclei were more frequent in LEC rats than in LEA rats, a sibling line of LEC rats. Binucleated hepatocytes were also more frequent in LEC rats than in LEA rats at week 4. Bi-, tri- and tetra-nucleated cells whose nuclei were sometimes different in size were observed when jaundice became manifest. The number of proliferating liver cells, determined by pulse labeling with 5-bromo-2'-deoxyuridine (BrdU), was higher in LEC rats than in LEA rats at 2, 4, 8, 12 and 14 weeks, with a maximum at week 4. A remarkable increase of BrdU uptake was observed at week 16, when jaundice developed. The possible involvement of abnormal cytokinesis and kariokinesis in the manifestation of hepatitis was suggested.

Pharmacological effect of KC-404 on leukotriene release from human leukocytes induced by ionophore A23187.

Release of leukotriene (LT) B4, LTC4 and histamine from human peripheral mixed leukocytes stimulated with calcium ionophore A23187 was evaluated. The amounts of LTB4 and LTC4 in supernatants were analyzed by high performance liquid chromatography and histamine was measured by an automated fluorometric technique. A time-course study demonstrated the characteristic patterns of the release of these mediators. LTB4 and LTC4 were more slowly released than histamine and the release of these LTs was inhibited by KC-404, a novel anti-asthmatic drug which has no effect on histamine release.

A case of paroxysmal ventricular tachycardia during pregnancy.

We report a case of a 37-year-old woman who had paroxysmal ventricular tachycardia (VT) during early pregnancy. She had severe hyperemesis, palpitation at 6 weeks of gestation and many episodes of paroxysmal VT, but no apparent organic heart disease. At that time she had a transient increase of thyroid hormone levels. With bed rest and without medication, her symptoms and episodes of VT disappeared in accordance with the improvement of hyperemesis and thyrotoxicosis. She demonstrated a rare course of arrhythmias in which the deterioration of VT was observed at transient thyrotoxicosis and hyperemesis.

Functioning parathyroid lipoadenoma--report of four cases: clinicopathological and ultrasonographic features.

Functioning parathyroid lipoadenoma (hamartoma) composed of abundant adipose or myxomatous stroma and epithelial cell nests is an unusual cause of primary hyperparathyroidism. We report herein four new cases. None of them belongs in the category of multiple endocrine neoplasia or familial hyperparathyroidism. The clinical manifestations and the laboratory findings are indistinguishable from those of the usual forms of primary hyperparathyroidism. Ultrasonography of the neck demonstrated an enlarged parathyroid gland as a hyperechoic mass in the two patients tested. At operation in each case, a single enlarged gland was found and resected, the weight being 3, 0.3, 0.45 and 1 g, respectively. The patients are normocalcemic 1 to 10 years after surgery. Pathological examination disclosed that the lesions were consistent with lipoadenoma or its variants. On reviewing 20 cases of functioning lipoadenoma which were reported in the literature, including the present cases, we found that the size of the tumors varied and a functioning lipoadenoma is hence by no means unusually large as previously reported. Without knowledge of this specific clinicopathologic entity, the lesion may be overlooked at the preoperative localization study and misdiagnosed as a normal or hyperplastic parathyroid.

Expression of insulin-like growth factor receptors in primary human thyroid neoplasms.

The presence of IGF-I receptors was demonstrated in normal and neoplastic tissues of human thyroid. Binding of (125I)IGF-I to thyroid membranes was dependent on time and temperature of incubation, and maximal binding was achieved at 4 degree C and 18 h of incubation. (125I)IGI-I binding was dose-dependently displaced by unlabelled IGF-I; half-maximal inhibition occurred at concentrations of 10-20 milligrams. IGF-II and insulin had relative potencies of 5 and 1% compared with IGF-I. Scatchard analysis of binding data revealed a single class of IGF-I receptors with high affinity (Ka: 1.2-8.6 x 10(9) 1/mol) in normal thyroid tissues. Affinity cross-linking and autoradiography demonstrated the type IIGF receptors. Specific binding of (125I)IGF-1 in thyroid cancer tissues (9.69 +/- 2.07% per 200 micrograms protein; mean +/- SEM, N = 8) was significantly (p less than 0.05) higher than that in the surrounding normal tissues (3.03 +/- 0.35%, N = 8). In contrast there was no difference in the binding between adenoma tissues (4.19 +/- 0.53%, N = 5) and the adjacent normal tissues (2.94 +/- 0.24%, N = 5). The higher IGF-I binding in cancer tissues was due to an increase in the binding capacity without any change in the affinity. The presence of IGF-I receptors suggests a possible role of IGF-I and its receptors in the growth of thyroid cancer cells.

[A combined treatment of carcinoma of the maxillary sinus--treatment method for T4 cases].

Between 1974 and 1986, 54 cases of squamous cell carcinoma of the maxillary sinus were treated combining surgery, radiotherapy and regional chemotherapy simultaneously. Using UICC (1987) system, these 54 cases were classified into one T1, four T2, thirty-three T3 and sixteen T4. The combined therapy consisted of 1) reduction operation followed by daily cleaning of the necrotic tumor mass; 2) external irradiation with less than 30Gy; 3) intra-arterial infusion of 5-fluorouracil (5-FU) and broxuridine (BUdR); and 4) final tumor resection under general anesthesia. As several modifications were made for the treatment procedure in 1979, the 54 cases were divided into two groups: Group A with 29 cases, which were treated before 1979 and Group B with 25 cases, which were treated after 1979. In this paper, the operative procedure, which we use for T4 cases is described. The first operation is performed after radiotherapy with about 10Gy and in this operation only the necrotic portion of the tumor mass is removed. The maxillary cavity is opened through the sublabial incision. This window is kept open and daily cleaning of the necrotic tumor mass is continued through the window. Three weeks after the simultaneous administration of radiation with 20Gy and 5 intra-arterial infusions of 5-FU and BUdR, the residual tumor is curetted thoroughly under general anesthesia. Surgery is performed through the sublabial window which has been kept open after the reduction surgery. Removal of the residual tumor mass which extends to the pterygomaxillary fossa, nasopharynx, anterior and medial skull base can be performed without difficulty, provided that the tumor mass is removed in pieces.(ABSTRACT TRUNCATED AT 250 WORDS)

J-pouch: change of a method over years.

Since 1978, J. Utsunomiya has performed total colectomy, mucosal proctectomy and ileoanal anastomosis (IAA) in 105 patients and analysed their results in 97 patients consisting of 61 of familial polyposis (FP) and 36 of ulcerative colitis (UC) those who have been observed for two months to 10 years after operation. The operative procedures were classified in the three modalities. I. the "end to end-long cuff", II. "J-pouch-long cuff" and III. "J-pouch-short cuff" procedure. The success rate has been improved to 93.0% in the procedure III. compare with 74.1% in the II and 61.5% in the I. This improvement was achieved by preservation of reservoir continence, reduction of pelvic sepsis and less damaging of the internal sphincter during operation. Through a series of evolutional modification, the surgical technique has been established to the present method which is constructed with the four principal components: direct anastomosis of J-ileal pouch to the anus, short rectal cuff mucosectomy, anoabdominal approach at prone jack-knife position and routine use of defunctioning ileostomy. With this technique, both FP and UC patients enjoyed excellent or good function in 90% of the cases. Sex of the patient did not influence the functional result, but older patients showed poorer results compared with the younger patients. This procedure can provide a function comparable with ileorectal anastomosis with a technical difficulty similar to that of the abdominal pouch. All patients with FP or UC who require colectomy are candidates for IAA unless they are 60 years or older. For UC a three stage surgical procedure of IAA proceeded by open rectal excluding colectomy could improve the result.(ABSTRACT TRUNCATED AT 250 WORDS)

[Radial flap: a valuable fasciocutaneous flap for intra-oral reconstruction].

The radial forearm flap, or the forearm flap, is called "Chinese flap" for its development of the chinese doctors, and is originally designed for the correction to the post-burn contraction of the face and neck. The radial forearm flap is one of the fasciocutaneous flap, supplied by the radial artery, and transferred as a single-stage reconstruction micro-surgically. In oral and maxillofacial region, the deltopectral flap (D-P flap) and the pectralis major myocutaneous flap (P-M-M-C flap) are mainly used for the reconstruction. These flaps, however, are sometimes too bulky and limited to transfer, and more require two-stage operations. On the other hand, as the forearm flap being thin and pliable, some doctors use this flap micro-surgically at single-stage free flap reconstruction. Before two years, we have begun to transfer the radial forearm flap for the intra-oral reconstruction. The operation method is as follows. Design and Elevation of the Radial Forearm Flap 1. Using the ultrasonic doppler flow meter, the radial artery and the subcutaneous forearm veins are marked on the skin. 2. The flap is designed 20% larger according to the pattern to be reconstructed, with the distal section of the radial artery as an axis on the forearm and the median vein of forearm inclusively. 3. Before the operation, Allen test must be performed in order to determine whether the hand will survive without a radial arterial in-put. 4. The operation is performed with a arm tourniquet. The margin of the flap are incised down to the deep fascia, isolating and preserving the proximal subcutaneous veins as required.(ABSTRACT TRUNCATED AT 250 WORDS)

Characterization of two possible forms of type IV collagen from human kidney cortex.

Type IV collagen was isolated from human kidney cortex according to the purification of mouse kidney type IV collagen (MKIVC) with minor modifications as described previously [Oikawa, T., et al. (1986) Chem. Pharm. Bull. 34, 789-797]. Chromatography of human kidney type IV collagen (HKIVC) on a column of DEAE-cellulose resulted in its separation into two fractions, i.e., one (HKIVC-1) passed through and the other (HKIVC-2) adsorbed to the column, similar to MKIVC. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and Bio-Gel A-5m chromatography revealed that both HKIVC-1 and -2 differed from each other in the ratios of major and minor components. Amino acid analyses also demonstrated that there was a significant difference in the contents of several amino acid residues between both type IV collagens and their 80-kDa components, one of the major ones. These results indicate the possibility that there exist two forms of type IV collagen in human kidney.

Inhibition of paraquat accumulation in rabbit kidney cortex slices by ascorbic acid.

Effects of ascorbic acid and Fe2+ on the accumulation of paraquat into rabbit kidney cortical slices were studied. Ascorbic acid showed dose-dependent inhibition of paraquat accumulation. Fe2+ had a more powerful stimulatory effect on the lipid peroxidation of cortical slices than ascorbic acid, but it did not reduce paraquat accumulation. These results suggest that ascorbic acid is able to inhibit the accumulation of paraquat independent of its peroxidative action, and that ascorbic acid may serve as an antidote for the toxicity of paraquat.

Hereditary hepatitis in LEC rats: accumulation of abnormally high ploid nuclei.

The LEC (Long-Evans with a cinnamon-like coat color) rat is a new mutant strain with hereditary hepatitis. The rate of DNA synthesis, the relative amounts of binucleated cells, and the polyploidizations of LEC hepatocytes have been analyzed. Markedly high polyploidy, such as 32n and 64n, were detected after manifestation of hepatitis; however, no aneuploidy was detected. Bi-, tri- and tetranucleated cells whose nuclei occasionally differed in size were observed after the manifestation of hepatitis. In addition to small hepatocytes and oval cells in the periportal area of the hepatic lobule, enlarged cells with huge nuclei were also labeled with BrdU, indicating that in LEC rats suffering from hepatitis abnormal mitosis may be relevant to high polyploidization and multinucleation. Polypeptide analysis using 2D-PAGE detected the apparent lack of expression of two polypeptides, p29.5 and p30, in LEC liver cells; however, linkage analysis indicated no correlation between these peptide defects and the manifestation of hepatitis.

[2 cases of intermittent hydronephrosis--usefulness of ultrasonography].

Case 1: An 8-year-old boy consulted our clinic complaining of left flank colicky pain. He had a past history of the same episode. Ultrasonographic study was promptly performed. Left hydronephrosis was detected. At operation, left aberrant renal vessel was resected, and he has been free of pain. Case 2: A 65-year-old woman consulted our clinic because of abdominal pain and fever, who occasionally had the same episode. The existence and disappearance of the right hydronephrosis could be observed by emergency ultrasonography and following ultrasonic study, and the stenosis of the right lower ureter caused of cystitis cystica and glandularis was pointed out by retrograde ureterography. Diuresis ultrasonography by furosemide was performed to evaluate urinary tract stricture. In case 1, an incomplete obstruction pattern was obtained after operation, and in case 2, a complete obstruction pattern was shown. The emergency ultrasonography and diuresis ultrasonography were useful for the diagnosis and observation of intermittent hydronephrosis.

[A case of testicular tumor presenting as acute scrotum].

A case of teratoma of the testis presenting as sudden spontaneous hemorrhage without previous injury is described. A 25-year-old male was admitted with right scrotal pain and swelling. Though acute epididymitis or testicular torsion could not be neglected on physical examination, tumor-like echogram was obtained. High orchiectomy was performed subsequently. Macroscopically, testicular tumor with subcapsular hematoma was evident. Histopathological diagnosis was mature teratoma (pT1, No, Mo). The patient, after combined chemotherapy including cisplatin, vinblastine and peplomycin, is alive well without metastases for 15 months after operation.
