Synthesis and absolute configurations of the cytotoxic polyacetylenes isolated from the callus of Panax ginseng.

Panaxacol (1) and dihydropanaxacol (2), cytotoxic polyacetylenes isolated from the callus of Panax ginseng, were synthesized starting from D-(-)-diethyl tartrate. The absolute configuration of 1 was determined to be 9R, 10R and the absolute configuration at C-3 of 2 was tentatively assigned as 3S by the application of the R(+)-alpha-methoxy-alpha-(trifluoro methyl)phenylacetyl (MTPA) method.

Presence of a dipeptidyl aminopeptidase III in Saccharomyces cerevisiae.

The soluble fraction of a cell extract of Saccharomyces cerevisiae was found to contain hydrolytic activity toward arginyl-arginyl-beta-naphthylamide (Arg-Arg-beta NA). Most of the contaminating Arg-beta NA hydrolysis activity was removed by diethylaminoethyl (DEAE)-cellulose column chromatography and the enzyme was partially purified. It is very similar in various properties to mammalian dipeptidyl aminopeptidase III (DAP III), although it differs slightly in some respects.

Evidence of two forms of basic arginine esterases in human male urine.

Two forms of basic arginine ester hydrolyzing enzymes were found in human male urine, and their partial purification was performed using lima bean trypsin inhibitor (LBTI) affinity adsorption and Cellulofine GCL-2000 gel chromatography. The forms tentatively called basic human urinary arginine esterase-1 (BHUAE-1) and -2 (BHUAE-2), had respective estimated molecular weights of about 4.5 x 10(4) and 1.8 x 10(4) daltons, with pH optima observed at 9.5 and 10.0. These two new enzymes in the human male urine were different from human renal urokinases.

Stromal fat content of the parathyroid gland.

The proportion of stromal fat cells to parenchymal cells in 100 normal parathyroid glands was determined by the image analyzing computer technique. The parathyroid glands were resected at the time of thyroidectomy in 86 patients with thyroid tumors. None of the patients had any evidence of parathyroid dysfunction preoperatively. In the histologic sections of the parathyroid glands, the average percentage of stromal fat cell content was 38%. The percentage of stromal fat cells was correlated with the age and the body constitution of the patients, but the percentages of fat cells varied widely among glands in the given age and body constitution ranges. It was therefore not possible to discriminate a normal parathyroid gland from an abnormal gland solely on the basis of microscopic determination of stromal fat cell content.

Trial of thyroid autotransplantation in patients with Graves' disease whose remnant thyroid has unintentionally been made too small at subtotal thyroidectomy.

Autotransplantation of thyroid tissue was carried out in 5 patients with Graves' disease in order to prevent postoperative hypothyroidism, because the amount of remnant thyroid tissue was estimated to be too small, i.e. from 3 to 5 g. Approximately 0.5 to 2 g of thyroid tissue was cut into small pieces and transplanted into the sternocleidomastoid muscles or the strap muscles. Although the postoperative serum TSH levels were normal or slightly elevated, the serum concentrations of triiodothyronine were within the normal range in these 5 patients at a follow-up study carried out 2 to 7 years after surgery. Thyroid scanning with I-123 or 99mTc-pertechnetate (Tc-99m) revealed radioisotope uptake at the sites of transplantation in 4 of the 5 patients. These findings verify that the implanted thyroid tissues were alive and functioning and that autotransplantation may be a way of preventing postoperative hypothyroidism in patients whose remnant thyroid tissue has unintentionally become too small.

A case of secundum atrial septal defect combined with aortic stenosis and coronary artery disease showing a single second heart sound.

We describe an extremely rare case of secundum atrial septal defect with aortic stenosis and coronary artery disease showing a single second heart sound throughout the respiratory cycle by echocardiogram with simultaneous phonocardiogram. Aortic valve closure corresponded to the single second heart sound. We were unable to detect pulmonary valve closure (PVC) on echocardiogram. Because of the presence of pulmonary hypertension, the pulmonary component of the second heart sound (P2) was presumed to be increased in intensity, and the PVC-P2 interval was thought to be abbreviated. Carotid pulse tracing showed a prolongation of the left ventricular ejection time. We concluded that the single second heart sound was due to both prolongation of left ventricular systole and pulmonary hypertension.

[Lymphocytic adenohypophysitis. Case report].

The patients was a 27-year-old female, gravida 0, para 0, with complaint of headache and visual disturbance for about 1 month. Ophthalmological examination showed impaired visual acuity and enlargement of Mariotte's spot on the left. A computed tomography scan disclosed an enhanced mass in the sellar and suprasellar region. Endocrinological examination revealed slight elevation of thyroxin, growth hormone, and prolactin. A trans-sphenoidal hypophysectomy was carried out with a preoperative diagnosis of nonfunctioning pituitary adenoma. Histological examination showed a marked infiltration of lymphocytes and interstitial fibrosis, diagnosed as lymphocytic adenohypophysitis. She received hormone-replacement therapy owing to postoperative hypopituitarism. Twenty-eight cases of lymphocytic adenohypophysitis including the author's are classified clinically into two types; fulminant type and chronic type. The former type occurs frequently with disturbance of consciousness because of the end organ insufficiency and proceeds to death in a fulminant course. The latter type occurs frequently with headache and visual disturbances, closely related to pregnancy and the postpartum period and survival occurs with hormone replacement therapy. This disease should be kept in mind in the diagnosis of sellar and suprasellar mass lesions.

[Endoscopic and fluoroscopic pyeloureterotomy for ureteropelvic junction obstruction and ureteral stricture].

Endoscopic or fluoroscopic pyeloureterotomy was performed in a total of 32 patients (34 ureters) with ureteropelvic junction obstruction (UPJO) or ureteral stricture from March 1985. Cold incision was performed, using a flexible knife over a guidewire, endoscopically on 23 ureters and fluoroscopically on 11 ureters. The mean operation time was 98.9 +/- 48.3 minutes. Twenty-five (74%) of the 34 ureters showed relief of subjective symptoms, normalized urinalysis and radiographical improvement of hydronephrosis with a mean follow-up period of 11.7 +/- 10.0 months (range: 3-43). The clinical results by etiology were as follows: primary UPJO, 13/16 (81%); secondary UPJO, 4/4 (100%); ureteral stricture, 6/7 (85%); obliteration of ureteral implantation, 2/7 (29%). No major complication was noted with this endourological technique. This endourological technique might be clinically useful for the treatment of UPJO and ureteral strictures.

Features of regenerated clones with or without fusion treatment between auxotrophic mutants of Streptomyces antibioticus and their antibiotic productivity.

During experiments on protoplast fusion of complementary auxotrophic mutants (194 and 11M-21) of Streptomyces antibioticus for strain improvement, the clones (typified by F-40) regenerated on minimal regeneration medium (MRM) were found to be prototrophs, and to produce an antibiotic different from those produced by the parent strain. The protoplast regeneration of each parent was examined as a negative control experiment. In the regenerated clones of 194, half of them produced actinomycins similar to those produced by the original mutant 194, but others (typified by R-20) seemed to produce antibiotics similar to those produced by F-40. In the taxonomic characterization of morphological, cultural, and physiological properties of each strain, F-40, R-20, and the parent mutant 194 had no significant differences with a few exceptions. The problem here is whether the antibiotic of R-20 is the same as that of F-40, which was first isolated and found to be a peptide antibiotic different from actinomycins, with activity against Gram-negative and Gram-positive bacteria.

[Establishment of monoclonal antibodies against purified mucin from human pancreatic cell line].

Thirty four murine monoclonal antibodies were produced against mucin which was isolated from nude mouse xenografts of the SW1990 pancreatic adenocarcinoma cell line. Twelve of them reacted with formalin-fixed SW1990 cells. The antigenic determinants of these antibodies were composed of carbohydrates and their immunoreactivities were predominant at CsCl fraction #6. The remaining 22 antibodies did not react with formalin-fixed cells, 5 epitope specificities of which antibodies are present in the polypeptide part or the part of linkage between oligosaccharide and polypeptide and the immunoreactivities were predominant at CsCl fraction #5. Immunohistochemical study showed that the incidence of epitope expression of the latter 22 antibodies were higher in pancreatic cancer tissues and lower in normal pancreatic tissues than that of the former 12 antibodies. The incidence of Nd2 (one of the 22 antibodies) expression was highest in pancreatic cancer tissues and lowest in normal tissues. ELISA showed that Nd2 antigen was non-secreting type. In vivo tumor distribution studies showed that Nd2 antibody tended to accumulate in cancer tissues. These results seem to support the clinical availability of Nd2.

Malignant schwannoma and melanoma occurring in the maxilla.

A rare case of malignant melanoma of the hard palate following malignant schwannoma of the left maxilla in a 47-year-old woman is reported. Twenty-two months after surgical excision of malignant schwannoma with postoperative radiation and chemotherapy, malignant melanoma appeared with symptoms of gingival bleeding and swelling. Although the patient was treated by the partial maxillectomy with radical neck dissection and postoperative chemotherapy and adjuvant immunotherapy, she died of multiple distant metastases 9 months after the oral manifestation. A possible histological relationship of both tumors which are considered to be derived from the neural crest cells is also discussed.

Human placental carboxypeptidase M is anchored by a glycosyl-phosphatidylinositol moiety.

Basic carboxypeptidase activity was released from human placental membranes on treatment with phosphatidylinositol-specific phospholipase C of Bacillus thuringiensis. The enzyme was successively purified to homogeneity by SDS-polyacrylamide gel electrophoresis. The molecular nature and some catalytic properties of the purified enzyme revealed that it is identical with recently described basic carboxypeptidase M (R.A. Skidgel et al. J. Biol. Chem. 264 (4) 1989 2236-2241).

Poxvirus infection in Nile crocodiles (Crocodylus niloticus).

An outbreak was encountered of numerous yellowish cutaneous nodules in one- to two-year-old farmed Nile crocodiles (Crocodylus niloticus) in Kasaba Bay Crocodile Farm at Lake Tanganyika in Zambia during 1988. Out of 4000 crocodiles of different age groups, 300 yearlings were affected and 82 of those affected died. The lesions were prominent on the head, especially around the eyelids, the nostrils, both sides of the mouth, ventral neck, ventral pale belly, limbs and the root of the tail. Histologically, the epidermal lesions revealed large focal areas of marked acanthosis accompanied by hyperkeratosis and parakeratosis. In the prickle cells, there were multiple small neutrophilic granular inclusions and large eosinophilic homogeneous cytoplasmic inclusions. Electron microscopically, there were numerous poxvirus particles and matrices in the cytoplasm of many prickle cells. Dark blue homogeneous cytoplasmic inclusions in toluidine blue sections consisted of an electron opaque matrix with mature viral particles (160 x 200 x 230 nm). Furthermore, there were clumps of granular matrix containing immature viral particles (200 x 399 nm) in the cytoplasm. Various features of viral developing processes were observed.

Severe primary hyperparathyroidism in a neonate having a parent with hypercalcemia: treatment by total parathyroidectomy and simultaneous heterotopic autotransplantation.

Neonatal primary hyperparathyroidism is a life-threatening disease because of marked hypercalcemia and severe respiratory distress caused by the hypoplastic thorax and occasional rib fractures. We report a 29-day-old girl treated by total parathyroidectomy and simultaneous autotransplantation of parathyroid tissue (one fifth of each of the two glands) in the femoral quadriceps muscle near the groin. At the time of operation, all four of the parathyroid glands were markedly enlarged, and their total weight was 900 mg. Part of the resected parathyroid tissue was cryopreserved for further autotransplantation should hypoparathyroidism develop. Two years six months after surgery, the infant was well and had normal levels of serum calcium and immunoreactive parathyroid hormone in the absence of any supplementary treatment. Asymptomatic hypercalcemia in the presence of abnormally low fractional excretion of calcium was found in the father. Based on our experience and review of the literature, we recommend total parathyroidectomy, autotransplantation, and cryopreservation for the neonate with primary hyperparathyroidism.

[A case of retroperitoneal tumor presenting with gross hematuria].

A case of retroperitoneal tumor presenting with gross hematuria is reported. A 62-year-old female consulted our clinic with the chief complaint of gross hematuria. On physical examination, a goose-egg sized tumor was palpable in the left flank region. Drip infusion pyelography and computerized tomographic scan showed left retroperitoneal tumor which deviated the left kidney upwards. Percutaneous needle biopsy of the tumor revealed no malignancy. Total resection of the tumor was performed subsequently. A yellowish solid tumor was macroscopically encapsulated by fibrous tissue, weighed 230 g and 6 x 7 x 10 cm. Histopathological diagnosis was malignant schwannoma. After operation, the hematuria stopped without any treatment and deviation of the left kidney was improved. Soft tissue tumor should be treated by adjuvant chemotherapy with irradiation because of its high frequency of recurrence and metastasis. Combined chemotherapy with VCR, ADR, CPM and DTIC (CYVADIC) was performed and she is in good health at 1 year after operation.

[Clinical application of Therasonic lithotripsy treatment system for upper urinary tract calculi].

Extracorporeal shock wave lithotripsy (ESWL) using a Therasonic lithotripter was performed on 30 patients with urinary tract calculi between July, 1989 and February, 1990. The results obtained in 31 cases (one patient had bilateral renal stones) were presented. There were 22 inpatients and 8 outpatients in this series. The stone location was: renal pelvis in 15 cases, renal calyces in 10 cases, parenchyma or diverticulum of renal calyces in 3 cases, ureteropelvic junction in 1 case, and upper ureter in 2 cases. Sixteen cases (52%) became stone-free and residual stones (less than or equal to 4 mm) remained in 9 cases (overall effective rate 81%) at one month after the treatment. There were no serious complications including bacteremia and renal hematoma. We concluded that Therasonic lithotripter is useful in the management of upper urinary tract calculi.

Effect of zonisamide on serum immunoglobulins.

Zonisamide (AD-810) was additionally administered to 19 patients with refractory epilepsy who had been receiving multiple antiepileptic drugs and the effect of zonisamide on serum immunoglobulins was investigated. Zonisamide was administered in daily dose of 200-700 mg (mean 389 mg) for the period of 82-1,470 days (mean 863 days). The serum levels of IgG, IgA and IgM were determined before administration and at intervals of 3 or 6 months over the period of a maximum of 48 months after initiation of administration. There was no significant change in immunoglobulin levels at any time after initiation of zonisamide treatment as compared with the pretreatment levels. No significant correlation was found between the dose or serum concentration of zonisamide and the immunoglobulin levels.

The effect of selenium ions on the catabolism of prostaglandin E2 in rabbit kidney cortex.

The effect of selenium ions on the catabolism of prostaglandin E2 by 15-hydroxy prostaglandin dehydrogenase and prostaglandin delta 13 reductase in rabbit kidney cortex was examined. Selenium ions had no effect on the 15-hydroxy prostaglandin dehydrogenase activity. On the other hand, selenium ions inhibited the prostaglandin delta 13 reductase activity at concentrations ranging from 0.05 to 0.5 mM. The effect was concentration-dependent. The inhibition by selenium ions was non-competitive with regard to NADH and 15-keto PGE2, respectively. These results suggest that selenium ions have the potential to increase 15-keto PG levels in the kidney by inhibiting the prostaglandin delta 13 reductase activity and that such action of selenium ions may participate in renal damage of selenium poisoning.
