Significance of the hepatic mitochondrial redox state in the development of posttraumatic jaundice.

The effect of the hepatic energy status on the development of posttraumatic jaundice (PTJ) was studied to clarify the mechanism of PTJ. Fifty-four patients with severe torso injury who were expected to develop PTJ on admission with an average Injury Severity Score of 27 were selected for this study. They were retrospectively divided into three groups according to their maximum bilirubin concentration by day 10: group H, 12 patients with marked elevation of serum bilirubin (> 8 g/dl); group L, 23 with mild bilirubinemia (2-8 g/dl); and group N, 19 with no bilirubinemia (< 2 g/dl). Group H patients, in whom trauma-related shock was severe and prolonged, developed severe hyperbilirubinemia, and their arterial ketone body ratio (AKBR), which reflects the hepatic mitochondrial redox state and is closely correlated to its energy production, was significantly lower throughout the first week. In contrast, the AKBR increased to an above normal level, indicating enhanced energy production in groups N and L. The serum direct/total bilirubin was also higher in group H. The abnormal hepatic energy metabolism is considered to have reduced the excretion of conjugated bilirubin from the hepatocytes into the bile canaliculi, which is a process that has to proceed against the bilirubin concentration gradient. The subsequent diffusion of the accumulated water-soluble conjugated bilirubin in hepatocytes into the blood is thus considered to be one of the causes of PTJ.

Coronary thrombosis induced by intracoronary acetylcholine injection in a patient with normal coronary myocardial infarction.

A 67-year-old male was admitted with acute myocardial infarction. Coronary thrombolysis was carried out because complete occlusion of the obtuse marginal branch (OM) in the circumflex artery was detected in emergent coronary angiography, and recanalization of the OM was obtained at 3 hours after the onset of chest pain. No significant stenosis of the OM was found in coronary angiography performed at the recovery stage. After intracoronary acetylcholine injection to the left coronary artery, coronary spasm was induced and coronary thrombosis was observed in the left anterior descending artery thereafter. These findings indicate the possibility that the etiology of myocardial infarction is coronary thrombosis induced by coronary spasm.

Coexistence of choriocarcinoma and hepatoid adenocarcinoma in the stomach.

The case is presented of a 46 year old woman who had a gastric tumor with components of choriocarcinoma, hepatoid adenocarcinoma and common types of adenocarcinoma. Although two histologic types of tumor producing carcinoplacental or carcinofetal proteins were contained within the tumor, immunohistochemical analyses, especially of placental alkaline phosphatase, clearly showed that each component was present separately within the same tumor. It was only hepatoid adenocarcinoma cells that permeated the lymph and blood vessels. After the recurrence, the serum level of alpha-fetoprotein (AFP) markedly elevated, but that of human chorionic gonadotropic beta-subunit (hCG-beta) was always within normal range. These findings indicate that in the present case the hepatoid adenocarcinoma component was more aggressive in growth than the choriocarcinoma component.

[Injury of pulmonary artery and intrathoracic artery--a surgical case report].

The patient was a 52-year-old man. Tube drainage was performed to the right traumatic hemothorax. Then, 2,000 ml of blood flowed out and bleeding stopped for a time. Four hours later after injury, massive bleeding occurred again. So emergency thoracotomy was performed and we found a laceration in the main pulmonary artery and injury of intrathoracic artery. Pulmonary artery disruption from blunt chest trauma is rare. Further discussion was made by reviewing the related literature.
