Morphological investigations on cerebellar "neuroblastoma" group.

Classic medulloblastoma is a relatively common and distinct clinicopathologic entity consisting of primitive multipotential cells with differentiating capacity to neuroblastic and/or glial cell lines. Desmoplastic medulloblastoma has some features in common with cerebellar neuroblastoma, in which ultrastructural evidence of significant neuroblastic differentiation is extremely rare. We studied three cerebellar tumors with evidence of neuronal differentiation as compared to four classical desmoplastic medulloblastomas. Two of three tumors contained the regions of different degrees resembling desmoplastic medulloblastoma and one consisted of neuroblastic cells exclusively. This spectrum of differentiation suggests a relationship between cerebellar neuroblastoma and medulloblastoma, especially of the desmoplastic type. The nature of cerebellar neuroblastoma and its nosology are briefly discussed.

Human leukocyte antigen-linked genetic controls for T cell-mediated cytotoxic response to mumps virus in humans.

The involvement of human leukocyte antigen determinants for the response of mumps virus-specific cytotoxic T lymphocytes in humans was studied. The cytotoxic T lymphocytes could only lyse virus-infected allogeneic cells with which they shared a particular human leukocyte antigen, e.g., Bw52 or B7. The existence of Bw52 in the subjects who received a booster immunization with live mumps vaccine was associated with a significantly higher cytotoxic T-lymphocyte response than that of subjects without the gene. Although a donor-dependent difference in the recognition of human leukocyte antigen-A2 suggests the complexity of the genetic mechanisms involved, the results are largely consistent with the concept of major histocompatibility complex-linked genetic control of virus-specific cytotoxic T-lymphocyte response.

Cerebrospinal fluid edema: a rare complication of shunt operations for hydrocephalus. Report of three cases.

Three cases are presented in which a rare complication occurred after a shunt operation for hydrocephalus. On postoperative computerized tomography (CT) scans, extensive low-density areas appeared in the white matter along the ventricular catheter. After shunt revision, gradual resolution or disappearance of the low-density area was clearly demonstrated on CT. In one patient, a collection of cerebrospinal fluid (CSF) was confirmed at operation and appeared to lie in the extracellular spaces of the white matter. The phenomenon is considered to be localized CSF edema, different from porencephaly.

[Clinical evaluation of cefotiam in internal medicine].

Cefotiam (CTM) was administered to 52 patients with infectious disease associated with respiratory system, hematological malignancy, urinary system and other system. Good clinical responses were obtained in 38 out of 52 cases (73.1%). Neither objective and subjective side effects nor extreme abnormalities of laboratory tests were observed in these patients. It can be, therefore, concluded that CTM is 1 of the most useful drugs for infectious diseases in respiratory system, hematological malignancy, urinary system and other system.

Vitamin D dependent rickets: decreased sensitivity to 1,25-dihydroxyvitamin D.

A patient with vitamin D dependent rickets with decreased sensitivity to 1,25-Dihydroxyvitamin D was observed. She suffered from bone pain of two years duration beginning at 12 years of age and was found to be suffering from hypocalcemia, secondary hyperparathyroidism and osteomalacia. Laboratory findings revealed normal serum 25-hydroxyvitamin D (27 ng/ml) and markedly elevated serum 1,25-dihydroxy-vitamin D (131.9 pg/ml). The hypocalcemia was refractory in spite of administration of 25,000 units of vitamin D2, but therapy with high doses of oral la-hydroxyvitamin D3 resulted in significant elevation of the serum calcium level. The clinical findings and course of the patient's disease were quite different from those of other patients with vitamin D dependent rickets reported by other authors.

Changes in lymphocytes as an early sign of relapse in adult acute leukemia.

This study was undertaken to find out some clue to detect early relapse of adult acute leukemia which had been in remission. Laboratory data such as LDH, erythrocyte sedimentation, or immunoglobulin level did not show any difference at the time of relapse from the remission period. Complete blood count with reticulocyte count was evaluated every two weeks retrospectively for 12 weeks before relapse. No significant change was observed at the time of relapse. However, both the percentage and absolute number of peripheral lymphocytes significantly increased, whereas percentages of both T and B cells markedly decreased at the time of relapse, suggesting some derangement of lymphocyte function. Results in bone marrow culture did not yield any remarkable difference. It was suggested from this study that it is the time of prerelapse of adult acute leukemia when lymphocytes show an increase in either percentage or absolute number in the patients who are followed under their maintenance therapy schedule.
