[Granulocytic progenitors in bone marrow autografts].

Granulocytic progenitor cells (CFC) are a better reflection of stem cell pools than precursors. Their total number can be estimated during hematologic steady-state (N = 70 +/- 29 x 10(5) CFC/kg); the amount of marrow necessary to perform a bone marrow transplantation is only 1% of the total pool. CFC are useful but have a mainly qualitative value in evaluating the viability of marrow stored for 24 h at room temperature (92%) or marrow thawed after DMSO cryopreservation (62%). Sequential study of CFC in patients receiving autologous marrow transplantation enables differentiation of engraftment from autologous recovery. In 11 recipients of autologous marrow transplantation, no statistical relation was observed between number of infused CFC and severity of granulocytopenia. Further studies will be necessary to evaluate a better marker of real hematopoietic stem cells.

[Thymic seminoma. Apropos of a case diagnosed in a woman].

Primary thymic seminoma is a rare germ cell neoplasm histologically identical to testicular seminoma. About 100 cases have been reported in the world literature. The most probable pathogenic theory is abnormal migration of germinal cells from the vitelline sac to the embryonic thymus. Management involves surgery and radiotherapy. The extreme radiosensitivity of the tumor is responsible for a 5-years survival rate of 75 per cent. Thymic seminomas are usually found in young men. We report the case of a 49 year female in whom the combination of surgery and radiotherapy followed by chemotherapy provided excellent results.

[Chemotherapy of extensive digestive cancers with 5-fluorouracil: relation between the clinical response and plasma clearance of the drug].

This study was designed to establish a possible relationship between the pharmacokinetics of 5-FU and response to treatment. Thirteen patients with advanced, histologically proven malignancies of the gastrointestinal tract were studied. 5-FU, at a single weekly dose of 15 mg/kg, was given in bolus or as an 8-h infusion to every patient. Plasma clearance of 5-FU after an i.v. bolus injection of 15 mg/kg was individually determined and found to vary widely among patients (from 0.39 to 2.55 l/min). A clearance greater than or equal to 0.86 l/min was seen in all the patients who did not respond to treatment. All patients (except one) with a partial response or a stable disease had a clearance less than or equal to 0.72 l/min. The mean clearance of non-responders (1.40 +/- 0.21 l/min) was significantly greater (P less than 0.02) than that of partial responders or patients with stable disease (0.73 +/- 0.15 l/min). These preliminary results deserve further investigation on a larger number of patients, before plasma levels of 5-FU may be accepted as an indication of response to chemotherapy.

[Cis-platinum-induced peripheral neuropathy. One case (author's transl)].

A case of peripheral neuropathy secondary to cis-platinum therapy is reported. On the basis of electrophysiological studies and of biopsies, the predominantly sensory disorders observed (paraesthesias, pain, sensory ataxia) could be ascribed to axonal degeneration. This exceptional complication of cis-platinum therapy deserves to be borne in mind, since the drug is increasingly used, notably in genital cancers.

2' and 3' ribonucleoside monophosphate in leukocytes of acute myeloid leukemia: markers for early diagnosis of relapse.

Levels of 2' and 3' purine and pyrimidine ribonucleoside monophosphates (2'-, 3'-NMP) in leukocytes from blood and/or bone marrow were measured in three adult patients with acute non-lymphoblastic leukemias. The measurements of 2'-, 3'-NMP were made by high-performance liquid chromatography (HPLC) at various times in the course of the disease. Complete remission (CR) was obtained for all three patients but two of these have since died after relapsing at 8 and 9 months, respectively. The third patient remains in CR at 1 1/2 year. The levels of 2'-, 3'-NMP in the leukocytes of the patient remaining in remission have not changed since the beginning of his remission. However, in the patients who relapsed 2'- and 3'-NMP levels increased first in bone marrow than in blood leukocytes. These increases occurred about 3 months before the relapse was detected by morphological criteria. These data suggest that 2'-, 3'-NMP measurements may have a prognostic value if used to monitor patients with acute myeloid leukemia in CR.

[Incidence, prognosis and prevention of septicaemias in patients under treatment for acute leukaemia (author's transl)].

Septicaemias are frequent and severe in patients with acute leukaemia under aplastic treatment. The present study concerns 69 such patients: 29 with acute lymphoblastic leukaemia (ALL), and 40 with acute non-lymphoblastic leukaemia (ANLL). All were treated in single rooms in the same hospital and in similar conditions. The overall incidence of septicaemia was 62%; it was 60% in patients with recently diagnosed ALL and 68% during relapses. More than 34% of ALL patients and 82.5% of ANLL patients had one or several episodes of septicaemia. Among the 74 pathogens isolated 50% were Gram-positive organisms, 45% Gram-negative organisms and 5% Candida spp.. The first episodes of septicaemias were predominantly caused by Gram-positive spp. (61%) and the subsequent ones by Gram-negative spp. (60%). The primary infection could only be diagnosed in 19% of the cases and was most frequently located in the digestive tract or perineal region. The most common focal complications were lung infections (18 cases), skin infections (12 cases) and septic shock (15 cases). Seventy-four p. cent of the patients survived with prompt and potent antibiotic therapy. Death occurred in 26% and was clearly related to the following factors: chemotherapy of relapsed leukaemia and/or blastic aplasia and/or successive episodes of septicaemia. The incidence and severity of septicaemias in leukaemic patients will only be reduced by improved prophylactic measures against infection and by less pronounced and shorter chemotherapy-induced granulocytopenia.

[Immunologic assessment and acoustic "stressing situation" in an experimental system BALB/c mice, the Tsc-3T3 tumor].

Previous results in Hamsters showed enhancement of tumor takes in animals exposed to acoustic aggression. Since this phenomenon could be due to immunodepression, we investigated whether this was the case in a better defined system of inbred BALB/c Mice. The results show that in these conditions, the tumor graft could not transgress a minor histocompatibility antigen nor decrease the protective effects of polyoma virus against tumor challenge and that all the immune tests investigated were unaltered.

Follow-up of a randomized trial for oat cell carcinoma evaluating the efficacy of peripheral intravenous nutrition (PIVN) as adjunct treatment.

A randomized trial was initiated to compare the effects of peripheral i.v. nutrition (PIVN)-associated chemotherapy (adriamycin, vincristine, VP-16-213, and cyclophosphamide) versus a chemotherapy control group in patients with oat cell lung carcinoma. Thirty-nine evaluable patients were randomized. The test group included 19 patients, whereas 20 were followed in the control group. PIVN was scheduled each day the patient underwent chemotherapy. Each patient received 1,550 kcal day which included 10% glucose, 20% lipids, and amino acids which may or may not have been mixed in the same infusion bottle. The results show ten PIVN patients presently in complete remission at the end of three courses of treatment, compared to nine over the same time period in the chemotherapy control group. Thirty-three percent of patients are still alive after 15 months after the beginning of treatment. There was no significant difference in either general health or side effects, frequency or duration of complete remission, or survival time. After 1 year of treatment, 8 of 19 PIVN patients are still in complete remission, compared to 7 of 20 patients in the control groups.

Maintenance chemoimmunotherapy of nonlymphoblastic acute leukemias.

A trial of maintenance chemotherapy of nonlymphoblastic acute leukemia led to a comparison of two groups of patients in complete remission. Group 1 (14 patients) received only monthly reinduction chemotherapy. Group 2 (17 patients) received identical chemotherapy together with weekly immunotherapy combining BCG and irradiated leukemic cells. While the duration of the first complete remission was unmodified, the overall survival time and, above all, survival after the first relapse were prolonged in group 2 chemoimmunotherapy. These results were all the more marked when a homogeneous group of patients having received the same induction chemotherapy were considered.

High-dose methotrexate: a clinical and pharmacokinetic evaluation. Treatment of advanced squamous cell carcinoma of the head and neck using a prospective mathematical model and pharmacokinetic surveillance.

Some of 66 patients with head and neck tumors were treated with high-dose methotrexate monochemotherapy. The use of a prospective mathematical model with pharmacokinetic surveillance proved to be reliable, practical, and useful. By this means chemotherapy could be individualized, with a resultant marked reduction in the frequency and severity of toxicity. The onset of clinical toxic manifestations was significantly correlated with a poor therapeutic response and poor prognosis. The patients were classified in to three groups according to poor, intermediate, and good pharmacokinetic parameters calculated after an intravenous identification dose of methotrexate. These group allocations had a very high prognostic value with regard to toxicity, and especially to the quality of therapeutic response to high-dose methotrexate. They are suggested as useful guidelines in the prescription of high-dose methotrexate chemotherapy.

[Acute leukaemia during immunosuppressive treatment. One case (author's transl)].

A 55-year-old man was treated during 22 months with chlorambucil (total dose : 4 218 g) for glomerulonephritis. Towards the end of the treatment, he developed acute myelomonocytic leukaemia and died of this complication. The authors underline the oncogenetic dangers of the association dysimmune disease/immunosuppressive treatment and draw attention to the almost regular preleukaemic phase with its well-known cytological, biochemical, immunological, cytogenetic and evolutive criteria.
