Prepubertal XY gonadal dysgenesis.

Two children had prepubertal XY gonadal dysgenesis. A 7-year-old girl with clitoral enlargement had a left ovarian tumor that contained a dysgerminoma; the right gonad proved to be a gonadoblastoma. The second child (a 2-year-old girl) showed poor physical development and slight virilization of the genitalia. Her bilateral dysgenetic gonads were removed at exploratory laparotomy. The occurrence of gonadal tumors in XY gonadal dysgenesis is increased. It is probably related to the hypergonadotropinism existing from childhood as well as to genetic predisposition of the cryptorchid testis in the presence of a Y chromosome. Our first patient is one of the youngest who had XY gonadal dysgenesis iwth gonadoblastoma reported. The indication of prophylactic gonadectomy in XY gonadal dysgenesis is emphasized.

Endocrine studies in Sertoli-cell-only syndrome.

Pituitary-gonadal endocrine functions were studied in 15 patients with Sertoli-cell-only syndrome and a comparison was made with other testicular diseases, such as Klinefelter's syndrome. Elevated levels of serum luteinizing hormone as well as follicle stimulating hormone and lowered levels of serum testosterone suggested the existence of Leydig cell failure in addition to germ cell failure. However, the degree of these endocrinological abnormalities in patients with Sertoli-cell-only syndrome was mild compared to that in patients with Klinefelter's syndrome. Reserve capacity of Leydig cells in patients with Sertoli-cell-only syndrome was considered to be preserved as suggested by the human chorionic gonadotropin stimulation test.

New trial for fiberoptic cystourethroscopy: the use of metal sheath.

To overcome the disadvantages of using the fiberscope to observe the lower urinary tract we devised a metal sheath, which is used in combination with the fiberoptic cystourethroscope. The metal sheath helps in a smooth introduction of the fiberscope through the urethra and also in a close approximation of the fiberscope to the lesion. Fiberoptic cystourethroscopes combined with the metal sheath are useful in observing the bladder neck, the pedicle of tumors, the inner surface of the diverticulum and the condition of the intestine anastomosed with the bladder, which the current rigid endoscopes cannot approach. Photographs of the lower urinary tract were easily taken with this metal sheath method. However, the fiberoptic cystourethroscopes have a much smaller field of view, lower resolution and mor obscurity in the view. These drawbacks should be improved in the future.

Morphologic and biochemical study of pulmonary changes induced by bleomycin in mice.

Pulmonary changes induced by bleomycin were investigated in mice by using light and electron microscopy and by phospholipid analysis of the alveolar washings. The damage began in the endothelium of capillaries and was accompanied by interstitial edema and an appearance of enlarged macrophages along with hypertrophy of type II epithelial cells. This condition was followed by degeneration of type I cells. The denuded epithelium appeared to be repaired by two different mechanisms. In areas away from the bronchioles, division of existing type II cells and subsequent transformation into type I cells appeared to be the pattern of epithelial repair. In areas near the bronchiole, downgrowth of bronchiolar undifferentiated cells and subsequent maturation of these cells to type II and type I cells appeared to be prominent. Biochemically, total phospholipids and disaturated lecithin in the alveolar wash increased along with the increase of the alveolar lining layer and the hypertrophy of type II cells. This was considered to be consistent with the view that the hyperactive type II cells secreted more surfactant in the early phase of the experiment.

Postoperative improvement of hypertension in primary aldosteronism and Cushing's syndrome.

In 24 cases of primary aldosteronism and 30 cases of Cushing's syndrome, the incidence of preoperative hypertension and postoperative improvement of hypertension was investigated. All the cases of primary aldosteronism showed preoperative hypertension. Postoperative improvement of hypertension was observed in 21 out of those 24 cases. In 14 of these 21 cases, hypertension improved within 1 month postoperatively. In Cushing's syndrome, 23 out of 30 cases showed preoperative hypertension which improved in 20 cases postoperatively. They needed longer periods of time before improvement of hypertension than the cases of primary aldosteronism. The pathological findings in renal biopsy specimens seem to be correlated with postoperative improvement of hypertension in both diseases.
