Synergism of leukemic blast growth factors in medium conditioned by human bladder carcinoma cell line 5637.

Leukemic blast growth factors (LBGFs) are necessary for in vitro growth of clonogenic cells from patients with acute myeloblastic leukemia. As the human bladder carcinoma cell line 5637 had previously been reported to secrete abundant LBGFs into the culture supernatant, the LBGFs in 5637-conditioned medium (5637-CM) were characterized. Measurement of LBGFs was done using an in vitro leukemic blast colony assay in methylcellulose culture. LBGFs in 5637-CM were fractionated by anion exchange chromatography, and two peaks of activity were recovered. Pool B (high-salt eluent) and/or purified granulocyte colony-stimulating factor (G-CSF) were added to the clonogenic leukemic blast cell assays. It was found that pool B was more active than G-CSF in the majority of cases examined and that the two types of activity were synergistic in some cases.

Three cases of chronic bromisoval intoxication: clinical symptoms and application of energy dispersive X-ray analysis (EDX) to detect bromine in serum, urine and cerebrospinal fluid.

Bromisoval has been used as a hypnotic for the past several decades, and its abuse was known to cause various neurological as well as psychiatric symptoms. Three patients showed a variety of symptoms which could not be explained neuroanatomically: nystagmus, gait disturbance and hyperreflexia of the limbs in all the cases, dysarthria, double vision, hypotonia, ataxic gait and disturbance of consciousness occasionally and auditory agnosia in one case. For the purpose of determining the diagnosis, an energy dispersive X-ray microanalysis (EDX) was used to detect bromine. Five microliters of specimens were placed on the carbon-coated mesh, and using a TN-2000 analyzer, characteristic X-ray peaks of bromine were detected in the serum, urine and cerebrospinal fluid. The sensitivity to detect bromine in the serum was 30 micrograms/ml.

Temporary corneal oedema after acute intake of alcohol.

A 65-year-old man had diffuse, bilateral corneal oedema after acute intake of alcohol. Three days later the total opacification began to clear from the peripheral area towards the centre of the cornea. One month later both corneas were completely clear, and visual acuity was 20/20 in both eyes. Specular microscopy showed in the right eye greatly enlarged endothelial cells, which subsequently shrank, and they showed some pleomorphism six months later. The pleomorphic endothelial cells in the left eye changed little with time. Our findings suggested that the corneal oedema in this patient resulted from temporary endothelial dysfunction with extensive cell loss.

Polycythemia of end-stage renal failure: no inhibition of erythropoiesis by uremic serum and markedly increased serum erythropoietin level.

This report describes the case of a 50-year-old male with end-stage renal failure accompanied with congenital heart disease and polycythemia. After he had received continuous ambulatory peritoneal dialysis for 1 year, he still remained polycythemic and his serum erythropoietin titer, assayed using fetal mouse liver cells, was markedly increased. An inhibitory effect on erythropoiesis was not detected by this method. Bone marrow examination showed erythroid hyperplasia. These phenomena could be explained by an overproduction of erythropoietin by the remnant kidneys or extrarenal organs, such as the liver, in response to persisting hypoxia. The patient's bone marrow was still responsive to erythropoietin.

Alterations in the proliferative response of T cells from aged and chimeric mice.

Factors involved in the nonspecific and specific proliferative responses of T cells were examined in the A/J strain of mice that were physiologically aged in the specific pathogen-free condition. In aged A/J mice, the production of IL-1 and IL-2 from optimally stimulated peritoneal macrophages and splenic T cells was significantly decreased, while the production of IL-3 by T cells was increased. T cells from old mice showed a higher threshold for concanavalin A stimulation for the induction of IL-2 receptors. The same T cells responded only slightly in the mixed lymphocyte reaction (MLR) stimulated by allogeneic class II antigens, while they showed a high background stimulation without stimulators. The addition of syngeneic accessory cells did not increase the proliferative response of T cells from aged animals. T cells from bone marrow chimeras constituted by transferring stem cells from young A/J into irradiated old B10.A mice (young A/J----old B10.A chimeras) showed a pattern very similar to that of old A/J T cells, whereas those from the old A/J----young B10.A chimeras exhibited a response pattern comparable to that of normal young A/J cells. These results indicated that the proliferative responses in aged animals are abnormal, partially due to the alterations in the interleukin cascade. Some changes are probably due to the effect of environment where T cells undergo early differentiation, because bone marrow stem cells from aged mice can generate T cells with normal functions when differentiated in the young environment.

[Study on the optimum number of trials and intensity of unconditioned stimulants and strain difference in the two-way shuttle-box avoidance test using rats].

In order to determine the optimun conditions suitable for a number of trials and the intensity of unconditioned stimulant (US) in the two-way shuttle-box avoidance test in Sprague-Dawley strain rats, which are used most frequently in reproduction studies, conditioned avoidance response was observed under various conditions for 30 and 60 trials and the low and high US levels. Investigation was also conducted in Wistar rats under a high US level with 30 and 60 trials. Latency time of the escape response in Sprague-Dawley rats was shortened with increasing trials. Body weight gains of both strains of rats in the high US level with the 60-trial group decreased during the observation period. These findings suggest that the high US level with the 60-trial group is not suitable for the two-way shuttle-box avoidance test. The rate and latency time of the avoidance response were lower in Wistar rats than in Sprague-Dawley rats, although those of the escape response were higher. Significant changes in the following were observed, mainly from first to third sessions: the avoidance rate of all groups in strains of rats, escape rate of 60-trial group in Sprague-Dawley rats, avoidance and escape latency time of the 60-trial groups in both strains of rats and escape latency time of the 30-trial group in Sprague-Dawley strain rats.

In utero morphological effects of hydroxyurea on the fetal development in Sprague-Dawley rats.

In order to investigate in utero morphological effect of hydroxyurea (HU) in Sprague-Dawley rats, HU was intraperitoneally injected to pregnant Sprague-Dawley rats at a dose of 100 or 200 mg/kg/day during the organogenetic period (days 9-12 of gestation). A dose of 200 mg/kg/day induced growth retardation, high mortality and high incidence of malformations, although a dose of 100 mg/kg/day produced no adverse effects in the next generation. In the HU 200 mg/kg/day group the incidence of malformations in pups at 4 days of age was low as compared with that in fetuses and pups at 21 days of age. Increasing perinatal mortality in fetuses and pups due to severe central nervous system (CNS) malformations and disappearance of some cases of ventricular septal defect after delivery were considered as the possible causes to induce difference in malformation rate in various stage of development. Latent effect on the development of CNS malformations was observed between 4 and 21 days of age. There was no sex difference in teratogenic effect. These findings were compared with those in Wistar rats exposed to HU 200 mg/kg/day. The incidence of perinatal malformations and the stillbirths were significantly higher in the Wistar rats as compared with those in the Sprague-Dawley rats. In addition, such morphological effects of HU as the exencephaly, dilatation of lateral ventricle, anophthalmia, cleft palate and micrognathia are less severe in Sprague-Dawley rat fetuses than in Wistar rat fetuses.

[Measurement of spontaneous motor activity by using the vibrator response method in rats].

To investigate the usefulness of spontaneous motor activity (SMA) measurement using the vibrator response method, the acute effects of drugs on SMA were observed in Sprague-Dawley male rats. There were no significant differences between four devices. Methamphetamine (0.3-1 mg/kg, i.p.) and 1-2 mg/kg (s.c.) of apomorphine increased the SMA, but 0.05-0.2 mg/kg (s.c.) of apomorphine decreased the SMA. Apomorphine at 1-2 mg/kg (s.c.) significantly increased the SMA when the vibrator response method was used as compared with the Animex method. These results suggest that the vibrator response apparatus is useful for the measurement of SMA in rats.

Adaptive changes of H+ and/or bicarbonate transport in chronic metabolic acidosis: its quantitative evaluation by using isolated perfused kidney.

We have recently improved the technique of the isolated perfused rat kidney by addition of amino acids to the perfusion solution. Utilizing this improved preparation, we examined to what extent chronic acid-base status can alter H+ and/or bicarbonate transport in the whole kidney and the effect of acetazolamide (10(-4) M) on this process. Kidneys from rats that had been drinking tap water (control), 1.5% NH4Cl (acidotic), or 1.5% NaHCO3 (alkalotic) were perfused with a solution of identical pH and PCO2. During the control period, the fractional bicarbonate reabsorption was higher in acidotic rats than in control rats by 1.0% and urine pH was lower than those in control. On the other hand, the two parameters were not different between control and alkalotic rats. Acetazolamide increased urine pH and decreased the fractional bicarbonate reabsorption in all, but these parameters in acidotic and alkalotic rats were not different compared with those in control. We conclude that adaptive changes of H+ and/or bicarbonate transport in chronic metabolic acidosis corresponds to about 1.0% as fractional bicarbonate reabsorption by the isolated perfused kidney and is largely carbonic anhydrase dependent.

Pupillary light reflex in patients with hemispheric cerebrovascular disease.

Pupillary dynamics were investigated in 41 patients with hemispheric CVD by means of computerized open-loop videopupillography. The patients were alert and showed either abnormal high- or low-density areas on CT scan. Fourteen cases were examined in the acute stage (within 7 days from onset), 28 cases in the subacute stage (between 8 and 28 days) and 11 cases in the chronic stage (over 29 days). In the acute stage, the parasympathetic nervous functions (maximum velocity and acceleration of pupillary constriction) were hyporeactive and showed no significant differences between the ipsilateral and contralateral side of the lesion. In the subacute stage, these functions were significantly more hyperreactive on the ipsilateral side than on the contralateral side, that is, the parasympathetic functions of the healthy hemisphere became significantly improved in comparison with those in the acute stage. In the chronic stage, no significant differences were observed between the two sides. In contrast, the sympathetic nervous function (maximum velocity of dilatation) showed no differences with respect to either laterality or the time course after onset. The above data suggest that bilateral parasympathetic hypofunctions exist in the acute stage of hemispheric CVD, with an ipsilateral improvement of these functions with the passage of time, and that such autonomic dysfunctions may play some role in the pathophysiology of CVD.

A case of rhabdomyolysis in chronic renal failure.

Rhabdomyolysis can be induced by a variety of physical and chemical insults to skeletal muscle. Though there are only a few reports of myositis associated with viral diseases, and no reported cases of virus-induced rhabdomyolysis in uremic patients. Chronic uremic states have been known to potentiate a variety of metabolic and immunological abnormalities. Present case had an acute, progressive and fatal rhabdomyolysis which was thought to be induced by virus infection. A 43-year-old man had received hemodialysis therapy for 8 years. He suffered from an upper respiratory tract infection 11 days before admission. On admission, he was diagnosed as rhabdomyolysis with severe lactic acidosis and marked hyperkalemia. Although intensive care had been performed, he died of uncontrollable hyperkalemia (10.0 mEq/L) 2 days after admission. Maximum CPK and GOT levels were 105,200 and 56,800 mU/ml, respectively. Most probable cause of rhabdomyolysis was thought to be Parainfluenza type-3 infection, though histological examination failed to prove virus infection.

Immunoglobulin subclass antibodies to varicella-zoster virus.

Commercially available mouse monoclonal antibodies to human IgG subclass (IgG1 to IgG4) were applied to an enzyme-linked immunosorbent assay to measure IgG subclass-specific antibodies to varicella-zoster virus in children naturally infected with varicella-zoster virus and in varicella vaccine recipients. In children naturally infected with varicella-zoster virus, IgG 1 antibody was detected 2 weeks after onset of the disease in all cases, its activity increased at 1 month after onset, and almost equal antibody value was maintained 10 years after infection. This pattern of antibody response was similar to that of total IgG antibody to varicella-zoster virus after natural infection. On the other hand, low antibody activity was found in IgG2 only at 1 month of the disease. The highest antibody level of IgG3 was shown 2 weeks after onset of the disease; then, it gradually decreased, and no antibody activity was detected 10 years later. IgG4 antibody was first detected 1 month after onset and an almost equal level of antibody was shown 10 years after the disease. After inoculation of children with a live varicella vaccine, in contrast, IgG subclass antibody responses to vaccine recipients were almost equal to those after natural infection.

Human bladder carcinoma cell line HTB9, which secretes a factor to stimulate clonogenic leukemic blast growth, expresses the granulocyte-macrophage colony-stimulating factor gene.

Media conditioned by the human bladder carcinoma cell line HTB9 contained high leukemic blast growth factor activity and also showed granulocyte-macrophage colony-stimulating factor (GM-CSF) activity. Northern blotting analysis of total RNA from HTB9 cells using GM-CSF cDNA as a probe demonstrated abundant expression of the GM-CSF gene. Thus, this cell line secretes GM-CSF, and this factor contributes to the proliferation of clonogenic leukemic blast cells.

[Clinical evaluation of combination therapy with Fortimicin and Cefotax in the treatment of complicated urinary tract infections].

Clinical efficacy of combination therapy using Fortimicin (ASTM) and Cefotax (CTX) was studied in 63 patients with complicated urinary tract infections. The patients received 200 mg of ASTM i.m. and 1 g of CTX i.v. twice a day. The overall clinical efficacy of the treatment was evaluated by criteria proposed by the Japanese UTI Committee. The overall clinical efficacy rate was 68.3%. The rate was 67.3% in the patients with single bacterial infection and 72.7% in those with mixed infections. Subjective side effects were observed in one of the patients. Drug related aggravation in laboratory tests was observed in 8 items in 8 patients, but these were mild changes. We concluded that combination therapy with ASTM and CTX was useful for the treatment of complicated urinary tract infections.

A case of acute diffuse lymphocytic meningoencephalitis.

"Acute diffuse lymphocytic meningoencephalitis" is characterized neuropathologically by swelling of the brain and an unproportionally slight degree of inflammatory findings. Although many cases of this encephalitis have been reported in Japan, it was scarcely reported in other countries. The present case is a girl who had been well until fifteen years of age. She became psychotic on the 7th day after she seemed to have caught a cold. She fell into coma on the 15th day, then neurologic symptoms, such as nuchal rigidity and athetoid movement became prominent. She developed the status epilepticus with a high temperature of 41 degrees C, and died on the 23rd day. Although there was an increase of mononuclear cells in the cerebrospinal fluid (32/microliters), various antiviral titers were normal both in serum and cerebrospinal fluid. Neuropathologic changes were edema, perivascular lymphocytic infiltration in leptomeninges and parenchyma, and proliferation of rod-shaped microglia particularly prominent in the cerebral cortex and hippocampus. It is peculiar that thickening of leptomeninges formed by collagen fibers and inflammatory cells was observed. On the systemic study, centrolobular fatty change of the liver was found. This case had to be differentiated from several other neuro-psychiatric illnesses. The problems were discussed clinically and neuropathologically.

A case of paroxysmal rate dependent intra-His bundle block.

We present a symptomatic bradycardia-dependent paroxysmal atrio-ventricular (A-V) block occurred within the bundle of His. Paroxysmal A-V block was caused by atrial or ventricular premature beats and also by sinus slowing. This case also showed tachycardia-dependent A-V block within the bundle of His. We also discussed the controversial mechanisms of paroxysmal A-V block.
