Six cases of heterochronous gastrointestinal cancer preceded by breast cancer.

Among 432 women with primary breast cancer, six (1.4%) were diagnosed as having gastrointestinal cancer more than six months after operation for the breast cancer. This paper presents these six cases. The patients ranged from 56 to 78 years of age at the time of breast cancer surgery, and the interval after surgery until diagnosis of the second cancer ranged from 7 months to 5 years 1 month. The second cancer was gastric cancer in 3, esophageal cancer in 2, and hepatic cancer in 1. All of the 6 patients had received postoperative adjuvant chemotherapy for breast cancer. The most frequent histological type of breast cancer was solid-tubular carcinoma (3 patients). Three patients died, due to the second cancer, 4 days, 2 months, and 6 months, respectively, after diagnosis of the second cancer, and the other patients are alive 2, 3, and 4 years after diagnosis. The Japanese literature regarding multiple cancer among breast cancer patients is reviewed. It is concluded that care should be taken to examine breast cancer patients with gastrointestinal symptoms, which are likely to be dismissed as a side effect of postoperative chemotherapy.

[Safety and ocular hypotensive effect of a new prostaglandin analogue, S-1033, in normal volunteers].

We conducted a prospective, placebo-controlled, double-masked study to investigate the ocular hypotensive effect and safety of a new prostaglandin analogue, S-1033, in healthy human volunteers. In a single-dose, dose-finding study, where 0.1%, 0.2%, 0.3%, or 0.4% of S-1033 and the vehicle ophthalmic solution was instilled in each one eye of six adult male volunteers and vehicle solution alone in the fellow eye, 0.3% solution showed a significant ocular hypotensive effect at 1, 2, and 8 hours following the instillation and 0.4% solution showed a similar effect at 4 hours after the instillation. The following multiple-dose study, where 0.3% S-1033 solution was instilled twice daily for 8 days in one eye of six volunteers and the vehicle in the follow eye, revealed a significant, continuous intraocular pressure (IOP) reduction. Mild, transient conjunctival hyperemia and slight ocular discomfort developed in some cases, but they were minor. These results indicate that S-1033 has a significant ocular hypotensive effect without substantial adverse effects and the new compound seems to deserve further investigation as a possible anti-glaucoma medication.

[Breeding control and immobilizing effects of wood microingredients on house dust mites].

The possible effects of essential oils as wood microingredients on house dust mites (Tyrophagus putrescentiae, Dermatophagoides farinae and Dermatophagoides pteronyssinus) were investigated. 1. Whether small pieces of 5 types of wood had any control effect on mite breeding was studied. Mite breeding using only normal feed was compared with breeding using feed mixed with small pieces of wood. In addition, mite breeding using feed mixed with small pieces of wood with no essential oil was studied. 2. Effects of 6 different wood essential oils in immobilizing mites were studied with regard to respired and contact toxicities. 3. The immobilizing effects of 10 ingredients in Hinoki oil were also specifically studied with regard to contact toxicity. The results of the 3 experiments were as follows: 1) It was confirmed that the small pieces of Hinoki, cedar, pine and Lauan had control effects on mite breeding. However, the small pieces of spruce did not demonstrate an effect. Woods which had no essential oils had reduced or no breeding control effects. 2) It was confirmed that the 6 different wood essential oils had mite immobilizing effects associated with respired and contact toxicities. Rosewood oil, White Pine oil and Taiwan Hinoki oil had strong immobilizing effects. Hinoki oil, however, had only a weak effect. 3) Among the 10 ingredients of Hinoki oil, specifically Linalool, Geranyl acetate and alpha-Terpineol had strong mite immobilizing effects.

Reappraisal of preoperative carcinoembryonic antigen levels as a prognostic factor in resectable colorectal cancer.

Preoperative carcinoembryonic antigen levels in sera were measured in 124 cases of primary colorectal cancer, and their usefulness as a prognostic factor were evaluated. There were no significant differences in the CEA levels with regard to age, sex, size of the primary tumor, histological differentiation, peritoneal dissemination or DNA ploidy, but CEA levels were significantly higher in patients with liver metastasis. By using Cox proportional hazard model, the most significant discriminant in prognosis was liver metastasis, followed by preoperative CEA level, depth of invasion and DNA ploidy pattern. Preoperative CEA measurements were applied as a parameter of biological behavior of colorectal cancer in the follow-up course.

[Basic evaluation of the new fully automated urine sediment analyzer (U-FCM) based on flowcytometric technology].

We developed a new automated urine sediment analyzer (U-FCM) based on Flow Cytometry with the cooperative technical aid of TOA Medical Electronics Co., ltd. Flow Cytometry using an argon laser (wave length 488 nm) is used as the method for detection. This instrument provides quick, easy and accurate screening for patients whose urinalysis abnormalities indicate the need for further testing. Abnormal patient samples are separated from patient samples whose parameters are totally normal, thus decreasing the need for additional testing. The instrument is useful as a diagnostic aid, and in monitoring the therapeutic effect. Also it is useful in the health checking service for visitors at the AMHTS office.

[Gene level diagnostics of infectious diseases].

Recent developments in medical technology have caused a great change in infectious diseases, as characterized by epidemics of antibiotics-resistant bacteria, opportunistic infection in compromised hosts, and blood-borne viral infections such as hepatitis virus and human immunodeficiency virus. In the diagnosis of such new aspects of infectious diseases, conventional immunological, culture, and microscopical techniques are not always practical. By contrast, evaluation of infectious agents using molecular biological technology frequently offers the rapid, most accurate and sensitive method of diagnosis. Amplification methods are particularly attractive for the detection of small numbers of microorganisms, as in latent conditions, or for the fastest identification of the pathogen without laborious isolation. For introduction of the tests into routine procedures, their standardization as well as simplicity and low cost are required. Gene level diagnostics should be applied appropriately to management of infectious diseases along with the conventional techniques, while further roles of the tests must be determined, on the basis of the molecular elucidation of infectious diseases.

[Electronmicroscopic study on the vitreous membrane of the Stickler syndrome].

The vitreous membrane obtained from a patient with Stickler syndrome, an 8 year-old female, was investigated electronmicroscopically. The cells found in the membrane had cilia, microvilli and cytoplasmic filaments. Gap junctions and basement membrane-like structures were also found. Several of the cells had phagosomes containing crystalline materials. Thus, these cells seemed to be glial cells.

[Clinical significance of flow cytometric DNA analysis in metastatic lymph node of colorectal cancer].

Significance of flow cytometric DNA analysis in metastatic lymph nodes for assessing malignant potential of colorectal cancer was investigated using paraffin-embedded materials of primary lesions and metastatic lymph nodes from 65 patients who had been treated between 1975 and 1990. The DNA ploidy patterns of metastatic nodes were identical in 61.5% with those of primary lesions. Diploid cancers were significantly more frequent in metastatic nodes than in primary lesions. There were significantly more aneuploid cancers in proximal nodes than in distant nodes. There was no relation between ploidy patterns in primary lesions and survival. However, a significant relation was found between ploidy patterns in metastatic nodes and survival. Diploid cancer in metastatic nodes had a significantly better survival than aneuploid cancer, in all patients as well as those with curative resection. In patients with stage III and in those with the same depth of invasion, the survival rate of diploid cancer in metastatic nodes was significantly higher. There was no correlation between ploidy patterns in metastatic nodes and clinicopathological variables in primary lesions, such as histological type, depth of invasion, nodal involvement, peritoneal or hepatic involvement and stage. These results suggest that nuclear DNA content in metastatic lymph nodes may be a prognostic indicator in colorectal cancer with nodal involvement.

Binding site of annexin XI on the calcyclin molecule.

We purified rabbit calcyclin of S100 family protein and a calcyclin associated protein which has proved to be a novel annexin, annexin XI. Using a co-precipitation assay of annexin XI with phospholipid, the binding site of annexin XI on calcyclin was examined. The peptide fragment of calcyclin, CNBr-3 (residues 1-57), digested with cyanogen bromide completely inhibited the interaction of native calcyclin with annexin XI, while CNBr-1 (residues 83-90) and CNBr-2 (residues 58-82) did not affect the binding. We then constructed and expressed recombinant cDNAs for wild type and four different deletion mutants lacking N-terminal portions. The wild type (wt) and mt1 mutant lacking three amino acids from N-terminal bound to annexin XI with phosphatidylserine and Ca2+, whereas mt2, mt3 and mt4 with seven, twelve and eighteen amino acids deleted, respectively, did not bind to annexin XI. Moreover, the truncated mutant from residues 4 to 7 (mt5) decreased the binding capacity. These observations suggest that four amino acids (residues 4-7) at the N-terminal portion of calcyclin play an important role in the interaction of calcyclin with annexin XI.

Low plasma apolipoprotein AII levels in human and mouse amyloidosis with mutant transthyretin (Met-30) gene.

We measured the serum apolipoprotein levels in patients with familial amyloidotic polyneuropathy (FAP). The serum apolipoprotein AII levels were much lower than those of the control subjects, while the levels in asymptomatic carriers of the FAP gene were normal. Other plasma apolipoprotein levels, such as apolipoproteins AI, B, CII, CIII, and E, were all within normal ranges. The decrease of apolipoprotein AII in the plasma of FAP patients correlated with the progression of the disease. In a transgenic mice model of FAP carrying human variant transthyretin gene (Met-30), serum apolipoprotein AII levels were decreased in 1.5-year-old mice compared with control mice, while the 3-month-old mice had normal levels. These results suggest that apolipoprotein AII may play an important role in lipid metabolism or amyloid formation in patients with FAP.

Activation of protein kinase C inhibits human keratinocyte migration.

The involvement of protein kinase C (PKC) in epidermal growth factor (EGF)-induced human keratinocyte migration was studied with the phagokinetic assay. It was concluded that PKC activation does not mediate, but rather inhibits, EGF-induced keratinocyte migration. The following experimental observations support these conclusions: 1) The PKC inhibitor H-7 did not inhibit EGF-induced migration but instead led to a modest enhancement. 2) PKC activators such as phorbol-12-myristate-13-acetate (PMA), phorbol-12,13-dibutyrate (PDBu), and 1,2-dioctanoly-sn-glycerol inhibited migration, but biologically inactive 4 alpha-PMA had no effect. 3) PMA did not inhibit keratinocyte attachment and spreading but blocked migration almost immediately after addition. 4) Migration of PKC-depleted cells, which were produced by prolonged treatment with PDBu, was enhanced similarly to normal cells by EGF. 5) PKC-depleted cells were not susceptible to the inhibitory effects of phorbol esters on migration. Additional experiments, in which cells were preactivated with EGF, suggested that PKC inhibits the EGF effect at a post-receptor level. The inhibitory effect of PKC on keratinocyte migration was not restricted to EGF-induced migration; PKC activation also inhibited keratinocyte migration induced by bovine pituitary extract, insulin, insulin-like growth factor-1, and keratinocyte growth factor.

Quantitative analysis of herpes simplex virus DNA in cerebrospinal fluid of children with herpes simplex encephalitis.

Herpes simplex virus (HSV) DNA in the cerebrospinal fluid (CSF) of children with herpes simplex encephalitis (HSE) was quantified and typed using the polymerase chain reaction (PCR) assay. During the acute phase, HSV-DNA was detected in the CSF of 13 patients with HSE, including 5 neonates. A restriction profile of the PCR products cleaved with the restriction enzymes XhoI and BglII showed that 2 neonatal samples were HSV-2, and the remainder were HSV-1. The amount of HSV-DNA in the initial CSF ranged from 10(2)-10(5) copies/ml. A significantly greater number of HSV-DNA copies was detected in neonates than in older children (mean 3.9 vs. 2.5, log 10 copies/ml p < .05). Except for one patient, the amount of HSV-DNA decreased gradually with acyclovir therapy. These results show that a quantitative PCR assay is applicable not only to the diagnosis of HSE but also for monitoring the response to antiviral drugs.

T-cell receptor V alpha region usage in the cerebrospinal fluid of patients with mumps meningitis.

To investigate clonality of local T cells in viral infections of the central nervous system, the T-cell receptor (TCR) repertoire was evaluated in T cells from the cerebrospinal fluid (CSF) of nine patients with mumps meningitis, using the polymerase chain reaction (PCR) assay. The usage of the variable region of TCR alpha chain gene (V alpha gene) in the CSF was widespread, and an average of 13 out of the 18 V alpha families were expressed. Quantitative PCR analyses showed that the V alpha gene expression was biased toward three or less V alpha families in the CSF of each patient. When compared with peripheral blood T cells, the average percentages of V alpha 11 and V alpha 12 gene expression were significantly higher in the CSF than in the peripheral blood. These results suggested that mumps-specific T lymphocytes with a restricted TCR repertoire are selectively recruited to the central nervous system in mumps meningitis, although polyclonal, probably nonspecific, T-cell populations are present in the CSF.

Immunohistochemical localization of lipocortins in normal and psoriatic human skin.

The distribution of lipocortin I, a steroid-induced inhibitory protein of phospholipase A2, was examined in normal and psoriatic human skin. Using immunoblotting analysis with specific antibody against human lipocortin I purified from human placenta, lipocortin I was detected as a 37 kDa protein in cultured epidermal cells, whole skin and epidermis. In the dermis and stratum corneum, lipocortin I was only weakly detectable by Western blotting. In contrast to normal skin, much less lipocortin I was detected by Western blotting analysis in psoriatic skin. Using immunoperoxidase immunohistochemical analysis, lipocortin I was demonstrated in the cytoplasm of keratinocytes in the upper and middle layers of the epidermis and in some infiltrating cells in the dermis in normal skin. In involved psoriatic skin, by contrast, lipocortin I was almost undetectable in the epidermis, although it was demonstrated in some infiltrating cells in the dermis. No immunostaining of lipocortin I was observed in the stratum corneum of normal or psoriatic skin. These results, together with the finding that phospholipase A2 activity is higher in psoriatic epidermis than in normal epidermis, suggest that lipocortin I plays an important role in the regulation of differentiation and proliferation of epidermal keratinocytes.

Family study of women showing development of antibody to human T-cell leukemia virus I and assessment of the risk of vertical transmission of the virus to their children.

When pregnant women were tested for antibody to human T-cell leukemia virus-I, some were found to be positive although they had been negative during the previous pregnancy. In these women, HTLV-I infection was found from pedigree studies to have been acquired from their mothers rather than from their husbands. Furthermore, some of them had apparently remained HTLV-I antibody-negative for long periods after infection. When the breast-fed children of these women, in whom HTLV-I was acquired from their mothers but who were in an HTLV-I antibody-negative state, were also examined for evidence of HTLV-I infection, none was found.
