The changes in cellular functions were analyzed in 4 rheumatoid arthritis and 2 biliary cirrhosis patients. Impaired T-cell functions (low lymphocyte transformation reactivity (LTR) to Con A), seen in one cirrhosis patient before MP, improved with MP. MP also removed inhibitory factor(s) for LTR from the plasma of this patient. RA patients tended to have decreased Con A response and enhanced PWM response. MP enhanced LTR in 2 patients, but lowered LTR in patient who had been taking immunosuppressive drugs. Changes in lymphocyte populations occurred and included a substantial increase of T cells. A primary effect was an increase of helper T-cells. During MP, early leukopenia accompanied by complement activation and secondary leukocytosis were observed. Phagocytic PMNs activated by this process had significant augmentation of their O-2 generating activity as measured by chemiluminescence. This was evident in severe RA and in 2 cirrhosis patients with low phagocytic cell function before MP. This activation of phagocytes by MP may lead to additional beneficial effects by further lowering MMs such as immune complexes from plasma. A better understanding of these phenomena is necessary to assess the overall therapeutic response to MP, especially in patients where cellular mechanisms are important in pathogenesis of their disease.

Newly developed hollow fiber membrane filters made of ethylene-vinyl-alcohol designated as EVAL 2A, EVAL 4A, and EVAL 5A, were tested in vitro and applied clinically. EVAL 2A had the lowest molecular weight cut-off (30% rejection rate for 10(4) dalton dextran and 90% rejection rate for 10(5) dalton dextran); sieving coefficients (SC) of 0.78 for albumin, 0.59 for IgG, and almost 0 for IgM indicate its usefulness for separation of IgG from albumin when the recirculation flow on the filter was introduced. EVAL 4A and 5A showed higher cut-off levels (25% rejection of 10(5) dalton dextran and 80% rejection rate for 2 X 10(6) dalton dextran); SCs for albumin IgG, and IgM were 0.89, 0.83, and 0.24, respectively, indicating their usefulness in the separation of MMs of molecular size greater than IgG. IgM type cold agglutinin antibody (CAA), IC, and RF were effectively removed by these filters from CAA induced hemolytic anemia or rheumatoid vasculitis patients. Using recirculating flow to prevent filter plugging, one EVAL unit was found to be sufficient to treat 2-3 L plasma. These filter properties demonstrate the possible clinical application of EVAL filters for selective MM removal, when specific MM removal is indicated for the treatment of specific diseases.
We studied the effect of postoperative long-term cancer chemotherapy (PLCC) on the survival of the patients with stage IV gastric cancer, subjected to non-curative resection. The study was carried out with 90 patients receiving the PLCC from 1970 to 1978, and 95 patients received intraoperative one shot I.V. injection of Mitomycin-C from 1964 to 1978, and 106 patients without chemotherapy from 1964 to 1970. The PLCC consisted of the following combination: MMC intermittent intravenous injection (20 mg of MMC during operation followed by 10 mg on the next day, 4th week and every 3 months respectively): postoperative oral administration of FT-207 (600 mg/day) and; PSK (3 g/day) for more than 4 months after operation. The background factors were not significantly different among 3 groups. Two year survival rate was 18.9% in PLCC group, significantly higher than that of 7.4% in MMC group and 2.0% in no chemotherapy group which was respectively (P less than 0.05). The PLCC also demonstrated its be effective mess in prolonging the survival of the patients with hepatic metastasis or peritoneal dissemination.
A 9-year-old boy having bronchial asthma showed fluctuation of his mental state for 14 days. EEG showed multiple spikes or irregular spike and wave complex bursts, the focus being in the left occipital region, and he was diagnosed as prolonged partial complex status epilepticus. Skull radiograms revealed the presence of radiolucent areas in the left parietal region and cerebral angiography showed a pear-shaped aneurysm and angioma in the distal part of the middle meningeal artery. Nontraumatic aneurysm of the middle meningeal artery has been rarely reported; only the reports on 2 old women with Paget disease are available now. The authors' case is the first child case.
Irradiation of tetrameric concanavalin A with a high-pressure mercury lamp in the presence of chloroacetamide and fractionation of the reaction product by affinity chromatography gave a lectin derivative showing a monomeric molecular weight at pH 7.4, with an overall yield of 25%. It retained the intact binding ability against methyl-alpha-D-glucopyranoside. Hemagglutination and glycogen precipitation data suggested the monovalent nature of the derivative. The results of amino acid composition analyses indicated that the photochemically induced alkylation at one or two tryptophan residues per subunit of concanavalin A may be responsible for the dissociation into monovalent monomers which are stable at physiological pH.
Dose for dose, AM-715 was at least five times as active as pipemidic acid and nalidixic acid against systemic and urinary bladder-kidney infections in mice.
Prostaglandin E2 (PGE2) concentration in arterial and renal venous plasma was determined by radioimmunoassay in anesthetized dogs, and the PGE2 secretion rate was calculated. Intrarenal arterial infusion of CaCl2 (0.68 meq/min) resulted in a biphasic effect on renal blood flow (RBF) and an initial increase followed by a gradual decrease below the preinfusion level. PGE2 secretion rate increased from 7.2 +/- 4.4 to 95 +/- 28 ng/min, with a marked increase of RBF after 3 min of CaCl2 infusion. An equivalent amount of NaCl infused into the renal artery did not affect either the RBF or the PGE2 secretion rate. One-tenth the dose of CaCl2 (0.068 meq/min) infused into the renal artery did not increase either RBF or PGE2 secretin rate, and the RBF gradually decreased after infusion. The transient renal vasodilation observed during infusion of a high dose of CaCl2 ceased with the combined intrarenal infusion of verapamil (100 micrograms/min), and was also abolished by pretreatment with indomethacin (5 mg/kg i.v.). Infusion of CaCl2 (0.68 meq/min) into the femoral artery did not enhance the PGE2 secretion rate from the hindlimb, and femoral blood flow decreased immediately after the infusion. These results suggest tht the renal vessels as well as other resistance vessels are essentially constricted by calcium and that endogenous PGE2 released by calcium may modify this renal vasoconstriction.
TSH receptor and adenylate cyclase activity of plasma membrane fractions of human thyroid tumors were examined. The mean (+/- SD) basal adenylate cyclase activity in normal thyroid tissues was 0.35 +/- 0.33 nmole/mg protein x 10 min. The activity rose to 280% (range, 270-310%) of basal with TSH (166 mU/ml). In adenomas, the activity rose to 600% (range, 530-650), which was significantly higher than that of normal thyroid (P < 0.005). In the differentiated carcinoma, TSH responsiveness of adenylate cyclase was heterogenous (range, 110-520), but was qualitatively similar to that of the normal thyroid. On the other hand, basal adenylate cyclase activity of undifferentiated carcinoma was significantly lower than that of normal thyroid (0.018 +/- 0.007 nmol/mg protein x 10 min; P < 0.05) and was not stimulated by TSH. LH, FSH, and ACTH did not stimulate the enzyme in either kind of carcinomas. The mean (+/- SD) of the capacity of the high affinity receptor of adenomas (0.72 +/- 0.64 pmol/mg protein) and differentiated carcinomas (0.77 +/- 0.84) was not significantly different from that of normal thyroid (0.92 +/- 0.84). The affinity constants of the receptors in these three tissues were much the same (1.6-2.4 x 10(10) M-1). On the other hand, high affinity receptor could not be detected in all of the undifferentiated thyroid carcinoma. It seems likely that the failure of adenylate cyclase to respond to TSH in undifferentiated carcinoma of the thyroid is due to an alteration at the level of the receptor site. These data suggest that growth and metabolic activity of undifferentiated carcinoma may be independent of TSH, while those of adenoma and differentiated carcinoma may be affected by TSH.

6-O-(3-Amino-3-deoxy-alpha-d-glucopyranosyl)apramycin (17) was prepared by glycosidation of a suitably blocked 5,6-dihydroxy derivative (11) of apramycin with a blocked 3-aminoglucosyl chloride (15). Ribosylation of the 5-hydroxy-6-O-tetrahydropyranyl (THP) derivative (19) of apramycin gave 5-O-(beta-d-ribofuranosyl)apramycin (24) along with the 6 alpha (25) and 6 beta (26) isomers. Similar reaction with the 6-hydroxy-5-O-THP derivative (20) or 11 gave only 25 and 26, but not 24. 17 was at least as active as apramycin against most Gram-positive and Gram-negative bacteria tested and more active than apramycin against most Gram-positive and Gram-negative bacteria tested and more active than apramycin against strains producing aminoglycoside-modifying enzymes. Strains of Pseudomonas aeruginosa were generally less sensitive to 17 than to apramycin. 24 was the most active of the three ribofuranosyl derivatives prepared though it was less active than 17.
A new derivative of rifamycin SV, R-75-1, inhibits RNA synthesis in Mycobacterium smegmatis ATCC 607 (M607) at a concentration 10 times lower than rifampicin (RFP). However, both R-75-1 and RFP inhibit RNA polymerase reaction by 50% at the same concentration level (0.05 approximately 0.1 microgram/ml). Both inhibit the initiation process of RNA synthesis. E. coli RNA polymerase of the RFP-resistant strain was resistant to R-75-1. RFP was not inactivated by M607 cell extracts. The inhibitory effect of R-75-1 is markedly diminished if mycobacteria are grown in the medium containing Tween 80. On the basis of these results, the greater activity of R-75-1 to mycobacteria is suggested to be due to the better permeability than RFP.
