[The role of hysterectomy in the therapy of gestational trophoblastic tumor].

OBJECTIVE: To evaluate the role of hysterectomy for patients with gestational trophoblastic tumor. METHODS: A total of 68 cases of gestational trophoblastic neoplasia treated by hysterectomy from 1985-1997 at PUMC hospital was retrospectively analyzed. Thirty-eight cases were diagnosed as choriocarcinoma and 30 as invasive mole. RESULTS: Twenty-three elder patients who didn't desire to preserve fertility were selected for hysterectomy after short courses of chemotherapy. Twenty-two of them had complete remission (95.6%). The average total course of chemotherapy was 4.2. Of twenty-seven chemo-refractory cases who were suspected of an isolated lesion in the uterus, delayed hysterectomy as an adjunct to chemotherapy was performed. Twenty of them achieved complete remission (74.1%), with an average 9.4 courses of chemotherapy. Emergency hysterectomy was indicated in 18 patients with uterine perforation or life-threatening hemorrhage. Seventeen of the emergent cases had complete remission (94.4%), who had received an average 7.6 courses of chemotherapy. CONCLUSION: Although the development of effective chemotherapy has resulted in improved survival of patients with gestational trophoblastic tumor, hysterectomy remains an important adjunct treatment in a selected subset of patients. Modified radical hysterectomy is recommended for the indicated patients.

[Comparison of closing T loop technique and sliding technique using preadjusted appliance].

OBJECTIVE: To compare the difference between closing T loop technique and sliding technique. In anchorage demand and treatment outcome. METHODS: Sixteen first premolar extraction cases were chosen and divided into two groups for prospective paired t-test. They were treated by one author using 0.022" x 0.028" preadjusted appliances. T loops were fabricated using 0.018" x 0.025" s.s. wire. Chinese NiTi coil springs and 0.019" x 0.025" s.s. arch wire were employed in sliding technique group. Treatment materials, steps and appliance design in both groups were standardized. Measurements of dental model and cephalogram were taken. RESULTS: In closing T loop technique group and sliding technique group, the ratio of anterior teeth movement was 0.55 and 0.53 respectively. The difference was not significant (P > 0.05). L1MP angle decreased by 5.7 degrees and 3.1 degrees respectively during treatment (P < 0.05). CONCLUSION: Demand on anchorage was not significantly different between two groups. Sliding technique was more favorable in maintaining anterior teeth inclination.

[Clinical observation on effect of step respiration exercise combined Chinese drug therapy in treating pulmonary heart disease and exploration on its therapeutic mechanism].

OBJECTIVE: To introduce an auto-electrocardial counterpulsation device, which is non-invasive, nonmedicative and easy to operate, to patients of pulmonary heart disease. And to observe the therapeutic effect of the step respiration exercise (SRE) induced by the device combined with Chinese drug therapy (CDT) on pulmonary heart disease. METHODS: Fifty cases of chronic pulmonary heart disease were divided into SRE group, CDT group and CDT combined SRE group. Blood gas analysis and color ultrasonic Doppler cardiovascular function test were carried out in patients before and after treatment. RESULTS: After 3 courses of treatment, the effect of CDT combined SRE group revealed better than that of the other two groups. CONCLUSIONS: Respiration activity has definite influence on cardiovascular function of patients. SRE can significantly lower the heart rate, reduce the afterload, increase the pressure on coronary orifice by raising arterial diastolic pressure, enhance stroke output and alleviate the work of the heart. CDT can enhance the effect of SRE by synergetically removing pathogens to liberate the Lung, arresting the reversed Qi flow to remove Phlegm and relieve asthma, and replenishing the Kidney to control the reversed Qi.

[Clinical observation of side effects of Tripterygium preparation].

OBJECTIVE: To observe the side effects of Tripterygium preparation (TP) in the course of treatment. METHODS: Two hundred and seventy-one cases of patients with rheumatoid arthritis (RA) were treated using various TP to observe the occurrence of adverse reaction. RESULTS: The influence on reproductive system with TP extracted from the leaf ester tab were less than that from the root (TP tab and TP multiglycoside tab, TPMG). In group of TPMG the rate of side effect of 60 mg daily dosage was more than 30 mg daily (P < 0.01). The main influence on digestive tract and irregular menstruation was before age of 50 years and on renal function was after age of 50 years. Those adverse reactions mainly occur within first ten years in long therapy period. The side effects of TPMG produced in the Dampness-Heat type of RA were lower than Yin-Deficiency of Liver and Kidney type of RA (P < 0.05). CONCLUSION: TP ester tab is available for young woman and TPMG tab is suitable for the Dampness-Heat type of RA with lower dosis maintenance therapy.

[Minimally invasive approach for heart valve replacement: report of 73 cases].

OBJECTIVE: To report a new minimally invasive approach for heart valve operation. METHODS: Through right minimal vertical infra-axillary thoracotomy incision (by intercostal space), 68 patients with rheumatic heart disease, 2 patients with subacute endocarditis, and 3 patients with congenital heart disease underwent heart valve replacement from March 1997 to Spring 1998. RESULTS: 30 double valve replacements, 33 mitral valve replacements, 10 aortic valve replacements and 14 DeVega tricuspid annuloplasty were performed. The length of the incision averaged (8.5 +/- 0.9) cm, arrest times averaged (88.6 +/- 33.8) min, cardiopulmonary bypass times averaged (117.0 +/- 45.9) min. Five mitral valve replacements were performed with ventricular fibrillation. Ventilator periods averaged (9.6 +/- 3.6) h, chest tube drainage averaged (264.5 +/- 184.3) ml. One patient died of cerebral bleeding in hospital. Two patients suffered SBE and one of them died. The results of follow up for other patients were fine. CONCLUSIONS: This minimally incisive valve operation is a new safe, small wounded and cosmetic approach for heart valve replacement.

[Operative results of tethered cord syndrome].

OBJECTIVE: To evaluate the treatment results of 22 cases of spinal tethered cord syndrome (TCS) using microneurosurgical technique. METHODS: Form 1994 to 1997, 22 cases of TCS verified by MRI were treated. All patients'tips of conus were below L(2) vertebra shown by MRI. In this group, 4 cases had lipid disease in the sacral canal, 5 piloidal sinus, and 13 thick filum. The patients were operated on using linear incision. The incision included the terminal conus and the whole filum. Under the microscope, the adhesions of the caudal equine were separated and the filum was found out. The caudal equine was well combed. Lipoma and piloidal sinus were removed totally or partially. Loosening the spinal conus and repairing the dural matter prevented re-adhesion and re-tether. RESULTS: Follow-up (0.5 - 3 years) showed that all of the patients were recovered in different levels. CONCLUSIONS: TCS should be found as early as possible and treated under the microsurgical technique.

[A study of expression of p53, c-myc and PCNA in retinoblastoma].

OBJECTIVE: To investigate the expressions of p53, c-myc (oncogene) and proliferating cell nuclear antigen (PCNA) in retinoblastoma (Rb) and the relationships of the expression to the degree of differentiation and optic nerve infiltration. METHOD: The expressions of p53, c-myc and PCNA in Rb tissues of 31 cases were analyzed by using LSAB immunohistochemical method. RESULTS: The positive rates of p53, c-myc and PCNA in Rb were respectively 51.6%, 45.2% and 67.7% and all the expressions were significantly related to the differentiation degree of Rb (all P < 0.05). The expression of PCNA was significantly related to the optic nerve infiltration of Rb (P < 0.05). Both the expressions of p53 and c-myc in Rb were markedly correlated with the expression of PCNA (all P < 0.05). CONCLUSIONS: The occurrence of Rb is the result of multiple gene mutations. The determinations of p53, c-myc and PCNA are of significance for evaluating the histologic characteristics and biological behavior of Rb.

[The significance of p16, cyclin D1 and pRb expression in soft tissue leiomyosarcoma].

OBJECTIVE: To evaluate the significance of p16, cyclin D1 and pRb expression in human soft tissue leiomyosarcoma (LMS) and the relationship among these three proteins. METHODS: 42 cases of LMS were studied for p16, cyclin D1 and pRb expression with immunohistochemical method. RESULTS: The total abnormality percentage of p16, cyclin D1 and pRb expression in all 42 cases of LMS was 97.6%. The negative rate of p16 and pRb protein expression was 35.7% and 47.6% respectively, and the overexpression rate of cyclin D1 was 57.1%. Overexpression of cyclin D1 was also noticed in 55% leiomyomas. In the 20 cases of LMS which were pRb negative, 19 cases were moderately or strongly positive and one faintly positive for p16. Contrastly, in the 15 cases of LMS negative to p16, 12 cases were moderately or strongly positive and 3 cases faintly positive for pRb. Higher rates of loss of pRb protein were observed in the low-differentiated group than in the high-differentiated one. CONCLUSIONS: (1) The abnormality of p16-cyclin-pRb pathway is highly related to the pathogenesis of LMS. (2) Loss of p16 or pRb and/or overexpression of cyclin D1 may be involved in the pathogenesis of LMS. (3) There is a negative reciprocity between p16 and pRb protein expression in LMS. (4) Overexpression of cyclin D1 is a common molecular alteration that may occur in the early stage. (5) Expression of pRb is inversely related to the differentiation degree of LMS. (6) The expression of the 3 proteins studied has no relation to tumor location, recurrence, invasion or metastasis.

[Protective effect of Radix Salvia Miltiorrhizae on radiation damage of the cochlea].

In order to evaluate the preventive and therapeutic effects of Radix Salvia Miltiorrhizae on radiation damage of the cochlea, guinea pigs were divided into 3 groups. Group 1 was treated by radiation added with Radix Salvia Miltiorrhizae. Group 2 was radiated alone, and Group 3 was control. Group 1 and Group 2 were radiated with a single dose of gamma radiation(60 Gy). Morphological and functional observation of the cochleae was performed in two weeks after radiation. The result showed that the changes of complex action potential(CAP) and the cochlear structure were slight in Group 1, but obvious changes were found in Group 2. There was a significant difference in CAP response threshold and incidence of the cochlear hair cell loss between Group 1 and Group 2 (P < 0.01). These results suggest that Radix Salvia Miltiorrhiza may prevent radiation-induced cochlea damage. Its protective mechanisms may be cleaning free radicals, blocking calcium channel and improving microcirculation of the cochlea.

[Relationship between the effect of carbamazepine on SCE frequencies and folic acid in epileptic patients].

Peripheral blood lymphocyte sister chromatid exchange(SCE) frequencies, serum folic acid(FA) levels were examined in 15 epileptic patients treated with carbamazepine(CBZ), and another 15 epileptic patients treated with CBZ and folic acid(FA). The untreated epileptic patients and the healthy subjects served as control. The results showed that SCE frequencies were significantly higher in CBZ Group, compared with CBZ plus FA Group and control(P < 0.01). Serum FA levels were lower in CBZ Group compared with healthy control(P < 0.01). It suggests that CBZ can induce the increase of SCE frequencies. Supplementation with FA may effectively prevent chromosome DNA damage induced by CBZ.

[Comparative study on livertaxis of lactosaminated human growth hormone and galactosyl-human growth hormone].

This study was conducted to compare the livertaxis of lactosaminated human growth hormone (hGH-L) with that of galactosyl human growth hormone (hGH-Gal), using radioactive tracer technique. hGH-L and hGH-Gal were first prepared. After hGH-L, hGH-Gal and hGH were labelled with 131I or 125I, the data on biodistribution in mice, imaging in rabbits and chicken were obtained. hGH-L and hGH-Gal had remarkable livertaxis and their percentages for uptake in liver were 68.83%-74.65% and 68.18%-74% respectively, which were about two times as high as that of hGH. All of the hepatic bindings were receptor-mediated. But, the synthesizing of hGH-L was more easy-to-do, more mild in reaction conditions, and more economical of raw material as compared with the synthesizing of hGH-Gal. Therefore, hGH-L is a more potential receptor-mediated hepatic targeting secreting somatomedian drug for treating dwarfism.

[Biological characteristics and damaging regularities of grubs endangering the growth of Ophiopogen japonicus (Thunb.) Ker-Gawl].

OBJECTIVE: To provide a scientific basis for prevention and control of the damage done to Ophiopogen japonicus by grubs. METHOD: Conducting laboratory observation along with field investigation. RESULT: The grubs endangering the growth of O. japonicus are mainly Hoplosternus incanus, Holotrichina paraullela and Anomala corpulenta, H. incanus takes two years to make a generation and overwinters in the form of second or third-instar larvae. H. parallela and A. corpulenta generate yearly and overwinter in the form of third-instar larvae. The extent of the damage done to O. japonicus by grubs is related to such factors as the number of sequential cropping years, the model of intercropping and the type of soil. CONCLUSION: Prevention and control of the pest grub should be based primarily on proper agricultural (cultivating) measures.

Gender differences in determinants of temporary labor migration in China: a multilevel analysis.

Data from a 1998 migration survey in Hubei province are used to examine gender differences in the determinants of temporary labor migration from a multi-level perspective. The authors find that community level factors play a key role in temporary labor migration; models omitting community level variables are poor in predicting temporary labor migration. Significant gender differences exist in determinants of temporary labor migration. For men, temporary labor migration is mainly a response to community level factors; individual or household characteristics have little predictive power. For women, by contrast, temporary labor migration is predominantly determined by individual characteristics; community level factors are not as important.

[Cloning and sequence characterization of heavy chain variable region gene from hybridoma cells secreting MAB against VEGF].

OBJECTIVE: To clone the variable region of heavy chain (VH) gene from hybridoma secreting monoclonal antibody against human vasoendothelial growth factor (VEGF) for construction of recombinant antibody (scFv). METHODS: One step method was used to extract total RNA and degeneration primer was used to amplify the cDNAs by retropolymerase chain reaction (RT-PCR), then the products were inserted to T-vector and its sequence was analysed. RESULTS: The VH gene contained 369 base pairs encoding 123 amine acid resides. Analysis of the nucleotide sequence of the light chain variable region revealed that the VH sequence was 87% identical to the sequence present in mouse Ig VH region. CONCLUSION: The VH gene was 369 bp. According to Kabat classification, the VH gene belonging to mouse kappa light chains subgroup I (A), resulting from VH-D-JH4 rearrangment.

[Design and synthesizing of human vascular endothelial growth factor (VEGF) peptide].

OBJECTIVE: To predict continuous epitopes of VEGF using synthetic peptide of VEGF as antigen to produce anti-VEGF-peptide monoclonal antibody. METHODS: According to amino sequence of VEGF189, antigen index program and Goldkey soft were used to predict VEGF epitopes, and the peptide containing one of predicted epitopes was synthesized by 430A Peptide Synthesizer. RESULTS: The NH2-terminal 1-26 amino acid sequence of VEGF was determined as synthetic peptide and synthesized. CONCLUSION: The predicted epitope of VEGF was synthesized, which provided an effective method when lacking of VEGF.

[Cell proliferation and programmed cell death in tooth development].

OBJECTIVE: To evaluate the role of cell proliferation and programmed cell death (PCD) in the tooth development by detecting programmed cell death and expression of proliferating cell nuclear antigen (PCNA). METHODS: Expression of PCNA and PCD were detected by immunohistochemical staining and Tdt-mediated dUTP nick end labelling (TUNEL) in different stages of the tooth development of Sprague-Dawley rats. RESULTS: The positive cells of PCNA and PCD predominantly appeared at the proliferating growth center in the bud, the cap and the earlier bell stages of tooth development. PCNA and PCD were also observed in ameloblasts and odontoblasts, especially in the stellate reticulum cells of enamel organ and dental papilla during the period of dentin formation. CONCLUSION: Cell proliferation and programmed cell death are interrelated and interact on each other in the development of teeth, and they both involve in sculpturing the shape of teeth.

[Gas chromatography/mass spectrometry analysis for determination of methyl alpha-(6-methoxyl-2-naphthyl) propionate from carbonylation].

alpha-(6-Methoxyl-2-naphthyl) ethanol can be carbonylated to form methyl-alpha-(6-methoxyl-2-naphthyl)propionate catalyzed by a catalyst in the pressnce of methanol. High efficiency capillary gas chromatography and GC/MS technique were used for determining products which were obtained from the carbonylation. An OV-101 fused silica capillary column (25 m x 0.2 mm i.d.) and a flame ionization detector (FID) were employed in the GC analysis. The column temperature was kept at 250 degrees C. Mass spectra were obtained by electron impact at 70 eV. The structures of four compounds were identified. The major product is methyl-alpha-(6-methoxy-2-naphthyl) propionate, and the two by-products are 2-vinyl-6-methoxynaphthalene and 1-methoxy-(6'-methoxy-2'-naphthyl)ethanol.
