[Cost-effectiveness analysis of Beijing Fangshan cardiovascular prevention program in 1992 - 1997].

OBJECTIVE: To determine whether a community-based cardiovascular disease (CVD) intervention program, undertaken over six years, was cost-effective. METHODS: Based on Beijing Fangshan Cardiovascular Disease Comprehensive Prevention Program, the cost for intervention and expenditure saved from caring for CVD in the communities with intervention from 1992 to 1997 were calculated, and cost-effectiveness analysis was performed using disability-adjusted life years (DALYs) gained as an indicator of effectiveness. RESULTS: The cost for one DALY gained reduced gradually from 1992 to 1997, with an average ratio of cost to effectiveness of four to one (4:1). It cost annually 1,586.00, 1,380.20, -2,350.80, -905.30, -1,495.60 and -1,766.70 RMB yuan for one life-year saved, from 1992 to 1997 respectively, in a gradually decreasing trend with the increase in length of intervention. After intervention for two years, ratio of cost to effectiveness has become negative since 1994, which meant a positive benefit from intervention. Sensitivity analysis showed that ratio of cost to effectiveness was little sensitive to the changes in discount rate, weight of age and increase in cost of hospitalization for stroke and coronary heart disease, which reflected its reliability. CONCLUSION: Community-based comprehensive intervention for CVD in rural population is cost-effective.

[Effects of oral calcium supplementation on blood pressure in population].

OBJECTIVE: To assess the effects of oral intake of calcium lactate tablet on blood pressure and serum levels of sodium, potassium, calcium and phosphors. METHODS: In a rural area with natural population with higher average blood pressure and prevalence of hypertension, and with high salt and low calcium intake in diet in Yu County, Shanxi Province, 112 volunteers aged 30 to 64 years were selected based on a mass screening for hypertension, and randomized into trial and control groups with double blinding, taking calcium lactate tablets (800 mg of calcium daily) and placebo respectively for five weeks. Ninety-eight of them complied with the medical order completely, 39 males and 59 females, and were followed-up for systolic (SBP) and diastolic blood pressure (DBP), and their fasting venous blood specimen and 8-hour nocturnal urine specimens were collected. Twenty-four-hour diet recall was obtained for three consecutive days from all of them. RESULTS: Reduction of 4.7 mm Hg (0.62 kPa) and 2.7 mm Hg (0.36 kPa) in systolic and diastolic blood pressure, respectively in the trial group was observed, as compared with that in control one, with P-values of 0.027 and 0.074, respectively, after 35 days of treatment. The higher their baseline BP level, the greater reduction of their BP after treatment. Urinary excretion of calcium increased and serum level of phosphorus reduced after oral administration of calcium. CONCLUSION: In an area with higher prevalence of hypertension and with high salt and low calcium intake in diet, change in diet composition or supplementation with calcium could reduce their average blood pressure and prevent them from hypertension for those with mild and moderate hypertension and in high risk with higher blood pressure.

[Inhibition of growth and metastasis of human giant cell carcinoma of the lung by transfection of antisense VEGF121 cDNA].

OBJECTIVE: To explore the effects of blocking the VEGF/VEGF receptor paracrine pathway on growth and metastasis of human lung carcinoma cell line PG and to evaluate its potential application in gene therapy of cancer. METHODS: The eukaryotic expression vectors bearing either sense-VEGF121 cDNA or antisense-VEGF121 cDNA was constructed and transfected into PG cells. In vitro and in vivo tests such as Northern blotting hybridization, Western blotting immunochemistry analysis, as well as xenografting in nude mice were used to analyze the effect of antisense-VEGF. RESULTS: The transfectants stably expressing antisense VEGF121 were observed to produce markedly reduced 3.3 kb VEGF mRNA and 45 KD, 41 KD, 32KD VEGF proteins. When xenografted s.c. into nude mice, growth and metastasis of the antisense-VEGF transfected cell lines were greatly inhibited when compared with control cells. CONCLUSION: Antisense VEGF gene significantly inhibited tumor growth and metastasis and may provide an experimental example for the development of antiangiogenic gene therapy.

[Analysis of the mental state of burn patients with postburn phobic neurosis].

OBJECTIVE: To analyze the mental state of burn patients with postburn neurosis, so as to pave the way for solving the problem. METHODS: The clinical symptoms and the mental states of forty-three burn patients with phobic neurosis were analyzed in terms of age, sex, severity of burn injury and methods of burn management in accordance with phobic objects. RESULTS: (1) Simple phobia and circumstance phobia were mainly found in children, whereas sick phobia and social phobia were in adults. (2) Phobic neurosis happened more often in female, especially those above 12 years old, than in male patients. (3) The burn injury severity was positively correlated with the incidence of phobic neurosis. (4) There was higher incidence of phobic neurosis in patients treated with bandaging than those with exposure method. CONCLUSION: It was very important to close burn wound as soon as possible, to correct deformity, to improve dysfunction, to perform cosmesis and plastic surgery and to rebuild patients self-esteem in order to prevent or decrease the incidence of postburn phobic neurosis.

[Exploration of gene expression profiles of CD(34)(+) hematopoietic stem/progenitor cells based on large scale sequencing].

OBJECTIVE: To set up a large scale sequencing system and explore the gene expression profiles of CD(34)(+) hematopoietic stem/progenitor cells (HSPCs). METHODS: CD(34)(+) cells were isolated from umbilical cord blood and subjected to cDNA library construction. A primary profile of gene expression in HSPCs was emerged by EST sequencing and bioinformatics analyzing. RESULTS: Among 9,866 ESTs thus obtained, 7 476 meaningful ESTs were clustered into 2,060 unique sequence species (USSs), representing 1,054 known gene species and 1,006 unknown gene fragments. The 1054 known genes were divided into 8 categories: (1) hematopoiesis associated: 73, (2) chromatin structure and cell division/apoptosis: 91, (3) signal transduction and receptors: 111, (4) cell structure/mobility: 48, (5) cell/organism defense/homeostasis: 41; (6) Gene expression (transcription, translation and modification): 265, (7) metabolism: 192; and (8) unclassified: 233. CONCLUSION: A gene expression profile including 1,054 known genes and 1006 new gene fragments of HSPCs was primarily obtained, which may lay a basis for the further study on the molecular mechanism of hematopoiesis regulation and provide candidates for new gene cloning.

[The changes of GFAP and PCNA after human brain contusion: an immunohistochemical study in forensic pathology].

In order to find out changes of GFAP and PCNA after human brain trauma, samples of 38 cases were studied by the immunnohistochemistry method combined with quantitative image analysis. The gray degree and area of PCNA positive cells increased to the maximum at the 4th day and then decreased gradually. GFAP positive cells began to increase significantly 12 hours after contusion, reached the maximum at the 4th day and stayed in high level until the 7th day. These results suggest that: (1) GFAP, PCNA show regular changes with survival time after brain contusion; (2) GFAP and PCNA can be used as time markers of brain contusion from 2-20 days.

[Expression of ET of early ischemic myocardium in rats].

To study the expression of ET of early ischemic myocardium, we designed a model of early myocardial ischemia (EMI) in rats. The study was conducted with SABC method and HE staining. Cardiac myocytes of ischemia showed a little ET-positive at 60-min. Cardiac myocytes, endothelium and smooth muscle of vessel showed ET-positive staining in the area of EMI after 120-min. Staining was negative for Ischemia myocardium at 30-min and for the control group. No change was seen with HE staining in all groups. The data indicates that ET-SABC method may reveal the injury of myocardial ischemia 60-min and longer and may provide a new marker to diagnose EMI in forensic pathology.

[A treatment combined Chinese with Western medicine for microcirculation diseases in tongue].

OBJECTIVE: Glossodynia is a common oral disease. The purpose of the investigation is to analyse the mechanism of microcirculation disorder in glossodynia, and find a new effective treatment. METHODS: 18 cases with microcirculation disorder in tongue were studied by histopathology, hemorrheology and nail microcirculation, treated by intravenous injection of angelica for promoting blood circulation and removing blood stasis. RESULTS: Hyper-viscosity of blood was found in this group of patients. Abnormal micro-vessels were observed in microcirculation examination, though vasculitis wasn't detected in tongue tissue. CONCLUSION: Microcirculation disorder may play an important role in the cause of glossodynia. For elderly patients, cardiovascular and mental diseases should be considered. According to the theory of the blood stasis in chinese medicine, these studies show that a treatment combined chinese with western medicine can be applied for the patients, and injection of angelica can improve microcirculation.

[Screening DPPH radical scavengers from Monascus sp].

A strain belongs to Monascus sp. was separated from hongqu(Chinese beni-coji) which was used to brew red wine in Changxin county, Zhejiang province, and its DPPH radical scavenging metabolites were studied. Results showed that the yield and radical scavenging activity of neutral extract with EtOAc were higher than that of acid and basic extract. After shaken at 30 degrees C, 100 r/min for 5 days, this strain produced largest amount of neutral extract. Further partitioning the neutral fraction with silica gel column chromatograph, LH-20 column chromatograph, MPLC and HPLC, we screened 15 free radical scavengers whose yield was more than 2 mg and purity was higher than 85% in HPLC (260 nm absorption). Several of them were analyzed with 1H-NMR and 13C-NMR spectrum from which we concluded that B1-3 and C3-1-7 were two kinds of substitute of benzene, and C3-1-7 may be 3-OH-4-OCH3-benzoic acid. The scavenging activity of 40 mumol/L B1-3 was about 65%, and that of C3-1-7 was lower than 56%, while as the control the scavenging activity of 40 mumol/L Vc and Ve was 80.4% and 78.4% respectively.

[Investigation on Schistosoma japonicum infection in rodents in Yunnan Province].

OBJECTIVE: To investigate the role of rodents in the transmission of schistosomiasis in endemic areas in Yunnan Province. METHODS: Kato-Katz method was used to study the infection rate of rodents, humans and domestic animals. RESULTS: The infection rate was 0.9% (32/3,411) for rodents, 15.6% (461/2,964) for humans, and 9.6% (239/2,482), for domestic animals. The EPG(mean) was 5.8 for rodents, 1.8 for humans, 0.1 for cattle, 0.1 for horses and 0.02 for pigs. CONCLUSION: The rodents played a minor role in the transmission of schistosomiasis in the plateau area of Yunnan province.

[Studies on Euphorbia fischeriana diterpenoide lactones inhibitory effect on human tumor cells in vitro].

In this paper, two diterpenoide lactones 16-hydroxypseudojolkinolide B and jolkinolide B were isolated from Euphorbia fischeriana Steud. They showed inhibitory effect on human tumor cells both the K562 cell and CNE2 cell and significant dose-effect relationship. The IC50 values of 16-hydroxypseudojolkinolide B on the two human tumor cells were near to that of ADM's.

[Study on extraction process for psoralen in compound prescription by orthogonal design].

The study on water volume, extraction time and times for the extraction of psoralen in compound prescription Yiniao Tong capsule has been carried out by orthogonal design. The process condition has been determinated, extracting prescription herbs with water for 3 times(2 hour and 16 time amount of water all told). The determination method used in this experiment was TLC-scanning.

[mRNA expression of keratoepithelin in cornea after photorefractive keratectomy in rabbits].

OBJECTIVE: To investigate the roles of keratoepithelin(KE) during healing after photorefractive keratectomy (PRK). METHODS: 20 of 24 rabbits underwent bilateral 193 nm excimer laser photorefractive keratectomy to correct 10 diopters of myopia, and the other 4 rabbits as normal group. The mRNA levels of KE in corneas of the rabbits were determined by the in situ hybridization technique with a Digoxigenin labled probe. RESULTS: In normal group, there was no mRNA expression of KE in cornea. on 7, 14 days after operation, mRNA expression of KE was observed in epithelium and stroma of operation group, while on 28 days mRNA expression of KE was observed only in epithelium. CONCLUSION: KE was involved in the synthesis of extracellular matrix during healing after PRK and may play an important role in the development of haze by maintaining proper spacing among collagen fibrils.

[Simultaneously corneal laceration repair, traumatic cataract removal and intraocular lens implantation].

OBJECTIVE: The study aimed to evaluate the outcome of patients who undrwent simultaneously corneal laceration repair, cataract extraction and posterior chamber lens implantation. METHODS: Traumatic cataract extraction and posterior chamber intraocular lens (PC IOL) implantation were performed in 36 eyes of 36 cases with corneal laceration. YAG laser was the postoperative procedure for embrane before the lens and after cataract. Penetrating keratoplasty was performed for central cicatrice of cornea. All the patients have been followed up for 3-12 months. RESULTS: The postoperative visual acuity s.c. was 0.1-0.3 in 8 cases(22.8%), 0.4-0.5 in 14 cases (40%) 0.6-0.8 in 13 cases (37.1%) on following up. There were no serious complication attributable to PC IOL implantation. CONCLUSION: As long as the range of surgical indication was known well, PC IOL implantation can get good clinical effects at the time of primary repair.

[Multiple factors analysis of myopic regression after photorefractive keratectomy].

PURPOSE: To investigate myopic regression after excimer laser photorefractive keratectomy(PRK) in patients with myopia. METHODS: PRK was performed to correct myopia errors from 1.00 D to 16.00 D diopters on 214 myopia eyes(121 patients). All eyes were divided into two groups: group I--1.00 D(-)-6.00 D (124 eyes), group II--6.25 D(-)-16.00 D(90 eyes). All the patients were followed up for two years. RESULTS: A Significant positive correlation was found between the regression and the extents of attempted correction. The rate of regression in mild to moderate myopia group was 9.7% while in highter group it was 27.8% (P < 0.005). The rate of regression of older age group in lighter myopia group was highter than that in younger case (P < 0.005). Relatively severe haze under the corneal epithelium often accompanies the regression. CONCLUSION: The main factors which caused the myopia regression after PRK was the extent of attempted correction, patients' age and corneal haze.

[Analysis of refractive state after excimer laser photorefractive keratectomy in myopia].

OBJECTIVE: To evaluate refractive state after excimer laser photorefractive keratectomy (PRK) in patients with myopia. METHODS: 234 cases(391 eyes) followed up for two years were divided into two groups, group I: mild and moderate myopia(< or = -6.00 D), 228 eyes; group II: High myopia(> -6.00 D), 163 eyes. The refractive state of patients at three, six, twelve and twenty-four months postoperatively were monitored. RESULTS: At two years after photorefractive keratectomy (PRK), refractive regression (> -0.5 D), overcorrection(> 1.00 D), undercorrection(> 1.00 D), postoperative residual astigmatism postoperative astigmatism and hyperopia shift in group I were 1.7%, 4.4%, 5.7%, 12.8%, 7.4% and 7.5% respectively, while in high myopia group were 15.9%, 4.9%, 23.3%, 29.7%, 31.1% and 6.7% respectively. CONCLUSION: The results prove that photorefractive keratectomy is an effective, precise, stable and safe method for correction of mild and moderate myopia, but insufficient for high myopia.

[A novel method for screening anti-inner ear autoantibody in patients with autoimmune diseases].

OBJECTIVE: To appraise the clinical value of a newly established method, rapid electric field immobilizing liquid phase molecule dot blot analysis (REILMD), for screening anti-inner ear autoantibody in patients with autoimmune diseases. METHODS: Seventy-one patients with 11 kinds of autoimmune diseases were chosen for the study. Both the general immunity and autoantibodies were tested. In the processes of detection of anti-inner ear autoantibody, REILMD was used for screening, and then the Western blot was used to define the molecular weight of inner ear antigen recognized by the positive autoantibody. RESULTS: Acceleration of erythrocyte sedimentation rate (ES), positive rheumatoid factor (RF), increases in C reactive protein (CRP), IgG and circulating immune complex (CIC) were found in most cases with rheumatoid arthritis (RA) and systemic lupus erythematodes (SLE). Some of these patients had increased IgA, IgM and C4. Two of 16 RA had anti-double-stranded DNA (dsDNA) and anti-mitochondria and 4/16 had anti-nucleus antibodies. Eleven of 16 SLE had antinucleus, 7/13 had anti-ribonucleoprotein (RNP), anti-Sjögren syndrome A (SSA) and anti-dsDNA, 3/13 had anti-smooth muscle (Sm) and 1/13 had anti-DNA topoisomerase I (Scl)-70, striated muscle and stomach acid cell antibodies. No autoantibody was detected in AS. Anti-inner ear autoantibody existed in 9 out of 71 patients (13%) with autoimmune diseases, in 2 of 21 patients (10%) with sudden deafness and only in 1 of 48 control subjects (2%, coronary heart disease). The anti-inner ear autoantibody was positive in 5 of 16 (31%) patients with SLE and 1 each in RA, AS, Behset's disease and streptococcus infection syndrome. In patients with positive anti-inner ear antibody, 67% had anti-nucleus antibody, 50% had anti-RNP and dsDNA antibody. The molecules recognized by the positive anti-inner ear antibody were defined as 52,000, 36,000, 31,000 and 15,000 molecules of inner ear antigen. CONCLUSION: REILMD is a feasible and easy method for screening anti-inner ear autoantibody. Several autoimmune diseases, particularly SLE, may be implicated in damage to the inner ear.

[Characteristic immunodeficiency syndrome of rapid fatal type of simian immunodeficiency virus infected monkeys].

OBJECTIVE: To observe the characteristic immunodeficiency syndrome of the rapid fatal type of simian immunodeficiency virus (SIV) infected monkeys. METHODS: Eighty rhesus monkeys and 4 cynomolgus monkeys were intravenously inoculated with SIVmac or SIVmac251. The virus isolation and viral titer, estimation by indirect immunofluroresence and viral antibody were determined periodically from monkeys' plasma; lymph node biopsies were performed for pathohistological examination. RESULTS: Twelve out of 84 macaque (14.2%) died of rapid progressive type after inoculation of SIVmac and SIVmac251 in the duration 3 to 4 months. Dying monkeys showed persistent high viremia and low level titre antibody. Eight of 10 pathohistological changes showed severe depletion of lymphoid tissue in spleen and lymph nodes, there were remarkable immunodeficiency with opportunity infection. The other two monkeys appeared moderate lymphoid tissue deletion and hyperplasia without opportunity infections. The survived monkeys' (72/84) lymph nodes biopsies revealed hypoplasia of lymphoid tissue. CONCLUSIONS: The characteristic immunodeficiency syndrome of rapid fatal type of simian immunodeficiency virus infected monkeys could be made with persistent high viremia, low level antibody, severe lymphoid tissue deletion in lymph nodes and spleen, as well as complicated opportunity infections.
