[Regulatory effect of antisense VEGF121 cDNA transfection on angiogenesis and metastasis of human lung giant cell carcinoma].

OBJECTIVE: To study the regulatory effect of antisense VEGF121 cDNA transfection on endogenous VEGF secretion and angiogenesis of human metastatic lung carcinoma cell line PG and explore the significance of microvessel density (MVD) in tumor growth and metastasis. METHODS: The eukaryotic expression vectors bearing antisense VEGF121 cDNA was transfected into PG cells. Human umbilical vein endothelial cells (HUVEC) were cultured in conditioned mediums from transfected cells, and proliferation was determined by methyl thiazolyl tetrazolium (MTT) and 3H thymidine incorporation (3H TdR) assays in vitro. Microvessel density (MVD) in xenografted tumors in nude mice was analyzed by immunohistochemistry. RESULTS: The transfectant of antisense VEGF121 cDNA exhibited a reduction in VEGF secretion. HUVEC grown in conditioned medium from the antisense VEGF transfected cells exhibited a decrease in capacities of DNA syntheses and cell proliferation. MVD of tumor with transfected antisense VEGF gene was significantly lower than that in control vector. CONCLUSION: Antisense VEGF gene transfection can inhibit vascular endothelial cell proliferation in vitro and tumor angiogenesis in vivo, which may explain its inhibitory effects on tumor growth and metastasis.

[A comparison of malignant fibrous histiocytoma of head, neck and extremities].

OBJECTIVE: To compare the epidemiology, local and regional invasion and prognosis of head and neck, malignant fibrous histiocytoma (MFH) and extremity MFH. METHODS: Between January 1, 1972 and December 31, 1993, 173 patients with MFH (71 head and neck, 102 extremities and trunk) were referred to the Cancer Hospital for surgical treatment. They were 101 men and 72 women, with age ranging from 13 to 83 years (median: 45 years). The extent of surgery was classified into radical, wide and local resection. RESULTS: For head and neck, local recurrence of MFH after wide resection was 80.4% (37/46), compared with 36.4% (8/22) after radical surgery (P = 0.000). The five-year survival was 50.79% in all the head and neck patients (74.75% in patients free of local recurrence, 37.74% in patients with local recurrence; P = 0.0181), compared with 70.71% in extremity patients (P = 0.0005). Repeated surgery after recurrence of MFH could cure 40.9% of the head and neck patients and 80.7% of the extremity patients. CONCLUSION: Inadequate resection of head and neck MFH in initial surgery is associated with a high incidence of local recurrence, which is always correlated with worse prognosis. Repeated surgery for recurrent head and neck MFH is not as effective as for recurrent extremity MFH. Therefore, we suggest that the initial surgery for head and neck MFH should be as radical as allowed to avoid a possible local recurrence.

Modified sternal elevation for children with pectus excavatum.

OBJECTIVE: To describe our experience in the treatment of pectus excavatum (PE) using a modified sternal elevation procedure. METHODS: From Oct. 1986 to Dec. 1997, 171 patients with PE were admitted to the Department of Pediatric Surgery of the First Hospital of West China University of Medical Sciences. All patients were diagnosed through a history and physical examination. Cardiopulmonary function was assessed by M-mode echocardiography and instrument of pulmonary function in 40 patients before and 4.2 years after surgery which was performed between 1989 and 1994. We performed the following three procedures in the sternal elevation: (1) forming the metal strut in a "arch" shape, (2) suturing the perichondrium into a "pipe" shape, and (3) encouraging patients to do chest expansion exercise after operation. All patients were followed up for 1 to 12 years. RESULTS: The normal contour of the costal cage was enlarged in all but one patient. Exercise tolerance was improved, and cardiac function recovered to the same level as in healthy children, while pulmonary function recovered very slowly after surgery. CONCLUSION: The normal appearance of chest wall can be recovered and normal cardiopulmonary function can be restored by the modified sternal elevation procedure in children with PE.

[Studies on pncA gene mutations in M. tuberculosis isolates].

OBJECTIVE: To understand the mutations of pncA gene in M. tuberculosis isolates, and to evaluate their clinical value. METHODS: Analyzing the pncA genes in 74 M. tuberculosis clinical isolates with PCR-SSCP and PCR-AS. M. tuberculosis strain H37Rv was used as control. RESULTS: 32 drug-sensitive isolates all displayed normal pncA SSCP profile. Of 20 non-pyrazinamide-resistant isolates, 4 had abnormal pncA SSCP profile, in which 2 isolates were sequenced, one was TTC-->TTA mutation at codon 58, another was TGG-->CGG mutation at codon 68. Of 22 pyrazinamide-resistant isolates, 10 displayed abnormal pncA SSCP profile, in which 2 isolates sequenced had TTC-->TTA mutation at codon 58. CONCLUSIONS: The mutation of pncA gene is a pyrazinamide-resistant molecular mechanism in M. tuberculosis. Detecting the mutations of pncA genes might diagnose pyrazinamide resistance rapidly in some M. tuberculosis isolates.

[A study of chemo-hand instrument removal of caries in vitro].

OBJECTIVE: To study the efficiency of chemo-hand instrument removal material. METHODS: The effect on caries removal of N-monochlorochloraminobutyric acid and the electrical dental instrument was detected by the caries detection, microscope and SEM. RESULTS: The positive for the caries detection and residual caries were zero in both groups. CONCLUSION: The effect on caries removal of N-monochlorochloraminobutyric acid is almost the same as the control.

[Clinical study on the expression level of lymphocyte function-related antigen of breast cancer patients].

OBJECTIVE: To explore the protein expression level of the lymphocyte function-related antigen CD11a, CD18, CD54 of breast cancer patients' blood, cancer tissue and drainage lymph node, and the effect of chemotherapy as well as Yiqi Yangxue Granule on its expression level. METHODS: The immunohistochemical assay and flow cytometry were used to determine the change of the protein expression level of CD11a, CD18, CD54 on the lymphocyte of blood, cancer tissue and lymph node between before and after chemotherapy and chemotherapy plus Yiqi Yangxue Granule in 72 patients suffering from breast cancer. RESULTS: In breast cancer patients, the level of CD54 was increased more obviously than that of control group, the CD11a, CD18, CD54 of lymphocytes in cancer tissue increased remarkably, while the CD54 expression positive rate reached 100%, the expression level of CD11a, CD18, CD54 which increased significantly in negative lymph node obviously higher than that of positive lymph node. CONCLUSION: The level of adherence cell was markedly abnormal in breast cancer patients, whose immune adherence function disturbed. Although chemotherapy kills the tumor cells, it damages the immune adherence function, Yiqi Yangxue Granule could improve the immune function and enhance the anti-cancer effect of patients.

[Protective effects of tongli gongxia herbs on gut barrier in rat with multiple organ dysfunction syndrome].

OBJECTIVE: To explore the protective effects of Tongli Gongxia (TLGX) herbs on gut barrier with multiple organ dysfunction syndrome (MODS). METHODS: The MODS models were induced by intraperitoneal injection of Zymosan A, and Dachengqi decoction (DCQD) was used in treating MODS models. The levels of endotoxin in the peripheral and portal blood, and the contents of diamine oxidase (DAO), xanthine oxidase (XOD), malondialdehyde (MDA), reduced glutathione (GSH) and tumor necrosis factor (TNF), the permeability of intestinal mucosa and enterocytes, and the histopathologic changes were observed. RESULTS: The gut barrier function in MODS rats were severely damaged. DCQD could significantly reduce the levels of endotoxin in the blood (P < 0.01), the contents of XOD, MDA and TNF in the blood and intestinal tissue were lowered (P < 0.01), but the levels of GSH in blood and intestinal tissue were raised (P < 0.05). Levels of DAO in the intestinal tissue was markedly increased (P < 0.01). DCQD could reduce the permeability of intestinal mucosa and enterocytes, and attenuate the histopathologic changes of intestinal mucosa in MODS rats. CONCLUSION: TLGX herbs have apparently protective effects on the gut barrier in MODS rats.

[Effect of xiaoqinglong decoction drug-serum on degranulation and histamine releasing of peritoneal mastocyte in sensitized rats].

OBJECTIVE: To study the mechanism of anti-allergic effect of Xiaoqinglong Decoction (XQLD). METHODS: The influence of XQLD drug-serum frozen powder on degranulation and histamine releasing of peritoneal mastocyte in sensitized rats was examined by morphologic and fluorimetric method. RESULTS: The drug-serum prepared by serum obtained 2 hours after medication for 15 min has obvious inhibitory effect on degranulation and histamine releasing of peritoneal mastocyte which showed a basically unified time-effect relationship. CONCLUSION: The anti-allergic effect of XQLD is related with the allergic medium (histamine) inhibition of releasing of peritoneal mastocyte.

[Clinical analysis of 224 cases of small intestine disease].

OBJECTIVE: To increase the knowledge of small intestine disease, improve the diagnostic rate and discusses the examination methods. METHODS: 224 cases of small intestine disease were studied and their major symptoms, etiology, diagnostic methods were analyzed. RESULTS: The major symptoms of small intestine disease were abdominal pain (58%), abdominal mass (18%), fever (17%) and hematochezia (12%). The most common disease of small intestine was malignant tumor (32%), others were diverticulum (29%), leiomyomas (7%), hemorrhagic necrotizing enteritis (6%), Crohn's disease (4%). Double-contrast enteroclysis and laparotomy were still the major diagnostic methods. CONCLUSION: Malignant tumor was the most common disease of small intestine. More attention should be paid to the diagnosis of small intestine disease.

[Fourier analysis of macular local electroretinogram in retinitis pigmentosa].

OBJECTIVE: To understand the sites of macular dysfunction in retinitis pigmentosa (RP) by evaluating local electroretinogram (LERG) and further evaluating fundamental and second harmonic components. METHODS: LERG were recorded in 98 eyes of 49 patients to a flickered sinusoid of 31Hz and 10Hz with a hand-held stimulator with direct visualization of the fundus through dilated pupils. RESULTS: Compared to controls, the patients showed losses of both fundamental and second harmonic amplitudes. The abnormality ratios of fundamental amplitudes in RP affected time < 5 years, 5 - 15 years, 15 - 25 years and > 25 years were 54%, 64%, 83% and 92% respectively; and the abnormality ratios of second harmonic amplitudes were 18%, 36%, 58% and 75% respectively. The fundamental-second harmonic ratio was much more higher in the autosomal recessive and sporadic patients than in the autosomal dominant patients (P < 0.01). On average, the fundamental-second harmonic ratio in the patients tended to increase with age. CONCLUSION: The results suggest that in the retinitis pigmentosa, both receptoral and postreceptoral sites contribute to macular dysfunction. The level of macular dysfunction in different types of hereditary patients is different. It would be of clinical value to obtain a fundamental-second harmonic ratio in order to assess the disturbance in different sites of macular.

[Long-term therapeutic effects of extended radical resection and radical resection of cancer of cardia and stomach fundus].

OBJECTIVE: To study the best range of radical resection in the treatment of cancer of the cardia and fundus of stomach. METHODS: 418 patients with cancer of the cardia and fundus of stomach underwent radical resection. Of them 192 were treated by extended radical resection (extended group), and 226 by radical resection (radical group). The 5-year and 10-year survival rates were followed up and compared in the two groups. RESULTS: Analysis failed to demonstrate significant difference between the two operations for TNM stage I and II (P > 0.05). For stage III, however, the 5-year and 10-year survival rates in the extended group increased by 14.2% and 15.9% as compared with those in the radical group (P < 0.05). The two groups survival rates were similar for stage IV (P > 0.05). CONCLUSIONS: To completely clean the lymph nodes of splenic hilus and artery and improve long-term survival rate, extended radical resection including spleen and body and tail of the pancreas should be recommended for stage III patients with cancer of the cardia and fundus of stomach when their serosa was involved or lymph node metastasis took place.

[Coronary venous variation in patients with portal hypertension].

OBJECTIVES: To observe the changes in the anatomic variation and the hepatofugal flow of coronary vein and to evaluate the clinical significance of these changes in patients with portal hypertension. METHODS: The anatomic variations and the retrograde flow of the coronary vein, and the portal antegrade flow and the portal pressure were observed by direct and indirect portography, direct measurement of portal pressure and Doppler sonography in 200 cirrhotic patients with portal hypertension. RESULTS: Single branch and two branches of the coronary vein were noted in 79.46% and 20.54% of the patients with portal hypertension respectively by direct portography. The sites of coronary vein arising from the portal vein, splenic vein or portosplenic junction were found in 62.33%, 27.35% or 8.07% respectively. The confluence of coronary vein and portal vein was found in most patients with variceal bleeding and recurrent bleeding after devascularization. The confluence of the coronary vein and splenic vein was observed in most patients with refractory ascites without variceal bleeding. Arterial portography demonstrated retrograde opacification of the coronary vein in 26.19% (grade II portal venous pattern), 56.64% (grade III portal venous pattern), 100% (portal thrombosis and action bleeding) 47.79% (controuing periods of bleeding), and 19.05% (refractory ascites). CONCLUSIONS: The anatomic variation of the coronary vein may be related to variceal bleeding in cirrhotic patients with portal hypertension and the retrograde opacification of the coronary vein is a significant sign of variceal bleeding.

[Comparison of TIPS, a modified Sugiura procedure and TIPS plus a modified Sugiura procedure for portal hypertension].

OBJECTIVE: Transjugular intrahepatic portosystemic shunts (TIPS), a modified Sugiura procedure and TIPS plus a modified Sugiura procedure were compared to evaluate their clinical results in treatment of portal hypertension. METHODS: Ninety patients with portal hypertension respectively underwen TIPS (group I, n = 30), a modified Sugiura procedure (group II, n = 30) and TIPS plus a modified Sugiura procedure (group III, n = 30). The rates of rebleeding and encephalopathy, and the mortality of patients with portal hypertension were observed. RESULTS: The early postoperative complications were observed in 23.33% of the patients in group I, in 30.0% of those in group II, and in 20.0% of those in group III. The early postoperative rates of rebleeding were 6.67% in group I, 10% in group II, and 0% in group III. Hepatic encephalopathy was seen in 16.67% of the patients in group I, 0% in group II and 13.33% in group III. The early mortality was 3.33% in group I. During follow-up for 1 - 36 months, the rates of rebleeding were 34.48% in group I, 33.33% in group II, and 3.33% in group III. The rates of encephalopathy and mortality were respectively 17.20% and 20.69% in group I, 3.33% and 13.33% in group II, 3.33% and 3.33% in group III. The shunt patency in group III was higher than that in group I. CONCLUSION: Combination of TIPS and a modified Sugiura procedure can be effective to prevent and treat variceal bleeding.

[Long-term effect on preoperative ulnaris arterial intubation chemotherapy in treatment of stage III breast cancer].

OBJECTIVE: To evaluate preoperative ulnaris arterial intubation chemotherapy as a step of multidisciplinary treatment of stage III breast cancer. METHODS: 109 patients with stage III breast cancer were studied. 64 patients (group A) were treated by preoperative ulnaris arterial intubation chemotherapy and others (group B) were not. The 5 and 10-year survival rates and disease-free survival rates were analyzed. RESULTS: In patients with stage III breast cancer, the overall response rate of group A was 76.6%, and the response rate to histological study was 89.1%. The 5-year and 10-year survival rates of group A were 64.0% and 47.9% respectively. The disease-free survival rate of group A was 59.4% at 5 years and 43.3% at 10 years. All were higher than the 5-year and 10-year survival rates (31.1%, 21.9%) and the 5-year and 10-year disease-free survival rates (22.2%, 12.5%) of group B (P < 0.05). CONCLUSION: Preoperative ulnaris arterial intubation chemotherapy is an effective measure for the treatment of stage III breast cancer. It can improve the long-term effects on stage III breast cancer and reduce local relapse and distant metastasis.

[The development of degenerative disc animal model in rhesus monkey].

OBJECTIVE: To develop the degenerative disc animal model with histological and MRI observation. METHODS: Eight adult Rhesus monkeys were divided randomly into two groups after the lumbar discs MRI scan. The operated group, 6 monkeys, was subjected to annulus resection. A 5 mm x 3 mm cut was made in the left anterolateral part of the fibrosus annulus on L(4,5), L(5,6) and L(6,7) discs. The operated discs were scanned with MRI at 1 week, 4 weeks, 8 weeks, 13 weeks and 20 weeks postoperatively and observed histologically at 4 weeks, 8 weeks and 20 weeks after operation. RESULTS: The histological structure of Rhesus monkey was similar to that of human being. The percentage of high signal intensity area in the operated discs reduced significantly at 8, 13, 20 weeks after operation (P < 0.05). The fibroplasia outside the annular tear could be observed in all 6 operated discs at 4 weeks postoperatively. Two of 6 operated discs underwent fibroplasia at 20 weeks after operation. No heal evidence in the inner tear was showed after operation. CONCLUSIONS: The animal model for human lumbar disc degeneration could be obtained by anterio-lateral annulus resection. The early-stage degenerative evidence could be observed without injury by measuring the percentage of the high intensity area of the operated discs on T2WI of MRI.

[The characteristics and curative effects of strabismus following retinal detachment surgery].

OBJECTIVE: To describe the clinical characteristics and evaluate the surgical results in strabismus eyes which all had undergone retinal detachment surgery. METHODS: This study covered 24 strabismus cases (28 eyes) following retinal detachment operations. For these, examinations involving prism with alternative cover test or Krimsky test, eyeball movement, and diplopia analysis were performed. Strabotomy was carried out combined with or without scleral buckling removal or secondary intraocular lens implantation. The post-operative follow-up period ranged from 1 month to 24 months. RESULTS: (1) The main clinical feature in 16 strabismus cases (19 eyes) having managed by rhegmatogenous retinal detachment surgery was that the character of strabismus was related to the location of scleral explants. (2) Diplopia completely disappeared in 40.9% and partly disappeared in 54.5% of the cases. Strabismus was completely corrected in 63.6% of 27 eyes and partly corrected in 31.8% of them. CONCLUSIONS: (1) The strabismus following retinal detachment surgery is a restrictive strabismus, the clinical characteristics of which are different from that of palsy strabismus. (2) Strabotomy is effective in the treatment of restrictive strabismus.

[Hostility behavior as risk factor of coronary heart disease].

OBJECTIVE: To explore the relationships between hostility, and the risks of coronary-prone behavior and the clusters of unhealthy behaviors in individuals with different levels of hostility. METHODS: A cross-sectional study was carried out among 1 010 male employees from two factories in Beijing Capital Steel and Iron Company in 1997. Data on major risks of cardiovascular, other social-demographic factors and levels of hostility by Cook-Medley Scales were collected. RESULTS: Body mass index, amounts of cigarette smoking and alcohol drinking were significantly increased (P < 0.01) with a characteristic of reduced social support from one's colleagues, when individual's level of hostility increased. In addition, less years of education, higher prevalence of other chronic diseases (P < 0.05) and clustering of various unhealthy behaviors were found related to the increase of individual's hostility. The odds ratio in the group with highest hostility was 2.37 times higher than in the lowest one, after confounding effects from age, blood pressure, education level, status of chronic disease and social support being adjusted. No significant differences on both systolic and diastolic blood pressures among groups with different levels of hostility were discovered. CONCLUSION: The association between hostility, coronary-prone risk behavior and cluster of unhealthy behaviors suggested that hostility might increase the risk of coronary heart diseases through affecting individual's behavior.
