Hyperproduction of erythropoietin in nonanemic lead-exposed children.

Lead (Pb) poisoning has numerous effects on the erythropoietic system, but the precise mechanism whereby high dose exposure causes anemia is not entirely clear. We previously reported that Pb exposure is associated with depressed serum erythropoietin (EPO) in pregnant women residing in a Pb mining town and in a nonexposed town in Kosovo, Yugoslavia. In a prospective study, we tested the hypothesis that blood Pb concentration (BPb) may be associated with depressed EPO in children. BPb, hemoglobin (Hgb), and serum EPO were measured at ages 4.5, 6.5, and 9.5 years in 211, 178, and 234 children, respectively. At 4.5 years of age, mean BPbs were 38.9 and 9.0 microg/dl in the exposed and nonexposed towns, respectively; BPbs gradually declined to 28.2 and 6.5 microg/dl, respectively, by age 9.5 years. No differences were found in Hgb at any age. At age 4. 5 years, a positive association between BPb and EPO (beta = 0.21; p = 0.0001), controlled for Hgb, was found. The magnitude of this association declined to 0.11 at age 6.5 years (p = 0.0103) and 0.03 at age 9.5 years (p = 0.39). These results were confirmed using repeated measures analyses. We concluded that in Pb-exposed children, the maintenance of normal Hgb requires hyperproduction of EPO. With advancing age (and continuing exposure), this compensatory mechanism appears to be failing, suggesting a gradual loss of renal endocrine function due to Pb exposure.

Sexual risk behavior changes among HIV+ and HIV-female injecting drug users over 4 years.

This paper examines the sexual risk behavior of female injecting drug users who participated in a 4 year longitudinal study. Both HIV+ and HIV- women showed increases in monogamy, decreases in the frequency of unprotected vaginal/anal sex, and decreases in a risk index score throughout the study. HIV+ women had fewer occasions of unprotected sex than HIV-. However, a substantial proportion of the sample continued to engage in unprotected sex. Among the HIV+ women, depressed mood was significantly related to abstinence and to fewer occasions of unprotected sex, but CD4, medical symptoms, neurological impairment, and memory test performance were not associated with sexual risk behavior.

Excitation of baroreceptors depresses A- and C-components of the somato-cardiac sympathetic reflex in anesthetized rats.

The effect of baroreceptor activation on somato-cardiac sympathetic reflex discharges was examined in urethane-anesthetized, vagotomized, and artificially ventilated rats. Single shock stimulation of myelinated (A) and unmyelinated (C) fibers in the tibial nerve of the left hindlimb elicited two separate excitatory reflex discharge components in a branch of the cardiac sympathetic nerve. They are termed the A- and C-components of the somato-cardiac sympathetic reflex discharges. When aortic nerves (AN) and carotid sinus nerves (CSN) were intact, a sudden increase in mean arterial blood pressure to about 150 mmHg induced by I.V. injection of phenylephrine (50 micro/kg) depressed the A- and C-components by up to 47 +/- 5.4 and 37 +/- 7.7% of the control values, respectively. However, bilateral sino-aortic denervation completely abolished the pressure-induced depression of both components. We conclude that baroreceptor afferent signals from the AN and CSN inhibit both A- and C-components of the excitatory somato-cardiac sympathetic reflex discharges. This and other previous evidence mentioned in the text indicate that inhibitory cardiac sympathetic reflexes originating from arterial baroreceptors and excitatory ones originating from somatic afferents interact, probably at the brainstem.

Antidotal effect of glucoside extracted from Astragalus membranaceus on dimethoate intoxication in guinea pigs.

Therapeutic effect of the glucoside extracted from the root of Astragalus Membranaceus (AM) was studied in guinea pigs intoxicated with 600 mg/kg doses of Dimethoate (D). Four groups of guinea pigs, each consisting of two males and two females, were treated with (a) D, (b) D + Atropine, and (c) D + AM, (d) D + Atropine + AM. The survival time of the animals increased from an average of 70 minutes without AM to an average of 235 minutes with AM. Severe changes in ECG were observed prior to respiratory distress in groups without AM, and Atropine did not modify such changes. Differently, severe ECG disorders appeared only after respiratory distress in groups treated with AM. Prolongation of the Q-T interval and changing of the T wave configuration were significantly mitigated in the AM treated animals, while, arrhythmias were minimized and postponed. Moreover, muscular fasciculation and fibrillation, seizures and secretion in the respiratory tract were also significantly reduced by AM treatment. Results have shown that AM could be a promising drug to be used after cholinergic crisis in the treatment of cardiac complications with severe organophosphate intoxication.

[Effects of iron supplement on metabolism of iron and folic acid in rat liver].

OBJECTIVE: To study the effects of iron supplement on metabolism of iron and folic acid. METHODS: Thirty-two weanling Sprague-Dawley rats were divided into two groups, iron-normal and iron-deficiency, feeding twice a day with synthetic fodder containing 800 micrograms and less than 20 micrograms iron per day, respectively. Half of the rats in each group were supplemented with 8,000 micrograms of iron daily, and the other half of the rats were fed unchanged. Finally, four groups of rats, i.e., iron-normal with iron supplement, iron-deficiency with iron supplement, iron-normal and iron-deficiency, were set up. The fodder for all the four groups contained less than 10 micrograms of folic acid per day. RESULTS: Iron deficiency affected the growth, body weight and hemoglobin level of the rats, and their liver weight and iron content in the liver were significantly lower than the normal level, and their folic acid level was significantly higher than that in iron-normal rats. The growth, weight and hemoglobin level improved significantly, and iron content in the liver increased rapidly and exceeded the normal level after supplement with iron, while folic acid level in the liver declined until that was significantly lower than the normal. CONCLUSION: Folic acid content in the rats with deficiency both in iron and folic acid declined further after iron supplement, and no significant change was found in iron-normal rats.

Immunohistochemical study of Fas antigen expression in synovial tissues from patients with rheumatoid arthritis.

OBJECTIVE: To investigate the location and expression of Fas (Apo-1, CD-95) antigen in synovial tissue from rheumatoid arthritis (RA). METHODS: Immunohistochemical technique was used to identify the location and expression of Fas antigen in synovial tissues from 27 RA, 11 osteoarthritis (OA), 7 ankloysing spondylitis (AS), 3 pigmented villo-hyperplastic synovitis, 1 juvenile rheumatoid (JRA) patients and 5 "normal" control subjects. RESULTS: Fas was strongly expressed by synoviocytes and infiltrated lymphocytes in approximately two-thirds of RA patients (16/27). However, only weak expression occurred on lymphocytes in 3 of 11 OA, 1 of 7 AS patients and 1 of 5 "normal" subjects. The stain-positive substance in the forms of rings, granules, or dust was deposited on the cell membrane and in cytoplasm. CONCLUSION: The expression of Fas may be involved in the mechanism of synovium proliferation and abnormal activation of local lymphocytes in RA.

[Two-stage operation in the treatment of severe cicatricial adhesion on submental-thoracic region].

In order to decrease the risk of operation, complete release of scar tissue and reduce the recurrence after operation, from February 1994 to March 1997, seventy-three cases of severe cicatricical adhesion on the submental-thoracic region were treated with release and resection of scar tissue, and delay skin graft. The grafted skin was survived completely after operation. The result from the release of scar tissue of the cervical region was good. The physiological angle of submental-thoracic angle was recovered. It was thought that two-stage operation for submental-thoracic cicatricial adhesion would decrease the risk of operation and be advantageous to the release of contracted soft tissue of anterior cervical region and reduce the recurrence of contracture. The interval between the two stages of the operation was 2 to 3 days, which did not increase the rate of infection of the wound. The shortcomings including increase of pain to patient and prolong the time in hospital.

Basic fibroblast growth factor protected forebrain against ischemia-reperfusion damage in rats.

AIM: To study the effect of basic fibroblast growth factor (bFGF) on acute forebrain ischemia-reperfusion injury in rats. METHODS: Both vertebral arteries were occluded by electrocautery and severe but transient bilateral cerebral ischemia was produced by clamping both common carotid arteries for 20 min in rats. The contents of dopamine (DA) and 3,4-dihydroxyphenylacetic acid (DOPAC) in striatum were determined by HPLC with electrochemic detector (HPLC-EC) at 3, 6, 12, 24 h after the onset of reperfusion. The contents of sodium, potassium in forebrain were determined by atomic absorption spectro-photometric method at 6-h reperfusion. Morphological changes of the striatum were also examined. RESULTS: At 6-h reperfusion, the DA content in striatum decreased from (99 +/- 16) micrograms.g-1 (protein) in sham-operation group to (70 +/- 20) micrograms.g-1 (protein); the water and the sodium contents in forebrain increased from 77.34% +/- 0.19% to 79.6% +/- 0.6% and from (9.3 +/- 0.6) to (10.5 +/- 0.6) mg.g-1 (dry weight), respectively. bFGF (i.v. 45 micrograms.kg-1.h-1 for 3 h) from the start of reperfusion prevented the decrease of DA in striatum and the increases of water and sodium contents in forebrain. Histological examination also indicated that bFGF ameliorated the injury of neurons. CONCLUSION: bFGF prevented the brain from injury of ischemia and reperfusion.

[Regional differences in capillary density of children's skin--an enzyme histochemical and stereological study].

OBJECTIVE: The purpose of this study was to investigate the capillary distribution characteristics of the human skin. METHOD: The alkaline phosphatase enzyme histochemical and stereological methods were used to measure the length density (Lv) of capillaries of normal skin. One hundred and twenty specimens were taken from 20 different sites in 6 children cadavers. RESULT: The results showed: 1. The capillary Lv of the human skin varied with the body region. Statistically significantly (P < 0.01) greater capillary Lv was found in the head-face/neck region both in the papillary and reticular dermis than in the other parts of the body (excluding palm and sole). The smallest dermal capillary Lv was noted in the lower limb. 2. Statistically significantly (P < 0.001) higher capillary Lv was found in the papillary dermis than in the reticular dermis. CONCLUSION: The study confirms the regional and laminal differences in capillary density of the normal human skin, and it demonstrates more accurately rich blood supply in the head-face/neck region, which supports the hypothesis that longer random-pattern skin flaps can be safely raised in the head-face/neck than in other parts of the body.

[Study on the characteristics of personality, emotion and the plasma arginine vasopressin levels in patients with Gan Yang Shang Kang Zheng].

Eighty patients with Gan Yang Shang Kang Zheng (GYSKZ) and the related Zheng were questioned with three psychological scales: Type A Behavior Inventory, STAI and SDS and plasma AVP (arginine ressopressin) levels were determined by RIA. The TAB, S-AI, T-AI scores and plasma AVP levels of the patients with GYSKZ increased consistently and the changes of all parameters in different diseases in the same Zheng were similar. The results suggest that in patients with GYSKZ, type A behavior is the dominant personality and anxiety is the chief emotional disorder. Patients with GYSKZ are in the state of high level of psychological stress. Patients with GYSKZ and GHSYZ have some common pathophysiological basis.

[Relationship between the toxigenicity and ribotype distribution of Pseudomonas cocovenenans subsp. farinofermentans].

By rDNA fingerprinting technique, ribotype discrimination analysis was conducted for exploring the relationship between the toxigenicity and ribotype distribution of 42 strains of Pseudomonas cocovenenans subsp. farinofermentans. The results showed that 73.8 percent of the strains were toxigenic, which were distributed within the most ribotype and isolated from either food poisoning samples or normal food samples. The other strains were not toxigenic; which were isolated from normal food samples and distributed only within a few ribotypes. Since the most clones of P. cocovenenans subsp. farinofermentans were toxigenic, their potential hazard should be concerned.

[Study on the molecular epidemiology characteristics of Pseudomonas cocovenenans subsp. farinofermentans isolated from China with rDNA fingerprinting].

After the chromosomal DNA digestion by Bgl II and EcoRV, rDNA fingerprinting for Pseudomonas cocovenenans subsp. farinofermentans was made using biotin-labeled 16S + 23S rDNA as a probe. Fifty one strains of P. cocovenenans subsp. farinofermentans isolated from some areas of China were divided into 20 ribotyes, 52.9 percent of which were RT6, 9 and 2. In most cases, there were 2 or more ribotypes in each province or city. Though same ribotypes were isolated from different provinces, each ribotype existed in part of an area. The ribotype of RT6 was detected in samples from 4 incidents of food poisoning in Guizhou, Jiangsu and Hebei provinces, which indicated that these incidents may be caused by the same clone. With discrimination and cluster analysis of ribotypes, the rDNA fingerprinting may effectively contribute to analyzing the homogeneity of different strains, tracing the transmission route, and exploring the evolution and epidemiological characteristics of P. cocovenenans subsp. farinofermentans.

[The risk of lung cancer and mesothelioma in farmers exposed to crocidolite in environment].

To assess the risk of lung cancer and mesothelioma after environmental exposure to crocidolite for 20-30 years, a retrospective cohort study was carried out in farmers who had been exposed to crocidolite in environment. 1610 subjects were followed up for 9 years (Jan. 1, 1987 Dec. 31, 1995). The control group consisted of 7646 farmers who resided in the noncrocidolite pollution rural area in the same province. The results showed that the annual mortality rate was 43.75 per 100,000 population for lung cancer, and 36.46 per 100,000 for mesothelioma. Significantly high risks of lung cancer (RR 5.67) and mesothelioma (RR 182.3) were noted. These results demonstrate a strong causal association between lung cancer, mesothelioma and exposure to crocidolite.

[A comparison study on the sensitivity of lung tumor short-term induction test in three strains of mice].

To compare the sensitivities of lung tumor short-term induction in different strains of mice, the carcinogenicity risk was evaluated with the incidence rate and the average number of tumors. Three different strains of mice (KM, BALB/c and A/J) were injected with a single different dose of urethane i.p. The results demonstrated that the lowest dose for increasing tumor risk was 100 mg/kg in KM, 200 mg/kg in BALB/c, and lower than 25 mg/kg in A/J strain. So, the sensitivity of A/J strain is the highest and that of BALB/c is the lowest.

[Regulative effects pentagastrin and somatostatin on growth of human gastric cancer cells in vitro].

In order to observe the regulative effects of pentagastrin (PG) and somatostatin (SS) on the growth of two human gastric cancer cell lines (HGC803 and HGC823) in vitro, we observed the effects of PG and SS on proliferation of human gastric cancer cells by means of MTT. The contents such as gastrin, insulin, and glucagon were determined by radioimmunoassay (RIA), and the hexosamine content was determined by Neuhaus' method. The results showed that the growth of the two human gastric cancer cell lines were obviously promoted by PG. On the contrary, the growth and secretion of gastrin and glucagon were inhibited by SS. In addition, the hexosamine content of human gastric cancer cells was significantly increased by PG (7.58 +/- 0.66 versus 4.20 +/- 0.39 pg/cell, (P < 0.05). But the hexosamine content was decreased by SS (2.62 +/- 0.29 versus 4.20 +/- 0.39 pg/cell, P < 0.05). These findings indicate that the growth of gastric cancer cells is regulated by PG and SS, nevertheless a host of problems need to be elucidated.

[Transvaginal ultrasound assessment of cervical changes before onset of labor].

OBJECTIVE: To study the clinical value of cervical measurement by transvaginal ultrasound after 36 weeks of gestation. METHODS: Eighty three normal nulliparous woman were studied from 36 weeks of gestation until delivery prospectivelly. Transvaginal ultrasound examinations were performed and cervical length (CL), cervical width (CW) and the diameter of cervical canal were measured weekly. 52 cases were delivered naturelly and 31 cases by cesarean sections (CS). RESULTS: From 36 weeks of gestation until delivery, there was a significant correlation between gestation age (GA) and CL and the diameter of cervical canal (r = -0.37, P < 0.00001; r = 0.16, P = 0.002). The time from examination to initiation of labor was significantly correlated with CL and the diameter of cervical canal (r = 0.32, P < 0.00001; r = -0.19, P = 0.02). The regressive formulas were CL (cm) = 13.77 - 0.27 x GA, the time to beginning of labor (week) = 0.64 + 0.54 x CL (cm). The cases with CL > or = 3.5 cm at 2-3 weeks before labor had a significant higher CS rate than those with CL < or = 3.5 cm (P = 0.002). CONCLUSIONS: Transvaginal ultrasonographic measurement of CL, CW and the diameter of cervical canal could reflect the maturity of cervix accurately. It could be used as predictors of labor course and outcome of delivery.

[The effect of enzyme-linked immunosorbant assay and polymerase chain reaction on the detection of maternal and fetal human cytomegalovirus infection].

OBJECTIVE: To evaluate polymerase chain reaction (PCR) and enzyme-linked immunosorbant assay (ELISA) on detection human cytomegalovirus (HCMV) infection in preghnant women and fetuses. METHODS: ELISA was used to screen for serum HCMV-IgM in 1881 pregnant women. Blood HCMV DNA was examined by PCR in 656 of them simultaneously. 99 fetuses or newborns of HCMV infected mothers and 47 of uninfected ones were detected by both methods. RESULTS: The positive rate of serum HCMV-IgM in pregnant women was 2.4%, and that of HCMV DNA was 12.0%, 17.2% of fetuses or newborns from infected mothers had either positive HCMV-IgM or positive HCMV DNA or both of them. The intrauterine transmission rate of HCMV in mothers with positive HCMV-IgM or positive HCMV DNA or both of them was 18.5%, 14.8% and 27.3% respectively. Simultaneous detection with ELISA and PCR in 656 pregnant women and 146 fetuses showed fine correlations and significant differences between two methods. PCR had significantly higher detection rates than that of ELISA. CONCLUSIONS: ELISA combined with PCR may raise the detection rate and diagnostic efficacy of HCMV infection in both pregnant women and fetuses.

[Endoscopic variceal ligation combined with partial splenic embolization: preliminary clinical results].

Endoscopic variceal ligation combined with partial splenic embolization (EVL-PSE) was performed in a group of 13 patients with esophageal variceal bleeding and hypersplenism due to portal hypertension from January 1997 to March 1998. PSE was performed one to two weeks before or one week after initial EVL, and a range of 30% to 60% of the splenic parenchyma was embolized. Repeated EVL was performed at two week intervals until the varices were eradicated. Active bleeding in the nine patients was successfully controlled and all the varices of the 13 patients were eradicated after EVL-PSE. Eradication of the varices required two to five(mean 3.1) EVL sessions, follow-up ranging from 2 to 16 months(mean 6.9 months). In all but one case, no rebleeding occurred. All patients after PSE showed a good response on peripheral blood cell count and reduction of splenomegaly. No major complication or death related to the combination therapy was observed. Preliminary results in this study show that this combination therapy may result in more rapid eradication of the varices and reduce rebleeding after endoscopic variceal ligation. However, more data and studies may be necessary for further evaluation.
